<em>The corona pandemic has caused global problems, including Indonesia. One way to reduce the spread of the coronavirus is to wash your hands frequently with soap or hand sanitizer. Many types of hand sanitizers were developed to support efforts to prevent the spread of the corona. For this reason, this paper proposes two types of Contactless Hans Sanitizer (CHS) prototypes. Function tests and user satisfaction tests were carried out to determine the performance of the CHS. The test results show that type A CHS which only works fully electrically is more reliable than type B which works with electrical and mechanical systems. Of the 10 respondents, 90% chose CHS type A.</em>

Purpose. Congenital anomalies and injuries of the hand are often undertreated in low-middle income countries (LMICs). Humanitarian missions to LMICs are commonplace, but few exclusively hand surgery missions have been reported and none have attempted to demonstrate their cost-effectiveness. We present the first study evaluating the cost-effectiveness of a humanitarian hand surgery mission to Honduras as a method of reducing the global burden of surgically treatable disease. Methods. Data were collected from a hand surgery mission to San Pedro Sula, Honduras. Costs were estimated for local and volunteer services. The total burden of disease averted from patients receiving surgical reconstruction was derived using the previously described disability-adjusted life years (DALYs) system. Results. After adjusting for likelihood of disability associated with the diagnosis and likelihood of the surgery’s success, DALYs averted totaled 104.6. The total cost for the mission was $45,779 (USD). The cost per DALY averted was calculated to be $437.80 (USD), which is significantly below the accepted threshold of two times the per capita gross national income of Honduras. Conclusions. This hand surgery humanitarian mission trip to Honduras was found to be cost-effective. This model and analysis should help in guiding healthcare professionals to organize future plastic surgery humanitarian missions.

Peace missions often focus on statebuilding policies, being statebuilding a key aspect in these interventions. However, beyond or parallel to the democratisation process, questions regarding global justice arise. In this paper we are going to analyse the situation in Timor-Leste. We are going to argue that, beyond the classic democratisation approach, there are a number of factors that still have to be addressed, and these can be intrinsically related to democracy. We will conclude that the focus of the international interventions, especially within multi-dimensional missions and, therefore, the priorities of the host state, should go beyond the formal democratisation aspect. Social and economic factors, much related to the idea of global justice, should also be present and be a key aspect and result of these interventions.

The World Health Assembly designated 2020 as the Year of the Nurse and Midwife, and the slogan of this year’s 5th of May was “Nurses and Midwifes: Clean Care is in Your Hands”. This 5th of May helped us to highlight the need for increased staffing and hand hygiene training. In this paper we aimed to raise awareness about the global impact linked to adherence to proper hand hygiene practices by nurses and midwives.

Ignaz Semmelweis the father of hand hygiene made this startling discovery of strong association of contaminated hands with incidence of puerperal fever and deaths in 1847 and embarked on aggressive campaign to stem the tide of maternal deaths at that time and eventually succumbed to death due to septicaemia in 1865. At present about 171 years since this discovery, the entire humanity is yet to fully embrace the full practice of hand hygiene and reduce as much as half of the morbidities and mortalities from communicable diseases across the globe. Sub-Saharan Africa remains the last stronghold of millions of avoidable deaths from poor hand hygiene and 156 million people from Nigeria are yet to have access to facilities for hand hygiene. Facilities for hand hygiene should be adequately provided in all hospitals and clinics, schools and all settings experiencing assemblage of crowds while health education and advocacy is stepped. This will serve as a mark of honour to Ignaz Semmelweis, the man that paid the supreme price saving the lives of millions across the globe in defence of hand hygiene.

Background: Healthcare-associated infections (HAIs) are common and harmful to patients. Effective hand hygiene can help prevent HAIs, however, suboptimal healthcare worker hand hygiene remains problematic across the globe. This study analyses the impact of organisational changes on hand hygiene. Methods: This observational study assessed hand hygiene by different professions before and after a merger of a recently combined infectious diseases (ID) unit coupled with a qualitative study about barriers to optimal hand hygiene. Direct observations were compared with previous data collected on both units before they merged. We also conducted focus groups with the doctors and nurses about hand hygiene. Results: After two ID units merged in 2013, we observed 681 provider–patient interactions. We compared these with a previous observation period in 2012. Hand hygiene adherence among nurses significantly declined after the merger (from 36% to 24%, P <0.001). However, adherence among doctors increased from 51% to 63% after the merger ( P = 0.004). Data from the focus groups revealed a gap between doctor and nurses perceptions of education and goal adherence rates. Conclusions: Our findings underscore the important role played by effective unit leaders to prevent infection. We found long-term sustainability of hand hygiene practices among doctors. However, adherence among nurses was substantially lower.

Abstract:
The objectives were to (1) assess the prevalence of hand-washing practices across 80 countries and (2) assess frequency of hand-washing practice by economic status (country income and severe food insecurity), in a global representative sample of adolescents. Cross-sectional data from the Global School-based Student Health Survey 2003–2017 were analyzed. Data on age, sex, hand-washing practices in the past 30 days, and severe food insecurity (i.e., proxy of socioeconomic status) were self-reported. Multivariable logistic regression and meta-analysis with random effects based on country-wise estimates were conducted to assess associations. Adolescents (n = 209,584) aged 12–15 years [mean (SD) age 13.8 (1.0) years; 50.9% boys] were included in the analysis. Overall, the prevalence of hand-washing practices were as follows: never/rarely washing hands before eating (6.4%), after using toilet (5.6%), or with soap (8.8%). The prevalence of never/rarely washing hands after using the toilet (10.8%) or with soap (14.3%) was particularly high in low-income countries. Severe food insecurity was associated with 1.34 (95%CI = 1.25–1.43), 1.61 (95%CI = 1.50–1.73), and 1.44 (95%CI = 1.35–1.53) times higher odds for never/rarely washing hands before eating, after using the toilet, and with soap, respectively. A high prevalence of inadequate hand washing practices was reported, particularly in low-income countries and those with severe food insecurity. In light of the present COVID-19 pandemic and the rapid expansion being observed in low- and middle-income locations, interventions that disseminate good hand-washing practices are urgently required. Such interventions may also have cross-over benefits in relation to other poor sanitation-related diseases.

Aim: COVID-19 is a new illness and a big threat to global health. No specific antiviral agents are available for its treatment. The finest approach to manage this threat is to clean our hands properly. Washing hands under running water is a better way to stop the spread of infections than using a hand sanitizer. The aim of the study is to show that washing hands is efficacious than using a dab of alcohol-based hand sanitizer. Materials and Methods: A search was performed using three databases, PubMed, Scopus, and Google Scholar published from 2010 to 2019. The keywords for the survey were “hand-washing,” “hand sanitizer,” and “effective.” After the relevant articles were found, the critical appraisal was made to select those that were suitable for the systematic review. The inclusion criteria of the study were surveys, clinical studies, no sample size restrictions, and only English papers. Any conference abstracts, case reports, and unpublished data were excluded from this study. Results: On the basis of the keywords, 22 relevant articles were found, and of those, 17 articles were selected for the systematic review. Conclusion: This study evaluated that washing hands with soap and water are more practical and efficacious than using the sanitizer on greasy and soiled hands. An alcohol-based sanitizer, if used correctly and in appropriate volume, can disseminate certain type of micro-organisms.

This article addresses Pancasila as a text of interfaith dialogue in Indonesian context. The plurality of Indonesian society bears religious uniqueness in social life together, while conflicts may take place between religious groups. Religious conflicts are caused by attitude of exclusivism, radicalism, fundamentalism of religions and political identity. Pancasila can hold the sense collectivity among Indonesian society to be united. This article employs Gadamer hermeneutic theory in viewing historical texts. History is not something that only happened in the past and is interrupted with the present contexts. The theory of global ethics-Hans Kung, also helps the importance of dialogue with the religious communities to create religious peace. The article concludes that plural and multicultural society must live together in a dialogue circle. Pancasila as the text of interfaith dialogue is always understood as dynamic, open and relevant in the development of religious society in Indonesia. As the national identity, Pancasila has more than one conception as the lighters and guide toward each dialogue strategy that has been undertaken. The values of Pancasilamaintain inclusive religious attitude, respect to human values, and become a solid foundation in maintaining the unity of religious people.

Abstract Background Volume restoration using filler in the dorsum of the hand is a simple and effective procedure to improve wrinkles and hide veins and tendons. Currently, calcium-hydroxyapatite (CaHA) filler is the only FDA-accepted material to use in the hand dorsum. However, it is not easy to inject due to swelling and redness. In addition, hand anatomy through sonography is wrongly described in Plastic and Reconstructive Surgery® Journal. Through incorrect marking in sonographic hand anatomy, physicians will conduct erroneous procedures and surgeries. Objectives CaHA filler and hyaluronic acid (HA) filler were both injected into the hand to compare intra-individually the effect of each filler. Through this study, the author introduced detailed and refined sonographic anatomy. Overall, the author identified the correct injection method and depth. Methods In this prospective, intra-individual, comparative study, patients were injected 1 cc of CaHA filler to the left hand and 3 cc (36 mg) of microphasic hyaluronic acid to the right. Outcomes were assessed by Merz hand grading scale, skin bio-parameters, dermascopic finding, and Global Aesthetic Improvement Scale (GAIS) with follow-up at 3, 6, 9, and 12 months. Results The average GAIS score improved in both hands and generally maintained over the course of the study. There was significantly more bruising and swelling in the CaHA-injected hands compared with the HA-injected hands. The skin roughness and appearance improved in both sets of hands on dermascope. The average Trans-Epidermal-Water-Loss decreased and the average Stratum Corneum Hydration increased in HA-injected hands. The biopsy study showed that the HA particles lasted for 9 months. Conclusions The author reestablished sonographic anatomy. Veins and tendons became less apparent after injections. Level of Evidence: 2

From the end of the fifteenth to the start of the seventeenth century these globes show a remarkable diversity which is not observed either earlier or later, and the current paper relates this to the vibrant cosmological, astronomical and also geographical discussions and discoveries of the period. Also discussed is the influence that the theological topic of salvation may have had on the artistic appropriation of cosmographical ideas and their adaption in depictions of the Salvator Mundi’s globe.

Abstract:
Background: Brain connectivity has shown to be a key characteristic in the study of both Parkinson’s Disease (PD) and the response of the patients to the dopaminergic medication. Time series analysis has been used here for the first time to study brain connectivity changes during motor activation in PD. Methods: A 64-channel EEG signal was registered during unilateral motor activation and resting-state in 6 non-demented PD patients before and after the administration of levodopa and in 6 matched healthy controls. Spectral entropy correlation, coherence, and interhemispheric divergence differences among PD patients and controls were analyzed under the assumption of stationarity of the time series. Results: During the motor activation test, PD patients showed an increased correlation coefficient (both hands p < 0.001) and a remarkable increase in coherence in all frequency range compared to the generalized reduction observed in controls (both hands p < 0.001). The Kullback­–Leibler Divergence (KLD) of the Spectral Entropy between brain hemispheres was observed to increase in controls (right hand p = 0.01; left hand p = 0.015) and to decrease in PD patients (right hand p = 0.02; left hand p = 0.002) with motor activation. Conclusions: Our results suggest that the oscillatory activity of the different cortex areas within healthy brains is relatively independent of the rest. PD brains exhibit a stronger connectivity which grows during motor activation. The levodopa mitigates this anomalous performance.

This article discusses religious imperative in contributing to humanitarian problems, and is responsible for bringing about world peace. To make religion a source or principle to bring about peace, then it takes efort to dialog a religion with reality. In the case it will be discussed with a interreligious dialogue in the perspective of Hans Kung. According to Hans Kung, there is no world peace without the peace of religions. No peace of religions without a interreligious dialogue, and no interreligious dialogue without dives for the foundation of religions. The purpose of the interreligious dialogue according to Hans Kung is to reinvent religious souls so that it can be donated to various humanitarian problems. Hans Kung offered a constructive dialogue to built a consensus together with the purpose to create world peace. In the transition from the modern era to the postmodern era, Kung suggested a modern theological model called a critical ecumenical theology. Hans Kung also offers the importance of seeking a global ethic. Kung effort to set up a global ethic has a dual purpose, that is, to promote peace between religions and to “cure” the world that have a crisis of meaning, value, and norm. In addition, Kung also invited the religious people to make changes in the culture of the ko-existence to pro-existence. Kung also seeks to build a culture without violence. According to Hans Kung, the true religion is not only that does not conflict with humanity but also perfect humanity.

A Ginevra elaborň vaste critiche rivolte soprattutto alla Societŕ delle Nazioni e un proprio progetto per assicurare la pace che raccolse in diversi scritti poco conosciuti. Particolarmente interessante appare il progetto per la pace che Kelsen mise a confronto con la scienza giuridica e collocň nel contesto politico. In tale documento il giurista viennese si sofferma per la parte teorica sulla giustizia internazionale e per la parte pratica traccia le linee per un Tribunale Internazionale, teorizzando al contempo il postulato della obbligatorietŕ della giustizia internazionale. The concept of global education is based on the recognition of the world as a network of interdependencies and on realizing the fact that the current direction of the development of civilization is leading to environmental and humanitarian disaster. On the other hand, there is a belief that there is still a chance to change this state of affairs by means of education. The purpose of education is recognized as not only a transfer of knowledge, but also as a transfer of morals that are able to generate a shift in attitudes. In order to strengthen the impact of the global education project, it is necessary to provide a coherent description and axiological analysis of the values behind it. In the paper, we argue that ethicists should work on building philosophical arguments for the development and implementation of global education programs. We argue further that such a philosophical framework should be based on the ideas of new humanism (proposed by Aurelio Peccei) and global responsibility (formulated by Hans Jonas).

Aim: To d etermin e the safety and efficacy of narrow band ultravoilet B radiations in chronic hand eczema. Methods: 62 patients were enrolled from OPD of Dermatology Unit II, KEMU/ Mayo Hospital, Lahore. They weregiven phototherapy treatment through NB UVB local chamber, t hrice weekly (on fixed days) for a total of 12 weeks or until clearance. Patients were followed up fortnightly for further 1 month and final assessment was done at the end of one month. Physician’s Global Assessment (PGA) Score was used to assess the sever ity of eczema affecting the hands. Results The data was collected from 62 patients with 0% dropout. There was only one patient who had improvement <25%, 4(6.45%) had improvement 25 50%, 8(12.90%) had 51 75% improvement and 49(79.03%)cases had improvem ent >75%. Minimal side effects were seen which included erythema, photosensitivity, itching and pain. Conclusion : NB UBV is a safe as well as effective treatment option for chronic eczema of hands. Keywords : Chronic Hand Eczema, Narrow Band UVB, Physician’s Global Assessment Score.

Two contrasting views of handedness can be described as 1) complementary dominance, in which each hemisphere is specialized for different aspects of motor control, and 2) global dominance, in which the hemisphere contralateral to the dominant arm is specialized for all aspects of motor control. The present study sought to determine which motor lateralization hypothesis best predicts motor performance during common bilateral task of stabilizing an object (e.g., bread) with one hand while applying forces to the object (e.g., slicing) using the other hand. We designed an experimental equivalent of this task, performed in a virtual environment with the unseen arms supported by frictionless air-sleds. The hands were connected by a spring, and the task was to maintain the position of one hand while moving the other hand to a target. Thus the reaching hand was required to take account of the spring load to make smooth and accurate trajectories, while the stabilizer hand was required to impede the spring load to keep a constant position. Right-handed subjects performed two task sessions (right-hand reach and left-hand stabilize; left-hand reach and right-hand stabilize) with the order of the sessions counterbalanced between groups. Our results indicate a hand by task-component interaction such that the right hand showed straighter reaching performance whereas the left hand showed more stable holding performance. These findings provide support for the complementary dominance hypothesis and suggest that the specializations of each cerebral hemisphere for impedance and dynamic control mechanisms are expressed during bilateral interactive tasks. NEW & NOTEWORTHY We provide evidence for interlimb differences in bilateral coordination of reaching and stabilizing functions, demonstrating an advantage for the dominant and nondominant arms for distinct features of control. These results provide the first evidence for complementary specializations of each limb-hemisphere system for different aspects of control within the context of a complementary bilateral task.

<p><strong>Abstract:</strong> This article is aimed at introducing Hans Küng’s global<br />ethic and its contributions to world peace. Various crises which<br />have hit and are hitting the world motivated Küng to again<br />struggle to bring peace about and to establish a new, better life<br />order. By a global ethic – a basic consensus on binding values, noncancellable<br />criteria, and basic attitudes which are affirmed by all<br />religions, although dogmatically they are different, and which can<br />actually be given by non-believers – Küng emphasizes important<br />roles of religions without disregarding non-religious communities,<br />which are principally present to bring peace about as the main<br />characteristic of their teachings. Specifically, Küng’s opinion is<br />relevant in an effort to bring peace about in Indonesia, which is<br />very plural. Realizing that threats against peace and plurality in<br />Indonesia often come and result from interreligious conflicts, a<br />dialogue is absolutely necessary. As a concrete form of a global<br />ethic, Küng suggests a critical dialogue. Representatives of the<br />religions in Indonesia need to step forward and sit together in a<br />give-and-take dialogue which introduces the special aspect and<br />main direction of each of the religions. A bioética de V. R. Potter comemora em 2020 os seus primeiros 50 anos. O cientista preocupado com os avanços das ciências, sem o devido saber ético, propõe um diálogo entre as ciências com as humanidades, de modo a se alcançar a sabedoria necessária de como usar o conhecimento em vista do bem comum, a fim de assegurar o futuro da humanidade. A partir dessa ponte inicial entre as ciências com as humanidades, Potter mais tarde propõe outras pontes, em especial com as ciências biomédicas, pois identifica que a saúde humana depende do ambiente em que o indivíduo está inserido, o que ele classifica como bioética global. A obra O princípio responsabilidade, de Hans Jonas, em 2019, celebrou 40 anos. Embora Jonas não tenha utilizado a expressão bioética, o filósofo dedicou parte da sua vida no trabalho interdisciplinar no Hastings Center, para refletir sobre temas que hoje são do universo da bioética. Desse modo, essa reflexão quer saber: como a filosofia de Hans Jonas contribui na construção de um estatuto epistemológico à bioética global de V.R. Potter? Trata-se de uma pesquisa bibliográfica, de caráter narrativo, a partir das obras do cientista e do filósofo e seus comentadores. Conclui-se que embora Jonas não tenha utilizado da expressão bioética em sua reflexão, o filósofo não ignorou os problemas emergentes de seu tempo, porém classificou-os dentro do universo da ética prática. Assim sendo, sua produção filosófica, construída principalmente a partir da prática interdisciplinar de trabalho, constitui um verdadeiro estatuto epistemológico à bioética, em busca de fundamentos éticos para proteger a vida humana e extra-humana no futuro, o que faz do filósofo, um verdadeiro bioeticista.

As a step toward understanding the developmental relationship between handedness and language lateralization, this longitudinal study investigated how infants (N = 21) move their hands in noncommunicative and communicative situations at 2 weeks and at 3 months of age. The authors looked at whether left-right asymmetry in hand movements and in duration of self-touch appeared across conditions and whether the direction of asymmetry depended on the communicative nature of the situation. The authors found that asymmetries appeared less consistently than suggested in literature and did not only depend on the communicative nature of the situation. Instead, hand activity and self-touch patterns depended on age, the presence of the mother, the degree of novelty of the situation, and the presence of an object. The results partly support previous studies that pointed out an early differentiation of communicative hand movements versus noncommunicative ones in infants. It is in terms of the amount of global hand activity, rather than in those of the laterality of hand movements that this differentiation emerged in this study. At 3 months, infants moved their hands more in the communicative conditions than in the noncommunicative conditions and this difference appeared as a tendency already at 2 weeks of age.
