Introduction: In long-term survivors after OLT, a reduction of the diffusion capacity (TLCO) may be noticed in absence of interstitial pulmonary changes (as observed in high resolution computertomography HR-CT). Also a diminution of the body muscle mass may be seen together with an increase in body fat mass. We tried to identify the origin of the pulmonary diffusion impairment as an alteration of the membrane factor or the capillary volume. We analysed the progression of the impairment in the course time and the effects of cardiac and respiratory function on VO2max. Methods: In 38 patients (67.7 month after OLT) we determined TLCO, membrane factor, capillary volume, ventilatory drive, maximal inspiratory pressure (PImax), maximal O2-uptake on exercise and breathing efficiency. Objectives: The ARDS (acute respiratory distress syndrome) is characterized by a diffuse inflammatory response, a pulmonary hypertension and a severe hypoxemia mainly because of ventilation-perfusion-mismatching. Despite of some new therapeutical strageties the mortality remains high. Almitrine bismesylate was first described to improve arterial oxygenation in COPD- patients primary by increasing alveolar ventilation due to a stimulating effect on peripheral chemoreceptors. Furthermore several experimental studies have confirmed that almitrine improves PaO2 possibly because of reducing the ventilation- perfusion-mismatching by enhancing the hypoxic pulmonary vasoconstriction (HPV). Controversal results have been reported concernig the doses of intravenous Almitrine to be beneficial. Furthermore it remains unclear to what extend the pulmonary hypertension is augmented by a drug which is inhancing or restroring the HPV. Therefore we determined a dose-response-curve of almitrine in a animal model of ARDS. Methods: After induction of anaesthesia the ARDS-Modell was induced by repeated saline wash-0,5-2 ?g/kgKG/minout of surfactant. 14 swine received almitrine or the solvent in a prospective randomized manner. During the experiment the volume controlled mode of mechanical ventilation remained unchanged (FiO2 1,0 , PEEP 5 cm H2O). After the increasing doses of 0,5, 1, 2, 4, 8 and 16 ?g/kgKG/min each for 30 min we investigated the effects of i.v. almitrine on the pulmonary gasexchange and the pulomanry and systemic hemodynamics. “Gas exchange during inhalation anaesthesia of horses - subject to immediate or delayed IPPV (intermittent positive pressure ventilation)” Large animal Clinic for surgery, faculty of Veterinary Medicine, University of Leipzig Handed in Januar 2007 93 pages, 18 figures, 15 tables, 127 references, appendix with 19 tables Keywords: anaesthesia, horse, ventilation, IPPV, gas exchange Aim of the present study was to prove that gas exchange, especially the oxygenation of the blood, can be improved in the anaesthetised horse, if intermittent positive pressure ventilation (IPPV) is started immediately after introduction of anaesthesia. Furthermore, we put into question if an uncompromising ventilation with low ventilation pressures and low respiratory frequencies can prevent anaesthesia- associated functional disorders of the lungs. The literature part of the present study describes the physiology of respiration and lung function in general anaesthesia, with focus on the effects on the horse. Different forms of ventilation are demonstrated, followed by an illustration of an extensive monitoring for surveillance of respiration and cardiovascular circulation. Thirty adult horses in the age from 2 to 15 years referred to the horse departement of the Tierklinik Wahlstedt with a body weight ranging from 335 to 650 kg bwt were divided into two comparable groups. After premedication with romifidine (0.08 mg/kg, Sedivet, Boehringer Ingelheim) and an introduction of anaesthesia with diazepam (0.1mg/kg, Diazepam.ratiopharm, Ratiopharm) and ketamine (3.0mg/kg, Ursotamin, Medistar), the 15 horses of group I were immediately ventilated with controlled IPPV. The tidal volume was fixed at 1.5l/100kg and the respiratory frequency at 5 breaths/min. The horses asigned to group II were breathing spontanousely for 40 minutes, after which they were switched to a controlled IPPV modus with 5 breaths/min. Anaesthesia was maintained with isoflurane (Isoflo, Essex) and approximately 100% oxygen in both groups. Following parameters measured during anaesthesia were statistically evaluated: heart rate, respiratory frequence, mean arterial blood pressure, endexpiratory concentration of CO2, expiratory concentration of isoflurane, inspiratory concentration of O2, ventilation pressure, tidal volume, minute volume of ventilation, consumption of dobutamine and the dynamic compliance (during ventilation time). Blood gas analysis of the arterial blood samples were realized 20, 40 and 55 minutes after initiation of anaesthesia. The arterial partial pressure of oxygen (PaO2), arterio-arterial difference of partial pressure of oxygen (AaDPO2), arterial partial pressure of carbon dioxide, arterial pH, base excess, arterial bicarbonate and arterial saturation of oxygen were measured. Afterwards, the alveolar dead space was calculated. The named parameters were statistically interpreted in comparision between group I and II and in a comparable way during the entire anaethesia period. The evaluation of the listed parameters resulted in following findings: the values of the oxygenation, PaO2 and AaDPO2 were significantly different in the two groups at all three measure points. In group I, the PaO2 values were higher (381±17 and 207±35 mmHg (20 min), 409±19 and 212±28 mmHg (40 min) and 415±24 and 176±33 mmHg (55 min)) and the AaDPO2 values were significantly lower (176±16 and 298±29 mmHg (20 min), 179±17 and 324± 29 mmHg (40 min) and 184±21 and 322±36 mmHg (55 min)) compared to those of group II. The values for arterial partial pressure of carbon dioxide of group I were significantly decreased at every moment in time in comparison to those in group II (46±1 and 58±2 mmHg (20 min), 45±1 and 63±2 mmHg (40 min) and 46±2 and 50±1 mmHg (55 min)). Referring to the calculated alveolar dead space, a significant difference between the groups was found at all time. This parameter was always increased in horses asigned to group II compared to those in group I (0,08±0,02 and 0,18±0,03 (20 min), 0,11±0,03 and 0,21±0,04 (40 min) and 0,10±0,02 and 0,19±0,02 (55 min)). The evaluation of the expiratory carbon dioxide showed a significantly higher value in group II than in group I in the measure periods 2 (41±1 and 46±2 mmHg), 3 (41±1 and 46±2 mmHg) and 5-8 (41±1 and 45±1 mmHg (5), 41±1 and 45±1 mmHg (6), 40±1 and 45±1 mmHg (7) and 40±1 and 46±1 mmHg (8)). From measure period 9 onwards, at which both groups were ventilated, no significant difference between the groups could be determined. At the measure points 20 (7,39±0,007 and 7,31±0,013) and 40 (7,39±0,01 and 7,28±0,013), the arterial pH values of group II were significantly lower than those of group I. The comparison of mean arterial blood pressure and the consumption of dobutamine in both groups showed no distinction. The value for inspiratory respiratory minute volume was significantly higher in group I than in group II in the measure periods 2 (39±2 and 32±3 l) and 6 (39±2 and 31±3 l). The same statement is true for the tidal volume in the measure periods 1-3 (7765±278 and 6181±695 ml (1), 7846±291 and 6074±534 ml (2) and 7921±310 and 6549±459 (3)). The measurement of inspiratory oxygen flow showed higher values in group I than in group II in the measure periods 1 (74±2 and 68±2 mmHg), 2 (79±2 and 73±2 mmHg) and 4-6 (84±2 and 78±2 mmHg (4), 85±2 and 80±2 mmHg (5) and 87±1 and 82±2 mmHg (6)). The other surveilled parameters showed no significant differences. The results of this study verify that an immediate start of IPPV reduces the development of atelectasis, resulting in a reduced intrapulmonary right-left shunting volume and a reduced dead space ventilation in horses under inhalation anaesthesia, without producing a situation compromising the cardiovascular system, which increases the risk of general anaesthesia

This thesis aims to increase our understanding about plant responses to K, Mg and N limitation and their relationship with fruit quality of tomato. In the control treatment (100% biomass growth), plants were grown at high nutrient concentration. In the nutrient-limitation treatments, either K or Mg or N was supplied continuously at rates, which reduced biomass growth to about 80% (mild deficiency) or 60% (severe deficiency) of the control. This experimental approach allowed comparing long-term plant responses to deficiency of K, Mg or N at well-defined intensities of nutrient limitation. Nutrient limitation was associated with changes of biomass and nutrient allocation among various plant organs, whereby these changes were nutrient specific (e.g., biomass allocation to leaves was increased in Mg deficient plants, and decreased in N deficient plants), but not much dependent on the intensity of nutrient limitation. Effects of nutrient limitation on root and shoot morphological traits were also nutrient-specific and independent on the intensity of deficiency. Effects of nutrient limitation on fruit quality, e.g, sugar, lycopene and ascorbic acid content, were also nutrient-specific, and for specific parameter dependent on the intensity of deficiency. Responses to Mg deficiency were studied in more detail. In the first 6 days after start of Mg limitation, Mg concentrations in all plant organs drastically decreased without any effects on growth and leaf photosynthetic rates. Mg-deficiency increased leaf sugar concentrations, but sugar accumulation was not directly related to leaf photosynthesis which only decreased after leaf Mg concentrations were below 0.10 to 0.20 mg Mg g-1 leaf fresh mass. In Mg-deficient plants, sugars accumulated in source leaves even when the source/sink ratio was strongly decreased by shading of basal leaves. This is in accordance with the suggestion that sugar export is reduced due to inhibition of phloem loading.

Background: We investigated the combined effects of lung protective ventilation and extracorporal lung assist (av-ECLA) on hemodynamics and pulmonary gas exchange in a lavage-induced porcine model of severe ARDS. Methods: Saline lung lavage was performed in 15 healthy pigs until the arterial oxygen partial pressure decreased to < 60 mmHg. Apart from one sham-operated animal, the animals were randomized after a stabilisation period of 60 minutes into two groups: 7 animals received lung protective ventilation with increasing positive end-exspiratory pressure (PEEP) every 15 minutes, 7 animals were additionally treated with av-ECLA. Results: With rising PEEP levels the mean arterial blood pressure (MAP) and the cardiac output (CO) decreased significantly in both groups. No significant decrease of MAP was observed by using the av-ECLA but pulmonary shunt fraction increased significantly. Normocapnia was only achievable with av-ECLA. Carbondioxid- elimination was highly-effective with av-ECLA, the contribution to oxygenation was antagonized by the increase in pulmonary shunt fraction. Conclusion: In this porcine model of severe ARDS, only the combination of av-ECLA and lung protective ventilation results in normokapnia. No significant decrease of MAP was observed by using av-ECLA

Background: During high-frequency oscillatory ventilation (HFOV) the applied tidal volume can be minimized and thereby reduce the risk of alveolar shear stress, however, higher oscillatory frequencies result in insufficient CO2-elimination with development of hypercapnia and respiratory acidosis. In this experimental study, the effect of different oscillatory frequencies in HFOV in regards to CO2-elimination with or without the combination with an arteriovenous extracorporeal lung assist (av-ECLA) was investigated in an animal-model of ARDS. Our hypothesis: the use of high oscillation frequencies and therefore the minimization of tidal volume requires the combination of HFOV with av-ECLA to maintain or reestablish normocapnia. Methods: Therefore eight healthy pigs (56.5 ± 4.4 kg) were anesthetized and intubated, followed by a pulmonary lavage to induce a severe iatrogenic ARDS. After a stabilisation period of 60 minutes (PaO2 continuously < 80 mmHg) a recruitment maneuver was performed and mean airway pressure was adjusted 3 cmH2O above the previously determined lower inflection point. The animals were then transferred to HFOV, randomized and ventilated with either ascending or descending oscillatory frequencies for 30 minutes alternating with and without av-ECLA. Results: At oscillatory frequencies of 9 Hz and above without av-ECLA, the animals rapidly developed a hypercapnia, which could only be counteracted by combining HFOV with av-ECLA. Due to the recruitment maneuver and the adjustment of the mean airway pressure to 3 cmH2O above the lower inflection point, the oxygenation could be significantly improved throughout the trial (p<0.05). The results of both groups (ascending vs. descending oscillatory frequencies) did not differ from each other. Conclusion: During high-frequency oscillatory ventilation (HFOV) with oscillatory frequencies of 9 - 15 Hz, normocapnia could only be maintained by the combination with an arteriovenous extracorporeal lung assist (av-ECLA). In addition, after recruitment maneuver and the adjustment of the mean airway pressure to 3 cmH2O above the lower inflection point, a permanent and significant improvement in the oxygenation could be observed, even at very high oscillation frequencies. Therefore, the av-ECLA unmasks the lung-protective potential of HFOV: the minimization of the applied tidal volume not only limits an alveolar over-inflation and therefore volutrauma; the application of higher mean airway pressures also enables pulmonary recruitment and therefore protects the lung from atelecttrauma
