Full textThe need of evaluating health care practices has begun in the 80’s and has led to establishment of several agencies of evaluation. This work focused on two different approaches to evaluate care practices in colorectal cancer. The first approach is based on descriptive epidemiology methods. It concerns the respective places of several practitioners in the health care system. In this work, we studied, on a registry-based population, the respective places of general practitioners and gastroenterologists in the follow-up of patients resected with curative intent. This study emphasized that the places of general practitioners and gastroenterologists are complementary in clinical follow-up and in prescription of surveillance tests recommended by the consensus workshop of 1998. The second approach is based on survival of cancer. Indeed, estimations of survival allow evaluating the impact of preventive, diagnostic and therapeutic management care of cancers, and through a better knowledge of prognostic factors, may lead to improvements. In this work, we have developed and implemented a new relative survival model which estimates the effects of continuous prognostic factors avoiding the assumptions of proportionality and log-linearity of current models. Indeed, these hypotheses are rarely verified in oncology. This flexible modelisation, using B-splines, of prognostic factor effects allowed re-assessing both the effect of age and of period of diagnosis on the colon cancer-related mortality. Full textDigestive cancers are among the most common cancers, with a high mortality rate. They most often develop from a benign lesion that slowly evolves and eventually becomes cancerous. Organized screening by endoscopy increases the chances of cure, thanks to early detection and treatment. With recent optical advances in endoscopic imaging, a wide variety of lesions can be visualized during colonoscopic examinations. We first demonstrated that, once detected, colorectal lesions need to be characterized in order to predict histology in real time during endoscopy and choose the best therapeutic option for each lesion. Biopsies (targeted or not) are unnecessary and may make subsequent resection more difficult. The many existing classifications have their limitations, however, and we have therefore merged the criteria of all the usual classifications into the CONECCT classification, which in a prospective evaluation demonstrated its good diagnostic performance, enabling appropriate treatment choices to be made for lesions. We then sought to develop the initial CONECCT classification, by adding lesions with a focal zone of deep invasion, showing that diagnostic submucosal dissection is feasible for these lesions, and by adding colonic submucosal lesions, showing that as most are not malignant, they do not warrant advanced endoscopic resection but could benefit from a step-up strategy starting with simple histological sampling with advanced resection for the rare malignant lesions in a second stage. As lesion degeneration is not homogeneous, we sought to improve detection of areas of interest by showing that an area stained green in virtual chromoendoscopy, or green sign, could be associated with a more pejorative lesion histology. All the more so as we have described the difficulty presented by AI systems in detecting flat lesions such as sessile serrated lesions and non-granular laterally spreading tumors. As the characterization of colorectal lesions is essential but very difficult, we then sought to improve the characterization training of French-speaking gastroenterologists, via a working group on a social network, featuring educational videos dedicated to characterization. However, passive use of the group, with a very low degree of user interaction, failed to show any progression among residents over one year. Finally, we have developed a free application on computer and smartphone, enabling collaborative characterization by a trio of experts of 20 colorectal lesions published by 12 gastroenterologists

Full textIn athletic horses, physical activity leads to an important increase of energetic requirements, which are covered by important dietary supply. Our hypothesis was that physical activity could impair energetic components digestibility. This study aimed at evaluating the long-term effect of endurance conditioning on the digestibility of dietary energetic components. For this purpose, we adapted the conventional methods to athletic horses undergoing a daily work. Firstly, we demonstrated that the collection period of feces could be reduced to three and four days for total tract digestibility and mean retention time measurements respectively. We validated the use of a partial collection of feces for MRT measurements and pointed out that “acid detergent lignin” (ADL) was an appropiate marker for a digestibility measurement by partial collection. Secondly, we showed that the second week post-race represented a optimal period to conduct our measurements. Once the technical adjustments done, the adapted modus operandi was used to study the impact of increasing training levels during on digestibility in endurance horses. The results were as following : the first year, an increase of the digestibility, probably due to a longer MRT, was observed, but not the second year. The increase of energetic components digestibility in trained horses, as well as the underlying mecanisms, need to be confirmed but could be taken into account when nutritional studies are conducted on horses at maintenance and results extrapolated to sport horses

Full textThis work allows us to define the condition of use of the additive Yea Sacc®1026 (Saccharomyces cerevisiae CBS 493. 94) for horses fed with a complete pelleted feed. This dose corresponds to 6 - 10 x 10E9 UFC/kg complete feed. Our general approach of the large intestin ecosystem of horses pointed out the occurence of S. Cerevisiae CBS 493. 94 in both the cecum and the colon of horses, and several interactions between the yeast supplementation and the diet composition. With an alfalfa diet (HF), the increase of apparent digestibility of cell wall constituents of the diet in response to a Yea Sacc® supplement was confirmed by a stimulation of the fibrolytic activity determined both in vitro and in vivo. With a barley diet (HS), increases of both the retention time of digesta and the relative proportion of straw fiber in the diet diminished the cellulolytic stimulation effect of Yea Sacc® on digestive performances. However, our results showed a stabilisation of the hindgut pH in response to a Yea Sacc® supplement which decreased the L-lactic accumulation in both the cecum and colon. Although a limited number of meal was fed per day (2/day), the high energy density of the HS pelleted feed did not induce neither digestive or behaviour troubles in horses. Thus, providing a starchy complete pelleted feed with a sufficient amount of long fiber (wheat straw) does not disturb the digestive physiology of horses

Full textGastric dilatation-volvulus is an acute life-threatening disease in dog. Its obscur etiology and the many isolated risk factors make non invasive prevention strategy hard to be introduced and don't guarantee its efficacy. Prophylactic antropyloric gastropexy seems to be an efficient tool. The laparoscopic surgical procedure described here was studied on 7 female dogs for more than two month. This not very invasive procedure gives strong adhesions without any morbid consequences on the gastro-intestinal tract physiology (induced inflammation and pain, gastric emptying, gastro-intestinal sugar absorption). The absence of secondary effects and the lack of post operative morbidity of the technique lead to plan its use in routine clinics
