Acute angiographical changes for preventing acute rebleeding on GDC treated cerebral aneurysms were evaluated. From December 2000 to November 2002, 48 total aneurysms in 44 consecutive patients with acute SAH. Acute angiographical evaluations were carried out in 46 aneurysms, including 42 ruptured and 4 unruptured aneurysms. Two cases were excluded because of poor medical condition. The three-dimensional Guglielmi detachable coil is a modification of the conventional Guglielmi coil. It has a unique complex structure, with alternating small and large loops at 90 degrees angle to each other during deployment. The enhanced coil complexity optimizes coil purchase on the aneurysm wall, promoting coil stability within the aneurysm sac. It may be the solution of the single catheter technique in the embolisation of wide neck aneurysms. We report our early experience in the embolisation of wide neck aneurysms using these complex coils.

Abstract:
This report describes the use of the Guglielmi Detachable Coil (GDC) (Target Therapeutics, Fremont CA) in the treatment of cervicocerebral arteriovenous fistulae. From March, 1993 to May 1995 we used GDCs in an IRB approved protocol as part of the endovascular treatment of 11 patients with arteriovenous fistulae of internal carotid, external carotid, or vertebral vessels. The patient population included four patients with dural arteriovenous fistulae, three patients with direct carotid cavernous fistulae, three patients with vein of Galen malformations, and one patient with a vertebral artery

We report a case of bilateral thalamic infarcts with no apparent cause other than unilateral partial occlusion of the P1 segment of the posterior cerebral artery caused by Guglielmi detachable coils in a basilar tip aneurysm. This case demonstrates that bilateral thalamic infarctions can result from a unilateral posterior cerebral artery stenosis or occlusion, and does not necessarily imply bilateral posterior cerebral artery abnormality.

The authors report a rare case of spontaneous left vertebrovertebral fistula associated with neurofibromatosis Type 1 in a young woman. The patient had undergone previous failed surgical attempts to close the fistula and presented to the authors with recurrence of the fistula and neurological deterioration. Successful obliteration of the fistula with combined transarterial and percutaneous coiling with Guglielmi detachable coils resulted in disappearance of the fistula and significant neurological improvement.

Here, we present a 32-year-old male with proptosis and chemosis of the left eye following a close head injury. Digital subtraction angiography of the left internal carotid artery showed a left carotid-cavernous fistula (CCF) associated with a primitive trigeminal artery (PTA) variant. The patient was successfully treated with transvenous Guglielmi detachable coils embolization via the inferior petrosal sinus. The PTA variant was preserved without cerebellar or brainstem infarct.

A persisting primitive trigeminal artery (PPTA) is present in 0.2 – 0.6% of all cerebral angiographies. 27 cases of PPTA aneurysms have been reported. We present a case with sixth cranial nerve palsy due to a non ruptured aneurysm on a persisting primitive trigeminal artery. The aneurysm was successfully occluded with Guglielmi Detachable Coils with preservation of the parent artery. The cranial nerve palsy resolved and recovery was uneventful.
