Abstract:
This chapter explains the principles of how best to use the main diagnostic tools in paediatric neurology in the context of evidence-based medicine. The description of neuroradiology includes the principles of DWI, SWI, MRS, ASL and fMRI, and the usefulness of ultrasound, CT and PET scanning; neuroradiological anatomy, terminology, common incidental findings and normal myelination patterns. An approach to white matter and developmental brain abnormalities is depicted. Neurogenetic testing discusses the capabilities and limitations of microarray for Comparative Genomic Hybridization (copy-number variants), gene panel testing, and whole exome and whole genome next generation sequencing. The chapter offers the theory, practicality and pitfalls of electroencephalography, peripheral neurophysiology and evoked potential testing. Common practical procedures are described, including lumbar puncture, muscle biopsy and shunt tapping with an understanding of the place of special investigations on CSF, blood, urine, and skin. The scope of neuropsychological testing is described.

Abstract:
In the next two decades epigenetics could revolutionize understanding and treatment of diseases of the central nervous system. New research already demonstrates that manipulation of epigenetic mechanisms in vivo and in vitro can ameliorate a host of pathogenic processes associated with neurodegenerative disorders such as Alzheimer’s disease (AD), Parkinson’s (PD), amyotrophic lateral sclerosis (ALS), Huntington’s (HD), and multiple sclerosis (MS), among others. These advances have come relatively rapidly for a field that is still in its infancy compared to the much longer history of epigenetics in developmental biology. Epigenetic modifications are all-encompassing, from nucleotides to amino acids. They are capable of altering transcriptional to biochemical activity in a consistent manner across thousands of genes and hundreds of biologic pathways, yet they can do so differentially even in individuals or cells with identical gene codes. As such, epigenetic modifications are likely to touch on virtually all the mechanisms described in this book.

Sedel, Frédéric. Krabbe Disease in Adults. Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780199972135.003.0051.

Full text
Abstract:
Krabbe disease (globoid cell leukodystrophy) is an autosomal recessive lysosomal storage disease caused by mutations in the lysosomal galactocerebrosidase (galactosyl ceramidase) gene. Krabbe disease usually presents as a severe leukodystrophy in early infancy and childhood. In contrast, adult patients usually present with progressive spastic paraparesis. Other signs of the disease include peripheral neuropathy, dysarthria, cerebellar ataxia, pes cavus deep sensory signs, tongue atrophy, optic neuropathy, cognitive decline. Cerebrospinal fluid protein concentration is moderately increased in adults. High signals of cortico-spinal tracts on brain MRI are highly suggestive and this typical aspect may be associated with hyper-intensities of optic radiations, and of the posterior part of the corpus callosum.

Full text
Abstract:
Staphylococcal species are common commensals of the skin and mucous membranes of humans and animals but only in very recent years has zoonotic infections been recognised. They can also be associated with infection and disease, especially coagulase positive organisms. Staphylococcus aureus is relatively frequently carried by humans in the nasal passages and is a cause of infections in people including bacteraemias in hospitalised patients. More recently some strains of Staphylococcus aureus have acquired a resistance gene (mecA) which renders them resistant to meticillin (meticillin-resistant Staphylococcus aureus, MRSA). MRSA isolates are of major importance in healthcare situations as well as increasingly in the community. Animals can also be carriers of Staphylococcus aureus although less frequently than humans and MRSA can be carried or infect several different host species. For companion animals such as dogs and cats, the most frequently isolated MRSA strains are similar to the common local human healthcare strains; thus for the UK, EMRSA-15 and -16. This suggests a reverse zoonosis with spill over from the human population into their companion animals. In horses the situation is different, with some horses carrying or infected with human epidemic strains but others infected with strains less frequently seen in people. For food-producing animals the picture is different again with a particular strain, ST398, which appears to circulate endemically in animal populations, such as pigs, and can spill over into the human population where it can cause carriage as well as infection and disease. The transmission appears to be by direct contact with animals rather than through the food-chain.Where risk factors for infection with MRSA have been studied in animals they appear similar to some of the risks for human infection. Therefore, for control of MRSA in animals measures such as improved hygiene and good antibacterial stewardship are important.

Abstract:
Genre categories are shaped by cultural context and can change over time. This case study of Henry Lovelich’s Grail and Merlin (Cambridge, Corpus Christi College, MS 80) accounts for the malleability of ‘romance’ by assessing in detail how perceptions of genre are mediated by form, politics, religion, archival methods, aesthetics, and pedagogy. When MS 80 was created in the fifteenth century, the English text was part of a multilingual tradition in which romance and history were inherently entangled and overlapping. In the sixteenth century, the Grail and King Arthur served as politically useful history. As the religious polemics of the Reformation subsided, Lovelich’s translation came to represent the beginning of English national romance. By the mid-twentieth century, it had been repositioned as a much maligned ‘bad romance’. Later, from the perspective of manuscript studies, evaluations became more positive. Now, early in the twenty-first century, the expansion of digital archives supports new approaches that challenge traditional distinctions between literary history and book history.

Abstract:
Rosen, Jeremy. Minor Characters Have Their Day. Columbia University Press, 2017. http://dx.doi.org/10.7312/columbia/9780231177443.001.0001.

Full text
Abstract:
How do genres develop? In what ways do they reflect changing political and cultural trends? What do they tell us about the motivations of publishers and readers? Combining close readings and formal analysis with a sociology of literary institutions and markets, Minor Characters Have Their Day offers a compelling new approach to genre study and contemporary fiction. Focusing on the booming genre of books that transform minor characters from canonical literary texts into the protagonists of new works, Jeremy Rosen makes broader claims about the state of contemporary fiction, the strategies of the publishing industry over recent decades, and the function of literary characters. Rosen traces the recent surge in “minor-character elaboration” to the late 1960s and works such as Jean Rhys’s Wide Sargasso Sea and Tom Stoppard’s Rosencrantz and Guildenstern Are Dead. These early examples often recover the voices of marginalized individuals and groups. As the genre has exploded between the 1980s and the present, with novels about Ahab’s wife, Huck Finn’s father, and Mr. Dalloway, it has begun to embody the neoliberal commitments of subjective experience, individual expression, and agency. Eventually, large-scale publishers capitalized on the genre as a way to appeal to educated audiences aware of the prestige of the classics and to draw in identity-based niche markets. Rosen’s conclusion ties the understudied evolution of minor-character elaboration to the theory of literary character.

Full text
Abstract:
Hypermobility-related syndromes constitute a family of heritable disorders of connective tissue (HDCT) that derive from abnormalities affecting genes that encode for the connective tissue matrix proteins such as collagen, fibrillin, and tenascin. They range from such commonplace though poorly recognized conditions such as the joint hypermobility syndrome (JHS) to the better-known, if more rare, eponymous syndromes such as Marfan's syndrome (MFS) and the different types of the Ehlers-Danlos syndrome (EDS). The more common presentations are with skin pathology (bruising, scaring), joint or spinal and/or muscle pain and instability with vulnerability to injury and chronic widespread pain, cardiac valve pathologies, and in MFS and vascular EDS, arterial dilatation with the risk of dissection and rupture. JHS (widely considered synonymous with the EDS hypermobility type) is further complicated by cardiovascular autonomic dysfunction such as orthostatic intolerance, palpitations, and syncope, and the recently described and commonly encountered pangastrointestinal dysmotility. The latter can manifest as gastro-oesophageal reflux, gastroparesis, slow-transit constipation, or rectal evacuatory dysfunction with rectal intussusception. In addition, HDCT are associated with bladder and uterine problems as a consequence of pelvic floor weakness. Such multisystemic conditions need to be managed by a multidisciplinary team able to draw on medical, surgical, physical, and psychological interventions by appropriately experienced specialists and therapists. This chapter introduces the reader to the epidemiology, genetics, classification, and clinical presentation of JHS, EDS, and MFS. It also describes the key investigations required to support a diagnosis and assess complications of an HDCT, as well as the multidisciplinary approach to management.

Abstract:
Two genetic technologies capable of making heritable changes to the human genome have revived interest in, and in some quarters a very familiar panic concerning, so-called germline interventions. These technologies are most recently the use of CRISPR/Cas9 to edit genes in non-viable IVF zygotes and Mitochondrial Replacement Therapy (MRT). The possibility of using either of these techniques in humans has encountered the most violent hostility and suspicion. Here, we counter the stance of the US NIH and its supporters by showing that differing global moralities are free to exist unimpeded under international biolaw regimes, which do not in any way represent unified opinion against such technologies. Furthermore, we suggest a more rational approach to evaluating them through analysis of similar technologies which have caused past controversy.

Abstract:
Isolated left ventricular non-compaction is a controversial entity which has only been reported in the past 30 years. It is becoming more frequently diagnosed due to the use of echocardiography and MRI. It can present in fetal life, infancy, childhood, and adult life. Clinically, the patient can present with cardiac arrhythmias, cardiac failure, systemic emboli due to thrombosis within the ventricles, and sudden death. It can be a genetic entity associated with mutations in many genes associated with hypertrophic cardiomyopathy, dilated cardiomyopathy, and arrhythmogenic cardiomyopathy. It is a rare entity found at autopsy and is more common in children than adults. In the past the prognosis has been considered worse in children then in adults. Treatment is usually empirical, dealing with the cardiac failure, arrhythmias, and thromboemboli.

Abstract:
Neuroimaging of psychotic disorders began in the 1970s with studies of brain structure, followed by studies of blood flow, glucose metabolism, and receptor chemistry. More recently, studies of functional activation, connectivity, and neurotransmission have been added. The widespread availability of MRI has liberated researchers from the constraints of postmortem research and the limitations of inferring brain abnormalities through neurochemical effects in plasma or urine. Neuroimaging researchers aim to establish diagnostic markers, explain abnormal mental states, test anatomical and neurochemical models, and study the association of risk genes with brain abnormalities. Most studies have been cross-sectional and limited in size. But the ability to assess patients longitudinally and with large-scale study designs will ensure a prominent position for this research domain in the study of psychotic disorders.

Full text
Abstract:

Abstract:

Hogan, Patrick Colm. Style in Narrative. Oxford University Press, 2020. http://dx.doi.org/10.1093/oso/9780197539576.001.0001.

Full text
Abstract:
Style has often been understood both too broadly and too narrowly. In consequence, it has not defined a psychologically coherent area of study. In the opening chapter, Hogan first defines style so as to make possible a systematic theoretical account through cognitive and affective science. This definition stresses that style varies by both scope and level—thus, the range of text or texts that may share a style (from a single passage to an historical period) and the components of a work that might involve a shared style (including story, narration, and verbalization). Hogan illustrates the main points of this chapter by reference to several works, prominently Woolf’s Mrs. Dalloway. Subsequent chapters in the first part focus on under-researched aspects of literary style. The second chapter explores the level of story construction for the scope of an authorial canon, treating Shakespeare. The third turns to verbal narration in a single work, Faulkner’s As I Lay Dying. Part two, on film style, begins with another theoretical chapter. It turns, in chapter five, to the perceptual interface in the genre of “painterly” films, examining works by Rodriguez, Mehta, Rohmer, and Husain. The sixth chapter treats the level of plot in the postwar films of Ozu. The remaining film chapter turns to visual narration in a single work, Lu’s Nanjing! Nanjing! The third part addresses theoretical and interpretive issues bearing on style in graphic fiction, with a focus on Spiegelman’s Maus. An Afterword touches briefly on implications of stylistic analysis for political critique.

Full text
Abstract:
In the tumultuous era of the late sixties and early seventies, several currents of American art and culture coalesced around a broad sensibility that foregrounded and explored the immediacy of lived experience as both an aesthetic and political imperative. But in films set in the historical past, this sensibility acquired complex additional resonances by speaking to the ephemerality of the present moment through a framework of history, myth, nostalgia, and other forms of temporal alienation and distance. The Presence of the Past explores the implications of this complex moment in Hollywood cinema through several prominent examples released in the years 1967 to 1974. Key genres are explored in detailed case studies: the outlaw film (Bonnie and Clyde and Badlands), the revisionist Western (Butch Cassidy and the Sundance Kid, McCabe and Mrs. Miller), the neo-noir (Chinatown), and the nostalgia film (The Last Picture Show and American Graffiti). In these films, “the past” is more than a matter of genre or setting. Rather, it is a richly diverse, often paradoxical concern in its own right, whose study bridges diverse conceptual territories within soundtrack studies, including the sixties pop score, myth criticism, media technologies, and the role of classical music in compilation scoring. Against a broader background of an industry and film culture that were witnessing a stylistic and aesthetic diversification in the use of music and sound design, The Presence of the Past argues for the film-philosophical importance of the soundtrack for cultivating an imagined experiential understanding of the past.

Abstract:
The Internet has allowed people to access information that previously was difficult to obtain. It is important to know the information is true and accurate and does not represent ‘fake news’ or alternative facts. Patents describe inventions and contain accurate information, as patents are examined and their accuracy can be challenged. This book shows how patents and the inventions they describe have shaped the modern world, that is the world in the twenty-first century. Patent documents that date from the mid-nineteenth century to the present time are used in the text and the subject matter covers many technical areas: for example, Morse code, the diode, triode, transistors, television, frozen foods, ring-pulls for soft drink cans, board games such as Monopoly, gene editing, metamaterials, MRI, computerised tomography, insulin and monoclonal antibodies such as Herceptin. Up to a page of text is used for each entry and the text is backed up by drawings from patent documents. Patent numbers are included to allow interested readers to trace the documents. Inventions described in the patents are placed in a historical perspective. For example, the role of the cavity magnetron and radar are described in the context of the Second World War, whereas the diode is discussed in the development of broadcasting at the beginning of the twentieth century. Entries cover examples from life sciences, engineering and physical sciences in the modern world.

Abstract:
This biography of Esther Zimmer Lederberg highlights the importance of her research work, which revealed the unique features of bacterial sex, essential for our understanding of molecular biology and evolution. A Hidden Legacy relates how, she and her husband Joshua Lederberg established the new field of bacterial genetics together, in the decade leading up to the discovery of the DNA double helix. Their impressive series of achievements include: the discovery of λ‎ bacteriophage and of the first plasmid, known as the F-factor; the demonstration that viruses carry bacterial genes between bacteria; and the elucidation of fundamental properties of bacterial sex. This successful collaboration earned Joshua the 1958 Nobel Prize, which he shared with two of Esther’s mentors, George Beadle and Edward Tatum. Esther Lederberg’s contributions, however, were overlooked by the Nobel committee, an example of institutional discrimination known as the Matilda Effect. Esther Lederberg should also have been recognized for inventing replica plating, an elegant technique that she originated by re-purposing her compact makeup pad as a kind of ink stamp for conveniently transferring bacterial colonies from one petri dish to another. Instead, the credit for the invention is given to her famous husband, or, at best, to Dr. and Mrs. Lederberg. Within a few years of winning the Nobel Prize, Joshua Lederberg divorced his wife, leaving Esther without a laboratory, cut off from research funding, and facing uncertain employment. In response, she created a new social circle made up of artists and musicians, including a new soulmate. She devoted herself to a close-knit musical ensemble, the Mid-Peninsula Recorder Orchestra, an avocation that flourished for over forty years, until the final days of her life.

Abstract:

Abstract:
Zaynab, first published in 1913, is widely cited as the first Arabic novel, yet the previous eight decades saw hundreds of novels translated into Arabic from English and French. This vast literary corpus influenced generations of Arab writers but has, until now, been considered a curious footnote in the genre's history. Incorporating these works into the history of the Arabic novel, this book offers a transformative new account of modern Arabic literature, world literature, and the novel. This book rewrites the history of the global circulation of the novel by moving Arabic literature from the margins of comparative literature to its center. Considering the wide range of nineteenth- and early-twentieth-century translation practices, the book argues that Arabic translators did far more than copy European works; they authored new versions of them, producing sophisticated theorizations of the genre. These translations and the reading practices they precipitated form the conceptual and practical foundations of Arab literary modernity, necessitating an overhaul of our notions of translation, cultural exchange, and the global. The book shows how translators theorized the Arab world not as Europe's periphery but as an alternative center in a globalized network. It affirms the central place of (mis)translation in both the history of the novel in Arabic and the novel as a transnational form itself.

Abstract:
Abstract:
This book argues that pornographic film relies on a particular "Victorianness" in generating eroticism—a Gothic Victorianness that is monstrous and restrained, repressed but also perverse, static but also transformative, and preoccupied with gender, sexuality, race, and time. Pornographic films enthusiastically expose the perceived hypocrisy of this Victorianness, rhetorically equating it with mainstream, legitimate culture, as a way of staging pornography’s alleged sexual authenticity and transgressive nature. Through an analysis of porn set during the nineteenth century and porn adaptations of Lewis Carroll’s Alice books, Robert Louis Stevenson’s The Strange Case of Dr. Jekyll and Mr. Hyde, Oscar Wilde’s The Picture of Dorian Gray, and Bram Stoker’s Dracula, this book shows how these adaptations expose the implicit pornographic aspects of “legitimate” culture while also revealing the extent to which “high” and “low” genres rely on each other for self-definition. In the process, neo-Victorian pornographies draw on Gothic spaces and icons in order to situate itself as this Gothic other, utilizing the Gothic and the monstrous to craft a transformative, pornographic space. These neo-Victorian Gothic pornographies expose the way the genre as a whole emphasizes, navigates, transgresses, and renegotiates gender, sexuality, and race through the lens of history and legacy.

Abstract:

Abstract:
