No. of bitstreams: 2 arquivo1982_1.pdf: 1649003 bytes, checksum: 1e1923dd1de26cd14dac820fd44e2ae8 (MD5) license.txt: 1748 bytes, checksum: 8a4605be74aa9ea9d79846c1fba20a33 (MD5) Previous issue date: 2008<br>Com os objetivos de estabelecer a ocorrência e os tipos de lesões gástricas na forma hepatoesplênica da esquistossomose mansônica (EMHE), e de verificar a existência de associação da gastropatia congestiva portal (GCP) com a intensidade da fibrose periportal (FPP) e com os sinais endoscópicos e ultra-sonográficos da hipertensão porta (HP), foram estudados prospectivamente 71 pacientes, selecionados no ambulatório de esquistossomose do HC, UFPE. Foram excluídos pacientes com antecedentes de tratamento cirúrgico para HP, escleroterapia ou ligadura elástica das varizes do esôfago, de alcoolismo, hepatites B e/ou C e pacientes em uso de beta-bloqueador. Todos os pacientes realizaram endoscopia digestiva alta (EDA), seguindo um protocolo para caracterização das varizes do esôfago, da gastrite e da GCP, realizando biópsia no corpo e antro gástricos; ultra-sonografia (USG) do abdome superior, seguindo protocolo do Cairo e Niamey para caracterização do grau e padrão da FPP e dos diâmetros das veias porta, esplênica e também do diâmetro longitudinal do baço. Foram calculados médias, valores máximos, mínimos e desvios padrão das variáveis contínuas, apresentados distribuição de freqüência e percentuais, e utilizados os testes de Qui-quadrado e Exato de Fisher para verificar as associações entre a GCP e a intensidade da HP e grau da FPP. Em todos os testes foram aplicados níveis de confiabilidade com valores de 95% (p<0,05). A freqüência de GCP pela EDA foi de 54,9% (39 casos), sendo 33 (46,5%) leve e 06 (8,5%) intensa. A de gastrite foi de 83% (59 casos), sendo 32 (45,1%) enantematosa e 27 (38%) erosiva. A análise histopatológica revelou 20 casos (28,2%) de GCP, 39 casos (54,9%) de gastrite, 07 casos (9,9%) de lesões associadas (GCP e gastrite) e 05 casos (7,0%) com mucosa normal. Não houve associação entre o diagnóstico endoscópico e o histopatológico da GCP. A elevada incidência de Helicobacter pylori possivelmente mascara o diagnóstico da GCP pela histopatologia. Igualmente, não houve associação da GCP com o calibre e sinais vermelhos das varizes do esôfago, assim como da GCP com o grau da FPP, nem com diâmetros das veias porta, esplênica e diâmetro longitudinal do baço. O grande número de circulação colateral observado em 33 pacientes (43,5%) foi possivelmente o motivo pelo qual não se obteve associação da GCP com a FPP, nem com os sinais preditivos de HP avaliados pela USG do abdome. Devido ao pequeno número de casos de GCP intensa (06 - 8,5%), conclui-se que, na EMHE sem antecedentes prévios de tratamento para HP, a GCP é uma condição benigna, com baixo risco de HDA

Abstract:

Portal hypertension (PH) is a serious consequence of several disease states affecting prehepatic, intrahepatic, or posthepatic portal circulation. Backpressure caused by PH transmits through the collaterals to form varices at various sites. PH also leads to hyperdynamic congestion and altered gastrointestinal mucosal immune response, resulting in portal hypertensive gastropathy (PHG), portal hypertensive enteropathy (PHE), and portal colopathy (PC). These PH associated phenomena may lead to torrential life-threatening bleed or chronic blood loss leading to debilitating chronic anemia. Endoscopy plays a pivotal role in the management of these patients both for diagnostic and therapeutic purpose. The choice of therapeutic strategy depends on many factors: severity of the disease, patient’s clinical performance, and whether it is done as an emergency or as a prophylactic approach. In this chapter, we evaluate the endoscopic management of patients with the gastrointestinal complications of PH.
