Abstract:
ABSTRACT: Gingivectomy Using Scalpel and Electrocautery in Gingival Enlargement Treatment of Puberty Female. Puberty Gingivitis is a gum inflamation due to certain conditions that are classified by etiologic factors and pathological changes. The increase in gingival size is a sign of gingival disorder. Gingivectomy is cutting the gingival tissue by removing the lateral wall of the pocket which aims to eliminate pockets and gingival inflammation thus obtaining physiologically, functionally and aesthetically good gingiva. In this case, 16-year old woman complained of the state of the upper and lower gums which were swollen since the last one year, easily bleeding and no pain. The handling of this case was designed to use the technique of gingivecto by using periodontal knives and gingivoplasty by gradually using electrocautery. Gingivectomy technique with a combination of scalpel and electrocautery in puberty gingivitis treatment gives satisfactory results in aesthetic and functional state in patients.

Abstract:
Smoking patients show reduction of inflammatory clinical signs that might be associated with local vasoconstriction and an increased gingival epithelial thickness. The purpose of this work was to evaluate the thickness of the marginal gingival oral epithelium in smokers and non-smokers, with clinically healthy gingivae or with gingivitis. Twenty biopsies were obtained from four different groups. Group I: non-smokers with clinically healthy gingivae (n = 5). Group II: non-smokers with gingivitis (n = 5). Group III: smokers with clinically healthy gingivae (n = 5). Group IV: smokers with gingivitis (n = 5). These biopsies were histologically processed, serially sectioned at 5 mm, stained with H. E., and examined by image analysis software (KS400), which was used to perform the morphometric evaluation and the quantification of the major epithelial thickness, the epithelial base thickness and the external and internal epithelial perimeters. Differences between the four groups were analyzed using ANOVA test and Tukey's test. The criteria for statistical significance were accepted at the probability level p < 0.05. A greater epithelial thickness was observed in smokers independent of the gingival health situation.

Abstract:
Background/Aim. Mast cells are mononuclear cells originating from bone marrow. They produce various biologically active substances, which allow them to actively participate in immune and inflammatory processes associated with periodontal disease. The study focused on distribution and density of mast cells in healthy gingiva as well as in different stages of periodontal disease. Methods. The material used for this purpose was gingival biopsies taken from 96 patients classified into 4 groups: healthy gingiva, gingivitis, initial and severe periodontal disease. Toluidine blue staining according to Spicer was utilized for identifying mast cells. Results. Basing on our study, the density of mast cells in the gingival tissue increases with the progression of the infection, which means they are more numerous in gingivitis compared to healthy gingiva, as well as in periodontal disease compared to gingivitis. Conclusion. Increase in the number of mast cells in the infected gingiva can be correlated with an increased influx of inflammatory cells from blood circulation into the gingival stroma, as well as with the collagen lysis, since these cells produce substances with collagenolytic potential. Based on the distribution of mast cells, it could be concluded that in the evolution of periodontal disease there are significant dynamic alterations in migration and localization of these cells.

Abstract:
Abstract Acute streptococcal gingivitis is an acute inflammation of the oral mucosa. Specific bacterial infections of the gingiva may be due to neisseria gonorrhea, treponema pallidum, streptococci, and other organisms. Streptococcal infections are seen rarely. This case report describes a patient who presented with severe gingival inflammation and pain that was diagnosed as an acute streptococcal infection. Bacterial cultures were obtained from the lesion, and biopsies were obtained from the gingiva of lower incisors for histopathologic evaluation. The patient was successfully treated using conventional periodontal therapy (scaling, root planning, curettage) and antibacterial agents. The reconstructive phase for this patient consisted of the fabrication of a heat-cured acrylic gingival facade to mask the gingival recession. The treatment of acute gingivostomatitis is of importance because of the possibility of systemic secondary infections. When esthetics is important, a gingival prostheses can be considered. The differential diagnosis, etiology, and treatment of acute streptococcal gingivitis are discussed and the literature is reviewed in this report. Abstract:
This randomized clinical study examined the influence of the gingival condition—healthy versus mild inflammation—on sulcus representation and possible gingival recession for two gingival displacement procedures prior to conventional impression making. The interventions double cord technique or a kaolin paste containing aluminum chloride were applied to 40 probands. The opposite quadrant served as intrapersonal reference (split-mouth design). Precision impressions were then made. Extraoral digitization of the plaster models resulting from the reference impression prior to gingival displacement, the intervention impression and control impressions were the basis for the computer-aided three-dimensional analysis. After six months, a mild artificial gingivitis was induced, and the contralateral quadrant (cross-over design) was examined for the intervention. The gingivitis deteriorated the sulcus representation for the double cord technique group but did not affect the paste technique group. The gingival condition had no influence on the marginal gingiva height changes. The minor extent of those changes, which were measured up to six months after intervention at the palatal study site, were not considered to be in the clinically relevant range for gingival recession. For healthy gingiva, the cord technique showed superior sulcus representation compared to the paste technique. This advantage was lost to a great extent under the conditions of mild gingivitis.

Abstract:
Abstract Hypoxia-inducible factor 1 alpha (HIF-1α) and vascular endothelial growth factor (VEGF) are proteins that stimulate the proliferation and migration of endothelial cells. These proteins have been described in many pathologic and inflammatory conditions, but their involvement in the development of periodontitis has not been thoroughly investigated. This study compared the immunohistochemical expression of these proteins, involved in angiogenesis and hypoxia, by imunnostained inflammatory and endothelial cells in periodontal disease and healthy gingival tissues. Gingival tissue samples were divided as follows: 30 samples with chronic periodontitis, 30 with chronic gingivitis, and 30 of healthy gingiva. Results were analyzed statistically by the Kruskal-Wallis, Mann-Whitney and Spearman correlation tests (p=0.01). Inflammatory and endothelial cells were found to express these proteins. Periodontitis showed median percentage of HIF-1α-positive cells of 39.6%, 22.0% in cases of gingivitis and 0.9% in the healthy gingiva group (p=0.001). For VEGF, median percentage of immunopositive cells was 68.7% for periodontitis, 66.1% in cases for gingivitis, and 19.2% for healthy gingival specimens (p<0.001). Significant correlation between VEGF and HIF-1α was also observed in healthy gingiva (p<0.001).The increased expression of HIF-1αα and VEGF in periodontitis, compared to gingivitis and healthy gingiva, suggests possible activation of the HIF-1α pathway in advanced periodontal disease. The correlation between HIF-1α and VEGF expression in healthy gingiva suggests a physiological function for these proteins in conditions of homeostasis. In periodontal disease, HIF-1 and VEGF expression may be regulated by other factors, in addition to hypoxia, such as bacterial endotoxins and inflammatory cytokines.

Abstract:

Gingivitis, caries, periodontitis, oral candidos, plaque and calculus are oral disorders that has been shown having a higher prevalence in children with diabetes in comparison to healthy children. The results in the studies varied to a certain extent due to different outcome measures and methods. Conclusion: Previous studies have shown a relationship between children and adolescents with type 1 diabetes and their impaired oral health, where plaque and gingivitis were most prevalent. More research is generally required on how diabetes type 1 affects the oral health of children and adolescents in general.

Abstract:
Objective: To evaluate in humans the efficacy of power driven interdental cleaning tools in addition to tooth-brushing compared to tooth-brushing alone or any non-power driven interdental cleaning tool in addition to tooth-brushing on dental plaque removal and prevention of gingivitis.   Introduction: Daily mechanical self-care disruption of dental plaque is considered important for oral health maintenance. Tooth-brushing, which is the most common method for removing dental plaque, has only a marginal effect on the interdental spaces between the teeth. Therefore, special cleaning tools are necessary for plaque removal in these areas. Dental floss has long been considered “the golden standard” of interdental cleaning and is often recommended by dental health professionals. Powered interdental tools are an alternative that in certain studies have proven to be efficient on plaque and bleeding.   Method: Two internet sources (PubMed and Cochrane Central) were searched for studies investigating the efficacy of powered interdental tools on the parameters of plaque, gingivitis and bleeding in comparison to tooth-brushing only or tooth-brushing and any non-powered interdental tool. The PRISMA 2009 protocol was adopted and studies included were analysed for results and homogeneity. Bias across studies was estimated according to a protocol by SBU.   Results: 14 articles were found relevant to the research question and analysed for data on plaque, gingivitis and bleeding. Studies were grouped according to type of device investigated. The groups consisted of oral irrigation with water on patients with natural teeth or undergoing orthodontic treatment plus a group on other interdental tools. The results show that power interdental tools have a significant ability to decrease plaque, gingivitis and bleeding.

Abstract:
Slutligen inkluderades 19 artiklar i studien.
The link between overweight/obesity and oral diseases in children and adolescents   Background: Overweight/obesity is a health problem that is increasing rapidly in Sweden and around the world. Overweight/obesity, dental caries and periodontitis are all chronic and multifactorial diseases. These diseases are common in both children and adolescents. The oral diseases are linked to unhealthy dietary habits that affect both oral- and general health, which in turn may affect the weight of children and adolescents. Aim: To study whether there is a possible link between overweight/obesity and oral diseases in children and adolescents. Me-thod: This work was a literature review. DOSS and PubMed were used for search of scientific articles. Total generated hits were 393 of which 50 articles were selected for fulltext review. Using a modified review the articles were then examined for probative value and analyzed to determine if the the articles are weak, moderate or strong. Finally, 19 articles were included in the study. Results: Socioeconomi, BMI in children and parents, level of education, ethnicity, hygiene, nutrition and health status was of great importance in the development of overweight/obesity and dental caries. Overweight/obesity had a direct impact on periodontal disease but because of the studies in this area being few the authors of this study believe that further research in this area is required. Conclusion: The majority of these 19 studies have shown a statistically significant association between overweight/obesity and oral diseases and the relationship was shown to be dependent on several factors.

Abstract:
AIM: The overall aim of this thesis was to study culture as an oral health determinant for dental caries and gingivitis in children living in Jönköping, Sweden, in relation to children living in Da Nang, Vietnam. MATERIALS AND METHODS: In 1993 and 2003, cross-sectional studies with clinical examinations and questionnaires were performed in Jönköping, Sweden, with a random sample of 130 children from each of four age groups; 3, 5, 10 and 15 years. The final study sample comprised 739 children, 154 (21%) with two foreign-born parents and 585 (79%) with two Swedish-born parents (Paper I). In 2000, all 15-year-olds (n=143) at one school in Jönköping, Sweden, were asked to participate in a questionnaire study connected to clinical data. The final sample comprised 117 individuals, 51 (44%) with foreign-born parents and 66 (56%) with Swedish-born parents (Paper II). In 2008, a cross-sectional study with clinical examinations and questionnaires was performed in Da Nang, Vietnam with 840 randomly selected children, 210 in each of four age groups; 3, 5, 10 and 15 years. The final sample comprised 745 individuals (Papers III and IV). RESULTS: In 2003, the mean number of decayed (initial and manifest) and filled tooth surfaces was significantly higher in all age groups in children with foreign-born parents compared with children with Swedish-born parents. The gap between children with foreign-born parents and Swedish-born parents increased over the ten-year period from 1993 to 2003. The odds ratio of dental caries development among 10- and 15-year-old children with foreign-born-parents was more than six times higher than for their counterparts with Swedish-born parents (Paper I). Fifteen-year-olds born in Sweden of foreignborn parents and those who had immigrated before one year of age had a caries prevalence similar to 15-year-olds with Swedish-born parents, whereas the caries prevalence in children who had immigrated to Sweden after 7 years of age was 2-3 times higher (Paper II). Among the 3- and 5-year-olds in Vietnam, 98% suffered from dental caries, compared with 91% of 10- and 15-year-olds (Paper IV). The distribution of the most frequent values of decayed and filled primary tooth surfaces (dfs) in 5-year-olds was 16–20, and of decayed and filled permanent tooth surfaces (DFS) in 15-year-olds was 1–5. The maximum dfs was 76–80, and significant numbers of children had dfs between 20 and 50. The percentage of tooth sites with plaque and gingivitis was higher for children in all age groups with foreign-born parents compared with children with Swedish-born parents, except among the 15-year-olds in 2003. In Vietnam, the prevalence of plaque and gingivitis was high in all age groups, especially in 10- and 15-year-olds. Fifteen-yearolds in Sweden with foreign-born parents had a higher intake of snack products between principal meals compared with 15-year-olds with Swedish-born parents (Paper II). In Sweden, most children in all age groups brushed their teeth themselves or with help from their parents twice or more than twice a day (Paper I). Among 3- and 5-year-olds in Vietnam, about half of the parents reported that their children brushed their teeth themselves or with help from parents twice or more than twice a day (Paper III). All 3-year-olds and 99% of 5-year-olds in Sweden brushed their teeth with fluoride toothpaste (Paper I). Among 15-year-olds in Sweden with foreign-born parents, 88% reported that they brushed their teeth with fluoride toothpaste at least twice a day compared with 98% of 15-year-olds with Swedish-born parents (Paper II). In Vietnam, 44–78% of the children used fluoride toothpaste for toothbrushing and 51% consumed sweets between principal meals at least once a day (Paper III). Sweetened milk was the most common source of this sugar intake for the 3- and 5-year-olds (Paper III). CONCLUSIONS: Culture is an important oral health determinant for dental caries and gingivitis in children. There is an urgent need to improve oral health care promotion and preventive programmes for children with foreign-born parents in Sweden, but also a great need for such programmes for children in Vietnam.

Abstract:
Gingivitis is the most common gingival inflammation in the oral cavity, and the most prevalent periodontal disease affecting 90% of the population in all age groups. Recently, a few research groups have investigated the possibility of using ultrasound in dentistry, particularly in diagnosing bony destruction in the more severe form of periodontal disease called periodontitis. This work investigates the feasibility of using ultrasound imaging to quantitatively assess gingival tissue inflammation. Signal and image processing of ultrasound data have been performed to quantitatively assess gingival tissue. A number of gingival scans were conducted in vitro to render ultrasound images of high-spatial and contrast resolutions. For each sample the B-mode images were matched with almost the same slices in histology. Results show that ultrasound scans for tissues with gingivitis exhibited low intensity of reflections (hypo echoic) at the inflamed tissues, while healthy dense epithelium layers exhibited higher reflections (hyper echoic). Histological diagnosis revealed good agreement with the ultrasound results indicating the usefulness of such ultrasound imaging in diagnosing gingivitis. In addition, a new design for an intraoral linear array ultrasound probe is demonstrated and utilized in our clinic in vivo. Analysis of the echogenicity patterns of the resultant images demonstrates the potential of using such a new probe in gingival health assessment, which would be feasible and clinical relevant for patient evaluations clinically.
