Abstract:
Laparoscopic gastric plication is infolding of the greater curvature of the stomach to reduce gastric volume. This chapter covers the complications of the procedure, their causes, and their management. The most common complication is gastric obstruction, and it is the main reason for reoperation. Most mild obstructions can be managed conservatively. Patients with signs of ischemia and those with poor response to medical treatment need reoperation. Mild bleeding is not infrequent after gastric plication but severe bleeding is rare. Gastric perforation can occur, and treatment options include co

This chapter discusses the main issues with a selected group of surgical complications after Roux-en-Y gastric bypass. The main focus is on diagnosis and management of anastomotic leak and stricture, marginal ulcers, and gastrogastric fistulas. Anastomotic leak is one of the most feared and potentially catastrophic complications. The most common site is at gastrojejunostomy. Avoiding devascularization of the gastric pouch, creating a tension-free anastomosis, and performing an intraoperative anastomotic leak test are crucial technical points in preventing leaks. Once the leak is highly suspect

Aspiration pneumonitis during the perioperative period is a serious complication and involves passage of sterile gastric contents into the airway resulting in alveolar damage. The mechanism of aspiration pneumonitis is characterized by a significant inflammatory reaction. The risk of aspiration is highest during anesthesia induction, but it is also present during emergence and extubation. The risk factors include delayed gastric emptying (gastritis, pain, pregnancy, obesity, elevated intracranial pressure), emergency surgery, upper abdominal surgery, and difficulty securing the airway. This chapter addresses the diagnosis, investigation, and management of anaemia due to a deficiency in iron, vitamin B12, or folate. Erythropoiesis requires an adequate supply of iron for haem formation, as well as vitamin B12 and folic acid (folate) to support high levels of DNA synthesis, and a lack of any of these will result in anaemia. Iron-deficient anaemias are typically microcytic, while a deficiency in vitamin B12 or folate results in megaloblastic haemopoiesis and a macrocytic anaemia. Iron deficiency results from poor dietary iron intake, poor absorption, increased demands, blood los
