AbstractThe objective of this study was to develop a structural-cognitive-behavioral model for error analysis of group B streptococcus (GBS) prophylaxis failure, classify delivery cases into this model, and examine compliance with treatment guidelines. A retrospective, cohort study was conducted of women with liveborn pregnancies greater than 24 weeks in April 2018 at a single hospital. We created a structural-cognitive-behavioral model of five assessments for adherence to GBS prophylaxis guidelines and then classified these into four distinct error stages. A descriptive analysis was performed to determine if the pregnancy had a perfect process, a GBS prophylaxis failure, or a fortuitous outcome. There were 313 women who met the study criteria. The rate of GBS positive was 12.8%, negative 37.4%, and unknown 49.8%. The most common errors were cognitive perception errors related to incorrectly documenting GBS status, 57.7% (N = 79). Of these errors, 15.2% (N = 12) led to GBS prophylaxis failure. Perfect outcomes occurred in 62.7% (N = 196) women, GBS prophylaxis failure occurred in 13.7% (N = 43), and fortuitous outcomes occurred in 23.6% (N = 74). In our study, we were able to identify structural, cognitive, and behavioral errors that contribute to GBS prophylaxis failures. In other cases, these errors may contribute to fortuitous outcomes.

Abstract:
ObjectiveWe conducted a retrospective review of patients with a diagnosis of Guillain-Barré syndrome (GBS) to assess the diagnostic impact of applying age-adjusted upper limits for CSF total protein (CSF-TP) supported by a systematic literature review.MethodsCases coded as GBS or inflammatory neuropathy for the period 2001–2016 at The Ottawa Hospital were reviewed. Cases were included if they met the Brighton criteria for GBS with a diagnostic certainty level 1 or 2 and had contemporaneous CSF-TP data. We excluded cases with CSF pleocytosis >50 and cases with Miller-Fisher syndrome. Age-adjusted reference limits were compared with conventional 0.45 and 0.6 g/L upper limits.ResultsOne hundred thirty-eight cases met the study criteria, with a mean age of 47 years. The mean interval from symptom onset to lumbar puncture was 7.9 days, and mean CSF-TP was 1.23 g/L. There was a strong correlation between rising CSF-TP and time to lumbar puncture. Age-adjusted CSF-TP had a significantly lower sensitivity of only 45% in the first week (32% in the first 3 days) compared with 70% in the first week for the 0.45 g/L limit. All upper limits gained high sensitivity after the first week.ConclusionsThe low sensitivity of CSF-TP for the diagnosis of GBS is exacerbated by age-adjusted upper limits. The main role of lumbar puncture in GBS in the first week may be to help exclude other inflammatory or neoplastic etiologies of acute neuropathy. After the first week, the magnitude of the CSF-TP rise reduces the effect of different upper reference limits.

Objective. To test the hypothesis that maternal obesity is an independent risk factor for rectovaginal group B streptococcus (GBS) colonization at term.Study Design. Retrospective cohort study of consecutive women with singleton term pregnancies admitted in labor at Barnes-Jewish Hospital (2004–2008). Maternal BMI ≥ 30 Kg/m2(obese) or <30 Kg/m2(nonobese) defined the two comparison groups. The outcome of interest was GBS colonization from a positive culture. Baseline characteristics were compared using Student’st-test and Chi-squared or Fisher’s exact test. The association between obesity and GBS colonization was assessed using univariable and multivariable analyses.Results. Of the 10,564 women eligible, 7,711 met inclusion criteria. The prevalence of GBS colonization in the entire cohort was relatively high (25.8%). Obese gravidas were significantly more likely to be colonized by GBS when compared with nonobese gravidas (28.4% versus 22.2%,P<0.001). Obese gravidas were still 35% more likely than nonobese women to test positive for GBS after adjusting for race, parity, smoking, and diabetes (adjusted OR 1.35 [95% CI 1.21–1.50]).Conclusion. Maternal obesity is a significant risk factor for GBS colonization at term. Further research is needed to evaluate the impact of this finding on risk-based management strategies.

Asymptomatic term neonates born to mothers who are Group B Streptococcus (GBS) unknown or GBS positive but “inadequately” treated prior to delivery do not require invasive laboratory evaluation. We conducted a retrospective cohort study of mother/baby dyads born from January 1, 2005 until September 30, 2007 at the Medical College of Georgia. Their current protocol is to obtain a Complete Blood Count with Differential (CBC with D), Blood Culture (BC), and C-reactive protein (CRP) after birth. Mother/baby dyads () that met inclusion criteria were reviewed. Of these 242 babies 25 (10%) were started on antibiotics after the initial lab values were known. None of the blood cultures were positive and the CRP's were normal. The 2002 GBS guidelines call for laboratory evaluation of “at-risk” neonates, but the workup of these babies is not only costly, it does not provide any advantage over old fashioned clinical observation for the evaluation and treatment of early onset GBS sepsis.

Introduction. The majority of patients with acute upper gastrointestinal bleeding (UGIB) are admitted for urgent endoscopy as it can be difficult to determine who can be safely managed as an outpatient. Our objective was to compare four clinical prediction scoring systems: Glasgow Blatchford Score (GBS) and Clinical Rockall, Adamopoulos, and Tammaro scores in a sample of patients presenting to the emergency department of a large US academic center. Methods. We performed a retrospective cohort study of patients during 2008–2010. Our outcome was significant UGIB defined as high-risk stigmata on endoscopy, or receipt of blood transfusion or surgery, or death. Results. A total of 393 patients met inclusion criteria. The GBS was the most sensitive for detecting significant UGIB at 98.30% and had the highest negative predictive value (90.00%). Adding nasogastric lavage data to the GBS increased the sensitivity to 99.57%. Conclusions. Of all four scoring systems compared, the GBS demonstrated the highest sensitivity and negative predictive value for identifying a patient with a significant UGIB. Therefore, patients with a 0 score can be safely managed as an outpatient. Our results also suggest that performing a nasogastric lavage adds little to the diagnosis UGIB.

Abstract Objective Imperfect culture sensitivity and increase of early onset neonatal sepsis (EONS) risk in preterm neonates raise concern that culture-based intrapartum antibiotic prophylaxis (IAP) may be insufficient after preterm premature rupture of membranes (PPROM). Our objective was to compare rates of EONS after empiric versus culture-based IAP in PPROM. Study Design This retrospective cohort study included women with a singleton gestation and PPROM between 23 and 33 weeks. Outcomes after culture-based IAP were compared with empiric IAP. The primary outcome was EONS. Secondary outcomes included group B streptococcus (GBS) bacteremia, bacteremia, and neonatal GBS infection. Bivariable and multivariable logistic analyses were performed. Results Of the 270 women who met inclusion criteria, 136 (50%) had culture-based IAP of whom 36 (26.5%) were GBS positive. There was no significant difference in bacteremia (2.2 vs. 4.5%, p = 0.30), GBS infection (0.8 vs. 0.7%, p = 1.00), or EONS (11.8 vs. 12.7%, p = 0.82) in infants of women with culture-based IAP compared with empiric IAP. Multivariable analysis confirmed a lack of advantage to empiric versus culture-based IAP in EONS risk (adjusted odds ratio [aOR] = 0.82, 95% confidence interval [CI]: 0.44–1.93). Conclusion In pregnancies complicated by PPROM, infants of women who received culture-based IAP had no significant difference in EONS or GBS infection compared with infants of women with empiric IAP.

Background: Group B Streptococcus (GBS) can cause life-threating invasive infections, yet GBS is also a normal component of the intestinal and genitourinary tract. Although it is regarded as a potential urinary pathogen, the morbidity and mortality associated with recovery of GBS from urine cultures of nonpregnant adults is not well understood. We evaluated characteristics and mortality among nonpregnant adults with urine cultures that grew GBS. Methods: Using administrative data from the Veterans’ Healthcare Administration (VHA), we conducted a retrospective cohort study of VA healthcare system users from January 1, 2008, through December 31, 2017, with monomicrobial urine cultures growing ≥100,000 colony-forming units of GBS. Urinary tract infection (UTI) cases were defined as urinalysis positive for leukocyte esterase and pyuria (≥10 white blood cells), an International Classification of Diseases (ICD) code for UTI, and an antibiotic prescription. Cases with colonization were defined as negative for leukocyte esterase and pyuria, no ICD code for UTI, and no antibiotic prescription. Cases not meeting either definition were deemed unclassifiable. We compared demographics, comorbidities, and all-cause mortality among these 3 groups. Results: Over the 10-year study period, 26,848 veterans had 30,740 urine cultures positive for GBS. Applying the definitions above, there were 2,807 cases of infection, 8,789 cases of colonization, and 15,252 cases that were unclassifiable. Patients with a GBS UTI were slightly older compared to those who were colonized, with a higher Charlson comorbidity index and greater burden of chronic renal disease (Table 1). Individuals with infection versus colonization had 30-day mortality rates of 1% and 0%, respectively, and 1-year mortality rates of 9% and 4%, respectively (Figure 1). Conclusions: The association of a greater burden of illness among veterans who met our definition of UTI compared to colonization might be more reflective of providers’ responses to patients with chronic medical conditions rather than a difference in GBS as a cause of UTI. Overall, the prospect of a urine culture that grows GBS does not appear to be associated with adverse long-term outcomes.Funding: NoDisclosures: None

Abstract Introduction/Objective Streptococcus agalactiae, Group B Streptococcus (GBS), is a major cause of neonatal sepsis and infections in pregnant women. However, incidence of invasive GBS infections has more than doubled in the last two decades with highest risk in adults 65 years or older. Other risk factors are diabetes, malignancy, and immunocompromised state. Bacteremia and skin soft tissue infections are the most common invasive infections in nonpregnant adults. Rarely GBS infection has a fulminating pyrogenic exotoxin-mediated course characterized by acute onset, multiorgan failure, shock, and sometimes death, referred to as toxic shock-like syndrome. Methods A 77-year-old hypertensive female with uncontrolled type 2 diabetes mellitus and a history of bilateral foot ulcers presented to the hospital in probable septic shock. Clinical diagnosis of necrotizing fasciitis was made and she underwent bilateral lower limb amputations. Results Grossly soft tissue appeared gray. Microscopically fascia was necrotic without neutrophils present and Gram stain revealed sheets of Gram positive cocci. These findings reflected histopathologic Stage III necrotizing fasciitis, which is associated with 47% mortality. Autopsy showed a similar histology of Stage III necrotizing fasciitis involving the surgical stump. Erythema and desquamation of the upper limbs bilaterally and multi-organ failure met the clinical picture of Streptococcal Toxic Shock Syndrome (STSS) and fulfilled the criteria for TSS due to Group A Streptococcus (GAS), defined by The Working Group on Severe Streptococcal Infections. Conclusion Group B Streptococcal Toxic Shock-Like Syndrome may have a similar outcome to STSS caused by GAS and other pathogens and, in limited studies, mortality has been 30% or greater.

Introduction. The proportion of women with severe maternal morbidity from obstructed labor is between 2 and 12% in resource-limited settings. Maternal vaginal colonization with group B streptococcus (GBS), Escherichia coli, and Enterococcus spp. is associated with maternal and neonatal morbidity. It is unknown if vaginal colonization with these organisms in obstructed labor women is associated with poor outcomes. Objectives. To determine whether vaginal colonization with GBS, E. coli, or Enterococcus is associated with increased morbidity among women with obstructed labor and to determine the risk factors for colonization and antibiotic susceptibility patterns. Methods. We screened all women presenting in labor to Uganda’s Mbarara Regional Referral Hospital maternity ward from April to October 2015 for obstructed labor. Those meeting criteria had vaginal swabs collected prior to Cesarean delivery and surgical antibiotic prophylaxis. Swabs were inoculated onto sterile media for routine bacterial culture and antimicrobial susceptibility testing. Results. Overall, 2,168 women were screened and 276 (13%) women met criteria for obstructed labor. Vaginal swabs were collected from 272 women (99%), and 170 (64%) were colonized with a potential pathogen: 49% with E. coli, 5% with GBS, and 8% with Enterococcus. There was no difference in maternal and fetal clinical outcomes between those colonized and not colonized. The number of hours in labor was a significant independent risk factor for vaginal colonization (aOR 1.02, 95% CI 1.00–1.03, P=0.04). Overall, 38% of GBS was resistant to penicillin; 61% of E. coli was resistant to ampicillin, 4% to gentamicin, and 5% to ceftriaxone and cefepime. All enterococci were ampicillin and vancomycin susceptible. Conclusion. There was no difference in maternal or neonatal morbidity between women with vaginal colonization with E. coli, GBS, and Enterococcus and those who were not colonized. Duration of labor was associated with increased risk of vaginal colonization in women with obstructed labor.

ENGLISH ABSTRACT: Grazing management in the semi-arid communal areas of Namaqualand is investigated in this study. Paulshoek, situated in the Leliefontein Communal Reserve area, is delineated as the study area and consists of 20 OOOha.Cattle farming is practised by most households primarily on a subsistence basis. It is also used as a source of income when financial problems arise. The management of grazing-land in terms of formal rules and regulations is non-existent with only mutual relationships between farmers. The aim of this study is to develop a spatial framework for the management of a sustainable grazing regime with the aid of Geographic Information Systems (GIS) technology. This aim is reached through three overlapping goals which are researched individually. The goals are to create a spatial image of the physical resource base as well as resource usage and from this the development of a computerised (Excel spreadsheet) spatial management framework for sustainable grazing management. Through the use of existing data available from the National Botanical Institute (NB!) as well as other sources and with the aid of GIS technology, coupled with Indigenous Knowledge, these goals were reached. The stockpost as individual entity is studied to give a broader understanding of how the herdsmen see their immediate environment. The grazing management system, as presented in this thesis, consists of a simple Excel Spreadsheet, with inputs from GIS technology and Indigenous Knowledge. The area is overgrazed, as is seen in the widespread occurrence of kraalbos cross the Paulshoek landscape. The recommendations are that this management system should be implemented, which could minimize the futher overgrazing of the area. The management system relies heavily on the cooperation of the community and the integration of existing management systems and policies. It will give a more detailed account of who may own how many stock and where they may graze in the communal rangelands which will also help in the establishment of new permanent waterpoints in the area.
AFRIKAANSE OPSOMMING: Weidingsbestuur in die semi-ariede gemeenskaplike gebiede van Namakwaland is in hierdie studie ondersoek. Paulshoek, geleë in die Leliefontein Gemeenskapsreservaat gebied, word as studiegebied afgebaken en beslaan ongeveer 20 OOOha.Veeboerdery word deur meeste van die huishoudings primêr as bestaansboerdery beoefen. Dit word ook gebruik as 'n bron van inkomste wanneer finansiële probleme ondervind word. Hierdie doel word behaal deur drie oorkoepelende doelwitte wat elk afsondelik behandel word. Die doelwitte is om 'n ruimtelike beeld van die fisiese hulpbronbasis asook die benuttingsregime van die hulpbron te skep, en hieruit 'n rekenaar-gesteunde (Excel sigblad) ruimtelike bestuursraamwerk vir volhoubare weidingsbestuur te ontwerp. Deur gebruik te maak van bestaande inligting afkomstig vanaf Nasional Botaniese Instituut (NBI) asook ander bronne en met die koppeling van GIStegnologie en Inheemse Kennis (IK) is die doelwitte bereik. Die veepos word as entiteit behandel om 'n beter verstandhouding op te bou van hoe die veewagter sy onmiddelike omgewing bestuur. Die weidingsbestuurstelsel, soos voorgestel in hierdie tesis, bestaan uit 'n eenvoudige Excel sigblad, gekoppel aan GIS-tegnologie en Inheemse Kennis. Die gebied is oorbeweid, soos gesien kan word aan die verspreiding van kraalbos regoor die landskap van Paulshoek. Die aanbevelings is dat die bestuurstelsel geïmplementeer moet word om te verhoed dat die gebied verder onderhewig sal wees aan intensiewe beweiding. Som bakgrundsmaterial användes tvåolika typer av data, dels Svensk MarktäckeData (SMD) och dels en vegetationskarteringspecifikt framställd för ändamålet baserat på IRF-bilder. Syftet med studien är att försökaidentifiera en optimal metod för att analysera och kartera ekologisk konnektivitet utifrån demöjligheter och begränsningar som respektive verktygslåda och kartmaterial erbjuder, medtekniska förutsättningar liknande en kommunal tjänstemans. Jämförelserna är gjorda både urett kvalitativt användarvänlighetsperspektiv, och ur ett kvantitativt resultatperspektiv däranalysernas kvalitet och resurseffektivitet bedömts.Resultatet av jämförelserna visar att en analys utförd med Matrixgreen, mjukvara designadspeciellt för att modellera ekologisk konnektivitet är mindre kvalitetsmässigt tillförlitlig än enanalys gjord med ODC. Anledningen till detta är att Matrixgreen i sin nuvarande form ärbegränsat av programmeringsfel, vilket delvis förtar dess användningspotential. I jämförelsenmellan de olika typerna av kartmaterial så är det min uppfattning att vegetationskarteringenutförd med stereotolkning är kvalitetsmässigt överlägsen marktäckesdata från SMD somunderlagsmaterial i ekologiska konnektivitetsanalyser. Den förstnämnda är dock såresurskrävande att framställa att den troligtvis bara är ett aktuellt alternativ i projekt med storbudget.
Two different tool boxes using GIS were compared with the purpose of modeling ecologicalconnectivity. Additionally, two different sets of landscape data which could be used in GISmodeling of ecological connectivity at a municipal level were compared. The toolboxes arebeing compared is the Matrixgreen 1.6.4, an extension to ArcGIS 9.3.1, and ODC (Object,Distance, Cost), a selection of tools from the ArcGIS Spatial Analyst. The map data beingcompared is the Swedish Land Cover Data (Svensk MarktäckeData, SMD) and a vegetationmapping prepared by remote sensing and stereo interpretation of infrared aerial photographs.The purpose of this study is to identify an optimal method for analyzing and mappingecological connectivity based on the potentials and limitations that each tool box and mapdata set provides, under technical conditions similar to those of a municipal official’s.Comparisons were made both from a qualitative usability perspective, and a quantitativeperformance perspective with analysis of the quality and resource efficiency assessed.The results of the comparisons indicates that an analysis performed with Matrixgreen,software designed specifically for modeling ecological connectivity is qualitatively lessreliable than an analysis performed with ODC. The reason for this is that Matrixgreen in itspresent form is limited by programming errors, which partly deprives its potential use as toolsto perform connectivity analysis at a landscape scale. In the comparison between the differenttypes of map data, it is my opinion that the vegetation mapping conducted by stereointerpretation is qualitatively superior to land cover data from the SMD as support material inan ecological connectivity analysis. Although, the former is so expensive to produce that it islikely only a relevant option in projects with large budgets.

GIS is a relatively new phenomenon in the subject for geography. In order for the students to get a good GIS education, proper technical equipment is required. The Swedish National Agency for Education show, through statistics about the costs for elementary school, that private schools generally spend more money per student on equipment than what is the case in public schools. The purpose of this study is to examine if the prerequisites for conducting GIS education differ between public schools and private schools. Are there noticeable differences in the education and the time spent on GIS education? To find the answers to this, six teachers from six different schools, who in some form teach GIS, where interviewed. The results show that the access to the technical devices is better at private schools. The way both school forms present the data that they have gathered is a bit different.

The endodermal diverticulum of the caudal foregut develops into the GB. The GB lies between liver segments 4B and 5 and is supplied by the cystic artery. Cholecystokinin and vagal stimulation result in GB contraction, delivering bile (synthesized in the liver) into the duodenum. More than 80% of gallstones (cholelithiasis) are of the cholesterol type. Porcelain GBs can be seen on plain radiographs. Unless GB cancer is suspected preoperatively, or dense adhesions are likely, laparoscopic cholecystectomy is the operation of choice. Injury to the CBD (0.5%), duodenum, and liver parenchyma is uncommon but is a serious complication.

Guillain-Barré syndrome (GBS) is an acute inflammatory polyradiculoneuropathy that is hypothesized to be most commonly caused through the autoimmune mechanism of molecular mimicry. Typical GBS symptoms include back pain, paresthesiae in the limbs, and ascending, symmetrical muscle weakness. Examination findings in GBS include muscle weakness, areflexia, and a usually mild sensory loss. The majority of GBS patients report an infection one to four weeks before symptom onset. Zika virus (ZIKV) has now been associated with GBS in multiple case-control studies. GBS patients must be monitored for signs of impending respiratory failure and autonomic dysfunction. The treatment for ZIKV-associated GBS is the same as with other GBS cases; intravenous immunoglobulin and plasmapheresis are both thought to be equally effective treatments in ameliorating the condition. The prognosis for recovery from ZIKV-associated GBS is very good, based on the previously reported case series.

The incidence of Guillain-Barré syndrome (GBS) is about 1 case per 100,000 per year, but it is twice that in elderly persons. Multiple antecedent events have been reported. GBS can become a major neurocritical illness. Admission to the intensive care unit is expected in 1 of 5 patients with GBS. Rapidly progressive limb weakness may extend to the oropharyngeal muscles or diaphragm, an effect that often leads to intubation and mechanical ventilation. Outcome in GBS is nearly always good, and severe disability remains in only a minority of affected patients.

Common clinical conditions include Guillain-Barré syndrome (GBS) and myasthenia gravis (MG). Recognition of impending respiratory failure for such patients is of great importance. This chapter describes general features of neuromuscular weakness and neuromuscular respiratory failure. In addition, it reviews the clinically important aspects of GBS, MG, and botulism. This paper describes considerations that must be addressed when designing a gas fuel clean up system to meet utility gas turbine fuel specifications. Each gas turbine manufacturer has a gas fuel specification that must be met in order to protect the equipment from the effects of burning poor quality gas fuel. With the introduction of advanced technology combustion systems and strict emission requirements, it has become increasingly important that clean, dry gas fuel be provided at the inlet to the gas turbine control system in order to maintain the equipment in proper working order. The ASME gas fuel standard B133.7M is representative of a specification that meets or exceeds most manufacturer’s requirements. This specification calls for superheating to avoid the condensation of moisture or hydrocarbon liquids and includes limits on particulate concentration and size. Issues relating to the fuel quality are discussed, including calculation and measurements of dew points, gas sampling and analysis and relative location of cleanup equipment required to meet this specification.

A program has been underway since 1986 by Westinghouse Electric Corporation, Textron, Inc., and the sponsoring agency, the Morgantown Energy Technology Center (METC) of the U.S. Department of Energy (DOE), as participants, to establish the technology base for coal-fired combustion turbines targeted for power generation (50 to 150 MW size units). The developed system must be able to burn unbeneficiated, low-cost, utility-grade coal and meet the EPA New Source Performance Standards (NSPS) for coal-fired steam generators (Thoman et al., 1987). Development of a high pressure (12 to 16 atms) slagging combustor is the key to making a direct coal-fired combustion turbine a commercial reality. In testing to date, a 6 atm slagging combustor, rated at 12 MMBtu/hr (12.7 MHkJ/hr) has demonstrated its ability to handle high- and low-sulfur bituminous coals, and low-sulfur subbituminous coal. The program objectives relative to combustion efficiency, combustor exit temperature and pattern factor, NOx emissions, carbon burnout, and slag rejection have been met. Today, Northern States Power, working with Westinghouse with assistance from Textron is developing a plan to commercialize a direct coal-fired advanced combined cycle (DCFACC). Included in this plan is a pilot plant (which does not include a combustion turbine) and a demonstration plant that would utilize a 50 MW combustion turbine. The first commercial DCFACC, which would Include a 100 MW combustion turbine, is scheduled to be operational by the year 2001. The cooperative effort among Northern States Power, Westinghouse, and Textron is financially independent of the work now sponsored by DOE/METC. This paper presents the status of the pressurized slagging combustor development program including recent work to reduce alkali, particulates and SOx levels leaving the combustor and gives an overview of our commercialization process and plan.

Contamination levels of liquid fuel entering the combustor of a gas turbine must be low to avoid high temperature corrosion and fuel system fouling. Accordingly, each of the major industrial gas turbine manufacturers has strict contamination limits which must be met in order to comply with the warranty of the turbine. The responsibility to assure compliance lies with the owner of the power plant and can only be guaranteed with on-site fuel analysis for contaminants and ash-forming metals. This paper discusses the various fuel analysis techniques available to the gas turbine user. It will review each technique’s ability to meet the need for fast and precise on-site analytical data. It will be demonstrated that the rotating disc electrode (RDE) atomic emission technique is the preferred analytical method for on-site fuel analysis. Actual field experience will be used to illustrate and discuss compromises that may be necessary to meet the objectives of the gas turbine user, fuel treatment supplier and turbine manufacturer.

Improvements in land-based gas turbine performance require materials capable of operating at increasingly higher temperatures. Innovative approaches to alloy and process improvement, such as directional solidification, can meet the more demanding conditions of new gas turbine designs. This paper outlines GE TBO’s multi-faceted research program for the development of directionally solidified GTD-111 first stage turbine buckets. Included are discussions of the theoretical background, casting program, mechanical tests and machining studies. The GTD-111 directional solidification program successfully met the technological challenges encountered in scaling up directional solidification experience from relatively small aircraft parts. Directionally solidified buckets are in production for several first stage product lines. Improvements in mold technology coupled with experience gained in the on-going research program indicate that more complex parts as well as single crystal parts are now feasible.

Research suggests that active learning strategies develop the critical thinking and problem-solving skills that are necessary for effective clinical reasoning and decision-making abilities. Unga uttrycker själva att de vill ha mer kunskap om psykisk hälsa och skolan lyfts ofta som en bra plattform. Många unga vet inte vart de ska vända sig för att få hjälp med psykiska besvär och de har behov av att bli lyssnade på. När-varande vuxna som lyssnar behövs, dock saknar många vuxna kunskap om psykiska besvär och diagnoser kopplat till psykisk ohälsa.Psykisk hälsa bland unga är en viktig samhällsfråga som de senaste åren har fått mer uppmärksamhet i samhällsdebatten. Flera satsningar har gjorts av civilsamhällesaktörer och offentliga aktörer. Samtidigt visar den internationella kartläggningen att det är svårt att påvisa långsiktiga effek-ter av alla de satsningar som görs. Det kan finnas flera orsaker till detta. Det finns dock insatser som verkar mer lovande och här finns en möjlighet för beslutsfattare att göra skillnad för ungas psykiska hälsa. Förutom att bespara unga onödigt lidande är dessa insatser ofta samhällsekonomisktkostnadseffektiva, eftersom de på sikt leder till minskat vårdsökande och bättre förutsättningar för unga att klara skolan och i förlängningen arbets-livet. Sådana satsningar går även i linje med Barnkonventionen artikel 24 om alla barns rätt till bästa möjliga hälsa, som sedan 2020 är lag i Sverige. Utifrån kartläggningen har vi följande rekommendationer 1. Rusta unga med färdigheter om psykisk ohälsaUnga bör få tillgång till de kunskaper och färdigheter som enligt forsk-ning visat sig lovande för att främja psykisk hälsa och förebygga ohälsa. Internationellt ges flera av dessa insatser inom ramen för sko-lan. De behöver utveckla sina färdigheter i hur man tolkar och förstår sina egna känslor och sin kropp och vad som är vanlig livssmärta, hur man kan hantera den och när man behöver kontakta vården. Därför bör ett nationellt initiativ som når alla barn tas för att främja barn och ungas kunskap och färdigheter om psykisk hälsa för att rusta dem för livet. Det bör även inkludera information om vart man kan vända sig om man behöver ytterligare stöd eller vård. Unga ska vara delaktiga i utformningen av ett sådant initiativ. 2. Ge vuxna kunskap och verktyg om psykisk hälsa – för att underlätta stöd till ungaBarn och unga behöver närvarande vuxna som har förmågan att lyssna, fånga upp och vägleda dem. Det är inte ovanligt att vuxna i barns och ungas närhet upplever osäkerhet och saknar kunskap om psykiska besvär och diagnoser. Därför behöver vuxna, framförallt föräldrar och yrkes-verksamma som jobbar nära unga, få bättre kunskap och verktyg för hur de ska möta unga på ett bättre sätt. Satsningar på att höja föräldrars kun-skap om psykisk hälsa bör göras, exempelvis i form av föräldrastödspro-gram och folkbildning. Lärare bör även ges grundläggande kunskaper om psykisk hälsa inom ramen för lärarutbildningen. 3. Insatser bör följas upp under en längre tidInsatser för att främja psykisk hälsa bör planeras, implementeras och ut-värderas med långtidsuppföljningar (>12 månader). Detta för att bättre kunna bedöma de långsiktiga effekterna och för att kunna utveckla och anpassa insatserna efterhand.

Review question / Objective: At this time, a comprehensive systematic review and network meta-analysis (NMA) was conducted to: (1) fill the research gap by giving rankings on treatment efficacy; (2) provide statistical evidence of not head-to-head comparisons; (3) seek out the best and up-to-date therapeutic strategy reported in latest RCTs; (4) address potential adverse events (AEs) of available treatments. Condition being studied: The incidence of glioblastoma (GBM) increases with age, until now, there has been less evidence on the optimal treatments for elderly GBM since only general GBM populations were included in clinical trials. Given the poor survival of elderly GBM, we collected randomized controlled trials about newly diagnosed GBM (ndGBM) and recurrent GBM, and conducted a Bayesian network meta-analysis on ndGBM regarding overall survival (OS) and progression-free survival (PFS). We revealed TTF + TMZ and TMZ + HFRT were likely to be best treatments for OS; BEV + HFRT and TMZ + HFRT were likely to be best options for PFS. Current study is the most comprehensive and powered network analysis on elderly GBM until now, it also provides more insights for elderly GBM management.

Die Nutzung verfügbarer Radverkehrsdaten auf GPS-Basis stellt eine preisgünstige Möglichkeit für Kommunen dar, einen Überblick über das Nutzungsverhalten ihrer Radfahrenden zu erhalten. Mit den vorliegenden Ergebnissen soll eine Lücke bei der Interpretation von GPS-basierten Daten geschlossen werden. Die Radfahrtypologie auf Basis des geäußerten Verhaltens kann dabei helfen, GPS-Daten auch ohne detaillierte Kenntnisse der zugrundeliegenden Nutzergruppen zielgenauer zu interpretieren. Damit können zukünftig Kommunen die Potenziale entstehender oder bereits vorhandener Angebote an GPS-Radverkehrsdaten zielführender nutzen und ihre Radverkehrsinfrastruktur besser darauf abstimmen. In einem ersten Schritt wurde auf Basis einer Befragung eine empirisch belegte und wissenschaftlich hergeleitete multidimensionale Typologisierung von Radfahrenden erstellt. Anschließend wurde eine umfangreiche heterogene Probandengruppe mit unterschiedlichen soziodemografischen Ausprägungen mit Geräten für die Aufzeichnung ihrer Radrouten ausgestattet. Das auf diesem Weg erhobene Radverkehrsverhalten wurde, gestützt durch kontinuierliche begleitende Befragungen, ausgewertet und anhand unterschiedlicher Indikatoren beschrieben. Damit wurden Präferenzen einzelner Gruppen, z. B. im Hinblick auf Geschwindigkeit, Streckenlänge, Typ der Radverkehrsinfrastruktur, Fahrtzweck oder Routenwahl identifiziert. Auf Basis einer Onlineumfrage konnten vier unterschiedliche Typen von Radfahrenden beschrieben werden, die sich hinsichtlich der Nutzungshäufigkeit, zurückgelegter Entfernungen, Fahrverhalten, Sicherheitsempfinden, Identifikation als Radfahrerende, Wetterabhängigkeit und in motivationalen Aspekten unterscheiden. Anhand der unterschiedlichen Ausprägungen in diesen Merkmalen werden sie als die ambitionierten, die funktionellen, die pragmatischen und die passionierten Radfahrenden bezeichnet. Bezogen auf das Verkehrsverhalten steigt die Nutzungshäufigkeit von ambitionierten über passionierte und pragmatische Radfahrende an. Funktionelle Radfahrende geben die mit Abstand geringste Fahrradnutzung unter allen vier Typen an. Hinsichtlich der angegebenen Distanzen, die zurückgelegt werden, liegen passionierte, pragmatische und funktionelle Radfahrende dicht beieinander. Ambitionierte Radfahrende gaben dagegen an, deutlich größere Distanzen zurückzulegen. Die Ergebnisse aus der Umfrage zeigten sich in einer anschließenden Felduntersuchung in abgeschwächter Form. Insbesondere der ambitionierte Radfahrtyp lässt sich durch höhere Tageskilometerwerte, Geschwindigkeiten und Beschleunigungen von den anderen Typen abgrenzen. Bei den anderen Typen ist eine Unterscheidung weniger ausgeprägt. Hier zeigte sich, dass vor allem die Zugehörigkeit zu einer bestimmten Altersgruppe einen Einfluss auf das Fahrverhalten hat. In Übereinstimmung mit bisherigen Erkenntnissen zeigte sich, dass mit zunehmendem Alter tendenziell etwas langsamer und stetiger gefahren wird. Ebenso radeln auch weibliche Personen etwas langsamer und stetiger als männliche Radfahrer. In der Nutzerbefragung zeigten sich geringe Unterschiede für die Präferenz bei der Infrastrukturnutzung zwischen den Typen, z.B. bei funktionellen Radfahrenden, die eine getrennte Führung im Seitenraum bevorzugen. In der Feldstudie wurde dies ebenfalls untersucht. Auch hier zeigten sich nur geringe Unterschiede. Die Ergebnisse werden auch vor dem Hintergrund eines, eventuell durch die Versuchssituation veränderten Fahrverhaltens der teilnehmenden Radfahrenden, diskutiert. Es konnte vor allem eine hohe Nutzungsfrequenz und Häufigkeit beobachtet werden, die die angegebenen Werte aus der Typenbefragung übertrafen. Für die Nutzung von GPS-Daten für die Radverkehrsplanung wird aus den Ergebnissen abgeleitet, dass eine mögliche Skalierung beziehungsweise Wichtung von Daten entlang soziodemografischer Faktoren die größten Potenziale bietet.

Abstract:
As of 2004 and continuously to this day, the annual growth rate of renewable sources has been greater than that of all fossil fuels combined. In the midst of this transition to cleaner energy, natural gas is the only fossil fuel that has increased its share in the global energy matrix. Technological changes in the LNG supply chain, as well as transformations in the global natural gas market, largely explain this growth. This publication provides evidence on the fundamental role that natural gas plays in the energy transition, given that: (i) its greenhouse gas emissions are substantially lower than those of oil and coal; (ii) it provides the firm power necessary to complement intermittent renewable energies; (iii) it is particularly safe compared to other fossil fuels. In line with these attributes, the International Energy Agency projects that the share of natural gas in the global energy matrix by 2040 will remain stable (around 24%), even in its Sustainable Development Scenario, which would allow to meet the goals established in the Paris Agreement.
