Abstract:
Objetivo: Determinar la asociación entre los niveles de hormonas tiroideas (HT) y el grosor de la íntima-media carotidea (GIMC) en pacientes con enfermedad renal crónica en hemodiálisis. Material y métodos: Se incluyeron 50 pacientes adultos que recibían hemodiálisis crónica y regular en el Centro de Hemodiálisis de la Universidad Peruana Cayetano Heredia en el período de enero a junio del 2011. Se determinaron los niveles séricos de colesterol total y fracciones, calcio, fósforo, PTH, albúmina, TSH, T4 libre, T3 total y T3 reversa (T3r). El GIMC se determinó por ecografía Doppler de carótidas. Resultados: La media de edad fue 55 años y 42% fueron mujeres. Las alteracionestiroideas estuvieron presentes en 50% de los pacientes, siendo más frecuente el hipotiroidismo subclínico. El 56% tenía GIMC >0,90mm y placas ateromatosas (>1,5mm) se encontraron en 50% de los pacientes. No se encontró asociación entre HT y GIMC. Conclusiones: Las hormonas tiroideas no están asociadas con el incremento en elgrosor de la íntima-media carotideo.

Abstract:
Introducción: El objetivo del presente trabajo fue determinar la relación entre la limitación de la movilidad articular (LMA) de pacientes con DM 2 con el grosor de íntima-media de arteria carótida (GIMC). Material y métodos: la LMA se diagnosticó a través de una maniobra de la plegaria positiva y se confirmó con el test de impresión de las palmas pintadas. Se incluyeron 80 pacientes diabéticos tipo 2 (40 con LMA y 40 sin LMA), con edades entre 40 y 80 años. El GIMC se midió con ultrasonografía de alta resolución en Modo B. Resultados: los pacientes de ambos grupos no tuvieron diferencias significativas en edad y sexo. El grupo de LMA tuvo mayor tiempo de duración de diabetes. La glicemia en ayunas y la HbA1c, no presentaron diferencia significativa en ambos grupos de pacientes. Tampoco hubo diferencias en relación con hipertensión arterial, tipo de tratamiento antihipertensivo y terapia antidiabética. Los pacientes con LMA tuvieron un GIMC significativamente mayor que sin LMA (arteria carótida común, con LMA 1,035 ± 0,313mm., vs sin LMA 0,856 ± 0,57 mm., p<0,05; arteria carótida interna, con LMA 0.876 ± 0.107mm. vs sin LMA 0,840 ± 0,150 mm., p<0,05). El GIMC combinado (promedio de todos los segmentos) también fue mayor (con LMA 0,956 ± 0,018 mm vs sin LMA 0,847 ± 0,139 mm., p= 0,007). Conclusiones: La LMA estuvo asociada a un mayor GIMC.

Abstract:
Introducción: el inicio de la prediabetes en edades pediátricas se ha convertido en un problema cada vez más frecuente, donde la obesidad juega un papel relevante. Los pacientes con prediabetes en la infancia presentan una alta prevalencia de señales aterogénicas tempranas (SAT), las cuales constituyen factores de riesgo para enfermedad cardiovascular, esto conduce a una alteración precoz de la función endotelial. Objetivo: determinar la asociación entre las SAT detectadas y la aterosclerosis subclínica en niños y adolescentes con diagnóstico reciente de prediabetes. Método: se realizó un estudio descriptivo y transversal con 31 niños y adolescentes entre 5 y 18 años de edad con diagnóstico reciente de prediabetes, atendidos en el Instituto de Endocrinología de Cuba, durante noviembre 2015 a noviembre 2016. Las principales variables estudiadas fueron: edad, sexo, antecedentes patológicos familiares y personales, presencia de obesidad abdominal, tensión arterial, lipidograma, resistencia a la insulina, disfunción endotelial (DE) y grosor íntima media carotideo (GIMC). Resultados: predominaron los adolescentes, el sexo masculino, los antecedentes familiares de diabetes mellitus tipo 2 y personales de obesidad. La obesidad abdominal, tensión arterial normal, lipidograma normal y resistencia insulínica caracterizaron a los pacientes. Se detectó DE en el 19,3 % de los pacientes y GIMC aumentado en el 58 %. En los pacientes con DE y GIMC aumentado, predominó la obesidad abdominal y la resistencia a la insulina. Conclusiones: desde el momento del diagnóstico de la prediabetes puede existir aterosclerosis subclínica en niños y adolescentes que presentan varias SAT asociadas.

El hipotiroidismo subclínico puede contribuir al desarrollo de patologías cardiovasculares y se ha demostrado que a partir de niveles de TSH &gt;2,5uUI/mL se desarrolla disfunción endotelial. <strong>Objetivos:</strong> Determinar el grosor de la íntima media carotídea (GIMC), la presencia de placas en carótidas y el perfil metabólico en mujeres de edad media asintomáticas con TSH ≥2,5 uUI/mL y compararlas con aquellas con niveles &lt;2,5 uUI/mL. <strong>Diseño:</strong> Estudio transversal y analítico. <strong>Lugar:</strong> Hospital Nacional Dos de Mayo e Instituto de Investigaciones Clínicas, Universidad Nacional Mayor de San Marcos, Lima, Perú. <strong>Participantes:</strong> Mujeres sin historia de enfermedad tiroidea, cardiovascular o diabetes. <strong>Intervenciones: </strong>En 60 mujeres sin historia de enfermedad tiroidea, cardiovascular o diabetes, con edad promedio de 53,8 ± 5,8 años, se determinó la tirotropina (TSH), colesterol total (CT), colesterol de densidad alta (HDL), triglicéridos (Tg), glucosa (G) basal y a los 120 minutos (TTGO), insulina basal (Ins-B); se calculó las fracciones de colesterol de densidad baja (LDL), el nivel de insulinorresistencia (HOMA-IR), la presión arterial, perímetro de cintura abdominal, el índice de masa corporal (IMC) y el GIMC mediante ecoDoppler. <strong>Principales medidas de resultados:</strong> TSH ≥2,5 uUI/mL, perfil metabólico y su relación con GIMC. <strong>Resultados:</strong> El 38,3% presentó TSH ≥2,5 y 61,7% TSH &lt;2,5 uUI/mL. El 56% de mujeres con TSH ≥2,5 y 65% con TSH &lt;2,5 uUI/ml fueron hipertensas, sin diferencia estadística. El perfil lipídico, G basal, Ins-B e índice HOMA-IR fueron semejantes en ambos grupos. Se observó niveles significativamente más altos del IMC, G a los 120 minutos, el GIMC en carótida izquierda y el mayor entre ambas carótidas en las mujeres con TSH ≥2,5 uUI/mL (p=0,03, p=0,01, p=0,008 y p=0,02, respectivamente). La presencia de placas en la carótida izquierda y en al menos una de las carótidas fue significativamente más frecuente entre aquellas con TSH ≥2,5 uUI/ mL. <strong>Conclusiones:</strong> Los niveles de TSH ≥2,5 uUI/mL en mujeres de edad media predispondrían a la alteración del metabolismo de la glucosa y a aterosclerosis subclínica.

Abstract:
Les diverses normes internationales obligent les constructeurs automobiles à optimiser les chaînes de propulsion conventionnelles mais surtout à développer d’autres alternatives de motorisation dont l’une des plus prometteuses est le véhicule électrique. Néanmoins ces nouvelles propulsions doivent garantir les mêmes performances et le même niveau de sureté de fonctionnement (fiabilité et sécurité en l’occurrence). La chaîne de propulsion électrique est conçue autour d’un nombre important de constituants (machine électrique, capteur(s), convertisseur(s) de l’électronique de puissance, etc.) qui peuvent être le siège de défauts. La détection et la localisation de ces défauts sont indispensables mais pas suffisantes pour assurer la sureté de fonctionnement du système. En effet pour assurer un fonctionnement en mode dégradé, il faut mettre en œuvre une architecture de commande tolérante aux fautes. L'objectif principal de cette thèse est de proposer des nouvelles architectures de commande tolérante aux défauts (Fault Tolerant Control en anglais) d’un véhicule électrique propulsé par une machine asynchrone, en présence de plusieurs types de défaut du capteur mécanique. Cette thèse est organisée en 4 chapitres.Le Chapitre 1 est un état de l’art exhaustif avec une analyse critique des architectures et des systèmes de contrôle commande tolérants aux fautes des chaînes de traction électrique ainsi qu'un état de l'art des différents défauts qui peuvent apparaitre dans la chaine de traction électrique. Le Chapitre 2 propose deux architectures de commande tolérante aux fautes : l’Hybride FTC et le GIMC (Generalised Internal Model Control) ; l’approche Hybride FTC est une combinaison de deux contrôleurs, le premier est un régulateur PI pour le mode sain et le second un correcteur robuste H infini pour le mode défaillant. L’architecture GIMC permet la restructuration de la loi de commande d'une manière adaptative. Elle est conçue afin d’assurer une bonne robustesse du système en présence de défaut grâce à une boucle interne faisant office de boucle de correction et de diagnostic.Le Chapitre 3 est consacré exclusivement à la commande tolérante aux fautes à base d'algorithme de vote, en faisant une étude comparative de 4 algorithmes avec trois topologies différentes : une première structure est proposée sur la sortie du système, une deuxième structure est appliquée sur la commande du système, et la troisième structure est une hybridation des deux précédentes.Le Chapitre 4 est dédié à la validation expérimentale des architectures décrites précédemment. The various international standards require automakers to optimize conventional power train but mainly to develop other alternatives to drive, one of the most promising is the electric vehicle. However, these new drives should guarantee the same performance and the same level of dependability (reliability and security in this case).Electric power train is built around a large number of components (electrical machine, the sensor, the converter, power electronics, etc.) which may be affected by defects. The detection and localization of these defects are essential but not sufficient to ensure the dependability of the system. Indeed, to ensure operation in degraded mode, you must implement architecture of fault tolerant control (FTC). The main objective of this thesis is to propose new fault tolerant control architectures of an electric vehicle induction machine power train in the presence of several types of mechanical sensor failure. This thesis is organized into four chapters.Chapter 1 is a comprehensive state of the art with a critical analysis of architectures and control systems tolerant to faults of electric powertrains and a state of the art of the various defects that may occur in the chain of electric traction.Chapter 2 proposes two architectures of fault tolerant control: Hybrid FTC and GIMC (Generalised Internal Model Control); Hybrid FTC approach is a combination of two controllers, the first is a PI controller for the sound mode and the second a robust controller for the failed H infinity mode. The architecture allows GIMC restructuring the control law in an adaptive manner. It is designed to ensure robustness of the system in the presence of a fault with an inner loop acting loop correction and diagnosis.Chapter 3 is devoted exclusively to the algorithm voting fault tolerant control based on comparative study of four algorithms with three different topologies: a first structure is proposed on the system output, a second structure is applied system control, and the third structure is a two preceding hybridization.Chapter 4 is devoted to the experimental validation of the architecture described above. Abstract:

This thesis is divided in to two different parts. The first part includes examination of the measurementsystem of an industrial robot using a resolver sensor. The main focus is on methods for suppressing noise in the angularvelocity signal without increasing timedelay. Five different methods are investigated. Three of these are based on oversampling: burstsamplingmethod, meanvaluemethod and correlationmethod. The meanvaluemethod and the correlationmethod have given good results. The two other methods are: extended kalmanfiltering and computation of the angular velocity without using numeric method to compute the angle. Extended kalmanfilter gives the overall best result.

In part two the control structure design of an industrial robot has been studied and how different sampling times and motorinertias affect the disturbance rejection and stability of the control loop. Different control structure designs have also been studied with the aim to suppress disturbances. Mainly H-infinity and GIMC designs have been compared with an ordinary PID controller. Here it can be shown that both the H-infinity and the GIMC controller yields good disturbance rejection. But both the methods lack in the robustness of model uncertainty.

Abstract:
La enfermedad cardiovascular es un trastorno complejo, multifactorial, poligénico y con un importante componente ambiental. Este grupo de enfermedades, y en particular la cardiopatía isquémica, ocasionan una gran morbilidad y mortalidad a nivel poblacional. Por lo tanto, la prevención es un elemento importante en todas las políticas de salud pública. En la última década se ha avanzado de manera notable en el conocimiento del componente genético de la cardiopatía isquémica y se han descubierto más de 50 loci asociados con el riesgo de presentar esta enfermedad. Muchos de estos loci no se asocian con los factores de riesgo cardiovascular clásicos, y el mecanismo etiopatogénico que explica esta asociación no se conoce. Por otra parte, la variabilidad genética tambén puede explicar parte de la variabilidad interindividual en la respuesta a diferentes fármacos y asociarse al pronóstico de la enfermedad.
Cardiovascular disease is a complex, multifactorial, polygenic disorder with an important environment component. This group of diseases, especially ischemic heart disease, causes significant morbidity and mortality at the population level. Therefore, prevention is a key element of relevant public health policies. Over the past decade there has been a significant advance in knowledge of the genetic component of ischemic heart disease; more than 50 loci associated with risk for this disease have been discovered. Many of these loci are not associated with traditional cardiovascular risk factors, and the etiopathogenic mechanism that explains this association is unknown. Moreover, genetic variability may explain part of the interindividual variability in response to different drugs and may be associated with prognosis.

MIFO (a Method for Investigation of Polluted Areas) is a Swedish investigationtechnique for polluted areas which is produced by the Swedish environmentalprotection agency. The purpose with MIFO is to standardize and simplify theevaluation of the situation in the environment in terms of the extent of pollution andit’s hazards and dispersal rate. A MIFO – analysis should lay the groundwork forfurther investigations and actions. The analysis gives a priority system where themore contaminated areas are processed with higher priority than areas with lesspollution or risks.The purpose with this report is to comleate a MIFO – 2 – analysis of Gimowoodpreservation factory in Östhammar municipality, Sweden. Gimowoodpreservation factory was during the 30’s to 50’s contaminated by the woodpreservative CCA (Copper, Chromium, Arsenic)The area has since then been investigated several times and remediated once.Copper, chromium and arsenic are breafly described in the report in terms of theireffects on humans and the environment and their general behaviour in water andsoil.Arsenic is a dangerous substance and is considered poisonous for humans and theenvironment. Copper and chromium are not considered as to be as hazardous.The amounts of the three substances in the water around Gimo are considered amoderate and low risk. The amounts of arsenic are in some places in the soil areabove the guideline values. The risk of spreading of the pollution is however low, andtherefore the result of the MIFO – analysis of Gimo woodpreservation factory isevaluated as “3 – moderate risk”.
MIFO (Metodik för Inventering av Förorenade Områden) är en nationell inventeringsmetod för förorenad mark framställd av Naturvårdsverket. Syftet med metoden är att få en gemensam nationell metod att enkelt och enhetligt bedöma miljökvalitén i olika områden i Sverige med avseende på föroreningars omfattning, samt skade- och spridningsriskerna dessa medför. En MIFO-analys skall ligga som grund för fortsatta undersökningar och eventuella åtgärder inom ett förorenat område. MIFO leder till ett prioriteringssystem där de mer kontaminerade områdena åtgärdas med högre prioritet än områden med lägre föroreningshalt eller spridningsrisker. Syftet med denna rapport är att genomföra en MIFO – fas 2 analys över Gimo träimpregnering, Östhammar kommun. Gimo träimpregnering kontaminerades under 30-50-talet med impregneringsmedlet CCA, ett impregneringsmedel innehållande koppar, krom och arsenik. Området har sedan slutet av 80-talet undersökts i flera studier samt sanerats. Koppar krom och arsenik beskrivs kort i rapporten med avseende på dess skadeverkan för människa och miljö, samt dess generella beteende i mark och vatten. Arsenik är ett skadligt ämne och är mycket farligt för människor miljön. Koppar och krom räknas inte lika miljöskadliga. Halterna av de tre ämnena ligger inom måttliga nivåer i grundvattnet kring Gimo, men riktvärden för arsenik överskrids till viss mån i marken. Spridningsrisken är dock låg i området och detta medför att MIFO-analysen av Gimo träimpregnering, gav resultatet ”Riskklass 3, måttlig risk”.

Portugal possui uma enorme e vulnerável faixa costeira. Estas zonas têm vindo a ser afectadas por actividade humanas exercendo uma forte pressão sobre o meio marinho. As zonas costeiras estão também associadas a actividades socioeconómicas significativas, tornando-se desta forma zonas bastante sensíveis. Surge assim, como pressão adicional a estas zonas, as alterações climáticas. Estas estão ligadas à subida do nível do mar, a alterações da frequência e/ou da intensidade das tempestades e furacões associados às consequentes cheias. Os impactes das alterações climáticas nas zonas costeiras serão assim provavelmente agravados pelo facto de nestas se localizarem as principais áreas metropolitanas do país. Assim o potencial para afectar um número elevado de pessoas é, nesta faixa, particularmente alto, tornando-se também um risco acrescido para os ecossistemas, e para a saúde humana. Com a realização deste trabalho, pretende-se, verificar as medidas de adaptação existentes na estratégia de gestão integrada portuguesa, no âmbito das alterações climáticas, efectuando uma comparação entre as medidas existentes em algumas das estratégias existentes no espaço Europeu e propondo algumas medidas a aplicar na estratégia existente em Portugal. ABSTRACT: Portugal posses a huge and vulnerable coastline that has been affected by human activities, putting its sea life to a high stress. The coastal areas are also associated with significant socio-economic activities, thus making them very sensitive areas. Therefore and as an additional problem comes the climate changes.These are combined with the rising sea levels, changes in frequency and/or intensity of storms and hurricanes associated with the resulting flood. Thus, the impacts of climate change on coastal areas will be, probably, aggravated by the fact that these areas are located in the major metropolitan areas of the country. As a result, the probability to affect a large number of people is particularly high, making it also increase the risk for ecosystems as for human health. The purpose of this thesis is to verify the adaptation measures, relative to the climate changes, within IZCM’s strategy. This is achieved by making a comparison between the existing measures in Portugal and in other European zones, and also, proposing some measures to implement to the current national strategy.
