Abstract:
Relevance . With growing economy and flourishing construction industries the comorbidities among construction workers are also raised. They are exposed to various dust, fumes, noxious gases and vapours making them susceptible to chronic airway diseases like Chronic Obstructive Lung Disease. The aim of the study was to evaluate the respiratory health status of construction workers in an unorganised sector. Materials and Methods . The data collected from National Capital of Delhi region in an unorganised sector of construction workers by using a Saint George Respiratory Questionnaire by the Institute of Occupational Health and Environmental research, Basaidarapur, Delhi, India. All the participant response was noted and the answers were evaluated to see respiratory health status of workers. Total 200 workers were evaluated. Total 182 males and 18 female participated in the study. Results and Discussion . 25% of workers reported poor health at the time of the survey, while only 4% of workers considered their health to be very good. Among the main complaints were indicated: cough, sputum production, shortness of breath, chest infections, attacks of wheezing. The overall mean of Saint George Respiratory Questionnaire core was 33.55. It increases with the working period in the construction field with 21.6% for 10years and 49.1 for 30years experienced workers, while workers with 11 to 20 years of experience, the score was 28.4. The Saint George Respiratory Questionnaire score was 35.1 in 21-30 years of experienced construction workers. Conclusion . As a result of the study, construction workers are found to be at high risk of various respiratory diseases and related disabilities. Participants in this study did not receive any treatment for respiratory problems at any clinic. This means the importance of occupational health education and the use of personal protective equipment and safe working conditions for construction workers.

Abstract:
Nowadays, tuberculosis continue to be one of actual problems of health care. The increasing of effectiveness of modern anti-tuberculosis therapy permitted to decrease mortality of patients with tuberculosis of lungs and to focus attention on problem ofprevention of their disablement. The purpose of study is to carry out comparative analysis of questionnaires for evaluating quality of life ofpatients with tuberculosis of lungs in process of successfully implementing of treatment with the view of selecting questionnaire most completely reflecting occurring alterations. The sampling of 42 patients with tuberculosis of lungs was examined at stage of completion of implemented treatment. All patients were applied spirometry. The following questionnaires were applied to analyze quality of life: SF-36, the Saint George hospital questionnaire (SGRQ) and questionnaire of chronic respiratory diseases (CRQ). The study was carried out twice in process of effective treatment with two months interim. The sensitivity of questionnaires was determined by effect size calculated by formula: [(average value at first examination - average value at second examination)/standard deviation at first examination]. In the process of treatment all patients with tuberculosis of lungs attained significant amelioration of quality of life that was demonstrated in questionnaire of SF-36 by all components besides one that decreased its effectiveness. The size of alterations of quality of life according general value of CRQ was low and amounted to 0.27. The Saint George hospital questionnaire (SGRQ) had maximal sensitivity to positive alterations resulting of applied treatment (effect size 0.39-0.49 for components and 0.47 for total value). Therefore, it was recommended as a tool of evaluation of quality of life in patients with tuberculosis in processes of implementing treatment.

Background: A major problem in developing countries is indoor air pollution and in India a major problem is the lack of awareness. A study says that flour mill workers, on average are exposed to such an environment for 8-10 hrs. a day. This prolongs exposure to flour mill dust and poor ventilation leads to respiratory complications. St. George’s respiratory questionnaire is one of the reliable methods to quantify health status in patients with chronic airflow limitation and also, peak expiratory flow rate (PEFR) has been proven to be a useful tool in assessing airway obstruction9 which is common in flour mill workers. Procedure: The 50 flour mill workers was selected for study according to the inclusion and exclu-sion criteria. The PEFR values and St. George Questionnaire was taken and the domains affected were seen. Result: The prevalence of respiratory problems in flour mill workers was 71%. Conclusion: There is a higher prevalence of respirato-ry problems in flour mill workers due to prolonged exposure to flour mill dust and poor ventilation.

Introduction: The six minute walk test (6MWT) is a practical and reliable measure of exercise tolerance that is widely used to assess the functional status of the patients with a variety of cardiac and pulmonary diseases, including heart failure, pulmonary hypertension, and Chronic Obstructive Pulmonary Disease (COPD). Objective: To evaluate the reliability validity and responsiveness of the six minute walk test and to measure the minimal clinically important difference (MCID) in South Indian IPF patients. Methods: The study population consisted of 30 patients diagnosed as IPF. Six minute walk test distance and other parameters were measured at baseline and at 18 week interval using a standard protocol. Correlation coefficients were used to evaluate the association between six minute walk distance and measures of dyspnea using Borg Scale rate of perceived exertion and Quality of Life questionnaire using St George Respiratory Questionnaire. The distribution based method was used to quantify the MCID. Results: The relationship between 6MWD with dyspnea and St George Respiratory Questionnaire showed good reliability (coefficient = 0.69 p<0.05). There was strong correlation between increase in 6MWD and decrease in Borg Scale of rate of perceived exertion and also decreased St George Respiratory Questionnaire indicating improvement in quality of life. The calculated MCID was 15 m. The decline in 6MWD indicated the prognosis of the disease. Conclusions: The six minute walk test is a clinically helpful measure of status of the disease and the prognosis in South Indian patients with IPF.

Tuberculosis can affect the patients’ quality of life, such as psychological, physical, and social functioning. St. George's Respiratory Questionnaire (SGRQ) is a special instrument which was widely used to measure the patients’ quality of life with respiratory disease. The objective of this study was to validate the Indonesian version of the SGRQ as instrument to collect data. A descriptive cross section design with 61 subjects was conducted at the Pulmonary Clinics and Primary Health Centers in the region of Yogyakarta within 3 months. The validation process included the known group validity, convergent and discriminant validity and factor analysis. There were 14 items question numbers which did not meet the criteria for convergent validity and 9 items which did not meet the criteria for discriminant validity. Known group validity analysis on gender showed that of the three domains of SGRQ, the activity domain gave statistically significant result. The factor analysis showed the result of Kaiser Meyer Olkin analysis (KMO) was less than 0.5. With a few modifications, the Indonesian version of SGRQ is valid and reliable for measuring quality of life in tuberculosis patients.

Improving these patients&#39; quality of life (QOL) is one of the primary treatment goals. The St. George&#39;s Respiratory Questionnaire (SGRQ) is used to measure the health status of patients with respiratory problems. This research aimed to assess the health status of patients with COPD after validating the Persian version of this questionnaire. <strong>Materials and Methods:&nbsp;</strong>The study was performed from Feb to Oct 2016 in Shiraz, Iran. First, the Persian version of the questionnaire was validated using a back-translation technique by two translators, supervised by two researchers, and finally approved by two pulmonologists. The test-retest method was used to investigate the questionnaire&#39;s reliability. The questionnaire was used to assess the health status of 158 COPD patients with FEV<sub>1</sub>/FVC ratio&le; 0.7 or FEV<sub>1</sub>&lt;80%, without exacerbation, cardiac disease, or recent hospitalization. <strong>Results:</strong>&nbsp;The reliabilities of the first part (questions 1-7) and second part (questions 8-14) of the questionnaire was 0.64 and 0.90, respectively. The current health was good, fair, poor, and very poor in 7.6%, 22.2%, 44.9%, 22.8%, and 2.5% of the patients, respectively. The mean&plusmn; standard deviation (SD) scores calculated for symptoms, activity, and impact were 54.216&plusmn;23.725, 41.477&plusmn;24.996, and 37.482&plusmn;26.390, and the total score was 133.176&plusmn;69.284. <strong>Conclusion:&nbsp;</strong>The Persian version of the SGRQ-C was valid and reliable. Summary Bronchial asthma (BA) is considered to be the disease of civilization. Life conditions of the people improve and liability to alergies becomes more intensive. Air polution determines, that asthma spreads fastly in all the world and has become a world problem of health. Asthma has exacerbated quality of life of many asthmatic people: it causes discomfort, psychological tension. Financial losses are much easier beared by a patient than social barrier, however, poor financial situation influences deeply the quality life of the asthmatic people. The goal of this work: to estimate the quality of life of asthmatic people. The following tasks have been set up in achieving this goal: 1. To evaluate the quality of life of astmatic people using SGPQ. 2. To evaluate the influence of BA to patient’s quality of life; 3. To define training impact of BA to quality of life. Investigations have been carried out using SGPQ. 100 patients - asthmatic people have been interviewed according to unanimous questionnarie for no less than one year. Totally the interview have passed 53 men and 47 women, i.e. 53 percentage of men and 47 percentage of women of totally interviewed. Age census of both gender groups was from 18 till 75 years old. Accordingly, the average age of the investigated group was 49,8. Not only the physical activity functions are damaged, causing many physical discomfort, but also negative to the psychological conditions, which in turn impacts on the quality of life. In addition to control the disease symptoms, the most important care is to improve patients’ quality of life. The quality-of-life instruments will help the health care providers to understand the degree of impact on patients, which in turn they can improve the quality of life of the patients using intervention. In addition, the global information exchanges have been increasing, cultural comparison should be emphasized. The purpose of this study tested reliability and validity of the Chinese version of the St. George’s Respiratory Questionnaire for COPD patients (SGRQ-C). The SGRQ-C had translated into Chinese version using a back- translation procedure. A purposive sample of 84 COPD outpatients were recruited from two medical centers in Taipei. Sixty-six outpatients from Cheng (2009) were included and 150 valid data in total. The question- naires used in this study included demographic characteristics, the Chinese version of SGRQ-C, WHOQOL-BREF Taiwan version. In addition, lung function and 6 minutes walk test were measured. Percentage, mean, score index, Pearson correlation, t-test had used for data analysis. p < 0.05 were considered as the statistical significance. The result revealed that the Cronbach’s α for total scale and three subscales were between 0.753 and 0.930. The Split-half reliability was between 0.687-0.883, and Test-retest reliability was between 0.517-0.616, CVI=0.95. The Chinese version of SGRQ-C was significantly correlation with the WHOQOL- BREF Taiwan version. The discriminating validity of the Chinese version of SGRQ-C was between -5.501~-8.464 (p<0.05). According to the disease severely, the Chinese version of SGRQ-C is more applicable to survey the quality of life of chronic obstructive pulmonary disease patients in Taiwan than the WHOQOL- BREF Taiwan version. In conclusion, the Chinese version of SGRQ-C has internal con- sistency and strong evidence of content, concurrent, and discriminating validity for COPD patient in Taiwan. These findings suggest that it is a useful disease-specific instrument and can be used in the future study.

This study compares the performance of the WHOQOL-BREF and the St George’s Respiratory Questionnaire (SGRQ) in patients with COPD. Methods: One hundred and thirty male patients diagnosed with COPD were interviewed in person using both the WHOQOL-BREF and SGRQ. The classic psychometric properties of two questionnaires were compared including internal consistency reliability and validity. The modern psychometric properties based on Item response theory (IRT) was used to calibrate the difficulty of items, and the item fit index was also obtained. Finally, item discrimination and item information were calculated to examine which items were adaptive for COPD patients. Results: The domain scores of the WHOQOL-BREF showed less ceiling and floor effect than the SGRQ. Both questionnaires had acceptable internal consistency (Cronbach’s >0.7). The WHOQOL-BREF had higher convergent and discriminant validity success rates (range, 66.7% to 100% and 92.8% to 100%) than the SGRQ (range, 54% to 94% and 82.1% to 91.7%). The cross-correlations showed that the physical domain of the WHOQOL-BREF correlated strongly with activity, impact, and total domain of the SGRQ (r= 0.53-0.59). The SGRQ are more sensitive in detecting patients’ clinical difference than the WHOQOL-BREF. In IRT analysis, the WHOQOL-BREF physical and SGRQ symptom, activity domain provide the appropriate level of difficulty to capture the patient’s latent trait. All Domains of the two questionnaires have less than one misfit items except the WHOQOL-BREF physical domain. The items related to medical characteristics in both questionnaires showed less item information. Conclusions: In general, both questionnaires showed acceptable reliability and validity psychometric properties. The SGRQ had better validity in assessing symptoms and disease-specific measurements. The WHOQOL-BREF provides more subjective and broader QOL measurements.

Abstract:
INTRODUCTION: Child maltreatment is a significant public health problem that affects all countries and cultures alike. Child maltreatment, which includes neglect, physical abuse, emotional abuse and sexual abuse, can result in negative consequences that are lifelong and irreversible. Previous studies have shown the prevalence of all forms of child abuse in India, which is also home to one fifth of the world’s children. However, adequate resources and efforts are not being made to understand the true scope of this problem. AIM: The present study utilized an existing measure, the Childhood Trauma Questionnaire (CTQ), to understand how parents from the South Indian culture defined acceptable child rearing practices and physical and emotional child abuse. The items from the physical and emotional abuse subscales on the CTQ were also tested for validity and reliability. It was hypothesized that fathers would report higher scores on the physical abuse subscale and mothers would report higher scores on the emotional abuse subscale. It was also hypothesized that parents would find spanking to be an acceptable form of discipline and note that both child physical and emotional abuse are not prevalent in the South Indian community residing in the United States. METHODS: This study recruited 41 participants (21 mothers and 20 fathers) to complete the CTQ. Of the total number of participants, ten were randomly assigned to participate in an in-depth interview, which focused on how parents in the South Indian community in Georgia interpreted the items on the CTQ and how parents defined child physical and emotional abuse. Reliability and validity testing was conducted using data analysis software SPSS 23.0. Qualitative analysis of the interviews involved Consensual Qualitative Research, identifying common themes among all ten interviews. RESULTS: Quantitative analysis revealed low to moderate internal consistency for the emotional abuse scale (α = 0.65) and moderate to high internal consistency for the physical abuse scale (α = 0.88). Independent t-test results showed that fathers reported higher scores on both the physical and emotional abuse subscale; however, these results were not significant. Using the Consensual Qualitative Research method, six domains were determined from the interviews. These included: (a) parent perspectives on child rearing practices, (b) spanking as a discipline practice, (c) country differences between India and the United States regarding discipline, (d) prevalence of abuse among the South Indian community in the United States, (e) reporting child abuse, and (f) parents’ awareness of resources to develop parenting skills. A majority of participants reported spanking as an acceptable form of discipline and believed that neither child physical nor emotional abuse was prevalent in the South Indian community in the United States. CONCLUSION: This study serves as formative research and encourages further investigation of different forms of child abuse in Indian populations, specifically child physical and emotional abuse. Understanding how a culture views children and child rearing practices is important in determining how abuse is defined within said culture. Societies that are more lenient and accepting of violence in general are at a greater risk for perpetrating the maltreatment of children. Establishing a concise definition of child abuse will aid in the development of valid measures that will determine the actual scope of the problem and create solutions, such as laws and policies that will shift a society’s view on appropriate interactions with children.

Abstract:
This chapter explores the influence of life stories and literature on English learners' critical thinking development by examining students' goal orientation, attitudes, and motivation to rhetoric passages with high sociolinguistic content. Specifically, this study, part of a larger project, applies a sociocultural pedagogical model to raise L2 learners' awareness of the relevance of developing their critical thinking competence to analyze reality and gain confidence in expressing their voice. As a dystopian life story, George Orwell's 1984 novel is used as critical thinking triggering strategy for first-year bilingual education baccalaureate students to achieve this goal. The study's methodical instruments included a three-dimensional analysis model, pre- and post-questionnaires, specific tasks on judgment and inference, guided interviews, rubrics, and field observations. This chapter reports on the study's qualitative findings and demonstrates the effect of literature and stories on students' engagement and awareness of their critical thinking skills.
