Most doctors are probably uncomfortably aware of the old joke that an alcoholic is someone who drinks more than his doctor. But how would one define a drug addict? Because of the taboo and illegal nature of drug misuse, if a similar doctor-based definition were coined, even in jest, it would cause a degree of consternation and indignation among the profession. However, the evidence that doctors abuse certain substances, in particular opiates and tranquillisers (Hughes et al, 1992), more commonly than the general population is well established.

The psychosomatic diseases belong to the group of the most spread disorders in the contemporary medical practice. Answer to the question--what is the spread of those disorders and depression with general practice physicists, we have looked for by means of a research performed in February 2008 in the Health Centers Nis and “Vozdovac”-Belgrade. As the research instruments, we have used an anonymous questionnaire about the psychosomatic diseases and depressivity self-estimation scale (ZUNG). Those papers were filled by 36 of physicists and general practitioners, male and female, aged 30-55 years.

AbstractThis case report presents the story of a young woman of Romani descent with a mixed dissociative (conversion) disorder within the contextual evidence-based and value-based medical framework. By painting the picture illustrating the course of her illness and the circumstances leading to the last clinical episode, compelling her most recent hospitalization, we delineate the contrast between common clinical phenomenology and the additional layers of the patient’s beliefs and values. Thus, we emphasize the importance of expanding the one-dimensional mainstream evidence-based approach, not only in cases of cross-cultural doctor-patient interactions but also in general medical practice, since the health attitudes and illness behaviors of every individual are influenced by their values and beliefs. In addition, the contemporary notion of medicine as a factual science requires a paradigm shift toward integrative multifaceted approaches if we as doctors are to treat human beings and not merely diseases.

The duty of confidentiality that a doctor owes to a patient is probably as old as the practice of medicine and is essential for trust between doctors and their patients. However, it is not an absolute duty and there are instances when a doctor is entitled, and in certain circumstances obliged, to disclose confidential information without patient consent to do so. This chapter is concerned with doctor–patient confidentiality and when medical information can reasonably be released. Confidentiality is covered by a mixture of UK statute, common law, tort law, and the European Convention on Human Rights. It is also a core component of the professional guidance given to doctors by the profession’s regulatory body, the General Medical Council, to which all doctors registered with it are expected to adhere.

This chapter will focus on the day-to-day issues encountered in the ED for an overall understanding of the scenarios new doctors are expected to face on the very first day of their exciting career. The first issue dealt with in this chapter is triage. Triage is the hub of clinical practice and used on a regular basis in one form or another. Although some departments have gradually developed the service of ‘see and treat’ and escaping triage, it is still applied formally or informally by a practising clinician. It is vital to prioritize patients attending with a wide range of clinical presentations. A few questions on this topic are included to give a flavour of what to expect when you join the ED. The other issue discussed in this chapter is legal medicine, which again a newly qualified doctor may encounter on their first entry to the ED. But, it must be emphasized that plenty of support is provided to newcomers to put them at ease so that they may use their initial few days for settling into the department. To overcome the dilemma of ethical and legal issues, doctors can also contact medical defence organizations (the Medical Defence Union, the Medical Protection Society, etc.) and almost every doctor subscribes to one of the unions for this kind of support. It is increasingly recognized that an appropriate level of communication is of the utmost importance for the safer and effective care of patients attending the ED. There is always the issue of when to refer a borderline case to a specialty peer for possible admission and further care. I am sure there are innumerable examples of a junior doctor feeling pressured to send a patient home inappropriately. Therefore the system of SBAR has been included in this chapter to remind every junior doctor as to how best to make effective referrals, no matter at what stage of their career they are or the clinical setting in which they work. Lastly, dealing with a situation involving a major incident or disaster is always at the heart of every ED.

People with DLB and PDD tend to be middle-aged and older. In this age group, selected general health issues deserve discussion. Comprehensive medical care is beyond the scope of this book, but several general medical topics should be addressed here. People with Lewy disorders commonly experience walking difficulties. The risk of falling may surface early or later in the condition. Fracture risk is also a concern, and preemptive action is wise. Falls can be tolerated if bones are strong; witness athletes who fall on a football field. Over the course of a lifetime, bones tend to lose their strength and become more prone to fractures. When the loss of bone integrity is substantial we categorize this as osteoporosis. There are specific criteria that doctors use to define osteoporosis. While bone integrity is measured in several ways, the most common and accepted measurement is by imaging with a nuclear medicine technique. A quick scan after injection of a radioisotope that is taken up by bones generates a picture and numeric data; these can be compared to those of normal subjects. This analysis is termed a nuclear medicine bone density study and sometimes is called a DEXA scan. Using the numeric measures from a bone density scan, reductions of bone integrity fall into two classes. We have already mentioned that substantial loss of bone strength is termed osteoporosis. A less severe reduction of bone integrity has been defined and termed osteopenia. Restated, mild bone weakening is osteopenia, and marked loss of bone integrity is osteoporosis. Conventionally, osteoporosis is treated with prescription medications, whereas the lesser problem of osteopenia is not. However, if there is a substantial fall risk, some clinicians would advise treating osteopenia with a prescription drug (see below). Who is at risk for osteoporosis? In the general population, advancing age is the major risk factor. Women over age 60 and men over age 70 fall into these risk categories, as well as those with very sedentary lives. In the context of DLB or PDD, osteoporosis is especially important to consider.

There is a vast array of presentations and every general practice surgery has a varying case mix ranging from physical to psychological ailments. General practitioners (GPs) are versatile, approachable, empathic and are familiar with yellow and red flags to justify a referral to hospital. GPs make home visits and run numerous specialist clinics to cater for an entire community. Nowadays, GPs are known to be more versatile than ever before as many tend to specialize (cardiology, dermatology, etc.) while others pursue a portfolio career consisting of commercial duties (e.g. event or ship doctors), academic research, and medical education. GPs are effective communicators and need to be able to build a rapport with patients from all walks of life regardless of age, sex, ethnicity, and faith. This chapter highlights the content and spread of general practice within the community. There is also discussion on career developments as a GP, how to make the most of your GP placement, and how to prepare for GP assessments, including communication models and triggers for patients to seek help from their GP.

Chapter 4 shows how a discriminatory professional environment limited doctors’ professional options and how their responses to this context contributed to defining the nature of British general practice. The entry of medical migrants into general practice was unplanned both by central government and doctors themselves. Understanding how doctors ended up working as GPs and being overrepresented in industrial parts of Britain involves developing an appreciation of the environment that the NHS provided at the time. Doctors’ decisions to stay in Britain were made in a professional context which was characterised by racism and what Albert Memmi has termed ‘heterophobia’ (i.e. the fear of difference). Being marked as non-white, and more generally as different directed South Asian doctors towards less prestigious roles in medicine, such as being a GP. A second layer of discrimination occurred within general practice, resulting in migrant doctors being disadvantaged when it came to obtaining GP positions in the more desirable areas.

The author explains why and how he decided to write this book. He says the idea of writing something that would bring the lived experience of doctors at war closer to the general public was originally broached by a high-ranking medical officer, who had been instrumental in helping to secure permission for the author's deployment to Afghanistan. The officer had read one of the author's previous books, The Last Amateurs, and wanted him to come up with a similar work about rear-located medical personnel. There was quite clearly a sense that a candid account of the everyday experience of combat surgical teams would be a welcome addition to the war literature. The result is a candid narrative that chronicles the commitment and fearlessness of the doctors and nurses at the field hospital “who have strong enough stomachs to mop up what we destroy.”
