Abstract:
The American Heart Association's Get With the Guidelines (GWTG) has established measures for how quickly a resuscitation team is to respond in a cardiac arrest with performance of cardiopulmonary resuscitation (CPR) and defibrillation. Two of the core measures for GWTG require initiation of CPR within 1 minute of cardiac arrest and defibrillation within 2 minutes in at least 85% of cases. The problem of interest in this Doctor of Nursing Practice (DNP) project was that the facility had not been able to reach 85% on these 2 measures. The purpose of this project was to achieve nursing response times for CPR and defibrillation to meet the core measures. Using the logic model, the project leader implemented mock code training over an 8-week period on the medical-surgical units. The study design for this project was a nonexperimental, retrospective chart review. Compliance data were obtained from the American Heart Association's GWTG database for the facility before and after implementation of mock code training. The results included a review of 10 cardiac arrest cases that occurred after implementation of training. There was 100% compliance with initiation of first compression within 1 minute of cardiac arrest. However, of 2 cases that required defibrillation, only 1 received the shock within 2 minutes. Quantitative descriptive analysis used percentages and a runs chart to compare response times prior to training with response times after training. The chart showed improvement in the area of first compression in meeting the goal of 85% compliance. As a result, the facility will continue to implement mock code training on a routine basis in its effort to improve patient outcomes, including survival and quality of life.

Abstract:
Background: The new guidelines for cardiopulmonary resuscitation states that the family should be given the opportunity to be present at the resuscitation of a close relative. A cardiac arrest is an emotional experience for the family. In addition to being part of the resuscitation effort, the nurse also needs to take care of the family. Aim: The aim of this study was to describe nurses' experiences of family presence during resuscitation. Method: A literature review has been made of 13 studies where differences and similarities were analyzed. The articles were published between the years of 2009 – 2013 Result: The nurses' experiences were divided into three categories; Factors that affect family presence, The nurse' experience surrounding the presence of the family, Factors affecting family presence in the resuscitation room. The result showed that, the feeling of confidence, the ability to work under stressful conditions, the reactions of the family and external prerequisites and the presences of a family support person were important. All nurses had unique experiences that influenced their attitudes towards family presence during resuscitation. Conclusion: Every resuscitation is unique and the family's presence should be evaluated. The need for a family support person was identified as an important factor to ease the family's presence. Local protocols should be created that clearly states the decision process and whom become the family support person. Travelbee's nursing theory facilitates the comprehension of the interactions between the family and the nurse.

Abstract:

The purpose of this study was to investigate factors associated with knowledge, overall and for sodium, of the 2015 Dietary Guidelines for Americans (DGA) among Dietetic Interns in Southern California. Specifically, a survey assessed whether Interns’ knowledge is related to their self-efficacy related to applying the DGA, belief that the DGA is a trusted resource for nutritional advice, eating patterns, and sodium intake behaviors. The survey was created for this particular project and administered via Qualtrics online Software. Pearson correlations demonstrated that there were statistically significant, positive correlations between self-efficacy and knowledge of the DGA, overall and for sodium. These results suggest that Interns’ self-efficacy may be related to their knowledge of the DGA, overall and for sodium.

This study is a production developed as a master's thesis, associated to the Post- Graduation Program in Education of the Federal University of Paraíba (UFPB), in the line of research in Educational Policies. It is an investigation about the policy of initial and continuing training of teachers of Basic Education in Brazil, guided by Resolutions CNE/CP 1/2002 and 2/2002, which establish the National Curricular Guidelines for the training of teachers of basic education; in addition to Resolutions CNE/CP 1/2006 and CNE / CP 2/2015 which deal respectively with the National Curricular Guidelines for the Graduation Course in Pedagogy and the definition of new guidelines for the initial and continuous training of teaching professionals of Basic Education. The methodology adopted was based on bibliographical and documentary research, with a qualitative approach. Our work was based on the studies of Freitas, H. (1999, 2002, 2003,2007a, 2007b), Sheibe (2001), Aguiar (2005), Brezezinski (1997, 2011), Freitas, L. (1992) and Dourado (2015). Our study discussed the teacher's initial and continued training and prioritized the National Common Base as a guiding principle of training. From the design of a new political project for teacher training, progress has been made in the legislation of the training policy of these professionals, and their profile has been re-dimensioned from the conception of teaching, which includes the articulated exercise in the teaching and learning processes and in the organization and management of Basic Education.
Este estudo constitui-se numa produção desenvolvida como dissertação de mestrado, junto ao Programa de Pós-Graduação em Educação da Universidade Federal da Paraíba (UFPB), na linha de pesquisa Políticas Educacionais. Trata-se de uma investigação relativa à política de formação inicial e continuada de professores da Educação Básica no Brasil, orientada pelas Resoluções CNE/CP nº 001/2002 e 002/2002, que instituem as Diretrizes Curriculares Nacionais para a formação de professores da educação básica; além das Resoluções CNE/CP nº 001/2006 e CNE/CP nº 002/2015 que tratam, respectivamente, das Diretrizes Curriculares Nacionais para o Curso de Graduação em Pedagogia e, da definição de novas diretrizes, para a formação inicial e continuada dos profissionais do magistério da Educação Básica. A metodologia adotada foi embasada em pesquisa bibliográfica e documental, com abordagem qualitativa. Este trabalho está fundamentado nos estudos de Freitas, H. (1999, 2002, 2003,2007a, 2007b),Scheibe (2001), Aguiar (2006), Brzezinski (1997, 2011), Freitas, L. (1992) e Dourado (2015). O estudo discutiu a formação inicial e continuada do professor e priorizou a Base Comum Nacional como princípio orientador da formação. Partindo do desenho de um novo projeto político de formação de professores, são apontados avanços na legislação da política de formação desses profissionais, redimensionando o seu perfil a partir da concepção de docência, a qual inclui o exercício articulado nos processos de ensino e aprendizagem e na organização e gestão da Educação Básica.

INTRODUCTION: One of the most frequent causes of sudden death in Europe is a sudden cardiac arrest. First above all providing urgent cardio-pulmonary resuscitation (CPR) is necessary for rescue of life. One of preconditions of successful resuscitation is its early start and knowledge of basic techniques. The level of knowledge of urgent resuscitation techniques, its proper performance, skillfulness and willingness of non-professional rescuers can decide about next life of people affected by sudden cardiac arrest. Every minute that the resuscitation is not performed a chance of survival of people affected with sudden cardiac arrest decreases by 10 - 12%. GOALS AND METHODOLOGY: The main research problem was an evaluation of the level of knowledge of techniques of cardio-pulmonary resuscitation with public in South Bohemia. The next goal was to map the interest of the public in South Bohemia in education in CPR. For working out of the empirical part the method of quantitative research was chosen in the form of a questionnaire which I made. The questionnaire contained 35 questions. The research sample was made of nonselected public in South Bohemia and the choice of informants was random. Abstract:
The thesis is devoted to the issue of non-professional first aid provision. Everyone should have knowledge of first aid procedures, because in the Czech Republic there is a statutory duty to provide first aid. It is therefore necessary to start training as soon as possible, already in school-aged children. The thesis is focused on pupils of 8th grade of primary schools in České Budějovice and their knowledge of first aid. The thesis is divided into two parts, theoretical and practical. In the theoretical part basic information on first aid is given. News and changes brought about by global directive in resuscitation Guidelines 2005. It also gives and account of individual diseases with brief descriptions, causes and emergency procedures. The last two chapters are focused on the Red Cross activities and first aid training in schools. The practical part examines the attitude of pupils to first-aid provision and their first aid knowledge. In this section, the following two objectives and hypotheses were stated: The first objective is to determine the pupils´ interest to get involved in first aid training. The other objective is to monitor the first aid knowledge and skills of primary school pupils before and after the training implementation. Hypothesis 1 assumes that primary school pupils are interested in obtaining information relating to first aid. The second hypothesis determines if the pupils´ awareness got improved after the training. To meet the goals of the thesis quantitative research was used. To pupils interested in the issue, professional instruction in first aid, led by a worker of the Red Cross in České Budějovice, was provided. Based on the pre and post test, a questionnaire survey was carried out when the pupils responded in writing to questions in two questionnaires. The first questionnaire contained 19 and the second 21 questions. Total 122 questionnaires were distributed. The goals of the thesis were met and after the questionnaires evaluation, both my hypothesis were confirmed. The research has shown the pupils' interest in the issue and a better understanding after the training. In order to improve basic first aid knowledge in primary school pupils it would certainly be beneficial to put emphasis on first aid training in schools.

Background: Cardiopulmonary resuscitation (CPR) guidelines have been revised in 2005 based on new evidence and expert consensus. However, the benefits of the new guidelines remain undetermined and their influence has not been published in Asia. This study aimed to evaluate the impact of implementing the new resuscitation guidelines and identify factors that influence discharge survival of out-of-hospital cardiac arrest (OHCA) patients in an Asia metropolitan city. Methods: This was an observational cohort study of all OHCA patients seen by the emergency medical service during period before (Nov 2003-Oct 2005) and after (May 2006-Oct 2008) implementing the new resuscitation guidelines. Detailed clinical information was recorded using the Ustein style template. Statistical analysis was done using X2 test or t-test for univariate analysis and logistic regression model for multivariate analysis. Results: There were 463 patients before and 430 patients after the new guidelines who received resuscitation. The rate of recovery of spontaneous circulation (ROSC), survival-to-intensive care unit (ICU) admission, and survival-to-hospital discharge all showed no benefits regarding the new resuscitation guidelines (ROSC: 42% v.s 39%, p=0.32; Survival-to-ICU admission: 33% v.s. 30%, p=0.27; survival-to-hospital discharge: 10% v.s.7%, p=0.09). The rate of ventricular fibrillation/pulseless ventricular tachycardia (VF/pulseless VT), rate of witnessed arrest, and rate of bystander CPR were much lower than in Western studies. After multivariate logistic regression, factors related to discharge survival were witnessed arrest and initial rhythm with VF/pulseless VT. The new resuscitation guidelines did not significantly influence the discharge survival. Conclusions: We did not observe any improvement in survival after implementing the new guidelines. Independent factors of survival-to-hospital discharge are witnessed arrest and initial rhythm with VF/pulseless VT. Because the rates of VF/pulseless VT and bystander CPR in Asia are low, popularizing CPR training programs and increasing the rate of bystander CPR may be more important for improving OHCA survival rates than frequent guideline changes.

Abstract:
The thesis is focused on the epidemic of Ebola between years 2014 and 2015 and response of the Czech Republic. The main objective of the thesis is to assess the strategy of the Czech Republic in order to eliminate import of Ebola. The thesis is using study of documents and its effort is a complex overview of the given matters. Used documents are especially information and reports of international and Czech organizations, regulations, expert articles, electronic inquiries and media reports. There was defined 5 research questions. First question is focused on the demographic and epidemiological characteristics of the epidemic in West Africa. Second question was analysing steps, which were performed within the system of preparedness for highly contagious infections in the Czech Republic. I have divided the whole strategy of the Czech Republic into 10 points. They consist of the enforcing of International Health Regulation (2005), Decision no. 1082/2013/EU of the EP and of the Council, National action plan for the case of event, which is subject to International Health Regulation and two directives, implementation of special measures on international airports, conclusion of cooperation agreements, transmission of information within the Czech Republic, simulation actions and humanitarian aid. Third question was analysing the respondence of arrival cards at the Václav Havel Airport in Prague. Respondence was approx. 40 %. (Jágrová, 2016a) Fourth question is assessing the efficiency of implemented measures using the recorded cases of suspicion. Although there was not any case of infection in the Czech Republic, it may be concluded, that implemented measures are efficient. Fifth question is focused on the assessment of implemented measures and suggestions, how the response of the Czech Republic may be improved. Preparedness of the Czech Republic was assessed as very good by experts. Also it may be concluded from results, that preparedness is sufficient. Suggested measures, how the response of the Czech Republic may be improved, are related to the system of rapid alert of the public, creation of team for humanitarian aid, maintaining of sufficient capacities and realization of simulation actions.

A palavra biotecnologia foi utilizada pela primeira vez em 1919 pelo engenheiro húgaro Karl Ereky, mas a sua influência na vida humana começou alguns séculos antes. Aliada à Indústria Farmacêutica, fez surgir uma nova classe de medicamentos conhecidos como medicamentos biológicos. Estes permitiram a descoberta de tratamentos para muitas patologias que até ao seu aparecimento não disponham de terapêutica disponível com os medicamentos convencionais. Na década de 80, foi aprovado o primeiro medicamento biológico e atualmente milhões de doentes conseguem tratar ou prevenir doenças crónicas ou graves, como é o caso da diabetes mellitus e do cancro. Com o término do período de exclusividade dos medicamentos biológicos inovadores, vários medicamentos biológicos similares ou biossimilares começaram a ser desenvolvidos e já estão no mercado desde 2006, existindo atualmente 19 biossimilares aprovados e comercializados na Europa. Ao longo desta monografia é discutido o quadro regulamentar dos medicamentos biossimilares, apresentada uma análise da situação mundial comparando várias potencias económicas e levantadas algumas questões que ainda não apresentam consenso relativamente à sua utilização.

The theme for this thesis is based on the European Resuscitation Council's guidelines for cardiopulmonary resuscitation of 2015. One of the important points of these procedures is the use of an automatic external defibrillator (AED). These devices were developed for informed responders without the knowledge of cardiac rhythm diagnostics and allow to perform an early defibrillation of the heart, before the arrival of emergency services. AED is a sophisticated device that is a part of the equipment of I - firefighters and policemen. Training of these so-called first aid responders performs the local emergency service team. Over the years 2015 and 2016, the majority of the professionally active firefighters and police officers in one selected Czech district who have the automatic external defibrillator in their equipment attended a training in how to use this device. The aim of this thesis is to determine whether the concept of the training courses is sufficient. If the theoretical part is well understandable for the respondents, and whether the practical training is sufficient - not only the actual compressions and ventilation, but also the use of an automatic external defibrillator, or whether it would be appropriate to extend this training. Este relatório, assim como o trabalho desenvolvido em estágio, tiveram como alicerce fundamental, a metodologia de projeto com todas as etapas inerentes á mesma, diagnóstico da situação, planeamento, execução, avaliação e divulgação dos resultados. Esta foi baseada numa metodologia de investigação, centrada num problema real identificado, com a implementação de estratégias e intervenções eficazes na sua resolução. Esta metodologia decorre da pesquisa, análise e resolução de problemas reais do contexto, promotora de uma prática fundamentada e baseada em evidência. (Estudantes do 7º Curso de Licenciatura em Enfermagem, 2010) As queimaduras são um dos problemas de saúde mais significativos em todo o mundo, levando à hospitalização prolongada e consequentemente a uma maior despesa para os clientes, famílias e sociedade. Atualmente, o prognóstico dos clientes com queimaduras depende do tratamento adequado, do sistema de saúde e profissionais de saúde, o que irá ter impacto na sobrevivência e na qualidade de vida posterior ao acidente. (Elsherbiny [et al] 2011) A temática sobre o grande queimado, contemplada no plano de ação do serviço e por ser uma área do nosso interesse, surgiu após reflexão, entrevistas informais com os colegas e com a enfermeira responsável, sobre as necessidades do serviço passíveis de intervenção, de forma a contribuir para a melhoria dos cuidados de enfermagem a estes clientes. Como produto final do projeto, emergiu um protocolo de cuidados ao grande queimado no SU da SE, de forma a dar resposta ao objectivo geral do projeto, ” Melhorar a qualidade e a segurança dos cuidados de enfermagem prestados ao grande queimado (Adulto) na sala de emergência do serviço de urgência do hospital onde desempenhamos funções.” Este relatório traduz, o trabalho desenvolvido no conjunto dos estágios que permitiu desenvolver competências comuns e específicas do enfermeiro especialista e de mestre em enfermagem médico-cirúrgica
Abstract:This report and the work on stage had the basic foundation, the project methodology with all stages inherent to it, situation diagnosis, planning, implementation, evaluation and dissemination of results. This was based on an research methodology, focused on a real problem identified with implementation of effective strategies and interventions in its resolution. This methodology arises from research, analysis and problem solving real context, promotes a practice based and evidence-based. (Students from the 7th Degree in Nursing, 2010) Burns are one of the most significant health problems in the world, leading to prolonged hospitalization and consequently to greater expense for clients, families and society. Currently, the prognosis for clients with burns depends on the proper treatment, the health system and health professionals, which will have an impact on survival and quality of live after the accident. (Elsherbiny [et al], 2011) The theme on the major burn, which is included in the action plan of the service and for being an area of our interest, came after reflection, informal interviews with colleagues and with the nurse in charge on the needs of the service subject to intervention, to contribute to the improvement of nursing care to these clients. As a final product of the product emerged a protocol of care to large SU burned in the SE in order to meet the overall objective of the project “Improving quality and safety of care provided to nursing major burns (Adults) in room Emergency hospital emergency department where we play functions.” This report reflects the work undertaken in all the stages that led to the development of specific skills and common nurse specialist, and master of medical surgical nursing.
