Abstract:
The Branch Davidians and Heaven’s Gate, two religious groups marked by apocalyptic worldviews, are compared to elucidate two types of trajectories of apocalyptic groups involved in violence. The Branch Davidians expected to be martyred based on David Koresh’s interpretations of the Bible’s prophecies about the events of the Last Days. Therefore, in 1993 they regarded assaults against their community by federal agents as verifying Koresh’s predictions. In 1997, the Heaven’s Gate “class” carried out what they regarded as an “exit” to The Level Above Human (TELAH) by implementing a collective religious suicide. In both cases, the interactions of outsiders with the group contributed to the resulting loss of life, although internal factors were more predominant in the case of Heaven’s Gate. The members of both groups lived and died in accordance with their respective worldviews and understandings of salvation.

Abstract:
This chapter presents evidence that at each level of the thalamocortical hierarchy the strength of our conscious perceptions increases. Conscious processes are not all-or-none effects, they are graded. Four factors may be particularly relevant for understanding the neural production of conscious experiences: (1) the actions of the thalamic gate; (2) the neural activity that anticipates an organism’s actions; (3) the activity of the hierarchy of cortical monitors; and particularly (4) the motor actions produced by the outputs of the cortical monitors and acting on the phylogenetically old parts of the brain: these serve to keep actions in accord with anticipations.

Abstract:
This chapter provides an overview of the healthcare delivery system. A figure illustrates how six different parts of the system relate to each other. The primary care level plays a key role in many countries by representing the gate, in which referrals to secondary care are being made. Tertiary care is principally of two types depending on patients’ prognosis: chronic care or rehabilitation. In addition to the three care levels, there are two parts with quite different roles: pharmacies provide pharmaceuticals, and sickness benefit schemes compensate the sick for their income losses. A recurrent policy challenge is to make each provider level take into account the resource implications of their isolated decisions outside of their own budgets. A brief discussion is included on the scope for ‘internal markets’.

Abstract:
Chapter 13 first looks at how changes at one level in the food supply chain may affect prices and quantities at another level via profit maximization. The chapter then considers firms that are closer to the consumer (e.g., restaurants) who will often be able to set their own prices and consider the analytics of profit maximization under this scenario. Utilizing this framework, the chapter considers the question: Are healthier foods more or less profitable than unhealthy foods? This leads naturally to a discussion of market segmentation, the limit of the market, and the distribution of healthy and unhealthy foods in the food system. As there are many calls for food firms to be more socially responsible and offer healthier foods, the chapter utilizes the framework to explore the implications of corporate social responsibility and how compatible that idea is with profit maximization.
