Abstract:
This study aims to investigate the occurrence, risk factors, clinical presentation, and management of gastric conduit necrosis following esophageal reconstruction, providing insights into its impact on patient outcomes and strategies for prevention and treatment. A retrospective analysis was conducted on patients who underwent esophageal reconstruction with gastric conduit creation between January 2010 and December 2023. Clinical records, including surgical notes, postoperative outcomes, and follow-up data, were reviewed to identify cases of gastric conduit necrosis. Risk factors such as surgi

Abstract:
Gastric necrosis is a rare condition, more so in children and can be potentially fatal. The sloughing of the necrosed wall results in perforations or large defects in the gastric wall. We report a rare case of extensive gastric necrosis in a 2 months old female infant, managed successfully. Child presented with non-bilious vomiting and excessive cry. There was history of recurrent upper abdominal distension. X-ray of abdomen showed massive pneumoperitoneum. Intra operatively, posterior gastric wall was sloughed off and lower end of feeding tube was lying in the peritoneal cavity. Necrotic wall

The acute respiratory distress syndrome (ARDS) is a syndrome of acute respiratory failure characterized by the acute onset of non-cardiogenic pulmonary oedema due to increased lung endothelial and alveolar epithelial permeability. Common predisposing clinical conditions include sepsis, pneumonia, severe traumatic injury, and aspiration of gastric contents. Environmental factors, such as alcohol abuse and cigarette smoke exposure may increase the risk of developing ARDS in those at risk. The major causes of morbidity and mortality in acute pancreatitis are organ dysfunction and infection of necrotic tissue. Management should aim to prevent, or to diagnose and treat, the complications of pancreatic inflammation, and any predisposing factors to avoid recurrence. Medical management is essentially supportive with oxygen, intravenous fluids, analgesia, enteral or parenteral nutrition, and correction of metabolic abnormalities. Patients with severe acute pancreatitis are unlikely to resume prompt oral intake so nutritional support is also required. Post-pyloric feeding is not requir

Abstract:
Gastritis and peptic ulcers are caused by Helicobacter pylori and can be treated with antibiotics. Helicobacter pylori is the only bacterium that can cause cancer. To grow at the pH of the stomach, the bacterium produces urease that cleaves urea to ammonia and bicarbonate. The ammonia neutralizes the stomach acid and damages the gastric mucosa. The urease requires nickel for action and Helicobacter produces nickel-scavenging proteins. The increased pH caused by ammonia liquefies the mucus layer and allows the bacteria to attach to the gastric epithelium where it uses a type VI secretion system

Esophagectomy is a challenging surgery that is known to be associated with high rates of morbidity. Anastomotic leaks, pneumonia, conduit necrosis and chyle leaks are the commonly reported complications. Perfusion assessment and tissue injection based fluorescence guided surgery (FGS) are the newer clinical applications of fluorescent dyes. With the advent and integration of real time fluorescence imaging with the existing minimal access platforms, the esophageal surgeon can employ these techniques to potentially improve outcomes. The impairment of liver clearance of activated coagulation factors, the release of thromboplastin-like activity from the necrotic liver cells and the hemodynamic changes due to expanded bollaterals may all contribute to activate the coagulation cascade. However, little is known about the mechanisms leading to this activation.
