This dissertation has explored the problem of a yawning gap between policy and the implementation of lay participation in health development activities in Ghana, using data from 577 households in 22 rural communities.<br>A Health Systems model has been applied to data, to explain relationships and four sets of variables--household need for health services, predisposing attributes, participatory patterns, enabling factors--on household use of available health facilities and services.<br>Overall, results indicate a complex interdependence of factors which influence modern health services use. Extending coverage to the informal sector is a key challenge to achieving universal coverage through contributory health insurance schemes. Ghana introduced a mandatory National Health Insurance scheme in 2004 to provide financial protection for both the formal and informal sectors through a combination of taxes and annual premium payments. As part of its election campaign in 2008, the current government (then in opposition) promised to make the payment of premium "one-time". This has been a very controversial policy issue in Ghana.

This dissertation investigates the mutual life project of Signe (1896-1988) and Axel Höjer (1890-1974), a married couple who were key actors in the construction of the Swedish welfare state.
