Full textThis work studies the psychical troubles of mentally handicapped adolescents. It refers to the attachment theory and psychoanalysis. The hypothesis is that mentally handicapped adolescents have an insecure attachment. The method used is an experimental one. The data analysis includes the results stemming from the experimental method and the analysis of the clinical data coming from a psychoanalytical approach of the material. All our results support our hypothesis. The adolescents assessed have an unresolved attachment. Their mental functioning attests the traumatism of the announcement of the mental handicap on their family. Mental handicap is looked on, by their psyche, as an evidence of the destructivity of aggressiveness. It appears either a crime, for which the handicapped adolescent must be punished or a punishment, evidence of a prior culpability that cannot be worked over. The reality of the handicap appears a hindrance to the fantastical working over of aggressiveness

Full textThis thesis proposes to investigate the psychological disability construction, based on three axes: historical, social and institutional. Studying ITEP (Pedagogic and Educative Therapeutic Institute) from Gironde, we interrogated the assignation process of the teenagers in these facilities and their backgrounds (social, familial, etc) that leads them to be designed as psychologically disabled people (2005 law). Consequently, when the collective attention is focused on the consequences of being handicapped to compensate, we want to study the origins of psychological disability that conduct them – or not – to be oriented in an ITEP. First, we realized an historical diagnosis. We studied five organisations managing ITEP by interviewing their directors, in order to compare the historical beginning of psychological disabilities and the development of Gironde organisations. The second axe consists of a social characterisation of ITEP teenagers by studying social data in all 20 Gironde ITEP (a pool of 965 teenagers). The study of the complex arrangement of ITEP teenager characteristics show that being psychologically disabled is affected by family social origins, showing a first assignation form. The third analytic axis is an ethnographic observation about how the Multidisciplinary Evaluation Team (EPE) operates in Gironde MDPH during a year. The results of this orientation procedure show how psychological disabilities is built between psychic disorder and social-family difficulties. Linking these three analytic parts allows for a better understanding about how the psychological disability is constructed, all invisible steps of the assignation processes and what legitimates them. In addition to having difficulties living an ordinary life, all conditions are combined to prevent these psychologically disabled teenagers and their families to have a space dedicated to social representation

Full textChildren with intellectual disabilities experience major inequality in the field of oral health. Opposition behaviors are very common. The care is then carried out sometimes in difficult conditions. This work examines the ethical issues faced by the dentist in the refusal of care in children intellectual disabilities. The objectives are: (1) to expose elements of contextualisation (2) to identify the practices of dental care in a national and international contexte and to initiate the philosophical reflection (3) to focus on the opinions and perceptions of practitioners to explain these practices and to identify areas of ethical tension (4) to propose a synthesis and a discussion. Our research is based on two studies, each of which is directed at dentists who are specialists in special care dentistry in children. A deductive approach was conducted to study practitioner profiles according to their practices and the opinions they have of these practices (online quantitative survey) and analyzed by MFA (Multiple Factor Analysis). An inductive approach based on focus groups was conducted to study the perception and opinion of practitioners. The thematic content analysis made it possible to understand the motivations and difficulties encountered. The results showed that most dentists are part of a teleological approach when they have to choose between respect for autonomy and beneficence. Nevertheless, if these constrained decisions are impregnated with a deontological scruple, this guarantees their ethical character. It would be necessary to develop education and the ethics of the discussion to reconcile the dual technical and emotional requirements of care

Even before it leads to a real handicap or impairment or discrimination, mental illness can generate tensions between the employee and his working environment ; this tensions can lead to him being put aside. It is difficult for a company to keep an employee that have this psychological disorders and especially for a big company as will be seen in this study. The observation of psychological handicap in a big company tends to reveal tensions between the employee and his job situation. Both professionals and experts have to take part and therefore this whole maintenance process is a very collective one. It can never be an individual process and even the working environment has to participate

Full textThe handicap psychique (psychological handicap) is a French expression which has imposed itself recently. It is the subject of various meanings. We place it in the very broad context of vulnerabilities and in the more restricted situations of handicap. While it is commonly accepted that psychological handicap is a social consequence of a mental disease, we show that people with handicap of psychological origin (HOPO) have no proven mental disorder and do not undergo any psychiatric treatment. We rely on the experience of a plan called Médiation logement. In this “Housing mediation” clinical psychologists meet people who are reclusive in their homes, or who exhibit behaviour or neighbourhood disturbances. Those people have no psychiatric history and we show that they share a common feature: a discrete incapacitating syndrome of psychological origin (DISOPO). This leads us to give a new definition of psychological handicap. Then we propose a clinical reading tool that would allow a team to make an early diagnosis of this DISOPO that appears in a person with a singular fragility when faced with personal and / or social traumatic of stressful event

Full textWith the purpose of an internal comparison concerning the motor handicap, the Rorschach test applied, according to J. Exner's comprehensive system, to a group of patients whose disability derives both from a physical trauma (exogenous) and from an internal somatic dysfunction (endogenous), shows on one side difficult mental working a frequent depression index, a strange living of human experience, and a sense of rupture. On the other side, we observe, a more furious internal rage for exogenous group, difficulty to integrate the emotional aspects, a more frequent use of abstraction and the loss of self-esteem, always for the exogenous. These general results will enable us to discuss the concept of dissociation. We after focus on motion mental images. Bett's shorter questionnaire mentions the failure of some patients confro9nted with kinesthetic solicitation. Besides the mental motion representation may have a defensive value. The results of Rorschach tests suggested in this connection: (i) the absence of "m" responses in inversely proportional to the intensity of anxiety,(ii) the "m"< in so far as they represent a process of devitalisation, can support the processes of perceptive apprehension. Furthermore, when we examine the variable "traumatic dream", chosen for its pathognomonic character, on realizes always through the Rorschach test, that the dream of anguish thoroughly confirmed by this perceptive-projective test, is not free for mental working. This result preserves all its importance in the psychotherapic perspectives; perspectives which will be evaluated in a second study

Full textThis thesis is based on multiple observations that paint a picture of a collection of restrictions on social participation for people with handicaps related to mental illness. In the absence of a legal context which encourages the social participation of these individuals, documentary evidence shows that there are roadblocks in various domains, such as domestic, social, economic, professional, political, cultural and artistic domains. In this context, and in line with international terminologies, our thesis aims to define the conditions which enable support groups for individuals with mental handicaps (known in France as Groupes d’Entraide Mutuelle, or GEM for short) to strengthen the social participation of their members. We aim to measure the strength of this involvement both on an individual and a collective level. After an overview of the concepts of participation and social participation, we will propose a theoretical model that covers the process and results of social participation, which we will compare and contrast to what we observe in the context of our research. This research is comprised of open-ended questionnaires administered face-to-face with members of GEMs, fragmented ethnographic observations in twenty GEMs, and extensive participant observation in two GEMs. The analysis of the data, which was in large part qualitative, points to different forms of participation on the individual and collective level. With these forms in mind, we identify various conditions necessary for the emergence of social participation, linked both with the individual characteristics of members and the specific contexts of each GEM, whether they be spatial, organisational, relational and/or professional.Our analysis leads us to understand the individual process of social participation as an experience, whereas at the collective level, the process is a publicization of a set of problems and points of view which transforms each of the actors involved. In terms of results, we identify three levels of social participation as well as the emergence of different types of expertise. In conclusion, we modify our original theoretical model and propose a new transactional model of the process of social participation and its results

Full textWork is a key-factor of recovery for people with severe mental illness. Various programs exist to support them to gain and maintain competitive employment. In France, the rate of employment remains low for this population and only scarce researches are developed in this field, although some French practices, such as the Ç ESAT de transition È (transitional workshops) of the MESSIDOR association, appear to be performing well. Theses transitional workshops support people with psychiatric disability to gain a competitive employment on the regular labor market while working in a sheltered activity. The goal of this research was to describe the Ç ESAT de transition È program, comparing it to international evidence-based practices, such as supported-employment programs, and to define how this program can facilitate a recovery process for people with a severe mental illness.The first part of this research is intended to describe the program with a mixed method (qualitative then quantitative) in order to compare it with international evidence-based practices and to describe the roles, tasks and competencies of the two professionals supervising people with a severe mental illness, counselors and supervisors, and to define how they contribute to a recovery process for them. A second part of the research is dedicated to clients of this program, with a longitudinal study (n=160) of clinical and psychosocial variables, at the beginning of the program, and in order to document predictive factors of their job tenure in the program, of their positive evolution to work integration on the regular labor market and of their recovery.These Ç ESAT de transition È are an hybrid form of a sheltered workshop, social enterprise and supported-employment program for people with a severe mental illness. In a workplace close to the context of regular labor market, thanks to a subtle relational posture, the two professionals supervising the workers help them to develop a new positive identity, self-esteem as a worker and hope in the future, which are key-factors of a recovery process. Clients have a positive perception of their situation in this program, on clinical and psychosocial variables, and this perception is stable on the 9 months follow-up of the study. They confirm that self-esteem as a worker, counselors and supervisors are key-factors to ensure their job tenure in the program, their positive evolution to work integration on the regular labor market and their recovery. Practical implications and recommendations from this research are the type of support that should be developed in France for people a with severe mental illness to help their work integration, as well as the type of training and supervision that should be offered to the professionals supporting them

Full textHaving a child with a disability confronts the parent with multiple difficulties. This thesis focuses on the psychopathological topics (parental distress and burnout) as well as the personal resources of parents of children with a disorder or handicap. Study 1 aims to understand, thanks to quantitative and qualitative approaches, the difficulties encountered in everyday life and in the moments initially dedicated to rest. Even when the child with a disability is an adult, the chronicity of difficulties and the time-consuming tasks have consequences on the personal life of the parent. The wishes for support are expressed (eg. home help) that can relieve the daily load. Study 2 focuses on a qualitative analysis of the experiences of parents of adolescents and adults with disabilities. Analyze of 14 interviews make it possible to identify changes (difficulties and resources) within different areas. The particularity of this study shows that parents maintain their parenting role despite the fatigue generated. Furthermore, they report concerns about the future of their child. Study 3 shows three clusters of parents of adults with mental disorders based on their level of parental distress and their participation in support groups. The clusters differed in the sense of loneliness, optimism and unconditional self-acceptance. The type of parental support would depend on their level of distress. Study 4 aims to study the various aspects related to the organization of family tasks and care of the child in connection with parental burnout. Of the 386 mothers of children with a disorder, 30% are exhausted and have more personal and family-related tasks than non-exhausted mothers

Full textThis work aims to demonstrate the relevance and the issue of a psychic elaboration work of the mentally handicapped adolescent subject in the setting of a psychotherapeutic caretaking. This research is supported by the clinical and theoretical articulation of psychoanalytical allegiance. The general assumption is as follows: in the setting of a psychotherapeutic caretaking, a mentally handicapped adolescent is able to produce a psychic elaboration of his handicap, which I name “work on the handicap”. The setting of the psychotherapeutic caretaking leads the mentally handicapped adolescent to build some representations of his own handicap. This research is supported by three psychotherapeutic care takings of mentally handicapped adolescents. Equally rich and complex, the clinical material coming from the three clinical cases demonstrates the carrying out of a work on the handicap, based on a co-construction between the therapist and the subject. The work on the handicap therefore could be the result of a psychic elaboration combining the work of mourning, the transformation of thought and representability. If the work on the handicap turns out to be painful in many regards for the mentally handicapped adolescent subject, it remains essential for his psychic construction
