Abstract:
Optimizing tissue perfusion by administering intravenous fluids presents a special challenge to the intensive care unit (ICU) clinician. Recent studies have drastically altered how we assess a patient’s fluid responsiveness, particularly with regard to upstream surrogates of tissue perfusion. Central venous pressure and pulmonary capillary wedge pressure have been found to be inaccurate markers of fluid responsiveness and have given way to methods such as cardiac output as assessed by echocardiography and the various forms of arterial waveform analysis. These newer techniques, such as stroke volume variation, systolic pressure variation, and pulse pressure variation, have been found to better delineate which patients will respond to a fluid challenge with an increase in cardiac output, and which will not. In addition, traditional methods of assessing the consequences of excessive fluid administration, such as pulmonary oedema and the non-anion gap acidosis of saline administration, have given way to more sophisticated measurements of extravascular lung water, now available at the bedside. Downstream markers of tissue perfusion, such as base deficit, central venous oxygen saturations, and lactic acid, continue to be useful in particular clinical settings, but are all relatively non-specific markers, and are therefore difficult to use as resuscitation targets for ICU patients in general. Finally, recent data on septic shock and ARDS have demonstrated the importance of conservative fluid strategies, while data in surgical populations have emphasized the need for judicious fluid administration and attention to the balance of blood products used in resuscitation efforts.

The topics discussed are all related to basic fluid mechanics. In these introductory notes I highlight some of the main features of fluid flows and their mathematical characterization. There is much physical intuition encapsulated in the differential equations, and one of our goals is to gain more experience (i) understanding the governing equations and various related principles of kinematics, (ii) developing intuition with approximating the equations, (iii) applying the principles to a wide range of problems, which includes (iv) being able to rationalize scaling laws and quantitative trends, often without having a detailed solution in hand. Where possible we provide examples of the ideas with ‘soft interfaces’ in mind.

This chapter explores beliefs about the importance of marriage. The authors explain how most emerging adults no longer think marriage is a needed institution but struggle describing why or what society would do without it. Marital importance can be divided into three broad categories of beliefs: an individual’s beliefs about the importance of marriage for society, beliefs about the specific importance of marriage in the individual’s own life, and the relative importance of marriage compared with other life goals. Most emerging adults believe that marriage is important, but they vary in the relative importance they place on marriage. When marriage is pitted against education and career importance, significant variation exists. The authors describe how many emerging adults now prioritize career success over marriage during their 20s.

This concluding chapter reviews the basic goods approach, its intellectual sources, and its practical importance. It emphasizes the role of basic goods as basic rights and connects these basic subsistence rights to the tradition of moral minimalism in political philosophy. It questions the claims of technological optimists and calls for a “rational pessimism” in the setting of global policy priorities. It also proposes a set of ten basic development goals to replace the current and often criticized sustainable development goals. It considers the feasibility of basic goods provision using back-of-the-envelope calculations, concluding that the goal of universal basic goods provision is indeed feasible.

Goal Focused Positive Psychotherapy presents the first comprehensive positive psychology psychotherapy model that optimizes well-being and thereby diminishes psychological distress. The theory of change is the Broaden-and-Build Theory of positive emotions. The therapeutic process promotes client strengths, hope, positive emotions, and goals. The book provides the foundational premises, empirical support, theory, therapeutic techniques and interventions, a training model, case examples, and future directions. A three-year study is presented that reveals that Goal Focused Positive Psychotherapy (GFPP) was as effective as cognitive-behavioral therapy and short-term psychodynamic therapies, which fits the meta-analyses of therapy outcome studies that no bona fide psychotherapy achieves superior outcome. However, GFPP was significantly more attractive to the clients. Descriptions are provided of the Broaden-and-Build Theory, therapy goals based upon clients’ values and personal meaning (i.e., approach goals and intrinsic goals), identification and use of clients’ personal strengths (including client culture), centrality of hope and hope theory, the implicit theory of personal change or the growth mindset, and finally Self-Determination Theory. The techniques and interventions of GFPP as well as the importance of the therapist’s intentions during therapy are presented. GFPP focuses upon the client and relationship while not viewing psychotherapy as a set of potent scripted treatments that acts upon the client. Goal Focused Positive Supervision is presented as a new model that supports the supervisee’s strength-based self-definition rather than a pathological one or deficit orientation. Training that includes the experiential learning of GFPP principles is underscored.

Because job search often is a lengthy process accompanied by complexities, disruptions, rejections, and other adversities, job seekers need self-regulation to initiate and maintain job search behaviors for obtaining employment goals. This chapter reviews goal/intention properties (e.g., specificity, proximity, conflicts, motivation type) and skills, beliefs, strategies, and capacities (e.g., self-monitoring skills and type, trait and momentary self-control capacity, nonlimited willpower beliefs, implementation intentions, goal-shielding and goal maintenance strategies) that facilitate self-regulation and as such may moderate the relationship between job search intentions and job search behavior. For each moderator, a theoretical rationale is developed based on self-regulation theory linked to the theory of planned job search behavior, available empirical support is reviewed, and future research recommendations are provided. The importance of irrationality and nonconscious processes is discussed; examples are given of hypoegoic self-regulation strategies that reduce the need for deliberate self-regulation and conscious control by automatizing job search behaviors.

An important feature of spiritual formation involves finding reliable processes, people, practices, and materials that put one in the best position to cultivate the spiritual life and to achieve the specified goals (e.g. purity of heart, discernment, and the vision of God). Accordingly, this chapter draws attention to some works that connect philosophy (including some epistemological themes and issues) and spirituality (including the topic of spiritual formation). It also spells out some of the main features of John Cassian’s (c.360–435 ce) account of spiritual formation. In particular, it focuses on his understanding of the penultimate and ultimate goals of spiritual formation, of the importance of relying on others for making epistemic progress in the spiritual life, and of the three indicators that help determine whether a person is a reliable spiritual authority. The chapter concludes by identifying briefly three areas that deserve further epistemological attention.

This chapter on “Treatment” examines the roles of evidence-based medicine and empirical evidence, including systematic reviews and therapeutic guidelines. The two ultimate goals of treatment are emphasized: to treat the disease and to treat the patient. How the neurologist can successfully structure treatment toward these goals is of paramount importance. The use of published criteria, randomized controlled studies, and trial-and-error in the selection of appropriate treatment are discussed, as well as the pitfalls of relying too heavily on statistical significance. The costs versus benefits of treatments and the issue of noncompliance are discussed, along with the neurologist’s interaction with the neurosurgical patient. Above all, the provision of reassurance is emphasized.

The importance of classical sources and a social approach to understanding questions of international security is developed in this chapter, alongside the concepts of trust and deterrence, all important in Freedman’s scholarly world, especially his Strategy: A History. The study begins with Sophocles and Plato, linking trust to friendship, and viewing both trust and society as co-constitutive phenomena. While deterrence and realist models of international relations assume that trust is in short supply, following the great figures of ancient Greece, in reality, trust will be more widespread the more robust the society is. The chapter explores the importance of honor and reputation. It concludes, after Plato, that trust comes the demonstrable willingness to do things for friends that have nothing to do with one’s own goals. Treating others as friends builds security through co-constitutive interaction. This applies to societies and states, as well as individuals.

Chapter 5, the book’s conclusion, draws comparative theoretical lessons from all three cases. It discusses six features of relationships between frenemies: risks to participants’ reputation; reliance on hybrid or compromise frames or goals; focus on single-issue or specific goals; the importance of emotional and personal narratives; lack of more extensive collaboration or institutionalization of the relationships; and outcomes that depend on the relative power of participants. The chapter discusses implications for ongoing policy regarding sex offenders, sex trafficking, and government surveillance. The paths of activism around the case studies have influenced recent issues of sexual assault, including in the military, in colleges and universities. Feminists have influenced these developments, but not alone. Frenemies, including both feminists and conservatives, continue to be engaged in these issues and to shape their paths.

This chapter reinterprets the history of twentieth-century US feminism by foregrounding the importance of the global. Both international events and transnational flows of people, ideas, and goods have shaped the development of feminism in the United States. Recognizing the importance of the global foregrounds the diversity of political goals and political actors within movements for gender equality. Also, acknowledging US feminists’ engagement with the global reinforces the need for new narratives and periodizations for the multiple histories of US feminisms. To explore these ideas, this chapter first analyzes definitions of feminism and existing historical narratives of US feminism. The second half examines the significance of the global for feminist movements seeking political equality, economic justice, and sexual liberation.

Zoltán Kodály (1882–1967), one of the foremost figures of Hungarian culture and choral music in the twentieth century, laid the foundations of a new music pedagogical approach during the times immediately preceding and following the years of World War II. His concept of music education can be summarized into two important goals: (1) to draw more people near to classical musical art while developing the necessary skills in them for its in-depth understanding and reception, (2) to create opportunities from these precious musical experiences for the shaping of personality and the creation of valuable community bonds. Kodály Zoltán’s art was centered around choral music; singing and choral singing are of paramount importance in his educational philosophy. According to him singing was the most important tool for the development of musical literacy. And choral singing was a gateway to life-long inspiration received from the performance of masterpieces.

Anesthetic techniques for procedures performed outside the traditional operating room are varied. General anesthesia, sedation, and regional anesthesia can all be delivered in this venue. The choice of technique is based on safety considerations and patient comorbidities. Perioperative monitoring such as pulse oximetry, end-tidal carbon dioxide monitoring, and electrocardiography and blood pressure monitoring protocols must be consistent with American Society of Anesthesiologists guidelines. Common procedures include elective office-based anesthetics, emergency room sedations, endoscopic retrograde cholangiopancreatographies in the gastroenterology suite, and minimally invasive interventions in the radiology department. Because most of these locations have limited postanesthesia care unit capabilities, the patient’s rapid return to baseline functioning and the ability to be discharged quickly, safely, and comfortably are important goals. Thus, anesthetic technique and the pharmacokinetics and pharmacodynamics of the anesthetics, analgesics, antiemetics, and local anesthetics are of utmost importance.

The United Nations Sustainable Development Goals initiative has the potential to set the direction for a future world that works for everyone. Approved by 193 United Nations member countries in September 2016 to help guide global and national development policies in the period to 2030, the 17 goals build on the successes of the Millennium Development Goals, but also include new priority areas, such as climate change, economic inequality, innovation, sustainable consumption, peace and justice. Assessed against common agreed targets and indicators, the goals should facilitate inter-governmental cooperation and the development of regional and even global development strategies. However, each goal presents considerable challenges in terms of collecting and analysing relevant data and producing the statistics needed to measure progress. Most governments in lower resourced countries simply do not yet have the systems and controls in place to produce high quality, reliable data and statistics, and it is questionable whether the quality and integrity of the available information is adequate to support meaningful decisions and set direction for the future. There are substantial implications: where progress cannot be measured accurately because of inadequate or flawed statistics, the result can be misguided decisions, doubts about achievement of the goals and significant wasted resources. Getting statistics ‘right’ depends upon the quality and integrity of the data used to produce them and on the quality of the processes for collecting, manipulating and analysing the data. Without a documentary records as evidence of how the data were gathered and analysed or how statistics were produced and disseminated, it is not possible to confirm that the statistics are complete, accurate and relevant. Various global organisations do recognise the importance of high quality data and statistics for measuring the SDG indicators reliably, but there has been little attention to the role of records in providing the evidence needed to trust the data and statistics. There is, moreover, a lack of awareness that digital information simply will not survive without policies and procedures to manage and preserve it through time. As a result, digital data, statistics and records are being lost regularly on a large scale, particularly in lower resource countries, where the structures needed to protect and preserve them are not yet in place. This book explores, through a series of case studies, the substantial challenges for assembling reliable data and statistics to address pressing development challenges, particularly in Africa. Hopefully, by highlighting the enormous potential value of creating and using high quality data, statistics and records as an interconnected resource and describing how this can be achieved, the book will contribute to defining meaningful and realistic global and national development policies in the critical period to 2030.

Richard Thaler and Cass Sunstein argue for “libertarian paternalism,” defined as the strategy to devise policy that will “maintain or increase freedom of choice” and at the same time “influence people’s behavior in order to make their lives longer, healthier, and better”. These two goals are often in conflict, and striking the right balance between them has proved difficult in both theory and practice. Where does Adam Smith fall in this debate? This chapter argues that Smith developed his own version of “libertarian paternalism.” It differs in important ways from that of Thaler and Sunstein, but it shares with them an attempt to balance respect for individual autonomy with a desire to help people lead better lives. Smith’s position accommodates the importance of both liberty and paternalism in enabling individuals to construct lives worth living, while avoiding some of the problems that have beset more recent versions of libertarian paternalism.

Abstract:
This chapter analyses the case of state-owned financial entities in Peru and argues that their development banking activities have decreased in importance as instruments of public policy since the structural reforms implemented in the early 1990s. Like all other state enterprises, they have since been subject to discriminatory legal regulation. The four state financial companies analysed have objectives and goals that are not integrated with one other, duplicate their efforts, and do not take advantage of the possible synergies to be obtained. Therefore, none of these institutions constitutes an effective state development bank such as exists in other countries.

This chapter summarizes what is new or edifying about the interpretive turn, and why this approach matters. It restates the case for drawing on the genres of thought and presentation common in the humanities, making the bold, contentious claim that the interpretive approach encourages creativity because puzzle-solving encourages imagination, even intuition, and a search for new connections. It replies to some of the more common criticisms of the interpretive approach, revisiting the claims that the author caricatures the political science discipline in Britain; accords too much importance to agency and not enough to structure; ignores the dominant political tradition in Britain; and omits institutions. Finally, it looks at the future of interpretivism in political science, focusing on the aesthetics of political science; and the importance of a tradition of scholarship free from the demands of relevance and impact. It argues that empathy, enabling conversations and edification, are equally worthy goals.

This chapter presents an overview of contemporary Indian Buddhism, broadly conceived, highlighting several historical developments, transregional influences, and Indo-centric adaptations within the colonial and postcolonial context. As the “homeland” of Buddhism and central to various contemporary revitalization movements, two themes are of particular analytical importance to this chapter: the recovery and reconfiguration of Buddhist material objects and the importance of reinvention among a range of Western and Asian Buddhist actors. After situating Indian Buddhism within the context of Indian historiography and discussions around the decline of Buddhism, this chapter examines various ways Indian Buddhist sites, artifacts, and structures are reimagined and reconfigured under colonization, nation-building, and changing socioeconomic interests. Also covered are Buddhist movements within India such as the Ambedkar-inspired New Buddhism, the role of Tibetan Buddhist refugees, and how the valorization of India’s Buddhist pilgrimage geography intersects with state goals toward tourism development and heritage diplomacy in Asia.

This chapter outlines the structure and critical elements of facilitator-led role play sessions. Elements include the importance of structuring the role play; recognizing the importance of established learning goals as expressed by each learner; navigating and directing the simulated patient (SP) to partake in a realistic clinical encounter relevant to the learner; managing an effective role play session with learner and SP; and providing meaningful feedback both directly to the learner, as well as overseeing balanced group feedback provided by observing peers in the small group. This chapter also provides helpful processes in training new and sustaining competent facilitators, keeping in mind available resources for training. Finally, this chapter points to necessary areas for continued development in facilitation.
