To the Editor.— Diseases of the gallbladder and bile ducts, while uncommon among infants and children, still occur with sufficient frequency that they must be remembered by all pediatric clinicians. One of the problems in this regard is that of terminology. If stones are present in the gallbladder or ducts there is no problem, though one has to know that pigmented and nonpigmented (usually cholesterol) stones are both found in children. Cholelithiasis, with or without cholecystitis, is the proper diagnosis.

The manifestation of gallbladder disease varies widely in terms of symptoms, clinical signs and severity. Gallstones, for example, affect 10–15% of the adult population, equating to more than 5 million people in the UK. Indeed, gallbladder calculi are the most common abdominal problem leading to hospital admission. In addition, the cholecystectomy is one of the most commonly performed surgical procedures, with 50 000 per year in the UK. Incomplete gallbladder removal usually leads to residual gallbladder, which can act as a nidus for stone formation in later life. Stump cholecystitis is a condition in which, such a large gallbladder remnant left after subtotal cholecystectomy becomes inflamed. When this happens, a completion cholecystectomy is advised. We present a rare case of residual gall bladder in a 44-year-old woman who underwent a subtotal cholecystectomy procedure 12 years ago.

Acute cholecystitis is the most common complication of cholelithiasis accounting for 14 to 30% of cholecystectomies performed in many countries. Symptoms in cholecystitis are due to impaction of stone and subsequent distention of gallbladder with inflammation. Study is aimed to clarify the role of ultrasound guided transhepatic gallbladder aspiration in the early management of acute calculous cholecystitis. Methods: The stu

Hydrocephalus is a prevalent health problem that is frequently encountered by paediatric neurosurgeons during infancy and childhood. We report a case of an 11-year-old boy with high cerebrospinal fluid protein hydrocephalus secondary to optic glioma that required a ventriculoperitoneal (VP) shunt. The patient had multiple failures of VP shunt and developed massive ascites. Alternatively, the hydrocephalus was treated by ventriculo-gallbladder (VG) shunt in the presence of sludge which was removed from the gallbladder before placement of the shunt.
