Abstract:
Background: The notch of Harty is a normal variant radiologically and incidentally diagnosed finding but can be mistaken as pathology. Objective: To identify the incidence and characteristic findings of the notch of Harty.Patients and Methods: A cross-sectional study was conducted on 121 patients referred to the Radiology department, MRI unit, to undergo ankle MRI through a one-year period (January to December 2020). Cases with significant ankle lesions were excluded. All MRI studies were performed on 1.5 T MR scanners (Aera, Siemens, Arlington).Results: The notch of Harty was present in 35.5% patients; their age ranged from 5-83 years with a mean age of 38.20 years. The average width of the notch was 5.65mm while the mean depth was 1.04 mm. There was no significant correlation between the presence of the notch of Harty and findings such as rheumatic arthritis, minor trauma, chondral thinning. However, there was significant correlation between the presence of the notch of Harty and subjacent subchondral sclerosis. Conclusion: The notch of Harty was present among a considerable number of cases with no sex predominance. The notch might be misinterpreted by radiologists as true osteochondral lesions, thus, familiarity with location and appearance of these pseudodefects will avoid their misinterpretation. Background: The aim of this study was to report the incidence of associated osteochondral lesions (OCLs) on postop CTs, which may benefit from arthroscopic treatment, in patients with rotational type ankle fractures with syndesmotic injury. The diagnosis and treatment of associated OCLs may be an additional benefit of the use of arthroscopy in the management of rotational type ankle fractures with syndesmotic injury. Methods: We retrospectively reviewed data of a prospective cohort study of patients who underwent open reduction and surgical fixation of an ankle fracture with syndesmotic injury. These patients underwent routine bilateral postoperative CT assessment. Two independent observers classified ankle fractures according to Weber and OCLs according to the classification system by Berndt and Harty. Fifty-nine patients were included. There were 19 (32%) Weber B type ankle fractures and 39 (66%) Weber C type fractures. One patient (2%) had fixation for a medial malleolus fracture and syndesmotic rupture without fibula fracture. Results: Talar OCLs were present in 8 patients (14%). In one patient, 2 OCLs were found, which resulted in a total of 9 lesions. Two lesions were found on the medial side, both anterior. The other 7 were located laterally, of which 1 was anterior, 3 central, and 3 posterior on the talus. According to the Berndt and Harty classification, 1 was classified as stage I, 4 as stage III, and 4 as stage IV. Conclusion: The prevalence of OCLs in ankle fractures with syndesmotic instability was 14%. We believe that lesions were arthroscopically accessible in 6 patients (10%). Moreover, most lesions were located on the lateral dome, and thus also potentially approachable through an anterolateral arthrotomy during open reduction and internal fixation. The majority of OCLs found in this series were Berndt and Harty type III or IV, and so likely would preferably have been addressed during the index procedure. Level of Evidence: Level III, diagnostic cohort study.

The First Symphony created an enormous impact, coming soon after the successes of the Viola Concerto of 1929 and the oratorio Belshazzar's Feast of 1931. The Symphony's vast scale, its emotional power and fierce utterance, allied to an impressive consistency of style, conspired to declare a work of originality and almost instantaneous appeal. An early Decca recording of it under Sir Hamilton Harty, who had conducted the first performances, enabled the Symphony to be heard worldwide.

Wide application of cell therapy particularly platelet enriched plasma (PRP) in modern orthopaedics enabled to improved treatment results in certain orthopaedic diseases and injury consequences. Treatment results for 7 patients (2 men and 5 women) with osteochondral lesions of talus (OLT) are presented. Mean age of patients was 26 years, mean duration of disease — 4 years. By Brendt and Harty roentgenologic classification I—II degree of OLT was diagnosed in 1 and III—IV degree — in 6 patients. In all patients mosaic chondroplasty of talus with implantation of PRP gel was performed. Postoperatively mean AOFAS index raised from 53 to 92. Follow up period made up around 2 years.

Surgical treatment of posteromedial talar dome lesions is frequently necessary for Berndt and Harty grade IV osteochondral defects and nondisplaced osteochondral fragments resistant to conservative modalities. When operative intervention is indicated, the approach and management can be complicated by the location and extent of the injury. The operative technique we advocate allows direct exposure of the lesion and minimizes damage to healthy articular cartilage and surrounding soft tissue. Use of a drill guide assists the surgeon in precisely placing a transmalleolar portal through the tibia for subchondral drilling of osteochondral defects when the lesions are inaccessible through traditional arthroscopic portals. Abstract:
Abstract Background The aim of the study was to investigate parameters influencing the preoperative pain intensity in patients with osteochondral lesions of the ankle. The evaluation covered patient-related parameters such as age, sex, body mass index (BMI), as well as defect-related parameters, such as localisation, size and stage (according to the classification of the International Cartilage Repair Society [ICRS] and the Berndt-Harty-Loomer classification). We also examined the correlation between the different surgical techniques and additional factors, such as debridement of an impingement or stabilisation of the ankle on the one hand, and the preoperative pain intensity on the other. Material and Methods 259 patients with osteochondral lesions of the ankle were operated in 32 clinical centres between October 2014 and December 2016 and enrolled consecutively in the German Cartilage Registry (KnorpelRegister DGOU). 151 patients were available for analysis. The preoperative pain intensity was assessed at the time of surgery with online questionnaires, using the Numeric Rating Scale for pain (NRS). Results The median preoperative pain intensity in the complete study population (n = 151) was 3 (range 0 – 10). There was no correlation between the age and the preoperative pain intensity (ρ = − 0.06). Further, there was not detected a difference between the two genders according to the preoperative pain intensity (p = 0.31). In female patients a higher BMI correlated with a higher preoperative pain intensity (ρ = 0.16). Within the group of patients with a solitary treated talus lesion there was no difference according to the preoperative pain intensity between the different localizations of the defect (medial vs. lateral talus) (p = 0.82). Within the group of patients with a solitary treated talus or tibia lesion there was no correlation between the defective area or the lesion stage according to the ICRS classification on the one hand, and the preoperative pain intensity on the other (ρ = 0.09, and ρ = 0.04, respectively). According to the Berndt-Harty-Loomer classification a higher lesion stage (stage four and five) was associated with a higher preoperative pain intensity (ρ = 0.13). There was no difference according to the preoperative pain intensity between the group of patients that received a debridement of a bony or soft tissue impingement in addition to the cartilage therapy and the group of patients without this kind of additional therapy (p = 0.10). Further, there was no difference according to the preoperative pain intensity between the group of patients that received a stabilisation of the ankle joint in addition to the cartilage therapy and the group of patients without a stabilisation procedure (p = 0.83). Conclusion Osteochondral lesions of the ankle can be associated with a moderate and in some cases high pain intensity. In female patients a higher BMI is associated with a higher pain intensity. Further, a higher lesion stage according to the Berndt-Harty-Loomer classification is associated with a higher pain intensity, which highlights the clinical relevance of this classification.

Background There is little information in the literature about the outcomes of retrograde drilling for osteochondritis dissecans of the talus in a pediatric population. The purposes of this study were therefore to investigate the efficiency of retrograde drilling among a young population and to identify factors that are predictive for failure of the treatment. The primary objective was to retrospectively evaluate clinical outcomes of pediatric patients with OCD of the talus who have undergone retrograde drilling. The criteria for failure of treatment was the necessity for subsequent surgeries. The secondary objective was to identify factors predictive of a failure of retrograde drilling. Therefore, we tried to establish correlations between clinical outcomes and a range of factors such as demographic factors (age, sex, BMI, etc.) or clinical characteristics (lesion size, lesion stability, etc.). Methods A retrospective study was done. The data used for this study came from the medical records of pediatric patients (? 18 years old at time of treatment) who were treated by the principal investigator at the CHU Sainte-Justine. All patients were treated between September 2013 and December 2017. The selected patient files were obtained either through Chartmaxx software or by physically consulting charts at the medical archives of the hospital. The inclusion criteria for patients were: (1) an osteochondritis dissecans of the talus (2) no other lower limb injuries (3) retrograde drilling of the OCD (4) available pre- and post-operative imaging (5) at least one postop follow up visit. The information collected from each patient were the following: date of birth, age at surgery, sex, height, weight, BMI, affected ankle, location of OCD on talar dome, duration of symptoms, dimensions of lesion, history of ankle injuries, stability, status of tibial physis, date of surgery, details of surgery, follow-up duration, number of follow-up appointments, patient status at the last follow-up appointment, post-operative complications, necessity of subsequent surgeries, time interval between additional treatments and retrograde drilling. Clinical outcomes for each patient, determined at the last follow-up, were classified according to the Berndt and Harty treatment result grading. Regarding the secondary objectives, Pearson’s correlations were performed between the different patient variables and the necessity for another surgery and as well between the former and the clinical result Berndt and Harty score measured at the last follow-up appointment. Results Twenty-one patients were initially identified for this study and their medical records were reviewed. Of these original patients, 4 were left out because they had undergone microfracture procedure. In our revised population of 17 patients, there were 16 girls and 1 boy and the average age at the time of the procedure was of 14.8 (?2.12) years old. The mean follow-up period for this group was of 9.82 (?9.84) months. In terms of the osteochondral lesion, 1 patient presented with an OCD on the lateral part of the dome of the talus while 16 patients had theirs medially. Table 1 illustrates more in depth the characteristics of the group. Among the patients studied, 4/17 (24%) required a secondary surgery after retrograde drilling signifying that they had a failure of the initial treatment. Moreover, based on the Berndt and Harty treatment result grading measured at the last follow-up appointment, 8/17 (47%) had good results, 4/17 (24%) had fair results and 5/17 (29%) had poor results. The initial Pearson’s correlations do not indicate any links between the various patient variables and the necessity for a subsequent surgery or between patient variables and the clinical result scores. However, there is a correlation between the clinical results scores and the necessity for another treatment thus illustrating the reliability of the Berndt and Harty treatment result grading. Conclusion Despite being in a pediatric population, it seems that the clinical outcome of retrograde drilling has a limited success rate with only 47% of patients from this study having good results at the last follow-up appointment. Those results emphasize the importance of more studies in order to understand what factors are predictive of a bad and good outcome. [Table: see text]

In the 1940s “Kentucky” was the greatest hit of Karl and Harty, one of radio's most popular country music duos during the heyday of live hillbilly music in Chicago. Soon after it was released in 1941, aspects of “Kentucky” were already being forgotten—indeed, were predicated on forgetting outmoded racial formations and modes of song transmission—though the song is explicitly about remembering the lost spaces of rural, southern youth. The nostalgic sentimentality of “Kentucky” occludes a secondary stratum of musical and textual qualities that evoke racialized modes of dance and entertainment. Through close analysis, interviews, and archival work, we examine the song's racial and geographical signifiers and source models to show how tensions between its dual dialectic of memory/forgetting and sentimentality/entertainment participated in the mid-twentieth-century decline of hillbilly music and rise of commercial country.

Glycine plays an important role as an inhibitory neurotransmitter in the ventral cochlear nucleus. However, little is known about the kinetic behavior of glycine receptors. The present study examines the kinetics of the native inhibitory glycine receptors in neurons of the ventral cochlear nucleus, using outside-out patches from acutely dissociated cells and a fast flow system. Steps into 1 mM glycine revealed fast phases of desensitization with time constants of 13 and 129 ms, that together produced a 40% reduction in current from the peak response. Slower desensitization phases also were observed. After removal of glycine, currents deactivated with two time constants of 15 and 68 ms, and these rates were independent of the glycine concentration between 0.2 and 1 mM. Recovery from desensitization was slow relative to desensitization itself. These results demonstrate that glycine receptors can exhibit faster rates of desensitization and deactivation than previously reported.

In 2011, Brooklyn-based artist Heather Hart introduced her interactive installation The Oracle of Epicure: Tooth for Tooth. For this, she transcribed a selection of recipes onto note cards and adorned each with an original drawing. Filed in a recipe box and set on top of a desk, viewers were invited to take one card in exchange for a recipe of their own that was to be written from memory on the spot. This article gives the particulars of Heather's installation with special attention given to the source of her recipes: Harry H. Hart's Favorite Recipes of Williams College, with Training Table Records, Notes and Menus (1951). Harry Hart, a chef at Williams College from 1917 to 1954, was Heather's great grandfather. Harry Hart, Jr., Heather's grandfather, was also a chef at Williams College from 1938 until circa 1959. By way of Tooth for Tooth, the Hart family's rich culinary history is conveyed for the first time here.

Due to the extremely high incidence of lesions and diseases in aging population, it is critical to put all efforts into developing a successful implant for osteochondral tissue regeneration. Many of the patients undergoing surgery present osteochondral fissure extending until the subchondral bone (corresponding to a IV grade according to the conventional radiographic classification by Berndt and Harty). Therefore, strategies for functional tissue regeneration should also aim at healing the subchondral bone and joint interface, besides hyaline cartilage. With the ambition of contributing to solving this problem, several research groups have been working intensively on the development of tailored implants that could promote that complex osteochondral regeneration. These implants may be manufactured through a wide variety of processes and use a wide variety of (bio)materials. This review aimed to examine the state of the art regarding the challenges, advantages, and drawbacks of the current strategies for osteochondral regeneration. One of the most promising approaches relies on the principles of additive manufacturing, where technologies are used that allow for the production of complex 3D structures with a high level of control, intended and predefined geometry, size, and interconnected pores, in a reproducible way. However, not all materials are suitable for these processes, and their features should be examined, targeting a successful regeneration.

They were scored using the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale, and lesions were graded using the system described by Berndt and Harty. Results: Between 1993 and 2002, 808 consecutive ankle arthroscopies were performed by the senior author, of which 215 were to treat osteochondral lesions of the talus. Of these, 12 had repeat arthroscopies because of unresolved symptoms. AOFAS scores improved from a mean of 34.8 prior to arthroscopy to 80.5 after repeat arthroscopy at a mean followup of 5.9 years (18 months to 11 years). Two patients returned to professional sports after the second procedure. Six patients returned to their preinjury levels of sporting activity and three returned to the same sports but played to a lesser standard or less frequently. One patient had already had a cartilage transplantation procedure. Conclusions: This is the first series specifically assessing patients who have had repeat arthroscopic debridement of osteochondral lesions of the talus, using the same debridement technique by a single surgeon. Our results question the assumption that repeat arthroscopic debridement yields poor results. They also provide a baseline for the newer chondral and osteochondral transplantation techniques to compare to at the medium term.

ABSTRACT The Pseudomonas aeruginosa quorum-sensing molecule N-3-(oxododecanoyl)-l-homoserine lactone (OdDHL) has been reported to have immunomodulatory activity in several systems, although the mechanism of that activity remains to be fully characterized. We demonstrate here, using a defined in vitro model of antigen responses by T-cell receptor (TCR)-transgenic mouse splenic CD4 T cells, that the effect of OdDHL on activation and cytokine production is complete within 4 h of antigen or mitogen stimulation and does not depend on the insertion of OdDHL in the cell membrane, despite a previous report that immunosuppression by homoserine lactones required a minimum acyl chain length of 11 carbons (S. R. Chhabra, C. Harty, D. S. W. Hooi, M. Daykin, B. W. Bycroft, P. Williams, and D. Pritchard, J. Med. Chem. 46:97-104, 2003). We also demonstrate that while OdDHL can have toxic effects on nonlymphoid leukocytes, it does not induce significant cell death in T cells at the concentrations (≤10 μM) used in these experiments. In addition, we show that primary and secondary antigen-specific cytokine responses are equally susceptible to inhibition by OdDHL and that the compound inhibits the differentiation of both Th1 and Th2 cells. However, the precise balance of cytokine production by CD4 T cells stimulated in the presence of OdDHL varies with both the antigen concentration and its affinity for the transgenic TCR. Thus, conflicting reports of the nature of the immunosuppression by OdDHL may be due in part to the differences in antigen affinity and concentration in different models.

Study Design Fourteen cadaveri specimens were sectioned to analyze the internal architecture of the human calcaneus. We described the arrangement and orientation of trabecular patterns within the calcaneus and made multiple measurements of its cortical thickness. Objective To characterize the internal architecture of the calcaneus and correlate these findings with well-described patterns of calcaneus fracture in order to better understand the fracture mechanics of this common fracture. Methods Fourteen dry, frozen, human calcanei were sectioned using a saw. In each the coronal, sagittal and axial planes, we sectioned separate specimens into slices of 0.5mm thickness. High-resolution radiographic images were taken of the sectioned specimens. The internal trabecular arrays were described and measurements of cortical thickness were recorded. The correlation between these findings and the known pattern of calcaneal fractures was analyzed. RESULTS A dominant trabecular pattern running anteroposteriorly along the long axis of the calcaneus was observed. In the posterior tuberosity the trabeculae were arranged parallel to the posterior border. There was an area of sparse or absent mineralization in the anterior part of the calcaneus corresponding to the “neutral triangle” described by Wood and Harty 10, 23. The thickest sites of the calcaneal cortex were the lower pole of the posterior tuberosity, the upper surface at the angle of Gissane, and the lateral surface below the anterior portion of the posterior facet. Conclusion The trabecular architecture of the calcaneus is created by applied stress in concordance with Wolff's law. The weakest plane of resistance to stress is parallel to these organized trabeculae or through areas lacking trabeculae. This study demonstrates that the primary and secondary fracture lines commonly encountered in calcaneus fractures correlates with the internal architectural map of the calcaneal trabecular patterns.

Introduction: Osteochondral injuries involving the ankle joint are unusual (incidence of 0.09% according to Berndt and Harty), third in frequency after knee and elbow location. They are described as a cause of chronic pain after ankle sprains in the active population (thought to occur in 2-6% of sprains). MRI is the gold standard diagnostic method. Therapeutic strategies include both conservative and surgical treatment. The aim of our study was to evaluate the clinical and functional outcome of patients with osteochondral lesions of the talus. Materials and Methods: We retrospectively reviewed 20 patients with osteochondral lesions of the talus treated in our department between January 2007 and December 2012. Sixty per cent were male with an average age of 42 years. Eleven patients were treated conservatively, one of them had clear surgical indication (LOC G III, as classified by Ferkel and Sgaglione) but refused to perform the procedure. Nine patients underwent arthroscopic surgery (debridement and microfracture), one of the procedures was a review of an arthroscopy performed in another service. No open surgery was performed. Clinical and functional evaluation was performed using the AOFAS score, Freiburg and VAS Score System. Results: Non-surgical treatment group had a pretreatment average AOFAS score of 58, which improved to 74.8 points; a Freiburg Score System that ranged from 65 to 79.3 points and a VAS average of 5,4. AOFAS surgical treatment group improved from 54.3 to 84.8 points, Freiburg Score System ranged from 60.6 to 81.4 points and VAS average was of 5,8. Discussion: It is difficult to compare our results with other series of patients, because we made a comparison between conservative versus artrhoscopic treatment, while other authors show results obtained when performing certain surgical technique Although surgical treatment has better results, we agree with the literature that conservative treatment presents acceptable results and should always be considered as the first option.

This was the central argument in our November 2007 Forum article to which Harry, Hart, Klingner, Cramer, and Sturges responded. In this rejoinder, we continue to assert the dramatic need to offer appropriate educational services to all children with EBD independent of their personal identity and characteristics. Although we identify areas of agreement with Harry et al., we challenge many of their assertions related to our article and continue to affirm that underservice of learners with EBD is the greatest challenge.

Na podlagi analize njihovih teoretičnih spoznanj proučujemo nujnost razvijanja strateške misli v obrambnem planiranju in nujnost obstoja obrambnega planiranja. Ta je v sodobnem svetu prepoznan kot eden od najučinkovitejših instrumentov države pri zagotavljanju njenih obrambnih potreb in obvladovanju negotove prihodnosti nacionalne varnosti. Abstract The article presents scientific and applicative findings of important 19th, 20th and 21st century theoreticians and authors in the field of strategy theory, war and warfare theory and philosophy, and military history, who have contributed to the acknowledgment and reinforced importance of defence planning (Carl von Clausewitz, Liddell Hart, Bernard Brodie, Colin S. Gray, Harry R. Yarger, Williamson Murray et al.). On the basis of the analysis of their theoretical findings, we examine the urgency of developing a strategic thought in defence planning and the urgency of its existence. Pisarze ci, wyrastający z ortodoksyjnej tradycji kalwińskiej, dominującej na północy Holandii, reprezentują zarówno zanikające społeczności i emancypujące się grupy społeczne, jak i jednost­ki wchodzące w ich skład, typowe dla społeczeństwa holenderskiego, które współcześnie cechuje wysoki stopień sekularyzacji. ‘T Hart wyszydza kalwiński literalizm biblijny, jak też, podobnie jak Franca Treur, kreśli portret zamkniętych ludowych kalwińskich nisz, podczas gdy Jana Siebelinka fascynuje tragiczna mistyka łaski. T.S. Eliot w eseju Religia i literatura zauważa, iż kultura odchodząca od źródeł religijnych rodzi literaturę, w której religia stopniowo zmienia swą pozycję — początkowo wszechobecna, z czasem staje się przedmiotem sporu, by ostatecznie być postrzegana jako anachronizm. Idąc jego śladem, zauważyć można, iż w prozie ‘t Harta kalwinizm a właściwie cała tradycja chrześcijańska jest na­dal groźnym ideologicznym wrogiem, którego potrzeba obezwładnienia uzasadnia wszelkie chwyty, z nieszczędzącą niczego zjadliwą satyrą włącznie. Prozę Treur i Siebelinka cechuje natomiast nuta nostalgii i dwuznaczność, co lepiej predestynuje ich niż ‘t Harta do pełnienia funkcji strażników dziedzictwa kulturowego swoich środowisk.

Background Glycine and gamma-aminobutyric acid (GABA) are inhibitory neurotransmitters that appear to be important in sensory processing in the spinal dorsal horn. Intrathecal administration of strychnine (strychnine-sensitive glycine receptor antagonist) or bicuculline (GABAA antagonist) was reported to induce allodynia. Although the strychnine-induced allodynia was shown to be mediated through the N-methyl-D-aspartate (NMDA)-type glutamate receptor, it is not clear whether the bicuculline-evoked-allodynia is mediated through the glutamate receptor system or how different the allodynia induced by strychnine and bicuculline are. Methods Male ddY mice weighing 20 +/- 2 g were used in this study. A 27-G stainless-steel needle attached to a microsyringe was inserted between the L5 and L6 vertebrae by a slight modification of the method of Hylden and Wilcox. Drugs in vehicle were injected slowly into the subarachnoid space to conscious mice at 22 +/- 2 degrees C. The volume of the intrathecal injection was 5 microliters. Studies on allodynia were carried out essentially according to the method of Yaksh and Harty. Results The intrathecal administration of strychnine or bicuculline in conscious mice resulted in allodynia elicited by nonnoxious brushing of the flanks. The maximum allodynia induced by strychnine was observed 5 min after intrathecal injection, but that induced by bicuculline was observed 10 min after intrathecal injection. Both responses gradually decreased over the experimental period of 50 min. The allodynia induced by strychnine was dose-dependently relieved by NMDA receptor antagonists (D-AP5, ketamine, and 7-C1-KYNA) and non-NMDA receptor antagonists (GAMS and CNQX) but not by metabotropic receptor antagonists (L-AP3 and L-AP4). On the other hand, allodynia induced by bicuculline was dose-dependently relieved by GAMS, L-AP3, and L-AP4, but not by D-AP5, ketamine, 7-C1-KYNA, and CNQX. Whereas the strychnine-evoked allodynia was dose-dependently relieved by the nitric oxide synthase inhibitor N(omega)-nitro-L-arginine methyl ester (L-NAME) and the soluble guanylate cyclase inhibitor methylene blue, the bicuculline-induced one was dose-dependently relieved by methylene blue but not by L-NAME. Conclusions These results demonstrate that both strychnine- and bicuculline-evoked allodynia were mediated through pathways that include the glutamate receptor and nitric oxide systems but in a different manner. the current study suggests that GABA and glycine may modulate responses to an innocuous tactile stimulus as inhibitory neurotransmitters at presynaptic and postsynaptic sites in the spinal cord, respectively.
