Abstract:
ObjectivesAdults with congenital heart disease in need of heart surgery frequently present with significant comorbidity. Furthermore, additional technical difficulties often related to redo operations increase the risk for postoperative mortality and morbidity. Hence, next to the type of the procedure, additional procedure-dependent and procedure-independent factors have to be considered for risk evaluation. The recently proposed grown-ups with congenital heart disease (GUCH) mortality and morbidity scores account for these additional risk factors. We sought to validate their predictive power in a large population operated in a single centre.MethodsData of all consecutive patients aged 18 years or more, who underwent surgery for congenital heart disease between 2005 and 2016, were collected. Mortality was defined as hospital mortality or mortality within 30 days following surgery. Morbidity was defined as occurrence of one or more of the following complications: renal failure requiring dialysis, neurologic deficit persisting at discharge, atrioventricular block requiring permanent pacemaker implantation, mechanical circulatory support, phrenic nerve injury and unplanned reoperation. The discriminatory power of the GUCH scores was assessed using the area under the receiver operating characteristics curve (c-index, including 95% CI).ResultsEight hundred and twenty-four operations were evaluated. Additional procedure-dependent and procedure-independent factors, as defined in the GUCH scores, were present in 165 patients (20.0%) and 544 patients (66.0%), respectively. Hospital mortality and morbidity was 3.4% and 10.0%, respectively. C-index for GUCH mortality score was 0.809 (0.742–0.877). C-index for GUCH morbidity score was 0.676 (0.619–0.734).ConclusionsWe could confirm the good predictive power of the GUCH mortality score for postoperative mortality in a large population of adults with congenital heart disease.

Abstract:
Grown-up congenital heart (GUCH) is a global challenge nowadays. The strategy of medical care for GUCH women dur-ing pregnancy, childbirth and the postpartum period is a topic of active discussion in the expert community. These patients have significantly increased risk of maternal and perinatal loss. A national obstetric cardiology and cardiac surgery multi-disciplinary team (OCCS) established in academic institutions in 2013 has provided medical support to 896 GUCH pregnant women over the last 7 years. In total, GUCH patients accounted for 36% of the cohort of all the examined pregnant women. Of these, 474 (53%) were primigravid. The mean age of the patients at the time of the first visit was 27.3 ± 5.7 years. Patients with uncorrected CHD accounted for 66.2% (n = 593), and 33.8% (n = 303) of pregnant women had undergone correction, including hemodynamic correction of complex CHD in 5 patients. Risk stratification was performed using several scores (mWHO, ZAHARA, CARPREG) for the comprehensive assessment of cardiovascular risk and prediction of pregnancy, deliv-ery, and postpartum period course. 82 patients were classified as having high cardiovascular risk (CVR) after the stratifica-tion. They needed admission to the cardiac surgery facility to receive different types of medical care. There were 2 (2.4%) cases of maternal loss and 3 (3.8%) cases of adverse perinatal outcomes in this group of patients. The article presents the algorithms for multidisciplinary care strategy choice in GUCH pregnant women with high CVR and their routing principles developed by the OCCS. These algorithms significantly reduced adverse outcomes of pregnancy and childbirth in this group of patients. Long-term results were evaluated in 69 patients (86.3%). The follow-up period ranged from 1 to 91 months, on average 34.4 ± 23.6 months. There were no long-term maternal losses or repeated cardiac surgeries. There was one case of unexplained death of a child 8 months after birth. The strategy of multidisciplinary medical care of a high-class GUCH pregnant woman should be personalized depending on the clinical data and in accordance with the ESC 2018 guidelines.

ObjectivesTo provide the first epidemiological lifestyle descriptions of the Italian grown-up/adult congenital heart disease (GUCH/ACHD) population by identifying the determinants of poor perceived health status.DesignCross-sectional pan-national survey.SettingItalian GUCH/ACHD patients who were members of the Italian Association of GUCH/ACHD.Primary and secondary outcome measuresTo discuss these lifestyle descriptions through an ad hoc developed questionnaire and health perceptions (ie, mental and physical health perception) through a short form health survey (SF-12).Results629 patients included; many investigated GUCH/ACHD lifestyles were determined similar to those of the general population — with the exception of the smoking habits, which were lower. The odds of the occurrence of inadequate physical health perceptions increased by more than two times in patients undergoing antiarrhythmic therapies (OR adjusted=2.045; 95% CI=1.201 to 3.479; p=0.008; n=629), more than 1.5 times in patients taking oral anticoagulants (OR adjusted=1.638; 95% CI=1.038 to 2.585; p=0.034; n=629) and roughly 1.7 times in patients treated with antiplatelets (OR adjusted=1.743; 95% CI=1.024 to 2.966; p=0.041; n=629). The odds of the occurrence of inadequate mental health perceptions increased by 1.7% for every year that the patients aged (OR adjusted=1.017; 95% CI=1.002 to 1.032; p=0.025; n=629).ConclusionParticular attention should be paid to these ageing patients’ increasing psychological needs, and additional research is needed to identify associations between their lifestyles and clinical outcomes.

Background: Grown-up congenital heart disease (GUCH) patients are unique and challenges especially at developing country. The numbers of diagnostic as well as interventional cardiac catheterization procedures in GUCH patients are growing. The aim of this study was to report the outcome of cardiac catheterization including intervention procedure in GUCH.Methods: The descriptive study was conducted at Sanglah Hospital, Denpasar, Bali, Indonesia. All patients (age of more than 12 years) who underwent cardiac catheterization from 2011 until 2017 were included in this study. Patients, characteristic, types of catheter procedures, immediate complications, and outcomes were documented.Results: A total 54 subjects were included with median age of 23 years and 70% were female. The first symptom that brought patients to hospital is dyspnea 46% and palpitation 32%. Five subjects underwent a diagnostic catheter procedure and 49 (91%) diagnostic and catheter based interventions. Transcatheter interventions procedures included atrial septal defect (ASD) (success rate of 20 per 21), patent ductus arteriosus (PDA) (success rate of 16 per16), ventricular septal defect (VSD) (success rate of 9 per 9), pulmonal stenosis (PS) (success rate of 1 per 2), and aortic stenosis (AS) (success rate of 1 per 1). The complications encountered were transient dysrhythmias in 15 subjects, device embolization in 4 subjects, massive bleeding in 1 subject, and overall mortality in 2 subjects.Conclusion: The number of the catheterization interventions in GUCH was 91% and ASD device closure was the most common procedure. Transcatheter intervention has a high procedural success rates (96%) and low procedural-related complications.

Bakgrund: I Sverige föds cirka 800 barn med hjärtfel varje år. Framstegen inom barnkardiologi och kirurgi har resulterat i att fler uppnår vuxen ålder än tidigare. Detta medför en växande patientgrupp, så kallade grown- up congenital heart disease (GUCH) patienter. Att leva med hjärtfel kan påverka det dagliga livet, såväl fysiskt som psykiskt. Syfte: Studien syftade till att undersöka vuxna patienters upplevelser av att leva med GUCH. Metod: En integrativ litteraturöversikt användes genomgående i uppsatsen. Sökningarna utfördes i databaserna Cinahl, PsychINFO, PubMed och SveMed+. Resultat: Fyra teman identifierades. Huvudfynden visade att patienter kände sig begränsade i sitt dagliga liv i varierande utsträckning. Oro, ångest och depression var upplevelser som de ställdes inför. De uttryckte även en känsla av att vården förbisåg viktiga delar av deras upplevelse. Slutsats: GUCH-patienter är en växande grupp som behöver uppföljning och vård livet ut. Kunskap om att leva med GUCH behövs för att kunna bemöta och stärka deras hälsoprocess. Sjuksköterskan bör bekräfta och se hela patienten utifrån dennes livsvärld samt hjälpa patienterna att finna en balans mellan vila och rörelse. Vidare forskning behövs då det finns ett behov av fördjupad kunskap om patienternas upplevelser av att leva med GUCH.

Abstract:
Self-loosening of bolts in non-permanent connections has been the subject of a myriad of investigations; however, because the vast majority of these studies focused on applications with long-term service conditions involving creep and/or cyclic (working or vibratory) loading, there are no analytical techniques that have been developed to address the load decay behavior of bolted assemblies with gaskets under a primary followed up by a secondary torque. The Ground Umbilical Carrier Plate (GUCP) includes a gasketed ASME-type flange assembly that transfers pressurized, cryogenic hydrogen from the Space Transportation System (STS). Although the threaded stud material is nominally elastic under service conditions, the gasket material, a polytetrafluorethylene (PTFE) matrix filled with 25% chopped glass fibers, undergoes viscous, strain hardening deformation. The consequence of over-torquing the assembly is yielding the stud. Alternatively, the consequence of under-torquing is premature loosening and subsequent fuel leakage. As such, identifying the interactions between assembly configuration, initial torque, etc. to relaxation behavior of the assembly has been identified as a means to reduce the dwell period (the time between initial torque and re-torque). Research is carried out to identify the optimal torque parameters that confer a minimal dwell period. This article documents the Gasket Relaxation and Re-Torque Optimization (GRRO) program used to modify procedures employed when connecting the flanges to the fuel tank of the Space Transportation System prior to a launch.
