A csontpótlást igénylő műtéti beavatkozások során a beültetésre kerülő csontpótló graft tulajdonságai meghatározzák az eljárás kimenetelét, rövid és hosszú távú sikerét. Munkánk első részében áttekintést adunk a modern csontpótló eljárások előnyeiről-hátrányairól, illetve részletesen foglalkozunk a gipsz szintetikus csontpótló graftként történő alkalmazásának lehetőségével. A kísérletes klinikai leírások biztonsággal és jó hosszú távú eredménnyel alkalmazható csonthiánykitöltő anyagként írják le a kalcium-szulfát-dihidrátot, azonban a gipsz csontsejtekre kifejtett hatása, a csontpótlás mechanizmusa nem ismert. Molekuláris biológiai módszerekkel vizsgáltuk a gipsz csontsejtekre gyakorolt hatását. Az egér-praeosteoblastok szaporodására ideális tenyésztőfelületnek bizonyult a gipsz, míg a klinikumban gyakran csonthiánykitöltő anyagként alkalmazott polimetil-metakrilát-csontcement gátolta a sejtek osztódását. A gipsz megváltoztatta a sejtek génkifejeződési profilját, a csontképződés irányába mutató gének expresszálódtak nagyobb mértékben a gipszes tenyészetekben, és ezekben a sejtkultúrákban emelkedett alkalikusfoszfatáz-aktivitást mértünk. Eredményeink molekuláris biológiai szempontból támasztották alá a gipsz szintetikus csontpótló graftként történő alkalmazásának létjogosultságát, a kalcium-szulfát-dihidrát új – a csontgyógyulást támogató – tulajdonságát is kimutatva.

This roundtable discussion features two leading scholars of critical race and digital media, Drs. Brooklyne Gipson and Kishonna Gray. The impacts of pleasure and danger on social network sites and digital media certainly predate these technologies yet are heightened in unique ways by their affordances. Both Gipson and Gray discuss how they center Black feminism, queerness, and intersectionality in their approaches to digital media studies and the conundrum of harm, pleasure, and nuance online. They discuss themes such as storytelling, methods, and play to push the boundaries of the binary and chart a path forward that is neither afraid of nor essentialist about the role of technology in our lives.

Abstract:
Treatment of humerus fractures is divided into operative and non-operative treatment Fractures of the diaphysis of the humerus heal well. Surgeons today have many opportunities to treat them. The decision on the type of treatment to be applied depends on the location of the fracture, the existence of associated injuries, the age and the general condition of the patient. Non-operative treatment is most often applied, although there are fractures in which surgical intervention is necessary in order to perform healing and prevent complications. Non-operative treatment of fractures of the diaphysis of the humerus gives good results, with little angulation and minimal or no shortening of the arm. Adequate repositioning, appropriate plaster immobilization and regular X-rays heal the fracture within the allotted time. Disciplined early physical therapy in terms of circular movements prevents shoulder contracture and allows later physical therapy to last significantly shorter. Non-operative treatment lasts from 7-11,5 weeks.

Abstract:
The purpose of this article is to show the possibility of using the GIPRO system, designed for individual pressure regulation in each coking chamber, on existing and new coking batteries. The prerequisites for the emergence of the technology of individual pressure regulation are described. The basics of processes occurring in coking chambers, which lead to undesirable environmental and production consequences, are explained. These negative effects can be leveled with the use of PROven® and SORRESO individual regulation systems developed in the world. The article provides a comparative analysis of various systems of individual pressure regulation, which are used in world practice, and the system of maintaining individual pressure in the coking chamber - the GIPRO system, developed by SE "GIPROKOKS" - with other existing global analogues. The history of the introduction of various systems of individual pressure regulation in the world is briefly presented. Design features of the systems themselves are described. The path of development of the "GIPRO" system from the idea to the experimental installation is described. The results of the design and installation of the "GIPRO" system at some factories in China and India are given. A description of the "GIPRO" system during operation with negative pressure in the gas tank is given, as well as other possible modes of operation of the system are presented. On the example of the developed projects, the possibility of using the "GIPRO" system for installation, both on large-capacity coke batteries with tamping technology that are being built, and on the local modernization of gas removal equipment on existing coke batteries, has been proven. Conclusions were made about the possibility and expediency of using the developed system for coke batteries of various designs. The developed system can be used to improve the environmental performance of existing coke batteries. Installation of the system on a working coke battery can be implemented with minimal loss of coke production on that battery. Health Monitoring System (HMS) is an excellent tool that actually saves lives. It makes use of transmitters to gather information and transmits it wirelessly to a receiver. Essentially, it is much more practical than the large equipment that the majority of hospitals now employ and continuously checks a patient's health data 24/7. The primary goal of this research is to develop a three-layered Ensemble of Classifier model on Edge based Healthcare Monitoring System (ECEHMS) and Gauss Iterated Pelican Optimization Algorithm (GIPOA) including data collection layer, data analytics layer, and presentation layer. As per our ECEHMS-GIPOA, the healthcare dataset is collected from the UCI repository. The data analytics layer performs preprocessing, feature extraction, dimensionality reduction and classification. Data normalization will be done in preprocessing step. Statistical features (Min/Max, SD, Mean, Median), improved higher order statistical features (Skewness, Kurtosis, Entropy), and Technical indicator based features were extracted during Feature Extraction step. Improved Fuzzy C-means clustering (FCM) will be used for handling the Dimensionality reduction issue by clustering the appropriate feature set from the extracted features. Ensemble model is introduced to predict the disease stage that including the models like Deep Maxout Network (DMN), Improved Deep Belief Network (IDBN), and Recurrent Neural Network (RNN). Also, the enhancement in prediction/classification accuracy is assured via optimal training. For which, a GIPOA is introduced. Finally, ECEHMS-GIPOA performance is compared with other conventional approaches like ASO, BWO, SLO, SSO, FPA, and POA.

Abstract:
In the Fall of 2015 the Office of the Wyoming State Archaeologist (OWSA) conducted archaeological investigations at the Gipson site, a historic campsite located in the Laramie Mountains of Wyoming. This project was undertaken at the request of the property owners and successfully synthesized research, outreach, and education. During the field session twelve students and volunteers ranging in age from 10 to 60 conducted survey, metal detecting, and test excavations. Data collected from this work has the ability to give clarity to the nature of the 19th century occupation and will add to the understanding of railroad building activities in the West. However, the benefits of this project extend beyond the informational value the collected data provide. Through the integration of students from the University of Wyoming and local volunteers, including the property owners, the Gipson site investigations proved to be a successful public outreach and archaeological educational tool. This paper will provide background on the project and discuss the challenges and benefits of incorporating outreach and education into a standard research project.

AbstractIn this paper an object-centric perspective on planning domain definition is presented along with an overview of GIPO (graphical interface for planning with objects), a supporting tools environment. It is argued that the object-centric view assists the domain developer in conceptualizing the domain’s structure, and we show how GIPO enables the developer to capture that conceptualization at an appropriate and matching conceptual level. GIPO is an experimental environment which provides a platform for exploring and demonstrating the range and scope of tools required to support the knowledge engineering aspects of creating and validating planning systems, both for classical pre-condition planning and hierarchical planning. GIPO embodies the object-centric view, leading to a range of benefits typically associated with object-oriented methods in other fields of software engineering such as highly visual development methods, code reuse and efficient, reliable development.

Tuberous sclerosis complex is a multisystemic genetic disorder with high phenotypical variability. Its progress frequently brings along autism (61%), epilepsy, intellectual disability (45%), and neurocognitive impairment (Gipson and Johnston, 2017). We are considering the case of an infant suspected with tuberous sclerosis complex by imagistic investigation in the prenatal period. The pre- and postnatal ultrasound, fetal MRI, ophthalmoscopy, and dermatological and neurological examinations were used for diagnosis and follow-up. The seven major and minor criteria were regarded as sufficient for accurate diagnosis.

Very Large Array Processors (VLAP) will be the need of the future for solving computationally intense Very Large Problems (VLP) common in pattern recognition, image processing and other related areas of digital signal processing. Design methodology of such VLAPs for massively parallel dedicated/general purpose applications is highly complex. Two companion papers (Part 1 and Part 2) on VLAP are presented in this issue. In Part 1, we propose a VLAP called Reconfigurable GIPOP Processor Array (RGPA). The RGPA is made up of high performance processing elements called the Generalized Inner Product Outer Product (GIPOP) processor. Unlike the traditional special/general purpose processors, ours has a totally different and new architecture and organization involving higher level functional units to match with the complex computational structures of numeric algorithms and suitable for massively parallel processing. We also present a strategy for mapping VLPs on VLAPs. In Part 2, we propose a novel VLSI design methodology for implementing cost effective and very high performance processors meant for special purpose applications and in particular, for VLAPs.

The ability to rapidly and accurately delineate open-pit granite mining areas is pivotal for effective production planning and environmental impact assessment. Over the years, advancements in remote sensing techniques, including the utilization of satellite imagery, LiDAR technology and unmanned aerial vehicles, have revolutionized the way mining areas are monitored and managed. Simultaneously, in the context of the open-pit mining area extraction task, deep learning-based automatic recognition is gradually replacing manual visual interpretation. Leveraging the potential of unmanned aerial vehicles (UAVs) for real-time, low-risk remote sensing, this study employs UAV-derived orthophotos for mining area extraction. Central to the proposed approach is the novel Gather–Injection–Perception (GIP) module, designed to overcome the information loss typically associated with conventional feature pyramid modules during feature fusion. The GIP module effectively enriches semantic features, addressing a crucial information limitation in existing methodologies. Furthermore, the network introduces the Boundary Perception (BP) module, uniquely tailored to tackle the challenges of blurred boundaries and imprecise localization in mining areas. This module capitalizes on attention mechanisms to accentuate critical high-frequency boundary details in the feature map and synergistically utilizes both high- and low-dimensional feature map data for deep supervised learning. The suggested method demonstrates its superiority in a series of comparative experiments on a specially assembled dataset of research area images. The results are compelling, with the proposed approach achieving 90.67% precision, 92.00% recall, 91.33% F1-score, and 84.04% IoU. These figures not only underscore the effectiveness of suggested model in enhancing the extraction of open-pit granite mining areas but also provides a new idea for the subsequent application of UAV data in the mining scene.

Abstract:
In order to inhibit the process of setting and hardening of building gypsum, retarding agents are used. They: decrease the solubility of gypsum semihydrate (e.g. ammonia) form compounds of lower solubility on the surface of gypsum grains (sodium and potassium phosphates, tetraborate, boric acid) are absorbed on gypsum grain surface and inhibit the formation of crystal nuclei (sulphite alcohol ferment, keratin). The materials mentioned above have some disadvantages: long duration of preparation (lying, boiling, filtering) and uncertainty of formulae (glue solutions) layering of gypsum additions mixtures and decrease of mechanical resistance of hardened gypsum (phosphates, borates) process complications caused by necessity of water heating that leads to uncertain influence of additions when the conditions for maintaining constant temperature of the mixture are absent (hot running water). New retarding agent, inhibiting the hardening of building gypsum without disadvantages listed above is created. The effect of the retarding agent is caused by the formation of clathrate compounds. The retarding agent consists of carbamide and inorganic stabilising addition, in ratio 40:1. The duration of the complete hardening of building gypsum can be prolonged by the retarding agent up to 90 min.

This paper aims at experimental and numerical research of delamination process in carbon/epoxy composite laminates with different fiber orientation angles in stacking sequence exhibiting the bending–twisting elastic couplings. Experimental specimens were subjected to the double cantilever beam (DCB) tests according to the ASTM D5528 regulations. Values of the mode I strain energy release rates were calculated by using three different data reduction schemes: the modified beam theory, the compliance calibration method and the modified compliance calibration. Determination of delamination initiation point was conducted in twofold way: by visual observation of crack tip using high resolution camera and by utilization of the acoustic emission technique. Numerical analyss were prepared in Abaqus/CAE Software environment by using the virtual crack closure technique (VCCT). The numerical beam model consisted of SC8R continuum shell elements. Obtained outcomes revealed that extensive fiber bridging phenomenon occurring during delamination process pronouncedly affected propagation values of strain energy release rate (GIprop) and numerically obtained load–displacement curves. Nevertheless, in initial stage of delamination, results obtained by using the VCCT were in agreement with experimental data. The greatest value of the mode I fracture toughness equal 0.56 N/mm was obtained for the BT45 laminate.

Az adaptáció egyik fontos eleme lehet az öntözéses gazdálkodási formák kiszélesítése, a víz- és energiatakarékos öntözési módok és módszerek előtérbe helyezése és nem utolsó sorban a használt vizek (szennyvizek, hulladékvizek, elfolyóvizek stb.), mint alternatív vízforrások öntözéses hasznosítása. Kísérletünkben egy intenzív afrikai harcsa-nevelő telepről származó használt termálvizet és Körös vizet használtunk fel energiafűz öntözésére egyéves liziméteres kísérletben. Az öntözéses kísérlet a Nemzeti Agrárkutatási és Innovációs Központ (NAIK) Öntözési és Vízgazdálkodási Önálló Kutatási Osztály (ÖVKI) szarvasi Liziméter Telepén valósult meg. A kísérletben 82-es fűz klón nyolcféle öntözéses kezelését alkalmaztuk. Eredményeink szerint a használt termálvíz minősége miatt öntözésre csak korlátozottan használható, mivel a szikesedésért leginkább felelős kémiai paraméterek (EC, SAR, nátriumszázalék és szódaegyenérték) értékei magasak. A vízminősége miatt (Körös víz felhasználásával) hígítással és gipsz javítóanyag hozzáadásával egy harmadik víztípust hoztunk létre (HG). A talaj kicserélhető kation tartalmában bekövetkező változásokat vizsgálva a használt termálvíz kiöntözése mellett a kicserélhető nátrium felhalmozódását mértünk valamennyi öntözési norma esetén. A hígított+kezelt öntözővíz (HG) mellett a talajban szignifikáns változás egy kicserélhető kation esetében sem történt. Körös víz öntözése mellett 15 és 30 mm öntözési norma esetén a hígított+kezelt vízhez hasonlóan jelentős változás nem történt, ugyanakkor a 60 mm-es öntözési norma mellett a nátriumtartalomban szignifikáns csökkenés volt kimutatható, amely a kilúgzásnak köszönhető. Az öntözetlen kezelésben a kicserélhető kalcium tartalom csökkenése mellett a magnéziumok mennyisége növekedett meg a talajkolloidok felületén, amely következtetésünk szerint természetes szikesedési folyamatok következménye. A talaj összes oldott sótartalma 0-40 cm mélységben a használt termálvizes öntözés után (60 mm öntözési norma) szignifikánsan különbözött a Körös vízzel öntözött kezelésekben mért értékektől, ugyanakkor a kezelt víz (HG) felhasználása mellett a talaj összes oldott sótartalma a Körös vízzel öntözött kezelésekben mért értékekhez hasonlóan változott és nem haladta meg a 0,05%-ot. A felhasznált öntözővizek talajra gyakorolt hatásának teljes körű értékeléséhez további talajvizsgálatokat tartunk szükségesnek, valamint a hosszú távú előrejelzések miatt a kísérletet folytatjuk több éven keresztül. Mindazonáltal megállapítható, hogy az elvégzett kísérletünk eddigi szakaszában az intenzív afrikai harcsanevelő telepről származó használt termálvíz jó minőségű vízzel való keverése és gipsz hozzáadása után felhasználható a talajvédelem szempontjából fenntartható módon, szolonyecesedés és sófelhalmozódás nélkül.

Az apajpusztai és zabszéki kísérleti területen megvizsgáltuk két 1 m-es talajszelvény ásványtani összetételét. A talajminták szemcseeloszlás szempontjából kozetlisztes (vályogos) agyag, agyagos kozetliszt (vályog), agyagos és kozetlisztes (vályogos) homok jelleguek, jelentos karbonáttartalommal és magas (9-10) pH-értékkel. A minták homokfrakcióját kétféle (vulkáni és metamorf eredetu) kvarc, földpát, muszkovit, biotit és járulékos ásványok, kozettörmelék (dolomit, mészko, metamorf palák, bontott riolit és egyéb vulkáni kozet) és organoklasztok (csigahéjtörmelék) alkotja. A minták röntgendiffrakciós és termikus elemzése kimutatta, hogy az agyagásványokat montmorillonit, klorit, illit, kaolinit és szmektit-illit kevertszerkezetu ásvány alkotja. Rajtuk kívül jelentos mennyiségu kvarc, karbonát és földpát, valamint kevés amfibol, pirit, goethit, hematit, gipsz és 2-4 % amorf fázis jelenik meg. A két teljes kémiai elemzésbol számított normatív ásványok a valódi összetételtol jelentosen különbözik, amely abból fakad, hogy minden bizonnyal a montmorillonit Na-jellegu, az igen nagy mértékben diszperz és rendezetlen agyagásványok Na+-adszorbciója jelentos, és számolni kell magas SiO4-4?AlO4-5 kihelyettesítéssel is. Az általánosan elfogadott szikesedési modell keretén belül a közlemény a szikesedés "finomszerkezetére" kíván fényt deríteni, s ezen belül az agyagásványok szerepére a folyamat megindításában és kifejlodésében. Feltételezve, hogy az eredeti állapotban a talajszelvény fo agyagásványa a piroklasztos eredetu Na-montmorillonit volt, amikor megindult a "kapilláris szivattyú" a normális pH-értéku, foleg Ca-hidrokarbonátos talajvízbol és a törmelékbol kioldott Ca++-ionok helyettesítették a szmektit köztes Na+-ionjait; ezek a felszínen különbözo, foleg karbonátos Na-sók alakjában kivirágoztak. Mikor beállt a csapadékos idoszak, ezek a sókiválások oldatba kerültek, a magas pH-jú oldat beszivárgott a talajba, a Na+-ionok helyettesítették a rétegközi Ca++-t, amely végül kicsapódott, kérgeket képezvén karbonátos és nem-karbonátos szemcséken a talajvíz szintje közelében. A folyamat, periodikus jellegébol adódóan számtalanszor megismétlodött, minek eredményeként a talajvíz felszínén egy sóoldat-párna keletkezett és a talajban a mészlerakódások egy többé-kevésbé összefüggo réteget képeztek. Ugyanakkor a felaprózódó agyagásványok szintjén általánosult a szerkezeti rendezetlenség. Mindezek alapján meg lehet állapítani, hogy a szikesedés kezdeti, evolutív vagy érett stádiumban van. Az apajpusztai és zabszéki mintavételi területek e szempontból a kifejlodés állapotában vannak. Ásványgenetikai szempontból a szikes területek valójában új ásványok bölcsojének tekinthetok. Itt jönnek létre a vízben oldható különbözo Na-sók, a Mg-tartalmú kalcit, a gipsz és itt esnek szét az agyagásványok, kolloid méretu és tulajdonságú rácsfoszlányokká, amelyekbol az üledékgyujtoben vagy a diagenezis során újból felépülnek a rendezett szerkezetu agyagásványok.

A unique high molecular weight protein (240,000 mol wt) has been purified from isolated desmosomes of bovine muzzle epidermis, using low-salt extraction at pH 9.5-10.5 and gel-filtration followed by calmodulin-affinity column chromatography. This protein was shown to bind to calmodulin in a Ca2+-dependent manner, so we called it desmocalmin here. Desmocalmin also bound to the reconstituted keratin filaments in vitro in the presence of Mg2+, but not to actin filaments. By use of the antibody raised against the purified desmocalmin, desmocalmin was shown by both immunoelectron and immunofluorescence microscopy to be localized at the desmosomal plaque just beneath the plasma membrane. Judging from its isoelectric point and antigenicity, desmocalmin was clearly distinct from desmoplakins I and II, which were identified in the desmosomal plaque by Mueller and Franke (1983, J. Mol. Biol., 163:647-671). In the low-angle, rotary-shadowing electron microscope, the desmocalmin molecules looked like flexible rods approximately 100-nm long consisting of two polypeptide chains lying side by side. The similar rodlike structures were clearly identified in the freeze-etch replica images of desmosomes. Taken together, these findings indicate that desmocalmin could function as a key protein responsible for the formation of desmosomes in a calmodulin-dependent manner (Trinkaus-Randall, V., and I.K. A drenagem ácida na mina de urânio de Caldas (MG) ocorre há mais de 20 anos, apresenta pH próximo de 3,0 e contém elementos metálicos e não-metálicos. Para o tratamento do descarte desse efluente, utiliza-se de cal para elevação do pH e precipitação dos contaminantes, gerando uma lama que é disposta na cava da mina, inundada de água ácida. O presente estudo consiste na caracterização de três amostras (Lama Antiga, Lama da Cava e Lama Fresca), utilizando análise térmica TGA/DTA, microssonda eletrônica e classificação de acordo com as normas NBR 10005 e NBR 10006. A caracterização enfocou a determinação das fases morfológicas e a composição elementar, para correlacioná-las com a capacidade de migração dos contaminantes. A etringita, a calcita e o gipso foram as fases identificadas como majoritárias. As lamas foram classificadas como não perigosas, mas não inertes. Estudos sobre a biodisponibilidade do urânio mostrou que este é facilmente solubilizado, pois a maior parte desse metal se encontra na fração lábil. A caracterização radiológica mostrou que todas as amostras apresentam valores abaixo do limite de isenção, de acordo com a Posição Regulatória PR-3.01/001 da Norma CNEN-NN-3.01.

The genetic-inspired particle swarm optimization (GIPSO) feature selection model efficiently discerns the relevant attribute from sophisticated financial databases, improving the model's discriminative power while alleviating dimensionality problems. Consequently, the SVNSTAO classifier leverages the features selected to discern complicated features inherent in fraudulent actions, which facilitates accurate diagnosis. Moreover, the COA parameter tuning mechanism enhances the SVNSTAO model's parameter, which ensures adaptability and optimum performance to varied fraud settings. Pacientų sveikatos mokymas turi įtakos atvykstan­čių pacientų srautų mažinimui priėmimo – skubiosios pagalbos skyriuje. Tyrimo tikslas – nustatyti pacientų sveikatos mokymo poreikį priėmimo – skubiosios pagalbos skyriuje. Tikslui pasiekti buvo taikytas kokybinis metodas, pusiau struk­tūruotas interviu. Interviu klausimai buvo sudaryti re­miantis išnagrinėta literatūra. Klausimynas slaugytojams sudarytas iš 18 klausimų, klausimynas pacientams – iš 14 klausimų. Tyrimas atliktas 2023 metais vasario – kovo mėnesiais. Tyrime dalyvavo 15 slaugytojų ir 13 pacientų. Slaugytojų interviu duomenimis, sveikatos mokymas priėmimo – skubiosios pagalbos skyriuje yra reikalingas, bet pirmumas teikiamas būtinajai medicinos pagalbai. Pacientui svarbu gauti sveikatos patarimų ir naudingos informacijos. Sveikatos mokymo būtinumą priėmimo – skubiosios pagalbos skyriuje lemia pacientų fiziniai, socialiniai ir psichologiniai poreikiai. Esant sunkiai paciento sveikatos būklei, prioritetas teikiamas būtinosios pagalbos teikimui. Pacientai tikisi greitų medicinos paslaugų ir greito svei­katos būklės pagerėjimo. Jie tikisi gauti aiškią informa­ciją apie sveikatos būklę, diagnozę, gydymo planą bei prognozę. Pacientams bendraujant su sveikatos priežiū­ros specialistais, svarbi jų empatija. Abstract Approximately 30-50% of Non-Small Cell Lung Cancer (NSCLC) patients already have CNS disease at time of diagnosis or else develop it during treatment, and there remains a need for better outcomes in these patients. The 3rd-generation covalent EGFR inhibitor osimertinib is an effective therapy for NSCLC patients, improving upon 1st-generation inhibitors gefitinib and erlotinib, and achieving enhanced brain penetration relative to those agents. However, the survival benefit relative to the 1st-generation inhibitors is observed primarily in patients with exon19 deletions rather than the L858R mutation. We developed a series of mutant-selective allosteric EGFR inhibitors that specifically target the L858R mutation (and subsequent resistance mutations), with high selectivity over wt EGFR and across the kinome. Early compounds including the isoindolinone JBJ-09-063 demonstrated good in vivo efficacy in mouse tumor models, despite limited bioavailability. Diversification and optimization of the chemistry, involving a scaffold hopping approach and the replacement of the thiazole amide with benzimidazole, delivered a set of compounds with promising potency and rodent PK. A subset of these achieved good brain exposure in mice and efficacy in an intracranial H1975 brain metastasis model. The mutant-selectivity and ability to co-bind with osimertinib makes an allosteric EGFR inhibitor an ideal partner for combination therapy, with the potential to deliver enhanced outcomes in L858R-driven NSCLC, especially for patients with CNS disease. Background Women who have experienced adolescent pregnancy and early motherhood are disproportionately affected in terms of their health and parenting capabilities, as well as their offspring’s health. Guided by Stress Process and Social Determinants of Health (SDH) Frameworks, which posit that multiple sources of stressors and structural determinants of adolescent pregnancy influence adolescent mothers’ subsequent health and quality of parenting (Xavier et al 2018, McLoyd 1998, Conger et al 2010, Gipson et al 2008). These dynamics then further impact offspring’s outcomes. Using an Integrated Stress-SDH Process for Health Disparities model and we test on whether early motherhood is associated with and subsequent maternal and child health from two informal settlements in Nairobi. Methods A cross-sectional design with 394 mothers of 2–16 years old children who sought maternal and child health services at Kariobangi and Kangemi public health centers between October 2015 to April 2016 were recruited. Participating mothers were asked questions related to their adolescent pregnancy history, their current health, wellbeing, and parenting practices, and their child’s health. Structural equation modeling (SEM) was utilized to examine hypothesized mediational pathways that adolescent pregnancy history has negative influences on women’s health and parenting during adulthood, which also influence their child’s health and development. Results Our study supports that women with a history of adolescent motherhood have poor physical and mental health outcomes as adults after adjusting for demographic confounders. SEM results partially support the Stress-SDH Process model that history of adolescent pregnancy had negative consequences on women’s adulthood health, which also negatively impacted offspring’s physical and mental health. Conclusion Consistent with the Stress Process and SDH literature, we found consistent cross-cultural literature that adolescent pregnancy set the stage for, subsequent poor maternal health and child outcomes. Although history of adolescent pregnancy and motherhood was not necessarily associated with negative parenting, consistent with parenting literature, negative parenting was associated with poor child mental health. Findings suggest importance of providing integrated care that address health and parenting needs to optimize offspring’s development in instances of early motherhood.

Abstract Outcome data from lung squamous cell carcinoma (SCC) show significantly lower overall and progression-free survival in Black patients. However, whether any genomic factors correlate with race and ancestry in this lung SCC, and whether this may contribute to disparities, has not been elucidated. We analyzed clinically annotated genomic data from 501 lung SCC tumor samples in the Cancer Genome Atlas (TCGA), with available ancestry admixture data. Here, we analyzed clinical data, gene expression profiles, mutations, and copy number changes to identify significantly differential events between tumors from patients of European (EUR) or African (AFR) ancestry. In tumors from AFR patients, we identified more frequent gain of chromosome arm 8q and/or the chr8q24 locus, where the oncogene MYC and its enhancers are located. 8q24 gain is associated both with global AFR ancestry and local AFR ancestry. Furthermore, expression of MYC targets is significantly increased in AFR samples relative to EUR samples, all of which suggest a role for the MYC pathway in differential cancer progression. MYC activity correlates with decreased immune infiltration and gene signaling, and patients with AFR ancestry are more likely to have decreased immune infiltrate and pathway activity. Our findings are consistent with observations in prostate cancer; this is the first study to find MYC enhancer amplification correlating with African ancestry in lung SCC. We are currently testing additional cohorts, including AACR GENIE, to validate these findings. Future studies will elucidate patient-specific treatment modalities. Etioloski faktori povređivanja su bili sledeći: saobraćaj 75 (38%), industrija i poljoprivreda 52 (26%), sport, padovi i ostalo 70 (36%) ispitanika. Sve prelome smo klasifikovali prema Gustilu. Prema ovoj klasifikaciji tipu I pripadalo je 34(17%), tipu II 85 ( 43%), tipu IIIA 48 (24%), tipu IIIB 20 (10%) i tipu IIIC 10 (5%) preloma. Rezulatati: Do potpune sanacije bez većih komplikacija došlo je u 120 (61 %) slučajeva dok je kod ostalih nastavak lečenja sledio intramedularnim klinom 33 (17%), aparatom po Ilizarovu 26 (13%) i Sarmientovim gipsom 18 (9%). Mekotkivna infekcija oko klinova se razvila kod 14 (7%) a infekcija u predelu rane kod 10 (5%) pacijenata. Nesrastanje i pseudoartroza su registrovani u 25 (13%) pacijenata, osteitis golenjače kod 16 (8%) a osteitis na mestu plasiranja klina kod 3 (1,5%) pacijenta. lzvršene su 2 (1 %) potkolene amputacije zbog teške konkvasantne povrede. Prosečno nošenje spoljnog fiksatora po Mitkoviću bilo je 23 (19-33) nedelje. Pun oslonac dozvoljavali smo u proseku nakon 9 i po nedelja. Zaključak: Preduslov za brže srastanje otvorenih preloma potkolenice uz što manje komplikacija je lečenje uz poštovanje svih principa primarne obrade rane, razumnu antibiotsku i antitetanusnu zastitu, pravilnu postavku spoljašnjeg fiksatora po Mitkoviću kao i stalnu kontrolu i eventualnu korekciju položaja fiksatora. Para avaliar o impacto das variações climáticas, de curta duração, sobre os ecossistemas de manguezais amazônicos, foi estudada a costa nordeste do Pará entre as coordenadas: 00º 32' 30" S / 00º 52' 30" S e 47º 28' 45" W / 47º 45' 00" W. Foram amostrados os sedimentos, águas superficiais e intersticiais com medidas de salinidade, potencial hidrogeniônico (pH), potencial de oxi-redução (Eh) e determinações mineralógicas por difração de raios X e microscopia eletrônica de varredura. Na água foram determinados o conteúdo de sulfetos dissolvidos, sulfato e cloreto, além da alcalinidade, sílica, ortofosfato, sódio, potássio, cálcio e magnésio, em meses chuvosos e de estiagem, sob marés de sizígia e quadratura. As variações sazonais do cloreto nas águas intersticiais mostram concentrações mais elevadas na estiagem que no período chuvoso, enquanto em superfície são mais elevadas nas marés de quadratura (período chuvoso) e de sizígia (período de estiagem). Os sulfetos dissolvidos foram encontrados em quantidades detectáveis somente a partir da profundidade de 10 cm o que indica exposição dos sedimentos aos fluxos advectivos de oxigênio atmosférico. Os teores de ferro dissolvido aumentam entre 0-10 cm e o potencial hidrogeniônico (pH) tende à neutralidade. A saturação das águas intersticiais na estiagem é indicada pelos minerais evaporíticos: gipso e halita. As variações pluviométricas são responsáveis por graduais mudanças nos teores de nutrientes e nas propriedades físico-químicas (pH, Eh e salinidade) das águas superficiais e intersticiais, no controle do equilíbrio salino das águas costeiras, na salinização e dessalinização dos sedimentos e na distribuição da vegetação de mangue no estuário. A exposição prolongada dos sedimentos na estiagem e as características morfológicas contribuem decisivamente para a oxidação total ou parcial dos sedimentos em superfície, o que modifica a mineralogia e as características químicas e físico-químicas das águas intersticiais.

A transmembrane extracellular matrix receptor of the integrin family, alpha 6 beta 4, is a component of the hemidesmosome, an adhesion complex of importance in epithelial cell-connective tissue attachment (Stepp, M. A., S. Spurr-Michaud, A. Tisdale, J. Elwell, and I. K. Gipson. 1990. Proc. Natl. Acad. Sci. USA. 87:8970-8974; Jones, J. C. R., M. A. Kurpakus, H. M. Cooper, and V. Quaranta. 1991. Cell Regulation. 2:427-438). Cytosolic components of hemidesmosomes include bullous pemphigoid (BP) antigens while extracellular components include a 125-kD component of anchoring filaments (CAF) and collagen type VII-containing anchoring fibrils. We have monitored the incorporation of the alpha 6 beta 4 integrins into forming hemidesmosomes in an in vitro wound-healing explant model. In epithelial cells recently migrated from the edges of unwounded sites over bare connective tissue, alpha 6 beta 4 first appears along the entire cell surface. At this stage, these cells contain little or no cytosolic hemidesmosomal components, at least as detectable by immunofluorescence using BP autoantibodies, whereas they are already positive for laminin and CAF. At a later stage, as cells become positive for cytosolic hemidesmosome components such as BP antigens as well as collagen type VII, alpha 6 beta 4 becomes concentrated along the basal pole of the epithelial cell where it abuts the connective tissue of the explant. Polyclonal antibodies to beta 4 do not interfere with the migration of epithelial cells in the explant. However, they prevent assembly of hemidesmosomal complexes and inhibit expression of collagen type VII in cells that have migrated over wound areas. In addition, they induce disruption of established hemidesmosomes in nonmigrating cells of the unwounded area of the explant. Monoclonal antibodies to alpha 6 have a more dramatic effect, since they completely detach epithelial cells in the unwounded area of the explant. Antibodies to CAF also detach epithelial cells in unwounded areas, apparently by inducing separation between epithelium and connective tissue at the lamina lucida of the basement membrane zone. These results suggest a model whereby polarization of alpha 6 beta 4 to the basal surface of the cells, perhaps induced by a putative anchoring filament-associated ligand, triggers assembly of hemidesmosome plaques.

Abstract Many translational studies suggest racial and ethnic survival disparities in lung squamous cell carcinoma (LSCC), with self-identified African-Americans showing significantly lower overall survival and progression-free survival than self-identified Caucasians. However, potential clinical and genomic factors contributing to this disparity have yet to be elucidated. Using clinically annotated genomic data from 501 LSCC tumor samples in the Cancer Genome Atlas (TCGA), we compared clinical data, gene expression profiles, mutations, and focal and arm-level copy number changes from tumors across ancestral groups to identify significantly differential events. Within this dataset, patients of AFR ancestry had worse survival and progression-free survival then patients of EUR ancestry. We applied the Cox Proportional Hazards model to predict overall and progression-free survival as functions of genetic ancestry alongside other genetic, molecular, and clinical variables. We found a significant difference in metastatic stage between tumors from patients of African (AFR) and European (EUR) ancestry, which was significant in predicting survival. Compared to tumors from EUR patients, tumors from AFR patients featured increased chromosome arm 8q gain and more frequent amplification of genetic locus chr8q24, which contains proto-oncogene MYC. Expression of MYC target genes was also significantly increased in AFR samples relative to EUR samples. These findings may suggest a role for the MYC pathway in differential cancer progression. We found more frequent amplification of genetic loci chr22q11 and chr16p11 in AFR samples compared to EUR samples. We identified top differentially mutated genes between groups, of which SPRR3, PDGFA, FRS3, and C21orf62 correlate with survival. We also found immune signaling gene sets enriched in the EUR group by gene set expression analysis (GSEA). These results suggest potential genetic and clinical factors implicated in racial survival disparities. Further studies and more diverse sampling of cancer patient populations are required to validate these differences and elucidate patient-specific treatment modalities. Further studies and more diverse sampling of cancer patient populations would allow validation of these differences and future elucidation of patient-specific treatment modalities. Bevezetés: Bizonyos gyermekkori törések minimálisan invazív műtéti megoldása a percutan Kirschner-drótos tűzés. Előnye, hogy kis műszerezettséget igényel, a drót eltávolítása egyszerű. Hátránya, hogy külső rögzítőt is igényel (gipsz, alumíniumsín). Célkitűzés: A dolgozat célja volt annak vizsgálata, hogy az epicutan (bőrfelszín felett) hagyott tűződrótok esetén alakul-e ki több szeptikus szövődmény, rövidebb-e az összes kórházi tartózkodás, fémkivételkor kisebb-e a narkózisok száma, mint subcutan hagyott drótok esetén. Módszer: A Heim Pál Országos Gyermekgyógyászati Intézetben 2017 és 2021 között megfigyelésen alapuló retrospektív kohorszvizsgálat történt. Beválogatási kritérium volt a 0–18 éves életkor, minden zárt, fedetten reponált törés, amelyet percutan tűzéssel rögzítettek. Kizárásra kerültek azok a betegek, akiknek egyidejűleg több műtéti ellátást igénylő vagy nyílt törésük volt, ahol többféle műtéti technika egyidejű alkalmazása történt, a nyílt repozíciók, ha frakcionáltan történt a dróteltávolítás, illetve ha hiányos volt a dokumentáció, vagy a beteg kezelése más intézetben indult/zárult. A betegek csoportosítása a drótok helyzete alapján történt (a drót bőrhöz való viszonya alapján epi- vagy subcutan). A vizsgált paraméterek közé tartozott a fémkivételnél alkalmazott érzéstelenítési mód, a kumulatív kórházi tartózkodás ideje, a szeptikus szövődmények aránya. Statisztikai analízisre a khi2-tesztet és a kétmintás t-próbát alkalmaztuk. Eredmények: A vizsgálatba 427 gyermeket vontunk be, átlagéletkoruk 10 (min.: 1, max.: 17) év volt. A drótok 138/427 esetben subcutan, 289/427 esetben epicutan helyzetűek voltak. Subcutan drótok esetén a kumulatív kórházi ápolási napok száma átlagosan 3,44 nap, epicutan esetben 1,92 nap volt (p = 0,0158). Narkózisra fémeltávolítás esetén subcutan drótoknál 33/138, epicutan drótoknál 19/289 esetben volt szükség (p<0,0001). Szeptikus szövődmény subcutan drótoknál 6/138, epicutan esetben 16/289 fordult elő (p = 0,6). Megbeszélés: Az epicutan drótok esetén rövidebb a kórházi tartózkodási idő, ritkábban van szükség narkózisra fémeltávolításkor, nincs több szeptikus szövődmény annak ellenére, hogy a bőrön kívül helyezkednek el. Következtetés: Az epicutan hagyott drótok alkalmazása előnyösnek tűnik: ritkábban igényel narkózist az eltávolításuk, így az összes kórházi ápolási nap száma alacsonyabb. Abstract There is growing interest in the use of human tumor primary cultures for the prediction of response to drugs and drug combinations in cancer medicine. While the field has witnessed numerous attempts to apply human tissue for the prediction of response, fundamental errors in methodologic approaches severely limited the predictive validity and clinical applicability of these important technologies. The first error was a focus upon drug induced cell growth inhibition. This lead to the failure of the clonogenic (human tumor stem cell) and H3*Thymidine incorporation platforms in the 1970’s and 1980’s. As the concept of apoptosis arose following landmark observations in 1972, cell death endpoints became predominant including mitochondrial function (MTT, XTT), ATP content (luciferase) and delayed loss of membrane integrity (DISC, EVA/PCD) among others. These improvements led to more biologically relevant measures. However, a second hurdle arose as investigators chose to amplify subculture and propagate tumors into what are now referred to as human tumor organoids. While these 3-D cultures are more reflective of tumor cell-cell interactions found in cancers, they cannot recreate the cell-vasculature, cell-stroma, inflammatory environment nor cancer associated fibroblast interactions with human cancer cells required for accurate response prediction. To address these shortcomings we developed the ex vivo analysis of programmed cell death (EVA/PCD) platform that applies native state tumor explants directly from surgical specimens to select drugs and combinations. To date we have reported 11,680 individual analyses. Positive prospective results have been reported in breast cancer (TTP p < 0.03); ovarian cancer (TTP p = 0.05), lung cancer (2-fold improvement in ORR; TTP p =0.035) and recently ovarian and uterine cancer (DFS p = 0.038) with two meta-analyses confirming the predictive validity in hematologic and solid tumors. With over 20 years of CLIA-approved laboratory experience in the study of human tumor primary culture explants, the EVA/PCD platform provides improved response and time to progression and offers the opportunity to streamline drug development, curtail costs and avoid futile care in patients with advanced and refractory malignancies. Al realizar una revisión, análisis e identificación de la arracacha (Arracacia xanthorrhiza Bancroft), algunos cronistas hacen referencia a aquellos alimentos y vegetales útiles, admitidos y apetecidos por los españoles o considerados alimentos asombrosos que describieron como de calidad y curiosidad exótica; en este caso inicialmente reportaron a la Arracacha esculenta, como una planta andina cultivada por los indios desde la más remota antigüedad, de la que se consume su raíz; historiadores hacen referencia a los muiscas, quienes cultivaban distintas clases de plantas, no es necesario hacer siembras propiamente dichas, se establece en huertos, en asocio; los indígenas muiscas la sembraban en lugares próximos a los de habitación y cumplieron un papel similar al quemodernamente presentan los cultivos de pancoger; tal es el caso de "las batatas" o del mismo "frijol". Esta tradición hoy en día conocida y estudiada como de economía campesina, contribuye a la seguridad alimentaria, el acceso a un alimento nutritivo y sano, este cultivo de importancia para muchas comunidades de pequeños productores de la región andina colombiana, entre ellas el departamento de Boyacá siendo el tercer productor después de Tolima y Norte de Santander. Para este trabajo de investigación, se partió de la necesidad de unificar e identificar el agroecosistema de arracacha Boyacá, a través de la aplicación de encuestas con un enfoque de investigación participativa y la observación directa. Este diagnóstico rural fue realizado con productores en los municipiosseleccionados y el Grupo de Investigación en Desarrollo y Producción Agraria Sostenible GIPSO de la Universidad Pedagógica y Tecnológica de Colombia; se describen los aspectos relevantes de este sistema de producción, a partir de la información recopilada sobre materiales cultivados, manejo agronómico y tecnologías locales de cultivo, iniciando desde el conocimiento tradicional de sus agricultores. Los resultados indicaron la existencia de más de diez materiales genéticos en Boyacá, (denominaciones locales), identificados con base en características morfológicas como forma y color de la raíz, color del follaje y el tallo. Es cultivada por pequeños agricultores en parcelas con áreas menores a 1 ha, adquiriendo a través de décadas, apreciables conocimientos en diferentes tópicos de producción.

Kutatásaink során tételesen megvizsgáltuk három tiszántúli mintaterület (Mikepércs, Hajdúsámson és Nyírbogát) erdős- és kontrollfúrásának felső egy méterén húsz cm-enként vett minták 0,1–0,2 mm-es homokfrakcióinak szemcséit, összesen 22.508 db szemcsét. Megállapítottuk a homokszemcsék ásványi összetételét, alakját és a felületükön látható bélyegeket. A vizsgálat célja az erdő- és kontrollterületi fúrásminták különbségeinek kimutatása, és a talajban végbemenő folyamatokkal történő összefüggésbe hozása.Mivel a területek földtani szempontból homogénnek tekinthetők, statisztikai módszerekkel vizsgáltuk, hogy a minták mennyiben hasonlóak fúrás és mintaterület szinten, és melyek azok a paraméterek, amelyek alapján az erdős és a kontrollminták elkülöníthetők.Az ásványi összetétel esetében összehasonlítottuk a kvarc/földpát részarányt (q/fp), a vas-hidroxidok és a gipsz mennyiségét. A szemcsemorfológia alapján a szögletes (éles, koptatásmentes) szemcsék részaránya, a szemcsefelület morfológiája esetében pedig a kioldás (korrózió), a gipszbevonatok és a szemcséket bevonó barna, limonitos/humuszos hártya gyakorisága hordoz információt a talajosodás során történő változásokról.Az erdővel huzamosan fedett területen az intenzívebb mállás miatt a q/fp arány nagyobb, mint a füves kontrollterületeken. A vas-hidroxid akkumuláció lényegesen nagyobb a folyamatosan tölgyerdővel borított területen, míg az akác- és a fiatal nyárerdő esetében csak a gyepterületek gyenge felszín közeli vasakkumulációja volt észlelhető. A tölgyes és a nyaras fás növényzet gyökérzónája alatt a szulfátok kismértékű feldúsulása észlelhető.A szemcsemorfológiailag éles-hegyes szemcsék az „in situ” felaprózódást jelzik, amelyek végig jelen vannak mind a hat fúrásmintában. A mikepércsi kontroll esetében pásztázó elektronmikroszkópos (SEM) vizsgálattal is valószínűsíthető a fagyhatás. A nyírbogáti 2. fúrás anyagában viszont feltételezhető, hogy a nyárerdővel való fedettség esetében a biológiai felaprózódás került előtérbe. A kontrollterületek fúrásaiban jól láthatók az eredeti (folyóvízi szállításra utaló) felületet felülíró korróziós kimarások, üregek, amelyek a hajdúsámsoni akácos 60 cm alatti mintájában is jelen vannak.Vizsgálatunk eredményeit összegezve megállapíthatjuk, hogy a homokos vázanyag részletekre kiterjedő vizsgálata felhívja a figyelmet a talajban végbemenő változásokra, ezen belül az erdős és a füves területek közötti különbségekre. Így az ilyen jellegű vizsgálatok jól kiegészítik a területekre vonatkozó cönológiai, talajtani és vízföldtani kutatásokat.Jelen munkát az OTKA NN79835 számú pályázata („Sekély talajvizű területen telepített ültetvények által a talajban és altalajban okozott sófelhalmozódás statisztikai és hidrológiai modellezése”) támogatta. Abstract:
A karbonátos szoloncsák szikesek megjavíthatóságának tanulmányozása céljából a Duna-völgy É-i felében lévo Szunyogpusztai Szikjavítási Telepünkön 33 éven át vizsgáltuk az 50 % CaSO4×2H2O-t tartalmazó gipsziszap növekedo adagjainak (20, 40 és 80 t/ha) talajjavító hatását. (A kísérlet elso 11 évében, Herke irányítása alatt elért eredményeket, könyvében ismertette (Herke, 1983). A kísérlet erosen meszes, homokos vályog összetételu, 65-75 cm szelvényvastagságú talajára a gipsziszapot a természetes gyep feltörése után 2/3 + 1/3 arányban megosztva, a kísérlet 1. és 4. évében szórtuk ki. A kísérletben szántóföldi növényeket termesztettünk, melyeket évente mutrágyázásban (50-90 kg N/ha + 60 kg P2O5/ha)és 3 esetben istállótrágyázásban (19, 19 és 32 t/ha) részesítettük. A 6 alkalommal végzett talajvizsgálatok eredményei alapján a következo megállapítások tehetok. A rendszeresen végzett, 18-20 cm mélységu talajmuvelés hatására a talaj megváltozott vízháztartása, nedvességdinamikája révén gipszezés nélkül is fokozatos és jelentos mértéku javulás következett be, elsosorban a muvelt talajrétegben. Különösen a sótartalom csökkenése volt nagyarányú. A gipsziszap adagjától függo mértéku és mélységu talajjavulást okozott, melyet a rendszeres talajmuvelés igen jelentosen elosegített. A javulás folyamata gipszezést követoen igen gyors volt, mely az évek során egyre jobban lelassulva, a gipszadagoktól függo ideig továbbfolytatódott. A 40, foként a 80 t/ha gipsziszapot kapott talajban a javulás még a 16. év után is folytatódott, foként az addig kevésbé megjavult mélyebben lévo talajrétegekben. Csak a 25-28 cm vastagságú talajréteg teljes megjavítására, a Herke-féle Na-érték alapján számított gipszadag (80 t/ha gipsziszap = 40 t/ha gipsz) idézett elo olyan mértéku talajjavulást, amely lehetové tette a szántóföldi növények eredményes termesztését. A javulás mértéke, a talajmuvelésnek köszönhetoen, a számítottnál sokkal nagyobb volt. A kísérlet 21. évében a talaj szelvényének pH-ja 8,11-8,82-re, szódában kifejezett lúgossága 0,02-0,13-ra, a Herke-féle Na-értéke 1,49-1,72 me/100 g-ra csökkent, míg a 20 órás kapilláris vízemelés nulláról 391-180 mm-re nott. A szántóföldi növények termése döntoen a talaj javulásának mértéke szerint alakult, tehát a gipszadagok növekedésével és az évek elorehaladtával nott, melyhez az agrotechnika színvonalának javulása is jelentosen hozzájárult. A 80 t/ha gipsziszappal javított talajon - az elso évek kivételével - a jó talajokon szokásos terméseredményeket értük el. A 33 éven át folytatott kísérlet eredményei bizonyítják, hogy a vékony szelvényu, homokos vályog összetételu szoloncsák szikesek kello mennyiségu gipsszel véglegesen, és jól megjavíthatók, melyeken megfelelo agrotechnikával jó talajokhoz hasonló termésátlagok érhetok el. A szikjavítás azonban nagyon költséges, ami nagyarányú alkalmazását nem teszi lehetové.
