Interstitial granulomatous drug reaction (IGDR) is a distinct inflammatory process that often presents as annular, violaceous plaques distributed on the extremities, proximal trunk, and intertriginous areas. The list of drugs implicated for inciting IGDR is growing, but most prominently includes ACE inhibitors, antihistamines, beta-blockers, antidepressants, and anticonvulsants. Ustekinumab is a human monoclonal antibody that targets inflammatory cytokines IL-12 and IL-23 and has been have shown to be effective in treating IGDR. However, we present a case that suggests ustekinumab can also act as an inciting agent for IGDR.

Red pigment is associated with a high sensitizing potential and is the most frequently implicated pigment inducing various types of histological reactions. Herein, we describe a patient with red tattoo pigment-induced granulomatous dermatitis that histologically revealed a very rare granuloma annulare–like reaction.

Bosutinib is a BCR-ABL tyrosine kinase inhibitor approved for the treatment of Philadelphia chromosome–positive chronic myelogenous leukemia. We report a case of bosutinib-induced interstitial granulomatous drug reaction in a 50-year-old Caucasian female with chronic myelogenous leukemia. Histologic analysis of a punch biopsy showed diffuse interstitial granulomatous infiltrates consisting of histiocytes amid thickened collagen accompanied by eosinophils. Her lesions improved with clobetasol 0.05% cream. No cases describing BCR-ABL tyrosine kinase inhibitor–associated interstitial granulomatous drug reaction were found in a search of the literature. It is important for physicians to be aware of the risk of interstitial granulomatous drug reaction associated with bosutinib treatment.

<p class="abstract"><strong>Background:</strong> Tattooing is nowadays very common practice especially among young people and we are witnessing a gradual increase of numerous potential reactions to tattoos. Purpose of this study was to identify the various tattoo reactions and to correlate it with histopathological examination and early interventions to decrease further morbidity.</p><p class="abstract"><strong>Methods:</strong> From December 2017 to September 2019, patients reporting with reactions due to tattooing were included in the present study after obtaining informed consent. A detailed history regarding the onset, duration and colour used for tattooing were collected and noted. Cutaneous examination and biopsy were done in all cases to know the type of reaction.<strong></strong></p><p class="abstract"><strong>Results:</strong> The analysis included 50 patients who had tattoo reactions. The most common age group affected was 16-30 years, with slight male predominance. Most of the cases (64%) with tattoo reaction presented to us within 1 to 4 months of disease presentation. Clinically, most cases had granulomatous reaction 23 (46%). On histopathology, granulomatous reaction was the most common type observed. Red colour dye was the most common colour associated with reaction seen. Clinico-histopathological correlation of various types of tattoo reaction revealed consistency with diagnosis in 17 (34%) patients, clinically as well as on histopathological examination.</p><p class="abstract"><strong>Conclusions:</strong> Lack of strict regulations by government and increased fashion trends increase complications associated with tattoos. Red inks are the most frequently associated with tattoo reactions. Granulomatous type reactions are a frequently reported pattern of inflammation seen in tattoo.</p>

The granulomatous cheilitis (GC) presents a heterogeneous group of disorders characterised by a granulomatous inflammation/reaction of the lips to various stimuli. Numerous etiologies have been proposed, including genetic, immunologic, allergic and infectious. Among the secondary causes of GC, an infection by Mycobacterium tuberculosis (MBT) should be considered. In such cases, the GC could be the clinical presentation of a tuberculid resulting from a hypersensitivity reaction to an underlying focus of active (ATBI) or latent tuberculosis infection (LTBI). This communication describes an immunocompetent patient diagnosed with GC resulting from tuberculid, who responded well to Isoniazid monotherapy.
