Abstract:

Introduktion: När bakteriellt plack får ligga ostört fyra till sju dagar reagerar gingivan med inflammation. Sjukdomstillståndet benämns gingivit. En kvinnas<img src="https://cdncache-a.akamaihd.net/items/it/img/arrow-10x10.png" /><img src="https://cdncache-a.akamaihd.net/items/it/img/arrow-10x10.png" /> graviditet präglas av en rad olika fysiologiska förändringar, den orala hälsan påverkas på olika sätt, kvinnan kan bland annat drabbas av graviditetsgingivit. Kunskapen om graviditetsgingivit är begränsad både bland blivande mödrar och hos olika professioner inom hälso-och sjukvård och tandvård. Syfte: Att beskriva begreppet graviditetsgingivit samt dess eventuella påverkan på det nyfödda barnet. Frågeställningar: - Vad har graviditetsgingivit för övergripande etiologi? - Vilken eventuell påverkan har moderns gingivit på det nyfödda barnet? Metod: Litteraturstudie. Resultat: Subgingivala bakterier, höga placknivåer i samband med en ökad närvaro av könshormonerna östrogen och progesteron samverkar så att en graviditetgingivit kan utvecklas hos gravida kvinnor. Resultatet påvisar även att för tidig födsel förekommer hos mödrar med gingivit och även att barn föds med låg födelsevikt. Konklusion: Den gravida kvinnans förändrade produktion<img src="https://cdncache-a.akamaihd.net/items/it/img/arrow-10x10.png" /><img src="https://cdncache-a.akamaihd.net/items/it/img/arrow-10x10.png" /> av könshormoner, förekomst av plack samt specifika bakterier har betydelse för utveckling av graviditetsgingivit. Kvinnor med graviditetsgingivit löper ökad risk att föda för tidigt och att barnet föds med låg födelsevikt. Det vetenskapliga underlaget om samband mellan graviditet och parodontit är rikligare än underlaget för sambandet med gingivit. Behov av sådan forskning finns.

Abstract:

Introduktion: Studier visar att gravida kvinnor löper större risk att drabbas av gingivit änicke-gravida kvinnor. Gingivit är den vanligaste formen av parodontal sjukdom vid engraviditet. Det finns bristande kunskaper hos olika vårdgivare om graviditet och kvinnorsorala hälsa och om dessa två saker kan påverka varandra. Syfte: Att beskriva orsaker till gingivit under graviditet samt hur tillståndet kan påverkadet ofödda barnets hälsa. Metod: Litteraturstudie Resultat: Studier visar att kvinnor med ett högt plackindex, subgingivala bakterier ochhöga nivåer av östrogen under en graviditets andra och tredje trimestrar var de som hadehögst procent av gingivit. Det finns ett samband mellan kvinnors parodontala hälsa ochprematur födsel samt låg födelsevikt. Gravida kvinnor med gingivit föder fler barn medlåg födelsevikt jämfört med gravida friska kvinnor. Konklusion: Plackförekomst, ökad ansamling av specifika bakterier och höga nivåer avkönshormoner har en väsentlig roll vid förekomsten av gingivit hos gravida kvinnor.Graviditetsgingivit kan möjligen innebära en ökad risk för prematur födsel och för att födabarn med låg födelsevikt. Behov av mer sådan forskning finns. Ökad kunskap om dettasamband rekommenderas till blivande mödrar och olika vårdgivare samt att informationoch munhygieninstruktioner bör ingå i mödrahälsovården

Abstract:

Sammanfattning Syftet med litteraturstudien var att undersöka förekomsten av gingivit hos gravida kvinnor.Metoden som användes var en allmän litteraturstudie där sökningen av vetenskapliga artiklargjordes i den medicinska databasen PubMed. Resultatet av litteraturstudien sammanställdesutifrån tolv vetenskapliga artiklar, där resultatet visade att under första trimestern (förstatredjedel av graviditetens nio månader) hade de gravida kvinnorna en gingivitförekomst på35,7%, under andra trimestern (andra tredjedel av graviditetens nio månader) vargingivitförekomsten 65,3%, och under den tredje trimestern (sista tredjedel av graviditetensnio månader) var gingivitförekomsten 73%. Slutsatsen i denna litteraturstudie var att gravidakvinnor löper en större risk att utveckla gingivit ju längre in i graviditeten de kommer.

Abstract:

Syftet med litteraturstudien var att undersöka tandtrådens effekt på plack och gingivit. Metoden som användes var en kvantitativ allmän litteraturstudie där sökningen av vetenskapliga artiklar gjordes i den medicinska databasen PubMed. Litteraturstudiens resultat sammanställdes utifrån elva vetenskapliga artiklar. Tandtrådens effekt på plack och gingivit utvärderades, i form av reduktion av: plackmängd och blödning hos såväl barn, ungdomar som vuxna individer. Tio av artiklarna visade att tandtråd hade en effekt på ovanstående parametrar medan den kvarstående artikeln inte kunde visa några sådana effekter. Slutsatsen är att användning av tandtråd i samband med tandborstning har en effekt med avseende på reduktion av plackmängd och gingivit. Ytterligare forskning krävs för en noggrann utvärdering av tandtrådens effekt.

Abstract:

Introduktion: Graviditet är det tillstånd då en kvinna bär på ett foster, vanligtvis i 9 månader. Då sker många förändringar i kroppen, både fysiska och hormonella. Parodontal hälsa ingår i den orala hälsan och påverkar både livskvalité och allmänna hälsan. Inom tandhygienistprofessionen ingår att motverka ohälsa i munnen genom kunskap baserad på vetenskaplig fakta. Syfte: Att undersöka hur förekomsten av gingivit och parodontit ter sig under graviditet, samt vilka faktorer som påverkar sjukdomarna under graviditet. Frågeställningar: Hur skiljer sig förekomsten av gingivit och förekomsten av fickdjup på 4 millimeter eller mer hos gravida kvinnor jämfört med kvinnor som inte är gravida? Vad finns det för faktorer som under en graviditet påverkar gingivitindex (GI) och den parodontala hälsan positivt, negativt eller som inte har någon påverkan? Metod: Litteraturstudie. Resultat: Resultatet visar att de parodontala sjukdomarna gingivit och parodontit är mer förekommande hos gravida kvinnor jämfört med icke gravida kvinnor. Faktorerna högt plackindex (PI), högt PI tillsammans med höga östrogennivåer, närvaro av specifika bakteriearter, högt body mass index (BMI), högt blodtryck, rökning och socioekonomisk status är de som påvisats ha samband med graviditet och parodontala sjukdomar. Konklusion: Den parodontala hälsan påverkas negativt under graviditet och det finns flera faktorer som påverkar sambandet.

Abstract:

Bakgrund: Övervikt/fetma är ett hälsoproblem som snabbt ökar i Sverige och hela världen. Övervikt/fetma, karies och parodontit är alla kroniska och multifaktoriella sjukdomar. Dessa sjukdomar är vanligt förekommande hos både barn och ungdomar. De orala sjukdomarna är förknippade med ohälsosamma kostvanor som påverkar både orala- och allmänna hälsan och som i sin tur kan påverka vikten hos barn och ungdomar. Syfte: Att studera om det finns ett eventuellt samband mellan övervikt/fetma och orala sjukdomar hos barn och ungdomar. Metod: Detta arbete var en litteraturöversikt. För sökning av vetenskapliga artiklar användes DOSS och PubMed. Totalt genererades 393 träffar varav 50 artiklar valdes ut till fulltextgranskning. Med hjälp av en modifierad granskningsmall granskades sedan artiklarnas bevisvärde och analyserades för att avgöra om graden på artiklarna var svag, måttlig eller stark. Slutligen inkluderades 19 artiklar i studien. Resultat: Socioekonomi, BMI hos barn och föräldrar, utbildningsnivå, etnicitet, hygien- kostvanor och hälsotillstånd var av stor betydelse till utvecklingen av övervikt/fetma samt karies. Övervikt/fetma hade en direkt inverkan på parodontit men då studierna inom detta område var få anser författarna till denna litteraturstudie att ytterligare forskning inom detta område krävs. Slutsats: Merparten av dessa 19 studier har visat ett statistiskt signifikant samband mellan övervikt/fetma och orala sjukdomar och detta samband visades vara beroende av flera faktorer. The link between overweight/obesity and oral diseases in children and adolescents Background: Overweight/obesity is a health problem that is increasing rapidly in Sweden and around the world. Overweight/obesity, dental caries and periodontitis are all chronic and multifactorial diseases. These diseases are common in both children and adolescents. The oral diseases are linked to unhealthy dietary habits that affect both oral- and general health, which in turn may affect the weight of children and adolescents. Aim: To study whether there is a possible link between overweight/obesity and oral diseases in children and adolescents. Me-thod: This work was a literature review. DOSS and PubMed were used for search of scientific articles. Total generated hits were 393 of which 50 articles were selected for fulltext review. Using a modified review the articles were then examined for probative value and analyzed to determine if the the articles are weak, moderate or strong. Finally, 19 articles were included in the study. Results: Socioeconomi, BMI in children and parents, level of education, ethnicity, hygiene, nutrition and health status was of great importance in the development of overweight/obesity and dental caries. Overweight/obesity had a direct impact on periodontal disease but because of the studies in this area being few the authors of this study believe that further research in this area is required. Conclusion: The majority of these 19 studies have shown a statistically significant association between overweight/obesity and oral diseases and the relationship was shown to be dependent on several factors.

Abstract:

Objective: To evaluate in humans the efficacy of power driven interdental cleaning tools in addition to tooth-brushing compared to tooth-brushing alone or any non-power driven interdental cleaning tool in addition to tooth-brushing on dental plaque removal and prevention of gingivitis. Introduction: Daily mechanical self-care disruption of dental plaque is considered important for oral health maintenance. Tooth-brushing, which is the most common method for removing dental plaque, has only a marginal effect on the interdental spaces between the teeth. Therefore, special cleaning tools are necessary for plaque removal in these areas. Dental floss has long been considered “the golden standard” of interdental cleaning and is often recommended by dental health professionals. Powered interdental tools are an alternative that in certain studies have proven to be efficient on plaque and bleeding. Method: Two internet sources (PubMed and Cochrane Central) were searched for studies investigating the efficacy of powered interdental tools on the parameters of plaque, gingivitis and bleeding in comparison to tooth-brushing only or tooth-brushing and any non-powered interdental tool. The PRISMA 2009 protocol was adopted and studies included were analysed for results and homogeneity. Bias across studies was estimated according to a protocol by SBU. Results: 14 articles were found relevant to the research question and analysed for data on plaque, gingivitis and bleeding. Studies were grouped according to type of device investigated. The groups consisted of oral irrigation with water on patients with natural teeth or undergoing orthodontic treatment plus a group on other interdental tools. The results show that power interdental tools have a significant ability to decrease plaque, gingivitis and bleeding. Syfte: Syftet med studien var att söka evidens för maskinella interdentala hjälpmedels effekt på plackborttagning, gingivit och blödning och om de har en tilläggseffekt utöver tandborstning jämfört med endast tandborstning eller tandborstning i kombination med manuella interdentala hjälpmedel på människor. Introduktion: Daglig plackborttagning är viktig för att uppnå god oral hälsa. Tandborstning är den vanligaste metoden för att ta bort plack men har endast en marginell åtkomst till det interdentala utrymmet mellan tänderna, därför behövs specielle hjälpmedel för plackborttagning i dessa utrymmen. Tandtråd har länge betraktats som ”en gyllene standard” för rengöring mellan tänderna och rekommenderas oftast av tandvården. Maskinella hjälpmedel är ett alternativ som i vissa studier har visat sig vara effektiva mot plack och blödning. Metod: Två databaser genomsöktes (PubMed och Cochrane Central) för studier av maskinella interdentala hjälpmedel och deras effektivitet på plack, gingivit och blödning i jämförelse med tandborstning, eller tandborstning med manuella interdentala hjälpmedel. Ett protokoll för systematiska litteraturstudier (PRISMA 2009) följdes och inkluderade studier bearbetades med avseende på resultat och risk för systematiska fel med hjälp av en mall från SBU. Resultat: Totalt 14 artiklar som var relevanta för syftet inkluderades i studien och data för plack, gingivit och blödning analyserades. Studierna grupperades i enlighet med vilken typ av hjälpmedel de utgjorde. Grupperna bestod av vattenflossers (oral irrigators) använda av patienter med egna tänder och/eller med fast tandställning samt en grupp övriga maskinella hjälpmedel. Resultatet pekar på att maskinella interdentala hjälpmedel har en signifikant effekt på plack, gingivit och blödning och kan rekommenderas till patienter som alternativ till manuella hjälpmedel.

Abstract:

Syfte: Syftet med litteraturstudien var att sammanställa evidens kring diabetes typ 1 och dess påverkan på barn och ungdomars orala hälsa. Metod: Studien var en allmän litteraturstudie. Inklusionskriterier var att de skulle ha publicerats mellan år 2008 till 2018, vara peer-reviewed granskade samt ha ett åldersspann på deltagare från 3 till 18 år. Efter en kvalitetsgranskning bedömdes 21 artiklar uppfylla kriterier till grund för studien. Artiklarna som användes har samtliga studerat diabetes typ 1 och dess påverkan på barn och ungdomars orala hälsa. Resultat: Resultatet påvisade att sjukdomen diabetes typ 1 har en påverkan på barn och ungdomars orala hälsa. Gingivit, karies, parodontit, oral candidos, plack samt tandsten är orala besvär som har visat sig ha en högre prevalens hos barn med diabetes i jämförelse med friska barn, enligt tidigare evidens. Resultatet i studierna varierade i en viss utsträckning beroende på olika utfallsmått och metoder. Slutsats: En sammanställning av studier visade ett samband mellan barn och ungdomar med diabetes typ 1 och en försämrad oral hälsa, där plack och gingivitförekomsten var som högst. Mer forskning generellt krävs kring hur diabetes typ 1 påverkar den orala hälsan hos barn och ungdomar, i syfte om att få en heltäckande bild. Purpose: The purpose of the literature overview was to study evidence for type 1 diabetes impact on children and adolescents oral health. Method: The study was a general literature study. The inclusion criteria were that they should have been published between 2008 until 2018, peer-reviewed and have an age range of children up to 18 years. After a quality review, 21 articles were used in this study. The articles that was used, have all studied diabetes type 1 and its impact on children and adolescents oral health. Result: The result showed that the disease type 1 diabetes has an effect on the oral health of children and adolescents. Gingivitis, caries, periodontitis, oral candidos, plaque and calculus are oral disorders that has been shown having a higher prevalence in children with diabetes in comparison to healthy children. The results in the studies varied to a certain extent due to different outcome measures and methods. Conclusion: Previous studies have shown a relationship between children and adolescents with type 1 diabetes and their impaired oral health, where plaque and gingivitis were most prevalent. More research is generally required on how diabetes type 1 affects the oral health of children and adolescents in general.

Abstract:

The aim of this study was to investigate the knowledge of caries and gingivitis among third to fifth grade school children. A questionnaire was distributed to 117 children of a primary school in the south of Sweden. The result of the study indicate that the children had good knowledge regarding development and prevention of caries and gingivitis. However, the children did not know the meaning of the words caries and gingivitis. The children also appeared to lack important information about dietary measures to achieve good dental health.

Abstract:

AIM: The overall aim of this thesis was to study culture as an oral health determinant for dental caries and gingivitis in children living in Jönköping, Sweden, in relation to children living in Da Nang, Vietnam. MATERIALS AND METHODS: In 1993 and 2003, cross-sectional studies with clinical examinations and questionnaires were performed in Jönköping, Sweden, with a random sample of 130 children from each of four age groups; 3, 5, 10 and 15 years. The final study sample comprised 739 children, 154 (21%) with two foreign-born parents and 585 (79%) with two Swedish-born parents (Paper I). In 2000, all 15-year-olds (n=143) at one school in Jönköping, Sweden, were asked to participate in a questionnaire study connected to clinical data. The final sample comprised 117 individuals, 51 (44%) with foreign-born parents and 66 (56%) with Swedish-born parents (Paper II). In 2008, a cross-sectional study with clinical examinations and questionnaires was performed in Da Nang, Vietnam with 840 randomly selected children, 210 in each of four age groups; 3, 5, 10 and 15 years. The final sample comprised 745 individuals (Papers III and IV). RESULTS: In 2003, the mean number of decayed (initial and manifest) and filled tooth surfaces was significantly higher in all age groups in children with foreign-born parents compared with children with Swedish-born parents. The gap between children with foreign-born parents and Swedish-born parents increased over the ten-year period from 1993 to 2003. The odds ratio of dental caries development among 10- and 15-year-old children with foreign-born-parents was more than six times higher than for their counterparts with Swedish-born parents (Paper I). Fifteen-year-olds born in Sweden of foreignborn parents and those who had immigrated before one year of age had a caries prevalence similar to 15-year-olds with Swedish-born parents, whereas the caries prevalence in children who had immigrated to Sweden after 7 years of age was 2-3 times higher (Paper II). Among the 3- and 5-year-olds in Vietnam, 98% suffered from dental caries, compared with 91% of 10- and 15-year-olds (Paper IV). The distribution of the most frequent values of decayed and filled primary tooth surfaces (dfs) in 5-year-olds was 16–20, and of decayed and filled permanent tooth surfaces (DFS) in 15-year-olds was 1–5. The maximum dfs was 76–80, and significant numbers of children had dfs between 20 and 50. The percentage of tooth sites with plaque and gingivitis was higher for children in all age groups with foreign-born parents compared with children with Swedish-born parents, except among the 15-year-olds in 2003. In Vietnam, the prevalence of plaque and gingivitis was high in all age groups, especially in 10- and 15-year-olds. Fifteen-yearolds in Sweden with foreign-born parents had a higher intake of snack products between principal meals compared with 15-year-olds with Swedish-born parents (Paper II). In Sweden, most children in all age groups brushed their teeth themselves or with help from their parents twice or more than twice a day (Paper I). Among 3- and 5-year-olds in Vietnam, about half of the parents reported that their children brushed their teeth themselves or with help from parents twice or more than twice a day (Paper III). All 3-year-olds and 99% of 5-year-olds in Sweden brushed their teeth with fluoride toothpaste (Paper I). Among 15-year-olds in Sweden with foreign-born parents, 88% reported that they brushed their teeth with fluoride toothpaste at least twice a day compared with 98% of 15-year-olds with Swedish-born parents (Paper II). In Vietnam, 44–78% of the children used fluoride toothpaste for toothbrushing and 51% consumed sweets between principal meals at least once a day (Paper III). Sweetened milk was the most common source of this sugar intake for the 3- and 5-year-olds (Paper III). CONCLUSIONS: Culture is an important oral health determinant for dental caries and gingivitis in children. There is an urgent need to improve oral health care promotion and preventive programmes for children with foreign-born parents in Sweden, but also a great need for such programmes for children in Vietnam. SYFTE: Det övergripande syftet med denna avhandling var att studera kultur som en oral bestämningsfaktor för karies och gingivit hos barn boende i Jönköping, Sverige, i relation till barn boende i Da Nang, Vietnam. MATERIAL OCH METOD: Under 1993 och 2003 utfördes två epidemiologiska tvärsnittsstudier i Jönköping med ett slumpmässigt urval av individer i åldersgrupperna 3, 5, 10 och 15 år. Samtliga individers orala hälsa undersöktes kliniskt och röntgenologiskt. Barnen, eller deras föräldrar, svarade även på ett frågeformulär avseende sociodemografi, attityder till och kunskaper om tänder samt munhälsovanor. Totalt omfattade studien 739 barn och ungdomar, 154 med två utrikes födda föräldrar och 585 med två svenskfödda föräldrar (Paper I). I en annan studie inbjöds 143 15-åringar på en kommunal skola i Jönköping att delta (Paper II). Av dessa deltog 117 personer; 51 med två utrikes födda föräldrar och 66 med två svenskfödda föräldrar. Alla ungdomar intervjuades med hjälp av ett strukturerat frågeformulär där kostfrågor förtydligades med visualisering, dvs exempel på förpackningar och varor. Information om karies (DFS) samlades in från journaler från Folktandvården, Landstinget i Jönköpings län. Under 2008 genomfördes en epidemiologisk tvärsnittsstudie i Da Nang, Vietnam med 840 slumpmässigt utvalda barn, 210 individer i åldersgrupperna 3, 5, 10 och 15 år, där det slutliga urvalet omfattade 745 individer. Samtliga individers orala hälsa undersöktes kliniskt. Barnen, eller deras föräldrar, svarade även på ett frågeformulär avseende sociodemografi, attityder till och kunskaper om tänder samt munhälsovanor (Papers III och IV) RESULTAT: År 2003 var antalet karierade och fyllda tandytor (dfs/DFS) statistiskt signifikant högre i alla åldersgrupper hos svenska barn med två utrikes födda föräldrar jämfört med barn med två svenskfödda föräldrar och klyftan mellan barn med utländsk bakgrund och barn med svensk bakgrund ökade under tioårsperioden 1993-2003. Risken för att utsättas för karies bland 10- och 15-åringar med utländsk bakgrund var mer än sex gånger högre jämfört med 15-åringar med svensk bakgrund (Paper I). Femtonåringar födda i Sverige med två utrikes födda föräldrar som hade kommit till Sverige före ett års ålder, hade en kariesprevalens som liknade den hos 15-åringar med svenskfödda föräldrar, medan barn som hade invandrat till Sverige efter sju år ålder hade en 2-3 gånger högre kariesprevalens (Paper II). Bland 3- och 5-åringar i Vietnam hade 98 % karies, hos 10- och 15-åringar var motsvarande siffra 91 % (Paper IV). De mest frekventa värdena av antalet kariesskadade och fyllda tandytor på primära tänder hos barn i 5-års-ålder var 16-20, och de mest frekventa värdena av kariesskadade och fyllda tandytor på permanenta tänder hos barn i åldern 15 år var 1-5. De maximala värdena av kariesskadade och fyllda primära tandytor var 76-80, och det fanns ett stort antal barn med mellan 20 och 50 skadade och fyllda primära tandytor. Procentandelen tandytor med plack och gingivit var numeriskt högre i alla åldersgrupper i Sverige hos barn med utländsk bakgrund jämfört med barn med svensk bakgrund, utom mellan 15-åringar år 2003. I Vietnam var förekomsten av plack och gingivit mycket hög i alla åldersgrupper och i synnerhet hos 10- och 15-åringar. I Sverige borstade de flesta av barnen i alla åldersgrupper sina tänder själva eller med hjälp av sina föräldrar två eller mer än två gånger per dag (Paper I). Bland 3- och 5-åringar i Vietnam rapporterade ungefär hälften av föräldrarna att barnen borstade tänderna själva eller med hjälp av föräldrar två eller mer än två gånger per dag (Paper III). Alla 3-åringar och 99 % av 5-åringar i Sverige borstade tänderna med fluortandkräm (Paper I). Bland 15-åringar med utländsk bakgrund uppgav 88 % att de borstade tänderna med fluortandkräm minst två gånger om dagen jämfört med 98 % av 15-åringarna med svensk bakgrund (Paper II). I Vietnam borstade 44 %-78 % av barnen tänderna med fluortandkräm (Paper III). Femtonåringar i Sverige med utländsk bakgrund hade ett högre intag av sötsaker mellan huvudmåltider jämfört med ungdomar med svensk bakgrund (Paper II). I Vietnam konsumerade 51 % av barnen sötsaker mellan huvudmåltiderna minst en gång om dagen. Sötad mjölk var den vanligaste källan till detta sockerintag för 3- och 5-åringar (Paper III). SLUTSATSER: Kultur är en viktig bestämningsfaktor för karies och gingivit hos barn. Det finns ett överhängande behov av att förbättra promotion och preventionsprogram avseende oral hälsa bland barn i Sverige med utrikes födda föräldrar men också ett stort behov för barnen i Vietnam.

Background: Traditional oral health education is seldom enough in order for dental care personnel to motivate their patients to good, long-lasting oral health care habits and there is a need for evaluation of other more efficient methods. One of these methods is motivational interviewing. An overview of its origin, definition, theoretical background and practical methods is given by way of introduction.Aim: The aim of the investigation was to review studies that had used motivational interviewing in order to obtain oral health, focusing on prevention and treatment of gingivitis, periodontitis and dental caries.Method: Systematic review. A search question was run through the databases ASSIA, ERIC, PsycINFO, Social Services Abstracts, Sociological Abstracts and PubMed. The articles were reviewed based on defined inclusion and exclusion criteria.Results: Five publications (based on three studies) met the inclusion criteria. Four of these (two of the studies) reported better outcomes for the intervention than the comparative method, both in short and long term. The fifth article had overall no clinically significant differences to report between intervention and control groups. As a result of the small amount of studies in addition to some methodological questions, no evidence is found regarding the use of motivational interviewing in dental care.Conclusion: There are too few studies of motivational interviewing within the area of dental care in order for any conclusions to be drawn and more studies are needed, preferably in a setting as close to the clinical everyday life as possible and with both objective and subjective outcome measures. The follow up period should be as extended as possible, and studies investigating the cost efficacy of the method are also required.

Abstract:

Syftet med denna litteraturstudie var att undersöka vilka effekter probiotika kan ha vid behandling av parodontala sjukdomar. Metoden som användes var en kvantitativ allmän litteraturstudie. Sökningar till litteraturstudien gjordes i databasen PubMed under mars månad 2015. Litteraturstudiens resultat sammanställdes utifrån åtta vetenskapliga artiklar. Litteraturstudien visar att behandling av gingivit med hjälp av mekanisk parodontal behandling i kombination med probiotika hade positiva effekter i form av minskning av inflammatoriska mediatorer, reduktion av mängd gingival vätska och reduktion av blödningsindex. Vid kronisk parodontit visade behandling med probiotika i kombination med mekanisk behandling positiva effekter på reduktion av plackindex, minskning av blödningsindex och fickdjupreduktion. Probiotika i kombination med parodontal behandling skulle kunna ge goda behandlingsresultat. Kunskapen om effekten av probiotika som tillägg till mekanisk parodontal behandling är allt för begränsad och delvis motstridig för att kunna ge patienterna någon behandlingsrekommendation. Det krävs mer forskning.

Bakgrund: Gingivit kännetecknas av svullet/lättblödande och ibland ömt tandkött. Gingivit är en inflammatoriskt orsakad reaktion som uppkommer i samband med närvaro av bakterier. För att motverka utvecklingen av gingivit rekommenderas munhygienvård regelbundet. Gingivit kan utvecklas till parodontit med skador på upphängningsapparaten och benstödet, orsaken till detta är idag dock ej helt klarlagd. Kliniska variabler (blödning vid sondering) är vedertagna diagnostiska metoderna för diagnostisk av gingivit idag. I pilotstudier har laktoferrinkoncentrationer setts vara förhöjda i stimulerad helsaliv och gingivalexsudat hos patienter med kronisk parodontit jämfört med patienter utan parodontit. Mål: Undersöka hur koncentrationen laktoferrin i ostimulerad helsaliv förändrades hos individer som utvecklade gingivit i en experimentell gingivitstudie och om C-reaktivt protein är en tillförlitlig markör för gingival inflammationsgrad.Material och metod: En experimentell gingivitstudie genomfördes med 13 försöksdeltagare där munhygienvanor kontrollerades och förändrades under 3 perioder á 2 veckor. Helsalivprov och mätning av CRP genomfördes. Resultat: Gingivalindex och CRP höjdes signifikant under försöksperioden Dag 0 till Dag 13(p=0,002 och p=0,027). Laktoferrin sågs först signifikant förhöjd under mitten av avslutningsperioden (Dag 20) (p=0,015). Laktoferrinkoncentrationen återgick inte till baselinevärdet under studien. Signifikant korrelation sågs mellan gingivalindex och laktoferrin från Dag 0 fram till Dag 13 (r=0,224, p=0,042). Utvecklingen av CRP sågs inte korrelera med någon variabel under försöket. Konklusion: Resultaten indikerar att utvecklingen av experimentell gingivit påverkar laktoferrinkoncentrationen över tid och kan därför vara en potentiell biomarkör för gingivit. De låga skillnader hos CRP visar att den inte är en tillförlitlig markör vid gingivit. Background: Gingivitis is characterized by swollen and bleeding tissue, an inflammatory reaction with presence of pathological bacteria. To counteract the development of gingivitis, regular oral hygiene care is recommended. Gingivitis can progress into periodontitis which damages the periodontal tissue and bone. Clinical variables (bleeding on probing, plaque occurrence) are diagnostic methods for gingivitis. Elevated concentrations of lactoferrin in stimulated whole saliva and gingival crevicular fluid has been seen in patients with chronic periodontitis compared to patients without periodontitis.Objectives: Investigate the salivary concentration of lactoferrin in subjects enrolled in the experimental gingivitis model, the response of capillary CRP was also investigated.Methods: Thirteen individuals participated in a 14-day experimental gingivitis model and 10-day gingival healing period, measurements of whole saliva lactoferrin and CRP was performed. Results: Gingival index and CRP was increased statistical significant during the induction of gingivitis (p=0.002 and p=0,027). Lactoferrin increased during the experimental phase and reached a peak at day 20, which was statistical significant compared to baseline (p=0.015). A statistical significant correlation between gingival index and lactoferrin was seen from day 0 to day 13 (r=0.224, p=0.042). CRP did not match any correlation to any other measured variable from day 0 to day 24.Conclusion: These results indicate that gingivitis may have a systemic effect, which in this study has been expressed as increased levels of salivary lactoferrin. Lactoferrin may be a potential biomarker for gingivitis. Due to low changes in concentrations and because CRP is systemically affected it makes an unreliable biomarker for gingivitis.

Abstract:

The aim of this study was to describe the prevalence of gingivitis, periodontitis and dental caries in individuals with type 2 – diabetes. The study was conducted in a form of a general literature review with the restricted to the following conditions: human studies, English, and articles published between 2007 - 2012. Articles lacking a control group were excluded. The data were collected through searches of the medical database PubMed. A compilation of 11 scientific articles was examined. In individuals with diabetes mellitus type 2, the presence of gingivitis was identified in 3 articles, periodontitis in 10, and dental caries in 3 articles. The 3 articles that showed presence of gingivitis demonstrated that individuals with type 2 – diabetes had a higher prevalence of gingivitis compared to non-diabetic individuals. A higher prevalence of periodontitis in individuals with type 2 – diabetes mellitus was found in 10/10 publications. Theses studies demonstrated in individuals with type 2 – a significantly higher incidence of deep pocket depths, attachment loss, recession defects and with higher plaque and gingival index scores. The three articles that showed the presence of dental caries also demonstrated a higher prevalence of dental caries in individuals with type 2 – diabetes mellitus. The articles included in the literature study showed statistically significant higher prevalence of gingivitis, periodontitis, and dental caries in individuals with type 2 – diabetes mellitus than in individuals without a diagnosis of diabetes. Introduktion: På grund av extremt hög fattigdom har de orala hälsofrågorna haft låg prioritet i Tanzania. Vid en konferens 2004 i Nairobi om den orala hälsan i Afrika ställde sig Tanzania, tillsammans med andra Afrikanska länder, bakom det gemensamma beslutet att hållbara nationella program måste utvecklas för att främja oral hälsa. Detta beslut har varit en drivkraft till att delta i och utveckla nedanstående projekt avseende munhälsofrämjande insatser. Endast ett fåtal studier finns redovisade vad gäller prevalensen av karies, gingivit och dental fluoros bland barn i Tanzania.Syfte: Syftet var att kartlägga antal tänder samt förekomst av karies, gingivit, dental fluoros samt munhygienvanor bland barn i Nkinga English Medium School och Nkinga Public School i Nkinga, Tanzania.Metod: Studien är en epidemiologisk tvärsnittstudie och bestod av klinisk undersökning av 244 skolbarn på 2 olika skolor. Undersökningen avslutades med tre muntliga frågor om munhygienvanor. Resultat: I medeltal hade eleverna 1,1 karierade tandytor (ds/DS) och 62,3 % av eleverna var kariesfria. Medeltalet tandytor med gingivit uppgick till 46,2 % och för plack till 51,5 %. Åttiofem procent av eleverna hade ingen eller mildare grad av dental fluoros. Förekomsten av svårare grad av dental fluoros var 15 %. Nio av tio elever borstade tänderna och av dessa använde 74 % tandkräm.Slutsats: Tandhälsan hos skolbarnen i Nkinga var ur kariologisk synpunkt allmänt god medan förekomsten av gingivit och plack var relativt hög. Studien visade att en bättre förebyggande tandvård behövs i Nkinga för att förbättra tandhälsan och egenvårdsvanorna hos skolbarnen.

Bakgrund: Individer med utländsk bakgrund tenderar ha sämre oral hälsa än svenskfödda individer. Detta kan bland annat bero på socioekonomiska faktorer såsom utbildningsnivå och kön. Invandringen till Sverige ökar och därför är det viktigt att studera nyanländas munhygienvanor samt kunskap om orala sjukdomar som karies, gingivit och parodontit för att kunna bemöta dessa individer utifrån deras behov. Syfte: Syftet med studien var att studera munhygienvanor samt kunskap gällande orala sjukdomar hos vuxna nyanlända med utländsk bakgrund. Metod: Studien är en kvantitativ tvärsnittsstudie med enkätundersökning som datainsamlingsmetod på SFI-skolor i Jönköpings kommun. Totalt 99 individer deltog i studien. Chi-två tester gjordes för jämförelse mellan grupper indelade efter utbildningsnivå. Resultat: Totalt 88,9% respektive 81,8% svarade att bakterier är en bakomliggande orsak till karies och parodontit. En statistisk signifikant skillnad visades vid jämförelse mellan högre och lägre utbildningsnivå avseende rökning som orsak till gingivit. Majoriteten av deltagarna borstade tänderna 2ggr/dagligen med fluortandkräm och 14,1% använde sig av approximalahjälpmedel 1gg/dag. Slutsats: Studiens resultat visade att deltagarna hade kunskap om orsaksfaktorer avseende karies och parodontit men bristande kunskap avseende gingivit. Deltagarna hade även goda tandborstningsvanor men bristande användning av approximalahjälpmedel och fluorpreparat. Background: Individuals with foreign backgrounds tend to have poorer oral health than Swedishborn. This may be depending on socioeconomic factors such as education and gender. Immigration to Sweden is increasing, and therefore it is important to study oral hygiene habits and knowledge of oral diseases among newly arrived immigrants. Aim: The aim was to explore oral hygiene habits and knowledge about oral diseases of newly arrived immigrants. Method: A quantitative cross-sectional with questionnaire survey at Swedish For Immigrants’ schools in Jönköping. A total of 99 people participated in the study. Chi-two tests were made for comparison between groups by education level. Results: Totally 88.9% and 81.8% replied that bacteria are an underlying cause of caries and periodontitis. A statistically significant difference was shown between higher and lower levels of education regarding smoking as a cause of gingivitis. The majority of participants brushed their teeth 2/daily with fluoride toothpaste and 14.1% were using auxiliary aids 1/day. Conclusion: The results of the study showed that participants had knowledge about causal factors regarding caries and periodontitis, but lack of knowledge about gingivitis. The participants also had good tooth brushing habits but lacked usage of use of interproximal aids and fluoridated agents.

Aim:The aim of the thesis was to study the effect of a self-care education in oral health for a group of dental nurse students in Danang, Vietnam. Methods: A quantitative intervention study was performed consisting of 53 selected participants, who were clinically examined to measure the dental biofilm and gingival inflammation. The intervention was a self-care education session where both information and instruction about the materials and methods of self-care in toothbrush technique and interdental cleaning were included. Results: The self-care education in oral health proved to have an effect on the students' oral hygiene. There was a statistically significant difference between the first and the second examination of the gingival inflammation. No statistically significant difference was found between the first and second examination of dental plaque but plaque score of buccal, lingual and distal surfaces significantly decreased before the second examination.The results did not show any significant difference between the group from the rural area and the group from the urban area. Conclusion:The result of the study shows that the self-care education in oral health had a positive impact on the participant´s oral hygiene. It would be beneficial to develop a self-care educational program for the population in Vietnam to improve the oral hygiene.Through knowledge and information about good self-care habits and its positive effects, more people can achieve a better and healthier oral health as a result. Syfte:Syftet med studien var att studera effekten av en given egenvårdsutbildning i oral hälsa för en grupp dental nurse studenter i Danang, Vietnam. Metod:En kvantitativ interventionsstudie utfördes, bestående av 53 utvalda deltagare som undersöktes kliniskt för att mäta dental biofilm och gingival inflammation. Interventionen var en egenvårdsutbildning där information och instruktion i material och metoder för egenvård i tandborstteknik och approximal rengöring inkluderades. Resultat:Egenvårdsutbildningen i oral hälsa visade sig ha effekt på studenternas munhygien. Det var en statistiskt signifikant skillnad mellan den första och den andra undersökningen av gingival inflammation. Ingen statistiskt signifikant skillnad uppnåddes mellan den första och andra undersökningen av dentalt plack. Dock sjönk plackförekomsten på de buccala, linguala och distala ytorna avsevärt innan den andra undersökningen. Resultaten visade inte någon signifikant skillnad mellan gruppen från landsbygd eller gruppen från tätort. Slutsats:Resultatet av studien visar att egenvårdsutbildningen haft en positiv inverkan på deltagarens munhygien. Det skulle vara fördelaktigt att utveckla ett egenvårdsprogram för befolkningen i Vietnam för att allmänt förbättra den orala hygienen. Genom kunskap och information om goda egenvårdsvanor och dess positiva effekter kan fler människor uppnå en bättre och hälsosammare oral hälsa.

Introduction. Le contexte de ce travail est le développement d’un capteur visant à détecter des protéines pro-inflammatoires (interleukines : IL-6, IL-8) dans le fluide gingival après pose d’une prothèse unitaire fixée. La détection de ces protéines en faible concentration sera assurée par des phénomènes physiques. Or, ces protéines ne se fixent pas directement sur la surface du capteur et il devient nécessaire de fonctionnaliser la surface. Ce travail préliminaire porte sur la validation du protocole de fonctionnalisation de surface. Matériels et méthodes : Plusieurs stratégies de fonctionnalisation ont été explorées les unes après les autres. Dans un premier temps, l’utilisation des nanobâtonnets d’or ou GNRs utilisés comme capteur sans marquage. L’interaction entre les protéines modèles et les GNRs ont été caractérisé par UV-vis. Dans un second temps, le protocole de fonctionnalisation consiste à accrocher directement les anticorps monoclonaux sur des surfaces d’or ou par l’intermédiaire d’une monocouche auto-assemblée simple (PEG ou DSP) ou mixte (MAU /MOH). Ce protocole a été caractérisé par QCM, SPR et PM-IRRAS et RAMAN.Résultats et conclusions : Les résultats trouvés avec les GNRs sont encourageants et méritent d’être approfondis d’une part et d’autre part les travaux de fonctionnalisation sont plus concluants. Ils ont permis de valider un protocole de fonctionnalisation d’une surface or par une monocouche auto-assemblé mixte couplé au greffage d’anticorps. Ce procédé est plus efficace car la sélectivité et la sensibilité du capteur est plus importante. Ce biocapteur une fois conçu, permet un champ d’application très large en recherche fondamentale et clinique et peut être un nouvel outil diagnostique et pronostique au cabinet dentaire. Os pacientes deveriam ter um ou os dois pré-molares inferiores com recessões classe II de Miller de um ou ambos os lados. Eles foram divididos aleatoriamente em um dos grupos de acordo com sua chegada: Grupo 1: Preparo do leito receptor e sutura do EGL apical a um colar de MG mantida ao redor do dente. Grupo 2: Preparo do leito receptor removendo-se toda a MG e posicionando o EGL sem recobrir a porção exposta da raiz. Grupo 3: Preparo do leito receptor semelhante ao do Grupo 2 e posicionamento do EGL no nível da JCE. Foram feitas medições pré e pós-operatórias aos 30, 90, 180 e 360 dias da altura da RG (HR), largura da RG (LR), quantidade de gengiva queratinizada (GQ), profundidade de sondagem (PS), gengiva inserida (GI) e nível de inserção clínica (NIC). O recobrimento inicial (RI) e o CA foram calculados com as medidas obtidas. Os dados foram submetidos à análise estatística. Aos 360 dias pósoperatórios, a porcentagem de recobrimento dos Grupos 1, 2 e 3 foi de 4,12%; 39,58% e 30,99% respectivamente. Não houve diferença estatística de RI entre os grupos. A quantidade de CA, aos 90, 180 e 360 dias pós-operatórios, não foi estatisticamente significante entre os grupos. Os Grupos 2 e 3 tiveram redução significativa no nível de inserção clínica aos 360 dias pós-operatórios. Trabajo de Investigación Requisito para optar al Título de Cirujano Dentista Introducción: Las enfermedades gingivales son una condición prevalente en la población. Los métodos actuales de vigilancia de enfermedades periodontales se basan en exámenes clínicos, los cuales requieren numerosos recursos para su implementación. Como alternativa se plantea el auto-reporte, opción de menor costo para estudios poblacionales, pero poco estudiado en población infantil. El objetivo de este estudio fue evaluar la relación entre sangrado gingival autoreportado, la condición clínica de salud gingival y su relación con la presencia de placa bacteriana en escolares de 6-12 años asistentes a colegios municipalizados. Material y métodos: Se examinaron 207 escolares de 6-12 años de edad asistentes a dos colegios municipalizados de la Región Metropolitana. Se registró Índice gingival de Löe y Silness simplificado (IG), Índice de higiene oral simplificado (IHO-S) y Examen Periodontal Básico simplificado (EPB), junto con una pregunta de auto-reporte sobre sangrado gingival durante el cepillado. Se determinó presencia de gingivitis mediante dos definiciones de caso (PG1=IG promedio≥0.5 y PG2=EPB>0) al igual que su severidad (SG1=según IG promedio y SG2=según índice de sangrado gingival). Se utilizaron pruebas de diferencia entre grupos, según edad y género, para los índices periodontales y test chi2 para determinar asociación entre presencia de gingivitis, severidad de gingivitis y condición de higiene oral respecto al auto-reporte. Se estimó sensibilidad (SS) y especificidad (ES) del auto-reporte, considerando buena validez si SS+ES≥ 120%. Resultados: La prevalencia de gingivitis fue PG1=98.6% y PG2=90.8%, registrándose severidades leve y moderada (SG1=54.6% y 43.9%; SG2=38.2% y 3.4%, respectivamente). El 60.9% presentó regular higiene oral. El 50.7% de los escolares con gingivitis auto-reportó sangrado a veces y el 10.6% reportó siempre, sin asociación según la condición de higiene oral. La definición PG2 fue más prevalente en quienes auto-reportaron sangrado gingival. Los mayores valores de SS+ES se obtuvieron para PG2 (127.0%) y SG2 moderada (128.7%). Conclusiones: El auto-reporte tendría capacidad para tamizar población enferma, especialmente con mayor severidad de gingivitis, pudiendo ser una prometedora herramienta para utilizar en vigilancia poblacional de gingivitis en población infantil y como método de detección precoz para instaurar medidas preventivas. Abstract:

IL-34 is a protein associated with bone degenerative diseases but the role in periodontal disease is unknown. The aim of this study was to assess the expression of IL-34 in primary human gingival fibroblasts (GF) and investigate if the expression is regulated by the pro-inflammatory cytokines interleukin-1 (IL-1β) and tumor necrosis factor α(TNF-α). We also investigated if IL-34 is detectible in gingival crevicular fluid (GCF) in healthy, gingivitis and periodontitis sites. Furthermore, we examined if healthy and inflamed gingival tissue contains IL-34. GF were stimulated by IL-1β300 pg/ml and TNF-α10 ng/ml. IL-34 mRNA was measured by quantitative real-time PCR (qPCR). GCF was collected from 11 healthy, 10 gingivitis, and 21 periodontitis gingival crevices. IL-34 protein was quantified using enzyme-linked immunoabsorbent assays (ELISA). Healthy and inflamed gingival tissue biopsies were collected and examined using immunohistochemistry (IHC). IL-34 mRNA was expressed in GF and the expression was enhanced 12x fold-change versus control by TNF-α10 ng/ml and 4x fold-change versus control by IL-1β300 pg/ml. IL-34 was also present in GCF but no significant difference in IL-34 protein was detected between the healthy, gingivitis, and periodontitis groups. Healthy and inflamed gingival tissue showed equal amounts of IL-34 protein in the epithelium while sub-epithelially the inflamed tissue showed higher levels of IL-34 protein. Pro-inflammatory cytokines stimulate IL-34 mRNA expression in GF. IL-34 protein is present in GCF and gingival tissue which demands further investigation about the eventual role of IL-34 in the pathogenesis of periodontitis.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Gingivitis is the most prevalent form among periodontal diseases. Evidence has shown the importance of gingivitis as a precursor of periodontal attachment loss and tooth loss. Despite its importance, often the diagnosis of gingival inflammation is overlooked. For maintaining gingival health, biofilm mechanical removal procedures should be perfomed effectively and periodically. Studies have shown oral hygiene frequencies of 12 and 24h are related to the gingival health maintenance. As important as frequency, it is the quality in which the procedure is performed. Studies assessing the association between toothbrushing frequency, effective biofilm removal and gingival clinical changes have an important role in the prevention and treatment of gingivitis. The aim of this study was to evaluate correlation between plaque and gingival inflammation in individuals underwent effective biofilm control at different frequencies. Data from this study were collected from a randomized clinical trial conducted in 2012 in Santa Maria, RS, Brazil. The sample consisted of 52 UFSM students, of courses not related to health, initially randomized into 4 oral hygiene frequencies and grouped into 2: Group G12/24 (12 and 24h) and group G48/72 (48 and 72h). In G12/24, even with an increase in plaque levels during 30 days, gingival inflammation levels were not statistically different from baseline. In G48/72, increase in plaque levels were accompanied by increasing levels of gingival inflammation. Positive correlation was observed in both groups, however, correlation is reduced in G12/24, demonstrating that increase in plaque levels were not sufficient to develop inflammatory response. In subjects who perform effective biofilm control, the correlation between plaque and gingival inflammation is affected by oral hygiene frequency. A gengivite é considerada a forma mais prevalente dentre as doenças periodontais. Evidências têm mostrado a importância da gengivite como precursora da perda de inserção periodontal e perda dental. A despeito de sua importância, muitas vezes o diagnóstico da inflamação gengival é negligenciado. Para a manutenção de saúde gengival, procedimentos de desorganização mecânica do biofilme devem ser realizados de forma efetiva e periódica. Estudos observaram que frequências de higiene bucal (HB) de 12 e 24 horas estão relacionadas a manutenção de saúde gengival. Tão importante quanto a frequência, é a qualidade em que esse procedimento é realizado. Estudos que avaliam a associação entre frequência de escovação, desorganização efetiva do biofilme e alterações clínicas gengivais têm um papel importante na prevenção e tratamento das gengivites. Portanto, o objetivo deste estudo foi avaliar a existência de correlação entre biofilme e inflamação gengival em indivíduos que realizaram efetivo controle do biofilme em diferentes frequências. Os dados deste estudo foram coletados de um ensaio clínico randomizado realizado no ano de 2012 na cidade de Santa Maria, RS, Brasil. A amostra foi composta por 52 estudantes de cursos não relacionados a área da saúde da UFSM inicialmente randomizados em 4 frequências de higiene bucal e agrupados em 2: grupo G12/24 (12 e 24h) e grupo G48/72 (48 e 72h). No G12/24, mesmo havendo aumento nos níveis de Índice de Placa (IPl) durante os 30 dias, os níveis de inflamação gengival não diferiram estatisticamente do baseline. Já no G48/72, aumento nos níveis de IPl foi acompanhado por aumento nos níveis de inflamação gengival. Houve correlação positiva em ambos os grupos, no entanto, a correlação diminuiu no G12/24, demonstrando que o aumento nos níveis de biofilme não foi suficiente para desenvolver resposta inflamatória gengival. Em indivíduos que realizam controle efetivo do biofilme, a correlação entre biofilme e inflamação gengival é afetada pela frequência de higiene bucal.

O objetivo deste estudo foi avaliar a situaÃÃo periodontal de todos os pacientes com Mucopolissacaridose, atendidos no Hospital Infantil Albert Sabin e no Hospital Geral CÃsar Cals, FortalezaâCE. Foram avaliados 18 pacientes, por um cirurgiÃo-dentista previamente calibrado, que registrou os Ãndice de Pernus, Ãndice de Placa Bacteriana VisÃvel, Ãndice de Sangramento Gengival, PSR e Ãndice CPO-D/ceo-d. Em relaÃÃo ao volume gengival, observou-se que 72,2% dos pacientes apresentaram Ãndice de Pernus nos escores 1, 2 ou 3, que corresponde Ã presenÃa de hiperplasia gengival. Quando se avaliaram os cuidados com relaÃÃo Ã higiene oral dos pacientes, observou-se que quanto ao Ãndice de Sangramento Gengival 77,8% dos pacientes apresentaram Ãndice maior que 10%, 66,6% dos pacientes apresentaram Ãndice de Placa Bacteriana VisÃvel maior que 40%. Com relaÃÃo ao PSR 94,4% dos pacientes apresentou escore 1, correspondendo a sangramento, e 5,6% escore 2, correspondendo Ã presenÃa de cÃlculo alÃm de sangramento, e quanto ao CPO-D/ceo-d 61,1% apresentou Ãndice maior que 5. De uma maneira geral, observou-se que a situaÃÃo periodontal dos pacientes portadores de mucopolissacaridose Ã uma situaÃÃo que inspira maior cuidado, mostrando caracterÃsticas prevalentes como o aumento gengival, alÃm de altos Ãndices de Sangramento Gengival, Placa Bacteriana VisÃvel e CPO-D/ceo-d. The Mucopolysaccharidosis (MPS) are a group of rare diseases characterized by enzyme deficiency that results in nondegradation of the glicosaminoglicans (GAGs). The aim of this study was to evaluate the periodontal status of all the patiente with MPS that are treated at the Hospital Infantil Albert Sabin and at the Hospital Geral CÃsar Cals, in Fortaleza, CearÃ, Brazil. A number of 18 patients were evaluated by a dentist that was previously calibrated, which registered the Pernus Index (IP), Visible Bacterium Plaque Index (IPB-V), Gingival Bleeding Index (ISG), PSR and Index that shows number of teeth that showed cavities, tooth loss or restoration (CPO-D/ceo-d). In relation to the gingival volume it was observed that 72,2% of the patients showed that the gingival was not in itÂs regular size. On the evaluation of dental hygiene, it was observed that 77,8% of the patients showed ISG greater than 10% and 66,6% of the patients showed IPB-V greater than 40%. In relation to PSR, 94,4% of the patients showed scored 1 which means gingival bleeding and 5,6% showed score 2 which means calculus besides gingival bleeding. And in relation to CPO-D/ceo-d 61,1% of the patients showed score greater than 5. In a general way it was observed that the periodontal status of the patients is a situation that requires more care, showing characteristics like gingival hyperplasias besides high scores of ISG, IPB-V and CPO-D/ceo-d.

O objetivo desta revisão sistemática da literatura foi avaliar a efetividade de diferentes procedimentos de cirurgia plástica periodontal (CPP) no tratamento de retrações gengivais localizadas e generalizadas e, determinar a existência de superioridade de uma determinada técnica cirúrgica sobre as demais. Foram identificados e incluídos apenas ensaios clínicos controlados aleatorizados (ECA), com período de acompanhamento mínimo de 6 meses de duração com as seguintes características: 1) diagnóstico clínico de retração gengival; 2) pelo menos 10 pacientes por grupo ao final do período de acompanhamento; 3) áreas de retração Classe I ou Classe II (MILLER 1985) > 3 mm de comprimento; e 4) áreas de retração que não apresentassem dentes com cáries ou restaurações. Nestes ensaios, avaliou-se a alteração da condição estética observada pelos pacientes, alterações na retração gengival, no nível clínico de inserção e no tecido queratinizado, sítios com recobrimento radicular completo, percentual de recobrimento radicular completo e recobrimento radicular médio. Na estratégia de pesquisa os bancos de dados Cochrane Oral Health Group\'s Specialised Register, CENTRAL, MEDLINE e EMBASE foram pesquisados até novembro de 2007. Busca manual e avaliação de literatura não publicada também foi realizada. A seleção e avaliação dos estudos foram realizados em duplicata, e os autores dos trabalhos incluídos foram contatados de forma a fornecer informações relevantes aos ensaios. Os critérios de seleção e análise dos resultados seguiram as diretrizes do Cochrane Oral Health Group (Cochrane Handbook for Systematic Reviews of Interventions 5.0.0). Os resultados mostraram que 21 ECAs se enquadraram nos critérios de inclusão entre 89 pré-selecionados. Destes, 12 foram utilizados na condução de meta-análises. Apenas quatro ECAs apresentaram alterações na condição estética. Verificou-se em uma das meta-análises, redução estatisticamente significante maior para o enxerto conjuntivo sub-epitelial (ECS) quando comparado a regeneração tecidual guiada, com uma diferença de médias entre os grupos de -0,39 mm [95% IC: -0,65, -0,12]. Alterações estatisticamente significantes foram observadas também para nível clínico de inserção (0,62 mm na comparação entre o enxerto de matriz dérmica acelular e o retalho deslocado coronariamente [95% IC: 0,09, 1,140]) e tecido queratinizado em três diferentes comparações. Em geral, todos os estudos incluídos, apresentaram redução estatisticamente significante na profundidade dos defeitos iniciais e ganhos no nível clínico de inserção e faixa de tecido queratinizado, independente do procedimento utilizado. Concomitantemente, observou-se um recobrimento radicular médio de 80,94% e um recobrimento radicular completo de 46,63% com uma grande variação nos resultados. Em conclusão, todos os procedimentos avaliados proporcionaram melhoras significantes nos parâmetros clínicos avaliados, independente da técnica empregada. Entretanto, o ECS apresentou melhores resultados. Em adição, é necessária a realização de novos estudos e que estes sigam as recomendações do CONSORT comparando os diferentes procedimentos de CPP ao ECS (como grupo controle), de forma a avaliar as alterações clínicas e estéticas dos pacientes. The objective of this systematic review was to evaluate the effectiveness of different root coverage procedures in the treatment of localized or multiple recession-type defects. Only randomised controlled clinical trials (RCTs) of at least 6 months\' duration were included if the participants met the following criteria: 1) a clinical diagnosis of localized or multiple recession-type defects; 2) recession areas selected for treatment classified as Miller (MILLER 1985) Class I or Class II; 3) recession areas containing teeth with no caries or restorations; and 4) at least 10 participants per group at final examination. In these trials we evaluated the aesthetic condition change related to patient\'s opinion, gingival recession change, clinical attachment change, keratinized tissue change, percentage of sites with complete root coverage and mean root coverage. We searched the Cochrane Oral Health Group\'s Trials Register, the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE and EMBASE. Handsearching included several dental journals. Grey literature included SIGLE search. Quality assessment of the included RCTs was carried out in duplicate and the authors were contacted for missing information. Selection criteria and evaluation followed the Cochrane Oral Health Group\'s statistical guidelines. Twenty one RCTs were identified between 89 in potential. From them, 12 were included in meta-analysis. One of them showed a statistically significant reduction in gingival recession for subepithelial connective tissue grafts (SCTG) in comparison to guided tissue regeneration (0,39 mm [95% CI: -0,65, -0,12]. Statistically significant changes were also observed related to clinical attachment level (0,62 mm acelluar dermal matrix allograft versus coronally advanced flap [95% CI: 0,09, 1,140]) and for keratinized tissue in three comparisons. Overall, All PPS procedures produced statistically significant improvements in gingival recession, clinical attachment level and keratinized tissue. Mean root coverage and sites with complete root coverage were 80.94% and 46.63%, respectively. In conclusion, all PPS produced statistically significant improvements in all outcomes. However, the SCTG presented the best overall results. In addition, more research is needed to compare different PPS to SCTG (as a control group) in order to evaluate clinical and aesthetic changes. Future RCTs should follow CONSORT guidelines.

O objetivo deste estudo foi utilizar biópsias minimamente invasivas para avaliar a expressão de mediadores inflamatórios e sua correlação com o fluido gengival em pacientes com periodontite severa. O grupo teste compreendeu 22 pacientes com periodontite severa (idade média 45,5  DP 8,9 anos), analisados por sítios rasos e profundos, e o grupo controle por 14 pacientes periodontalmente saudáveis (idade média 39,35  DP 16,5 anos). As amostras do fluido gengival foram coletadas com papel absorvente, nos mesmos sítios de onde foram realizadas as biópsias, e quantificadas, por Luminex, para IFN- γ, IL 1-β, IL-6, IL-21, IL-22, IL-23, IL-25, IL-31, IL-33, IL-4, IL-6, IL-10, IL-17A, IL-17F, sCD40L e TNFα. Foram coletadas biópsias, com um punch de 2mm de diâmetro nos grupos teste e controle. A avaliação imuno-histoquímica foi semiquantitativa, nas células epiteliais, plasmócitos, macrófagos, fibroblastos e células endoteliais, para a IL1-β, IFN-γ, IL-6 e IL-17. Foram observadas marcações imuno-histoquímicas, em todos os grupos celulares analisados, tanto nos pacientes com periodontite como nos controles, sem diferenças significativas entre eles. O fluido gengival apresentou maiores quantidades para os marcadores IL-1β e IL-23 nos sítios profundos. Não foram encontradas correlações significativas entre as marcações imuno-histoquímicas e o fluido gengival para os subgrupos analisados. Na análise comparativa entre as biópsias e o fluido gengival, a IL1-β mostrou alta concordância nos sítios rasos e profundos. Concluindo, a utilização padronizada de um punch de 2mm de diâmetro para biópsias em tecido periodontal mostrou- se viável, para estudos com imuno-histoquímica. O fluido gengival pode não expressar todos os marcadores do tecido correspondente, com variações dependendo do marcador analisado e das condições inflamatórias locais. The aim of this study was to use minimally invasive biopsies to evaluate the expression of inflammatory mediators and their correlation with gingival fluid in patients with severe periodontitis. The test group comprised 22 patients with severe periodontitis (mean age 45.5  8.9 years SD), analyzed by shallow and deep sites and the control group for 14 periodontally healthy patients (mean age 39.35  SD 16.5 years). Gingival fluid samples were collected with absorbent paper in the same sites where biopsies were performed, and quantified by Luminex to IFN-γ, IL-1 β, IL-6, IL-21, IL-22, IL-23, IL-25, IL-31, IL-33, IL-4, IL-6, IL-10, IL-17A, IL-17F, TNF and sCD40L. Biopsies were taken with a 2 mm diameter punch in both groups. Immunohistochemistry was semiquantitative evaluated, comprising epithelial cells, plasma cells, macrophages, fibroblasts and endothelial cells, to IL1-β, IFN-γ, IL-6 and IL-17. Immunohistochemical markers were observed in the analyzed cells groups, both in periodontitis patients as in controls, without significant differences between them. The gingival fluid showed higher amounts for IL-1β and IL-23 in deep sites. No significant correlations between immunohistochemical markers and GCF were found. In the comparative analysis between immunohistochemical markers and GCF, IL1-β showed high concordance in shallow and deep sites. In conclusion, the use of a standardized punch 2mm diameter for periodontal tissue biopsies proved to be feasible for studies with immunohistochemistry. The gingival fluid may not express all the markers of the corresponding tissue, with variations depending on the analysed marker and local inflammatory conditions.

Mediante este contexto, o objectivo geral desta monografia foi realizar uma revisão de literatura para actualizar conhecimentos no que concerne aos métodos de retração gengival utilizados em Medicina Dentária. For all printing procedures it is necessary to retract the gum tissue so as to record the finishing lines subgingival, there is, for this purpose, three gingival retraction methods that have been classified as mechanical, chemi-mechanical and surgical. Through this context, the general objective of this thesis was to conduct a literature review to update knowledges regarding methods of gingival retraction used in dentistry. As specific objectives: The exploitation of gingival retraction methods used in Fixed Prosthodontics, defining and comparing their advantages, disadvantages, indications and clinical applicability, as well as the requirements for the success of this; define gingival retraction and the forces involved in the same and clarify and alert dental professionals to the need for proper selection of the method of using gingival retraction.

Abstract:

Conceptualment el dismorfisme defineix una alteració morfològica, congènita o adquirida, d'una part o de la totalitat d'un aparell o d'un òrgan. Així l'engrandiment gingival és un dismorfisme per excés d'aquest teixit que s'ha relacionat amb diferents factors: inflamatoris, farmacològics, neoplàsics i d'altres. Clàssicament els fàrmacs que s'havien relacionat amb el sobrecreixement gingival eren els antiepilèptics, fonamentalment la fenitoïna. A partir de la dècada dels vuitanta aquesta alteració morfològica també s'ha relacionat amb l'administració d'altres fàrmacs com la ciclosporina A i els bloquejadors dels canals del calci: dihidropiridines, verapamil i diltiazem. L'engrandiment gingival induït per fàrmacs presenta unes característiques clíniques i anàtomo-patològiques similars, tot i tractar-se de fàrmacs amb una acció terapèutica molt diferent. Això ha fet pensar a molts autors de l'existència d'alguna característica comuna entre ells que expliqués la patogènia del sobrecreixement gingival. En la literatura existeixen nombroses hipòtesis que pretenen explicar aquest dismorfisme, y sembla ser que el mecanisme patogènic seria la combinació de diferents factors: placa bacteriana, presència de fibroblastes gingivals predeterminats genèticament "responder" i l'acció del propi fàrmac. Una característica farmacodinà.mica comuna dels fàrmacs reconeguts com inductors de l'engrandiment gingival és la d'alterar el flux de calci transmembrana, que alhora modificaria el metabolisme dels fibroblastes del teixit connectiu gingival, produint-se un increment dels components de la matriu extracel.lular: fibres de col.làgena i/o substància fonamental amorfa. És per això que el concepte que millor definiria aquesta alteració des d'una vessant clínica seria el d'engrandiment o sobrecreixement gingival, encara que el terme hiperplasia gingival hagi estat utilitzat en la majoria de publicacions científiques. El sobrecreixement gingival que habitualment s'inicia a la regió de la papil.la interdental, pot afavorir l'aparició de símptomes i signes clínics que inclouen: dolor, sagnat i friabilitat dels teixits, moviments anormals de les dents, alteracions estètiques, fonètiques i de l'oclusió, a més a més d'afavorir l'aparició de càries i d'altres disfuncions periodontals. Constatàrem que la majoria d'individus que presentaven engrandiment gingival aquest havia passat desapercebut tant pel pacient com pel clínic, encara que els afectats per un sobrecreixement més sever si que mañifestaren alguns d'aquests símptomes. L'engrandiment gingival induït per fàrmacs, i en el nostre cas pels calcioantagonistes, s'ha demostrat que pot ser reduït o previngut amb un bon control de la placa bacteriana dirigit a eliminar la inflamació gingival; i en els casos més severs la cirurgia periodontal resectiva és el mètode utilitzat per l'eliminació de l'excés de teixit. La majoria de publicacions referents a l'engrandiment gingival induït per calciantagonistes, són series de casos amb un nombre reduït de pacients que els realment necessaris en funció de la prevalença estimada per aquest dismorfisme; tampoc s'hi reflecteixen valors que indiquin el risc atribuïble al consum d'aquests fàrmacs, ni comparacions respecte a una població control. Desenvolupàrem un estudi clínic transversal en l'àmbit de l'atenció primària, CAP Rambla de Terrassa (Barcelona), on s'examinaren els pacients que de forma consecutiva eren visitats en les consultes de medicina general i cardiologia, per conèixer la prevalença, severitat i risc d'engrandiment gingival en la població tractada amb calciantagonistes, i que es comparà amb una població que no rebia cap dels fàrmacs reconeguts com inductors. En l'estudi s'inclogueren 265 pacients: 72 tractats amb dihidropiridines fonamentalment la nifedipina, 32 amb diltiazem, 14 amb verapamil i els 147 subjectes restants foren els controls de la mostra. Es valorà el grau de sobrecreixement gingival en un sentit àpico-coronari (vertical) amb l'índex d'Angelopoulos i Goaz modificat per Miller i Damm -GO-, i en un sentit àntero-posterior (horitzontal/nòdul-papil.lar) amb l'índex de Seymour modificat per Miranda i Brunet MB-. Es considerà que existia engrandiment gingival quan el valor d'algun dels dos índexs era >0 en qualsevol localització. També s'enregistraren: l'índex gingival, l'índex de placa i la profunditat de sondatge, així com les variables sòcio-demogràfiques i qualitatives de tots els individus. Estadísticament s'establiren les diferents comparacions de les variables analitzades amb la prova de Chi Quadrat i quan fou necessari s'utilitzà el Test exacte de Fischer. Es realitzà l'anàlisi bivariant dels factors que poguessin exercir alguna influència sobre els índexs de sobrecreixement gingival. S'evaluà mitjançant un anàlisi mu1tivariant de regressió logística l'Odds Ratio o risc atribuïble al calciantagonista ajustat per les variables de confusió. I finalment es determinà el test de concordança Kappa dels dos índexs utilitzats en la valoració de l'engrandiment gingival (GO i MB). La prevalença d'engrandiment gingival en la població control fou del 4.1% per l'índex GO i del 7.5% pel d'MB. La prevalença d'engrandiment pel grup casos -bloquejadors dels canals del calci- dels diferents subgrups farmacològics fou: per les dihidropiridines 33.3% (GO) i 51.4% (ME); pel diltiazem 31.3% (GO) i 50% (MB); i pel verapamil 21.4% (GO) - 35.7% (MB); siguent les localitzacions anteriors ínfero-vestibular i súpero-vestibular les més freqüents per ambdós registres. Degut a que l'inici del sobrecreixement habitualment es localitza a nivell de la papil.la interdental, i degut a que l'índex d'MB enregistra l'engrandiment en aquesta regió, això ens explicaria aquesta diferència de freqüències. En el subgrup de pacients tractats amb nifedipina (n=65) es relacionà la dosi acumulada d'aquest fàrmac amb el grau de sobrecreixement, i encara que a dosis acumulades altes existia una major prevalença d'engrandiment per ambdós índexs, aquest dismorfisme no el podem considerar dosi-depenent. L'edat i el sexe no semblen ser factors determinants en el desenvolupament d'aquest dismorfisme; tampoc per les altres variables qualitatives registrades. Pel que fa a la salut gingival els valors elevats de l'índex gingival (inflamació severa) condicionaren de forma significativa la presència d'engrandiment gingival. Respecte a l'acúmul de placa bacteriana, ambdós grups -casos i controls- presentaren uns índexs similars. Ja que l'engrandiment induït per fàrmacs és per tant un engrandiment combinat, explicable per la pròpia acció del fàrmac i a la inflamació gingival sobreafegida, és important determinar en quina proporció aquest engrandiment és provocat per un factor o un altre. D'aquesta manera en el grup tractat amb dihidropiridines, quan no es considera la dosi, el risc atribuible al fàrmac seria de 10.6-14.4 (GO-MB) i l'atribuïble a l'alteració de la salut gingival seria de 9.6 per ambdós índexs. En el subgrup tractat amb nifedipina a dosi acumulada alta, el risc atribuïble a aquest fàrmac fou superior, entre 17.4-23.6 (GO-MB) i el relacionat amb la salut gingival es mantingué .invariable per ambdós índexs (9.4). Respecte als altres calciantagonistes, diltiazem i verapamil, el risc fou més superior per l'alteració de la salut gingival que no pas per la presència del fàrmac inductor; aquest fet podria ser degut segurament a la reduïda grandària de la mostra d'aquests dos grups. Es varen detectar diferències de freqüencia d'engrandiment per ambdós índexs; l'índex MB detectava un sobrecreixement en les fases incipients de l'engrandiment que no diagnosticava l'índex GO, encara que després de determinar el grau de concordança o Kappa constàrem que ambdós índexs eren fiables. L'engrandiment gingival és un dismorfisme habitualment poc conegut pels clínics responsables d'aquests pacients; és per això important difondre valors de prevalença i risc de sobrecreixement gingival pel bon control de la salut periodontal en la població tractada amb bloquejadors dels canals del calci. En un futur caldrà analitzar mostres més àmplies de pacients tractats amb el diltiazem i sobretot amb el verapamil, per així poder determinar les freqüències de sobrecreixement gingival i valorar altres aspectes estudiats amb la nifedipina. Finalment és de suposar que el major consum de dihidropiridines diferents de la nifedipina, permetrà determinar si aquests també són fàrmacs inductors, tal com ja alguns autors estableixen en estudis experimentals i en casos clínics aïllats, i alhora establir en cada cas la seva prevalença. Gingival enlargement is an abnormal growth of the gingival tissue. It is mainly associated with dental-plaque-related inflammation, some diseases and drug therapy. It has been reported with the use of phenytoin, cyclosporin, and calcium channel antagonist, including dihydropyridinederivatives, veraparnil and diltiazem. This research was designed to study the prevalence of gingival enlargement in general population and in patients taking calcium antagonists. Subjects (n=265) were selected using predefined inclusion-exclusion criteria. The non-treated-control group included 147 patients attended in a general practice center (none had been treated with calcium antagonists). The treated-group included 118 patients taking calcium antagonists (at inclusion time and at least during the last six months). It included 72 patients taking dihydropyridines (65 receiving nifedipine), 32 treated with diltiazem, and 14 with verapamil. Medical history and a complete oral and odontological exploration were done in all patients. It included plaque index, gingival index, probing depth and measures of gingival enlargement in the vertical (GO) and the horizontal (MB) directions. The prevalence of gingival enlargement in the non-treated-control group was 4.1-7.5% when measured by GO and MB, respectively. The patients receiving dihydropyridines had an enlargement prevalence of 33.3-51.4%. Those under diltiazem were 31.3-50% and in the case of verapamil 21.4-35.7%. The most frequents locations for both GO/MB indexes were the anterior lower and the upper-buccal jaw. In the case of nifedipine, the enlargement showed a dose/response relationship when total drog exposition was considered. High rates of gingival index (severe inflammation) produced significant differences in relation to gingival enlargement (p<0.01). No differences were observed in the plaque index. Age and gender did not seem to be determinant factors in the development of overgrowth, the same results were obtained for all other qualitative variables registered. Multivariate statistical analysis showed that the attributable risk (Odds Ratio, OR) of gingival overgrowth related to dihydropyridine treatment was 10.6-14.4 (OR for GO/MB), and the risk related to gingival inflammation was 9.6 for both GO/MB.

Abstract:

Background Practicing good oral hygiene is pivotal to achieving superior oral health and overall quality of life. Controlling the plaque biofilm build-up on tooth surface has been the keystone for prevention of gingival diseases for decades. As gingivitis is associated with a number of diseases including heart disease, a review of the treatment approaches is conducted to identify a comprehensive set of guidelines for consumers to achieve better oral health. Aim To systematically review the evidence based approaches and evaluate their effectiveness in reducing gingivitis. Methods A search for relevant articles was conducted on PubMed, Medline and Cochrane in May 2014. In addition, the search included articles from journals, research groups and organizations. Results Based on the reviews, powered brushing using Triclosan/copolymer tooth paste was found to have an advantage over manual, with rotation oscillation brushes were proficient in minimizing plaque and gingivitis. Use of Inter-dental brushes was more favourable than flossing alone. Hexiditine (0.14%) mouth rinse reduced more plaque compared to Chlorhexidine with no side effects. Conclusion A lifestyle that includes powered brushing using Triclosan based tooth paste, Hexidine mouth rinses after the meals and interdental tooth brushes instead of flossing will reduce gingivitis. Dental professionals need to educate patients on these approaches. Consumers should adopt the combination of these to achieve their health goals. Comprehensive policies aimed at providing fair access to dental care for all people is essential. Trabajo de Investigación Requisito para optar al Título de Cirujano Dentista Autor no autoriza el acceso a texto completo de su documento Introducción: Las enfermedades periodontales constituyen un grupo de enfermedades infecciosas que comprometen los tejidos que circundan los dientes y que afectan a un gran porcentaje de la población. Es así como la gingivitis se presenta como la segunda enfermedad en prevalencia en salud bucal en la población infantil. Por otra parte, estudios en niños reportan la presencia de patógenos periodontales como parte de la microbiota bucal pero no se ha determinado el período en que se produce la colonización por dichas bacterias. El objetivo del estudio fue determinar la ocurrencia de algunos patógenos periodontales como Porphyromonas gingivalis, Prevotella intermedia/nigrescens, Fusobacterium nucleatum y Actinobacillus actinomycetemcomitans en un grupo de escolares de la Región Metropolitana y su asociación con gingivitis. Materiales y métodos: se tomaron muestras de placa bacteriana de surco gingival, dorso lingual y saliva completa a 30 escolares de 8 a 11 años de edad, provenientes de dos colegios de la Región Metropolitana las que fueron cultivadas en medios no selectivos (agar Columbia) y selectivos (Agar CVE y Dentaid-1). La identificación de los patógenos se realizó mediante la morfología colonial, pruebas rápidas y uso de baterías bioquímicas comerciales. Resultados: todos los niños evaluados presentaron gingivitis: 60% gingivitis leve y 40% gingivitis moderada. Se detectó la presencia de Porphyromonas gingivalis en un 47% de los niños y de Fusobacterium nucleatum en un 73,3%. Prevotella intermedia fue identificada en el 3,3% de los niños. Sin embargo, Actinobacillus actinomycetemcomitans no se encontró en ningún niño de la muestra. No hubo diferencias significativas en la ocurrencia de patógenos periodontales en gingivitis leve y moderada. Conclusiones: Patógenos periodontales están presentes como parte de la microbiota bucal, en niños con gingivitis. Esto puede constituir un factor de riesgo importante para el inicio de Periodontitis en la adolescencia o posteriormente en la edad adulta.

Trabajo de Investigación Requisito para optar al Título de Cirujano Dentista Autor no autoriza el acceso a texto completo de su documento La gingivitis es la enfermedad de mayor prevalencia en la población infantil luego de la caries dental. Los estudios epidemiológicos existentes en nuestro país reportan prevalencias que van desde el 40% al 98%, pero no existen estudios de vigilancia epidemiológica ni datos muy precisos sobre la prevalencia de esta patología en niños. Se llevó a cabo un estudio de prevalencia en una muestra de 1.637 niños escolares de 6 a 8 años de edad de la Región Metropolitana. El estado gingival fue estimado utilizando el Índice Periodontal Comunitario de Necesidades de Tratamiento, junto con la observación de Criterios Clínicos de inflamación. La prevalencia de gingivitis encontrada fue de 68,42%, lo cual es mayor que lo encontrado en un estudio referencial efectuado por Mella en 1992 (37,7%) (53) , con diferencia estadísticamente significativa (P=0,00001). La prevalencia aumentó al aumentar la edad, con diferencia estadísticamente significativa (P=0,0001). La prevalencia fue similar en ambos sexos, sin diferencia estadísticamente significativa (P=0,838), y sin asociación de riesgo (OR=0,97; 95% IC=0,78-1,21). La prevalencia se mantuvo relativamente constante entre los tres niveles socio-económicos, sin diferencia estadísticamente significativa (P=0.417). Según el IPCNT, la prevalencia de gingivitis fue de 29,57%: 27,49% de niños con hemorragia gingival + 2,08% de niños con cálculo gingival. Se sugirió reforzar las acciones promocionales y preventivas existentes en nuestro país relacionadas con las enfermedades periodontales en niños.
