Abstract:
The purpose of this article was to trace the growth and development of the Guild of St. Cecilia. This late nineteenth century organisation was founded by Frederick Kill Harford in London to provide music therapy to hospitalised patients. All information was derived from letters written by Harford and editorials that appeared in British medical and music periodicals. Initially, the Guild enjoyed great success and was endorsed by important people such as Florence Nightingale and Sir Richard Quain, physician to Queen Victoria. The Rev. Harford was astute in his observations that the effects of music must be tested to find the most beneficial ways for it to be used as therapy. He envisaged an association that would provide live and transmitted music via telephone to London's hospitals. Ultimately, due to the lack of support from the press, limited financial resources and Harford's ill health the organisation failed to prosper. Despite this, the Guild of St. Cecilia remains important because it kept alive the idea that music could be used therapeutically to benefit physically and mentally ill people.

Abstract:
Demuth and Harford (1999) contend that in Bantu relatives, the verb raises from I-C if the relative morpheme is a bound morpheme while the subject remains in spec-IP resulting in subject –verb inversion. Ikalanga, a Bantu language spoken in Botswana has no subject verb inversion in relatives although the relative morpheme appears to be a bound morpheme. This observation challenges the conclusion reached in Demuth and Harford (1999). This raises the question, What then is the structure of the relative clause in languages like Ikalanga and Luganda? This paper argues that Ikalanga relative clauses differ from other Bantu relative clauses in that the projection that houses the relative feature (RelP) projects below TP while in Bantu languages where subject verb inversion is observed such as Shona it projects higher than TP. Thus, the variation in the structures of Bantu relative clauses can be accounted for if we understand that there is a parametric variation in the position in which RelP projects; lower than TP or higher than TP.

Abstract This study examines the role that Michelangelo’s letters have played in biographies of the artist. It focuses on two periods — the Renaissance and the late nineteenth and early twentieth centuries, the latter witnessing an outpouring of biographies on Michelangelo. The first section of the study examines the way in which Giorgio Vasari edited the letters he received from Michelangelo in his 1568 vita of the sculptor. In the second section I analyze how the availability of the letters, through a long and fitful process, influenced the way in which biographers from John S. Harford to Giovanni Papini characterize Michelangelo and his world.

Abstract:
Pyrethrum (Tanacetum cinerariifolium), a small perennial plant, has been commercially grown for production of pyrethrins (Py) in Tasmania since 1983. Two field experiments were conducted on clay loam and sandy clay loam krasnozem soils at Harford and North Motton in the northern region and a two-year experiment on an alluvial sandy loam at Ouse in the southern region of Tasmania to study its potassium (K) requirements. Potassium, at rates up to 200 kg K ha-1, was band-placed at planting (basal K) of pyrethrum splits. At Ouse in the spring of the following year up to 100 kg K ha-1 was side-dressed in the subplots. No achene yield response was observed at Harford with the soil Colwell K (Kc) of 80 mg K kg-1 and North Motton (Kc = 132), but at Ouse (Kc = 50) in the first year a maximum increase of 86% in the achene yield was attained at 50 kg K ha-1. In the second year at Ouse, the achene yield response occurred with residual basal K up to the 200 kg ha-1 rate, where the achene yield was 3.9-fold greater than the control. In addition to the increase gained from the residual K in the second year, 50 kg side-dressed K ha-1 increased the achene yield by a maximum of 50%. Basal K did not have any effect on the Py concentration of the first year crops, but it increased the Py concentration in the second year crop at Ouse. Side-dressed K also increased the Py concentration. The Py yield at the highest rate of 200 kg basal and 25 kg side-dressed K ha-1 was more than 6.5-fold that in the control treatment. The Kc and the concentration of K in apical tissues were correlated to the rates of applied K, achene yield and Py yield, and their potential as techniques to predict pyrethrum response to K application is discussed.

This paper investigates the controversial morphemes that occur both at the clause level and within the DP in Bantu languages such as Ikalanga, which have been analyzed as "agreement morphology" by some (Baker 2002, Demuth and Harford (1999) and as resumptive/incorporated pronouns by others (Bresnan and Mchombo 1987, Zwart 1997). The paper proposes a unified analysis of this phenomenon, analyzing both clausal and DP-internal occurrences of these morphemes as agreement morphology which holds between the head of an XP and its predicate, or between the head of an XP and its modifiers. In both instances, the agreement relation is instantiated after movement of the relevant category from its base position to a specifier position which enables the moved category to enter into a checking relation (i.e. a spec-head configuration) with another category that has matching features (Chomsky 1995).

Most of us are familiar with the scenario of institutional belt-tightening and the effect that it sometimes has on staffing. Reductions and reallocations of human resources can make it difficult to accomplish our work and can affect morale—but not always. In this column, Andie Craley describes the net positives of that experience at her institution. While still facing the reality of doing more with less, Craley identifies how its inherent challenges can present definite opportunities and foster a community of collaboration.—EditorHaving started as a library technician at Harford Community College Library (HCC Library) just seventeen years ago, I already find myself the third in seniority. Library staff size seems to diminish as additional job functions are assimilated into existing positions, reallocated as new library services are introduced, redeployed to other departments, or completely eliminated as staff retire or resign. The concept of wearing many hats is common, as is the expectation to take on “other duties as assigned.” From networking with colleagues at other Maryland community college libraries, I know this situation is not unique. The current staffing reality both mirrors the evolution of the library’s facilities and demands doing more with less, but it also provides individual and collective opportunities for growth, supporting a community of collaboration.

e18015 Background: CML is a malignant clonal disorder of the hematopoietic stem cells that results in an increased myeloid cells but also erythroid and platelets in the peripheral blood and marked myeloid hyperplasia in the bone marrow. The management of this condition in a rural community clinic is a challenge due to the complexity in the routine monitor of these patients with molecular studies. In addition, data from Hispanic population treated with imatinib in USA is limited. Methods: A total of 15 patient with CML followed in a private clinic form June 2001 to January 2012 were included in the analysis for the study. Results: The patients included 10 males and 5 females. Median age was 51 years. All patients received imatinib 400mg. There were 4 patients with initial WBC > 50,000/mm3. All patients were in chronic phase with less 1% of blasts, except for one that had 7%. Twelve patients had a low Harford score, 2 with moderate and 1 high risk. Conclusions: Our data suggest that imatinib is highly effective in this population of patients. The CCR at 6 months was 80% and the MMR at 12 months was 80% compared with the data from the IRIS (CCR at 6 months, 74.7%) and DASISON (MMR at 12 months, 28%) trials. This group of patients demonstrates high CHR, CCR and MMR in comparison of what is reported in the literature. The study also present a group of Hispanic patients were the information about imatinib response is limited. The possibly that certain ethnic group has a different pattern of response as compared with Caucasian is intriguing. Lastly, it is evident that adequate care could be offer event in rural community clinics. [Table: see text]

Abstract Background Technology can extend Infection Disease Consultants (IDCs) into resource limited small community US hospitals. We compared in-person infectious disease (ID) consults with Tele-ID consults to determine whether length of stay, antibiotic usage, drug cost, or readmission rates will remain the same between both groups over a 3-month period. Methods University of Maryland Harford Memorial Hospital (UM-HMH) is a 95-bed hospital including a 6-bed ICU. From May to August 2017 (study period) there was no IDC at UM-HMH. During study period, IDCs from a regional UM hospital provided formal Tele-ID consults through an HIPAA compliant secured Skype Business account. Patient history and wound examination were done by the IDC via video monitor while a bedside nurse assisted in performing the physical examination. Laboratory and radiological data were reviewed in real time as both hospitals shared the same electronic medical record and IT infrastructure (Meditech 6.15). A formal consultation was dictated and computer orders were entered by the IDC within 24 hours of the consult request. Daily Tele-ID follow-up rounds were conducted. IDCs had the authority to transfer a patient to the regional hospital for in-person care if deemed necessary. Study period was compared with a baseline period (May–August 2016) when IDCs were providing in-person consults at UM-HMH. Results Baseline period had 148 inpatient stays and study period had 148 inpatient stays. Despite similar case mix index in both groups, there was no statistical difference between the clinical outcomes. Results are shown in Table 1. Conclusion Tele-ID at our hospitals was noninferior to in-person ID consults. An integrated computer system, nursing support, and daily follow-up are key components of a successful Tele-ID program. Abstract Background Many US hospitals have implemented CAUTI prevention bundles (CPB) but few have achieved the goal of zero CAUTIs for 12 consecutive months. We report our journey to zero harm at two community hospitals that have each successfully eliminated CAUTIs from all their units, including the intensive care unit (ICU), for over 12 consecutive months. Methods From April 2015 to March 17 (Period A), CPB was implemented at University of Maryland-Upper Chesapeake Medical Center (UM-UCMC) and University of Maryland-Harford Memorial Hospital (UM-HMH), each with 195 and 128 beds, respectively. UM-UCMC has a 15-bed ICU while UM-HMH has a 5-bed ICU. The CPB included placement of urinary catheters only for approved indications, use of two persons (buddy system) for catheter insertion, Nurse Driven Protocol for catheter removal, and silver-impregnated cloths for perineal care. A massive frontline engagement campaign “You can’t have a CAUTI if you don’t have a foley” was launched from April 2017 to March 2019 (Period B). The focus was intensified on reducing catheter utilization rates. Real-time feedback on new CAUTIs cases was provided to leadership at the daily safety briefs and to nurses and physicians at the unit-based huddles. The number of CAUTIs and “days without a CAUTI” was shared with team members via small posters and whiteboards. Results A statistically significant decrease in utilization of urinary catheters was observed (Table 1). Both hospitals and their respective ICUs remained CAUTI free for >12-consecutive months (Figure 1). Conclusion Eliminating CAUTIs for 12 consecutive months in acute care community hospitals is possible and serves as a step toward the journey to zero harm. Reducing catheter utilization is a key strategy. Humanizing each infection and providing real-time feedback to the frontline staff and leadership in whole numbers (instead of the old paradigm of reporting CAUTI rates) may have resulted in greater engagement. Abstract Background Behavioral health units (BHU) have been implicated in influenza outbreaks due to group activities, low availability of alcohol-based hand gels and unique host factors. We describe the management of an unusual influenza outbreak, which started in the BHU and then spilled over to the acute care hospital (ACH). Methods University of Maryland Harford Memorial Hospital is a 95-bed ACH with a 14-bed closed-door adult BHU located on the fifth floor. Two cases each of hospital-acquired influenza were identified in our BHU during 2016 and 2017. In January 2018, however, hospital-acquired influenza cases in the BHU spilled over to the adjacent ACH to cause an outbreak. A case was defined as a patient with fever of >100.4°F, presence of influenza-like illness, and a positive influenza test >72 hours after admission. Outbreak control measures included twice daily fever screening, enhanced droplet precautions, visitor restrictions, discontinuing community activities, enforcing hand hygiene at all hospital entrances, and hospital-wide chemoprophylaxis with oseltamivir. Results On January 15, 2018, the index patient developed influenza in the BHU followed by a second case in BHU 4-days later. Over the next 10 days, five more patients on the third and fourth floors of ACH tested positive. Attack rate was 3% and average length of stay was 8.9 days. Chemoprophylaxis with oseltamivir 75 mg orally once a day was given to 71% of all eligible hospitalized patients for a week (at a cost of $17,000). All seven patients yielded influenza A, subtype H3N2 and were successfully treated with oseltamivir 75 mg orally twice a day for 7 days. The outbreak lasted 11 days. Figure 1 shows the epidemiologic curve. Conclusion Special attention should be paid to influenza prevention in the BHUs due to the risk of spillover effect to sicker patients in the adjacent ACH. A short, 7-day course of hospital-wide oseltamivir chemoprophylaxis, in addition to promptly implementing the infection prevention measures was effective in controlling the outbreak. Stormwater management ponds are common best management practice (BMP) and green infrastructure (GI) for flood attenuation and water quality treatment in highway projects. Originally designed to provide storage volume for flood detention, stormwater ponds today often employ additional retention volume at pond bottom in a hope to improve water quality via sedimentation and other pollutant-removal mechanisms. It is commonly assumed that sediment accumulation and topographic variations (such as erosion, channelization, and in-pond plant growth and decay) over time often decrease the capacity of stormwater ponds. However, differences between design capacities and field capacities over time have never been verified and quantitatively analyzed before. This study presents such analysis using conventional topographic survey techniques and remote sensing data (topographic light detection and ranging digital elevation model [LIDAR DEM]) for 10 highway stormwater ponds along Interstate Highway-95 (I-95) systems in Baltimore City, Cecil County, and Harford County, Maryland, United States, with facility service life ranging from 14 to 26 years (1990–2015). Data derived from LIDAR DEM were compared with those from topographic survey; the LIDAR DEM data appear to be effective in measuring flood detention capacities and identifying silted ponds, but not in estimating the remaining retention volume for water quality treatment. Data from topographic survey indicate that the total volume in the ponds was relatively unchanged compared with the design, with increases in some instances. The increase typically occurred at the pond’s upper stages. Nonetheless, the water quality treatment capacity at pond bottom (wet pool volume) was drastically less (up to 100% of the design). As current maintenance practice of stormwater ponds relies heavily on visual inspection, the storage volume variations are often overlooked. As such, the findings prompt uncertainty on the long-term effectiveness of watershed implementation plan and models in the Chesapeake Bay watersheds, as many of them depend on wet pool volume design in BMP and GI.
