Abstract:
En el presente trabajo revisamos los últimos avances en la comprensión de la inmunosenescencia o envejecimiento del sistema inmunitario. Este proceso es producto de la interacción de factores innatos (genéticos) y, adquiridos (nutrición, estilos de vida e interacción huésped-parásito). Entre los factores genéticos destacan la expresión controlada genéticamentede las moléculas del sistenia HLA en las células inmunesy como la expresión de determinados alelos puede estar ligada a una mayor longevidad. Otros factores importantes son las modificaciones del sistema inmune en el geronte, a lo que contribuye la compresión de la involución tímica, así como los cambios en la distribución de las subpoblaciones linfocitarias y la calidad y cantidad de éstas. Otro factor de la inmunosencencia lo constituye la modificación en la producción de citoquinas, hormonas reguladoras del sistema inmune Es claro que de la comprensión adecuada de los factores mencionados se obtendrá estrategias dirigidas a la prevención, seguimiento y, por último, a la inmunointervencion en los pacientes gerontes.

Abstract:
Objetivo: Determinar la significativa utilización de servicios de salud por los adultos mayores usuarios del Hospital Nacional Cayetano Heredia entre 1990-97, mediante la comparación de dicho instrumento y sus características, entre pacientes geriátricos y de menor edad. Material y métodos: En este estudio retrospectivo, se incluyeron pacientes de 14 años y más, atendidos en hospitalización entre 1990-96, y en consulta externa y emergencia durante 1997. La información fue relevada de libros de alta y consolidados estadísticos, siendo tabulada por grupos de edad, para efectos comparativos entre pacientes mayores y menores de 60 años. Resultados: Hospitalización: los adultos mayores constituyeron 28.11% de los atendidos [p<0.01]. Entre 1990-96 hubo incremento de pacientes geriátricos (+19.4%) y disminución en los menores (-6.41%) [R2=0.5768]. A nivel geriátrico predominaron infecciones (33.9%) y enfermedades del aparato circulatorio (32.9%). La mortalidad geriátrica fue predominante (12.8%)[p<0.01]. Entre 1990-96 hubo decrecimiento de mortalidad geriátrica (-25%), sin observar cambios en los menores [R2=0.4045]. Consulta externa: los adultos mayores constituyeron 16.76% de los atendidos, con una población femenina que casi duplica a los varones [p<0.01]. Los gerontes continuadores predominaron sobre los menores y aquellos gerontes que consultaron sólo una vez [p<0.01]. La tasa de retorno y razón de atenciones/atendidos favoreció a los adultos mayores. La morbilidad geriátrica ambulatoria predominante fueron las cardiovasculares (20.14%). Emergencia: los pacientes geriátricos constituyeron 6.2% de los atendidos. Las entidades traumatológicas fueron predominantes en ambos grupos de edad, sin evidenciar diferencias significativas. Conclusiones: La utilización de los servicios de hospitalización y consulta externa realizada por los pacientes geriátricos, fue significativa en relación a los más jóvenes.

El dolor crónico en el adulto mayor es uno de los temas más álgidos, su atención debe ser permanente y la experiencia emocional de quien la padece lleva a una afectación no sólo de quien lo sufre, sino de aquellos que le acompañan. En el asilo San Pedro Claver, se manejan aspectos relacionados con las manifestaciones del dolor crónico el cual afecta la calidad de vida de los gerontes. El dolor se presenta como un evento incapacitante, ya que impide realizar actividades como: el caminar, permanecer sentado y permanecer acostado, también se ven afectados los patrones de sueño, la autoestima y las relaciones con la familia.

Resumo: O hipotireoidismo subclínico (HTSC) é uma enfermidade caracterizada pelo aumento dos níveis de hormônio tireoestimulante associado a níveis normais de tiroxina livre, habitualmente sem sinais e sintomas do hipotireoidismo clinicamente manifesto (HTC). A literatura mostra prevalência mais alta de HTSC em mulheres e nos mais idosos. No presente trabalho, estudou-se 76 mulheres portadoras de HTSC, com 65 anos ou mais, seguidas no Ambulatório de Endocrinologia da Faculdade de Medicina de Jundiaí, São Paulo. Foram excluídas mulheres com idade inferior a 65 anos, pacientes com diagnóstico prévio de HTC e HTSC em tratamento, Diabetes Mellitus ou outra endocrinopatia que possa cursar com elevação dos níveis lipídicos , hepatopatias, alcoolismo, nefropatias, tabagismo, uso de drogas hiperlipemiantes e pacientes com antecedentes de Doença Vascular Aterosclerótica (DVA). Avaliou-se: idade: 70.43 ± 4.84 anos; IMC: 27.52 ± 3.07 kg/m², pressão arterial sistólica (PAS): 136.97 ± 17.44 mmHg; pressão arterial diastólica (PAD): 87.89 ± 10.87 mmHg; tiroxina livre (T4L): 1.28 ± 0.25 ng/dl; hormônio tireo-estimulante (TSH): 12.41 ± 11.19 mIU/L; colesterol total (CT): 257.12 ± 61.22 mg/dl; HDL-colesterol (HDL-c): 39.30 ± 8.78 mg/dl; LDL-colesterol (LDL-c): 146.49 ± 42.94 mg/dl; VLDL-colesterol (VLDL-c): 41.13 ± 27.41 mg/dl e triglicérides (TG): 224.46 ± 123.30 mg/dl. Foi feito o estudo dos perfis clínico-laboratoriais presentes nesse grupo de pacientes, através da análise comparativa dos agrupamentos, que mostrou a existência de dois perfis: 1. Mulheres idosas com HTSC com maior valor de T4L e HDL-c e menor valor de TSH, CT, TG, LDLc, VLDL-c, CT/HDL-c e LDL/HDL-c; 2. Mulheres idosas com HTSC com valor menor de T4L e HDL-c e maior valor de TSH, CT, TG, LDLc, VLDL-c, CT/HDL-c e LDL/HDL-c. A aparente relação entre HTSC e dislipidemia resultou em uma elevada relação CT/HDL-c (p < 0.001) e também na relação LDL-c/HDL-c (p < 0.001) no subgrupo 2, o que traduz um risco cardiovascular aumentado, associado à DVA. Em conclusão, o estudo relaciona o HTSC em um grupo de mulheres idosas com a dislipidemia, mostrando dois perfis endócrinometabólicos, com risco de desenvolvimento de aterosclerose mais elevado naquele que apresenta menores valores de T4L e HDL-c
Abstract: Subclinical hypothyroidism (SCH) is a disease characterized by an increase in the thyrostimulating hormone (TSH), which is associated to normal levels of free thyroxin (FT4), which frequently does not present signs and symptoms of clinically manifested hypothyroidism. Literature reports a higher prevalence in women and elderly people. In this work, 76 elderly women with SCH were studied in the Endocrinology Clinic of FMJ - Jundiaí (São Paulo, Brazil), excluding patients in treatment to hypothyroidism and who presented other endocrinopathy or conditions that cause dyslipidemia. They were analyzed according to: Age (Average ± OR): (70.43±4.84), BMI (27.52±3.07), Systolic Blood Pressure (136.97±17.44), Diastolic Blood Pressure (87.89±10.87), FT4 (1.28±0.25), TSH (12.41±11.19), Total Cholesterol (TC) (257.12±61.22), HDL-c (39.30±8.78), LDL-c (146.49±42.94), VLDL-c (41.13±27.41) and Triglycerides (224.46±123.30). The clinical-laboratorial profiles present in this group was carried out through the clusters¿ comparative analysis, demonstrating the existence of two profiles: 1. Higher levels of FT4 and HDL-c and lower levels of TSH, CT, TG, LDL-c, VLDL-c, CT/HDLc and LDL/HDL-c; 2. Lower levels of FT4 and HDL-c and higher levels of TSH, CT, TG, LDL-c, VLDL-c, CT/HDL-c and LDL/HDL-c. The apparent relationship between SCH and dyslipidemia resulted in a high CT/HDL-c (p<0.001) and LDL-c/HDL-c (p<0.001) ratios in subgroup 2, which implies in increased cardiovascular risk, due to atherosclerotic vascular disease (AVD). In conclusion, this study relates SCH with dyslipidemia in a group of elderly women, revealing two endocrine-metabolic profiles, in which that with smaller FT4 and HDL-c levels presents a higher risk of developing atherosclerosis

Abstract:
La osteoporosis es definida como la alteración esquelética de la microarquitectura de los huesos, lo que se traduce en una disminución de la resistencia ósea. El presente estudio busca determinar el grado de recuperación, según densitometría, al año de tratamiento con Alendronato (Fosamax®) versus Calcio/Vitamina D (Caprimida D Forte®), en gerontes mujeres con osteoporosis tratadas en el Hospital Militar Geriátrico (HMG) durante todo el 2003. Durante el periodo de estudio se intervinieron un total de 237 gerontes mujeres con osteoporosis, de los cuales 149 recibieron tratamiento con Alendronato (1 tableta por día) y 88 con Calcio/Vit (Calcio elemental 500 mg + Vitamina D3 400 U.I., 1 capsula al día). Se compararon diversos aspectos en los dos grupos y se concluyó que tanto el Alendronato 70mg (Fosamax ®) como Calcio/Vit. D (Caprimida D Forte®) al año de tratamiento en gerontes mujeres del HMG mejoraron significativamente la densidad mineral ósea así como el score t a nivel de la columna, ambos mejoraron la DMO a nivel de la cadera. Así mismo Alendronato fue mucho más eficaz respecto a la recuperación de la DMO y el score t a nivel de la cadera que la terapia con Calcio/Vit D.

Abstract:
This thesis traces the development of The Dream of Gerontius from its genesis in the writings of John Henry Newman, through his early sketches and fair copies to the edition published in 1866. It then considers the possible reasons for Edward Elgar’s decision to set the poem to music and traces its development into an oratorio, exploring the extent to which the oratorio reflects the ideas contained in the original poem. Chapter One examines how the poem emerges out of the broad and immediate context of Newman’s life, considering (inter alia) both the position of the Roman Catholics in nineteenth-century England, and Newman’s experience of writing the Apologia, the work immediately preceding the poem. Chapter One also investigates the compositional process and the circumstances surrounding the publication of the poem. Chapter Two traces the development of the ideas of the poem through its early texts; taking three sections of the poem as examples, it provides a detailed comparison of five different versions of the poem. Chapter Three explores the poem’s philosophical and theological ideas, examining the extent to which Newman endeavours to make Roman Catholic theology acceptable to a Protestant readership. The reasons for Edward Elgar’s choice of the poem and his redaction of it into the libretto are considered in Chapter Four. Chapter Five looks at Elgar’s compositional processes and, through an examination of his early sketches, traces the development of his early ideas, especially in relation to the Soul’s appearance before God and how his ideas were altered as a result of the intervention of August Jaeger. The extent to which Newman’s original themes are still reflected in the oratorio is the subject of Chapter Six which also includes a description and account of the music of the oratorio.

At the current time of demographic changes in the world , it is observed that the health sector requires restructuring of its practices in caring for the elderly , in order to enable a comprehensive and humane care . In this sense, we identify the need for qualified professionals to assist the elderly and, therefore , it is important that training curricula are adequate to this reality . The study aims to analyze how it develops teaching gerontological nursing component in undergraduate nursing . This is a descriptive , exploratory study with a qualitative approach, in two higher education institutions in the city of Natal / RN . The population involved corresponded to the coordinators of graduate courses in nursing and teachers who teach content of health care for the elderly, a total of seven participants . The data collection instruments were used: a questionnaire containing questions related to the characterization of the participants and the teaching of gerontological nursing in undergraduate courses in nursing and a form with items of analysis for pedagogical projects . The results were presented descriptively in tables and analyzed from coming themes of the main themes of the instruments defined in: knowledge about the research participants , knowledge of teaching gerontological nursing component and knowledge of pedagogical projects , discussed under Resolution paragraph . 03 of 7/11/2001 , which establishes standards for the nursing program , the educational projects of educational institutions , the Theoretical Donald Shoon and thematically relevant literature . The results indicate that the majority of participants were female (four), all have graduate though , none of them stated in the qualification of aging or elderly health area; the organization of the contents of the component in the curriculum, workload theoretical- methodological and practical teaching strategies presented was heterogeneous and sometimes diverging at the same institution . On the definition of gerontological nursing component was identified that is not well defined along the pedagogical design of educational institutions surveyed . It was concluded that in the context of this study , teaching gerontological nursing is present in the course of nursing education, but needs reevaluation and updating in order to teach a wider range of driving and health practices
No atual momento das mudan?as demogr?ficas populacionais no mundo, observa-se que o setor sa?de necessita de uma reestrutura??o de suas pr?ticas no atendimento ? pessoa idosa, de maneira a possibilitar um cuidado integral e humanizado. Nesse sentido, identifica-se a necessidade de profissionais qualificados para assistirem a pessoa idosa e, para isso, ? importante que os curr?culos de forma??o do enfermeiro estejam adequados a esta realidade. O estudo tem como objetivo geral, analisar como se desenvolve o ensino do componente enfermagem gerontol?gica em cursos de gradua??o de enfermagem. Tratou-se de um estudo descritivo e explorat?rio, com abordagem qualitativa, realizado em duas institui??es de ensino superior no Munic?pio de Natal/RN, sendo uma institui??o p?blica e outra. A popula??o envolvida correspondeu aos coordenadores do curso de gradua??o em enfermagem e professores que lecionam conte?dos de aten??o ? sa?de do idoso, totalizando sete participantes. O instrumento de coleta de dados utilizado foi um question?rio contendo quest?es relacionadas ? caracteriza??o sociodemogr?fica e profissional e ao ensino da gerontologia nos cursos de gradua??o em enfermagem. Os resultados foram apresentados de forma descritiva em quadros, e analisados a partir de eixos tem?ticos oriundos das principais quest?es nominados em: conhecendo os participantes da pesquisa, conhecendo o ensino do componente enfermagem gerontol?gica e, conhecendo os projetos pedag?gicos. A an?lise descritiva foi do tipo comparativa, em fun??o dos resultados incluir duas institui??es de ensino, com base na Resolu??o n?. 03, de 7/11/2001, que disp?e sobre as diretrizes m?nimas para o curso de enfermagem e nos projetos pedag?gicos de ambas as institui??es de ensino. Os resultados mostraram que a maioria dos participantes (cinco) eram do sexo feminino, quase todos possuem p?s-gradua??o embora, nenhum deles declarou qualifica??o na ?rea do envelhecimento ou sa?de do idoso; a organiza??o dos conte?dos do componente na matriz curricular, a carga hor?ria te?rico-pr?tica e as estrat?gias metodol?gicas de ensino apresentaram-se de forma heterog?nea e, algumas vezes divergentes em uma mesma institui??o; Sobre a defini??o do componente enfermagem gerontol?gica, identificou-se que n?o est? bem definido ao longo do projeto pedag?gico das institui??es de ensino pesquisadas. Abstract:
This narrative inquiry traces and recounts an epiphaffic experience of a registered nurse on entering gerontic nursing, and her subsequent three-decade journey through the complexities and mazes of this nurse specialty. Such inquiry seeks to enable a better understanding of the realities of ageing and caring for older adults by opening up thinking and beliefs underpinning gerontic nursing work. Modern aged healthcare involves complex gerontic nursing actions, requiring highly skilled nursing personnel, but on the whole, gerontic nursing is dimly perceived and misunderstood by professional colleagues and the general public. Much of this misunderstanding is a legacy of an outdated ideology of gerontic nursing; yet these public beliefs, attitudes and interpretations are extremely powerful in determining aged care policy. As the population ages and more elderly people access healthcare services, society will be faced with an array of complex political and socioeconomic factors. This thesis aims to untangle such choices by pursuing the questions of: How have the constructs of ageing impacted on gerontic nursing practice and education?, What type of gerontic nurse will be required to provide future elderly care? and How will these people be educationally prepared for their new roles? Many of the constructs explored are dialectical in nature; that is, they have developed by inner conflict, the scheme of which is thesis and antithesis, or an original tendency and its opposing tendency. Such dialectical thinking has underpinned much of this thesis and in many instances, particularly in chapter 7, has taken the next step to the unification of these opposing tendencies; that is, synthesis, to create new understanding or meaning. Issues explored relate to: the ontology of ageing; the meaning of life; gerontophobia; Australia's changing population profile; changing aged healthcare systems; gerontic nursing cultural dilemmas; workforce planning; elder health in the future and gerontic nursing practice and education shifts. In a theoretical and methodological context, increasing difficulty with conventional epistemologies and the science founded on them is leading nurse theorists ever nearer to a postmodernist position. Narrative becomes a means through which gerontic nursing can accumulate and express cultural knowledge and critique procedure. The thesis exemplifies narrative's profound potential for underpinning the reconceptualisation of gerontic nursing practice and education. It is narrative's capacity to foreground the relationship between daily practice and knowledge that makes it a critical tool for the future of gerontic nursing inquiry. Narrative facilitates the paradigm, or more ontological shift from the dominant medical model of aged healthcare and 'tender loving care' rhetoric, to a therapeutic, caring-healing approach which has been in the margins in gerontic nursing practice. In the context of gerontic nurse education, narrative pedagogy offers new ways of thinking even in the midst of oppressive practices. Many issues remain unresolved about how gerontic nurses can be educated for future gerontic nursing practice. It would seem that aged care in Australia is a site of such organisational and cultural change, it threatens to undermine knowledge, care and understanding and shift care to untrained staff. The thesis illustrates how such approaches cloak much of gerontic nursing practice and devalue the intimate work of caring intelligently, emotionally and physically for frail older adults. However, while such tensions abound in gerontic nursing practice, the 2l~ century offers skilled gerontic nurses the opportunity to become key components in the refigured and redesigned aged healthcare delivery system. Research indicates that because few know enough about the sum of the future to impede well-constructed attempts at engaging in any new model design, taking any action is infinitely better than none. It is on this premise that Chapter seven posits a new model design for residential long-term aged care for older adults, believing that by imagining a different future, it can then be created and become a reality.

OBJETIVO: Adaptar transculturalmente o Exame Geronto Psicomotor (EGP) e analisar suas evidências de validade. MÉTODOS: A amostra foi constituída por 51 idosos, 10 participaram da adaptação transcultural e 41 da etapa de avaliação das propriedades psicométricas do EGP. As variáveis analisadas foram: Validade de conteúdo, concordância entre juízes, consistência interna, estabilidade teste reteste e validades convergente e divergente. As etapas de adaptação transcultural foram: tradução para a Língua Portuguesa, síntese das traduções, tradução reversa, treinamento dos avaliadores, avaliação do comitê de especialistas e pré-teste. Para coleta de dados utilizou-se: Questionário social, Mine Exame do Estado Mental (MEEM), Sênior Fitness Test (SFT) e a versão brasileira do EGP. Na análise dos dados e tratamento estatístico foi utilizado o programa SPSS 22. A pesquisa foi aprovada pelo Comitê de Ética sob o parecer nº 1.223.636. RESULTADOS: Após a análise das equivalências e as modificações propostas pelo comitê de especialistas, a versão pré final do EGP passou por um pré-teste (Piloto) em um grupo de 10 idosos. Durante a adaptação transcultural foram realizadas diversas mudanças no EGP, desde o layout até os comandos do teste. O coeficiente de k demonstrou alto grau de concordância entre os juízes. O valor do alfa de Cronbach foi (α=0,69) e não houveram itens cuja retirada impactaria na melhora desse índice. A correlação entre os dois momentos de aplicação mostrou que o escore global da EGP possui boa estabilidade (r=0,98; p<0,01). CONCLUSÃO: O EGP foi adaptado com êxito para uso no Brasil e demonstrou ser um bom instrumento para traçar perfis individualizados, já que engloba tanto a parte quantitativa quanto a qualitativa da avaliação. Contudo, a aplicação com grande número de indivíduos se torna complexa.
OBJECTIVE: To Adapt cross-culturally the Geronto Psychomotor Exam (EGP) and analyze its evidence of validity. METHODS: The sample consisted of 51 elderly, 10 participated in the cross - cultural adaptation and 41 of the stage of evaluation of the psychometric properties of EGP. The variables analyzed were: contente validity, judges greement, internal consistency, test retest stability and convergent and divergent validities. The stages of cross-cultural adaptation were: translation into Portuguese, synthesis of translations, reverse translation, training of evaluators, evaluation of the committee of experts and pre-test. For data collection, were used: Social Questionnaire, Mine Mental State Examination (MEEM), Senior Fitness Test (SFT) and the Brazilian version of EGP. In the data analysis and statistical treatment the SPSS program 22 was used. The research was approved by the Ethics Committee under opinion nº 1,223,636. RESULTS: After the analysis of the equivalences and the modifications proposed by the expert committee, the pre-final version of the EGP underwent a pre-test (Pilot) in a group of 10 elderly people. During the cross-cultural adaptation several changes were made in the EGP, from the layout to the test commands. The coefficient of k showed a high degree of concordance between the judges. The value of Cronbach's alpha was (α = 0.69) and there were no items whose withdrawal would impact the improvement of this index. The correlation between the two application moments showed that the overall EGP score had good stability (r = 0.98, p <0.01). CONCLUSION: The EGP was adapted for use in Brazil and has proved to be a good tool for drawing individual profiles, since it encompasses both the quantitative and the qualitative aspects of the evaluation. However, application with large numbers of individuals becomes complex.

Abstract:
Melania the Younger died more than a decade before the Council of Chalcedon (451 CE) and the ensuing bitter conflicts between Chalcedonian and miaphysite Christians. Nevertheless, the Greek Vita by Gerontius portrays her as actively involved in numerous religious and political controversies surrounding bishop Nestorius preceding her death. This chapter argues that the historicity of her alleged anti-Nestorian activities in the Vita must remain in doubt. The anti-Nestorian Melania of the Greek Vita appears to support Gerontius’s miaphysite condemnation of the politically dominant Chalcedonian Christians, providing him with a useful weapon in the pressing Christological battles he faced following her death. While the anti-Nestorian stance Gerontius attributed to Melania remained orthodox in Greek Christianity, his anti-Chalcedonian views, which he considered the natural extension of Melania’s, did not. Gerontius’s Vita Melania thus serves as a microcosm of the complex and highly politicized fifth-century disputes over the definition of Christian orthodoxy.

In the Life of Melania, Gerontius explains that Melania was impelled to adopt an ascetic life because she had been “wounded by divine love” and that, later in life, that “wounding” prompted Melania to a more rigorous form of asceticism. The concept of “divine wounding”—a prodding and provoking by God that drove ascetics to deeper devotion, discipline, and piety—had become relatively well-defined and common in the fourth and fifth centuries. This chapter demonstrates that wounding was an exceedingly “usable” image in ascetic texts given the widespread bodily experience to which the figurative language referred and given its relationship to other wound metaphors pervasive in earlier Christian discourse.
