Abstract:
This article discusses the effectiveness of digital health services by looking at the challenges and obstacles for society. One form of public services implemented by the government for the community is meeting the need for quality public services and having an orientation to the effectiveness of services to the community that are needed. In the health sector itself, advances in information technology have greatly supported health services. This online system is of high quality to be implemented because the public and members of the medical system can receive and provide detailed and fast information with the help of computerized technology. Information technology has reached various fields of life including the health sector to help the efficiency and effectiveness of health services. Information technology that is properly implemented can support the management process to be effective and efficient. The corona pandemic is changing people's habits in visiting and consulting doctors at hospitals. However, digital-based health services have not been effectively accepted by the public from various service units. They still need improvements in terms of connectivity, content and clarity of regulations.

Abstract:
This article reviews the hegemonic understanding of health services research in the United States and in the Anglo-Saxon world, taking as a point of reference the recently published PAHO anthology, Health Services Research: An Anthology (which contains the classics in health services research). The author outlines the main characteristics of health services research in the United States, and criticizes the focus on medical care of most health services research literature and the narrow spectrum of positions presented in that literature. He concludes that there is an urgent need for analyses of the socioeconomic and political forces that determine the level of health and the type of health services that exist in any society, analyses that are usually avoided in health services research.

Abstract:
Aged people are challenged by serious complications from chronic diseases, such as mood disorder, diabetes, heart disease, and infectious diseases, which are also the most common causes of death in older people. Therefore, elderly care facilities are more important than ever. The most common causes of death in elderly care facilities were reported to be diabetes, cardiovascular disease, and pneumonia. Recently, the coronavirus disease 2019 (COVID-19) pandemic have a great impact on blind spots of safety where aged people were isolated from society. Elderly care facilities were one of the blind spots in the midst of the pandemic, where major casualties were reported from COVID-19 complications because most people had one or two mortality risk factors, such as diabetes or cardiovascular disease. Therefore, medical governance of public health center and hospital, and elderly care facility is becoming important issue of priority. Thus, remote health monitoring service by the Internet of Medical Things (IoMT) sensors is more important than ever. Recently, technological breakthroughs have enabled healthcare professionals to have easy access to patients in medical blind spots through the use of IoT sensors. These sensors can detect medically urgent situations in a timely fashion and make medical decisions for aged people in elderly care facilities. Real-time electrocardiograms and blood sugar monitoring sensors are approved by the medical insurance service. Real-time monitoring services in medical blind spots, such as elderly care facilities, has been suggested. Heart rhythm monitoring could play a role in detecting early cardiovascular disease events and monitoring blood glucose levels in the management of chronic diseases, such as diabetes, in aged people in elderly care facilities. This review presents the potential usefulness of remote monitoring with IoMT sensors in medical blind spots and clinical suggestions for applications.

Abstract:
Introduction. In the system of measures to improve citizens' social guarantees, their right to health care occupies an important place. The European Social Charter obliges states to create a health care system that will contribute to improving the level of public health, reduce the state's susceptibility to crisis phenomena, and increase labor productivity and the standard of living of all segments of the population. The health care system, like the economy of our country, is in a state of war, has a number of significant shortcomings that make it ineffective and needs to be adjusted taking into account the problems it is experiencing. World experience shows that there may be different ways of solving this problem. In general, they differ in the structure of health care financing sources and the organization of the provision of medical services. In this regard, the study of the development of health insurance as an effective tool for providing guarantees for the population regarding health care issues becomes important in this regard. Practice shows that the current budgetary model of financing the health care sector is not able to independently cover the needs of the population in terms of the amount of free medical care guaranteed by the country's legislation. Due to the lack of a clear distinction between guaranteed volumes and medical services provided to citizens on a paid basis, free medical care is replaced by various paid medical services, in particular, medical insurance. If we consider health insurance from a socio-economic point of view, it is one of the most important components of the effective functioning of national health care systems, the most effective form of social protection against risks that threaten the health and life of citizens, ensures availability, quality and complete satisfaction of various needs of the population in providing and receiving medical services. Purpose. To analyse the health index of citizens according to various indicators; to summarize the contents of the Medical Insurance Programs against COVID-19; to systematize the structure of types of health insurance as an important component of social guarantees of society and to determine directions for its improvement based on indicators of insurance premiums and insurance payments. Method (methodology). The authors used the abstract-logical method and the method of observation to form theoretical conditions and conclusions; structural method, comparative method for the analysis of health insurance and its development. Results. The article examines health insurance as a component of the development of social guarantees of society; the "health index" of citizens is highlighted; a rating of insurance companies providing voluntary health insurance services was carried out based on the indicators of insurance premiums and insurance payments; types of medical insurance were analyzed by gross/net insurance premiums and payments; the dynamics of health care expenditures in Ukraine are characterized; the content of the COVID-19 insurance programs is characterized; it was determined that the dynamics of wages is important for the wide use of social security services.

Abstract:
Since its reform and opening in 1978, China’s economy has grown rapidly and human life and physical well-being have improved significantly. The public health care system is an imperative guarantee of people’s life and physical health. It is necessary to establish a reasonable planning, a reasonable division of labor, and an efficient medical service system to improve the overall efficiency of health care services and ensure the physical health of the people. The balance of public health care services plays a very important role. It plays a very important role in safeguarding people’s basic right to health, regulating social fairness and justice, promoting the sustainable development of medical and health services, and building a harmonious society. Based on 5G sensor technology, this paper combines relevant research on sensor technology at home and abroad and comprehensively uses related theories such as sensing principles and health care services to discuss the internal operation mechanism of China’s public health care service system. It conducts experiments to compare health care services with 5G sensor technology with existing health care service systems. It compares its bed occupancy rates, hospital admissions and discharges, outpatient and emergency department volumes, and average inpatient costs. Abstract:
All countries of the world confront complex dilemmas when dealing with their respective health sectors. In the industrialized democracies a seemingly insatiable demand for health care is outstripping supply, despite a relentless increase in the latter's share of national budgets and family incomes. Yet, there is little corresponding rise in general health indices, or even in human happiness about the quality of medical services. The inability of health services to deliver greater health for more money has ironically not blunted the public's appetite for them; rather, it has perversely increased it. Some of the evident reasons for this paradox are the following: (a) affluent humanity is less prepared than ever before to suffer minor ailments without drugs or other medical help; (b) demand for health care has been further stimulated by both new treatments for curable diseases and expanded coverage throughout the poorer levels of society; (c) new cures for old diseases come with ever higher price tags for their sophisticated technology so that much additional spending still saves few lives; and (d) the elderly, whose relative numbers in society are growing, require more routine medical care than the young. Clearly, health services in the developed North are victims of their own successes.

Aim. The aim of this paper is to present the possibilities of application and use of modern information and communication technologies that can improve multidimensional, holistic nursing care of the patient. Methods. The data was collected from literature reviews. Results and Conclusion. Tele-diagnosis, teleconsultation and tele-nursing, i.e. services for monitoring the patient's condition and life parameters, and facilitating therapy and rehabilitation at a distance, are a response to the growing demand for modern medical services in an ageing society. This paper discuss e-services as e-consultations, which are a direct and quick possibility for health check, Internet Patient Account (IKP), e-prescriptions, e-sick leave and also the project "TeleNrsCare”, which create a network for the exchange of knowledge, technology, experience and best practices. E-health and information and communication technologies can bring benefits to the whole society by improving access to medical services and improving their quality. An ageing society and an increasing number of those who are chronically ill and stay at home result in the need for a highly qualified staff and relevant technology to quickly and efficiently provide them medical assistance.

Abstract:
The relevance of the research topic is determined by the importance of medicine for modern society. The right of a citizen to timely medical care is enshrined in the Constitution of the Russian Federation and is one of the most important social obligations of the state. The need for legal regulation of this sphere arises from the variety of types of medical care and the variety of medical services. Taking into account that medicine affects the health of citizens and if the quality of services is inadequate, it can lead to significant harm to the patient, up to death, the legal basis for providing medical services should be elaborated in detail and avoid double interpretation of various legal norms. With the introduction of market principles in Russia, a new sphere of providing medical services to the population – paid. This allowed us to solve a number of problems related to improving the quality of medical services, their availability in terms of receiving highly specialized care, and reducing the burden on the insurance medicine system. At the same time, the need for a detailed legal justification for the provision of such services has become obvious, and, first of all, the issues of the contract for paid medical services, as the main form of business relations, enshrined in civil law. It is necessary to pay attention to the fact that in modern Russian legislation there is a certain discrepancy between the social significance of the health care industry, as well as the constitutional status of the right to health protection and an unreasonably low level of theoretical justification for the branch legal regime in this area of health care. Therefore, it is necessary to further modernize certain provisions of medical law that regulate various aspects of the provision of medical services. This includes issues related to the contract for paid medical services.

Abstract:
Medical schools realize their social accountability in cultivating future doctors as decision-makers of medical care in the future society as an axis of society and carrying out communication, research, and medical service (medical care). This study summarized the social accountability status of eight medical schools in Korea, including the characteristics, activities, and programs in the sociocultural context. The data from the self-evaluation research reports of eight medical schools were classified and organized according to education, research, and medical care services. The medical schools offer related subjects, community service activities, and clinical practice, allowing active participation in local health promotion and disease prevention. Community health information research was conducted considering the difference between the local natural environment in which the medical school is located and the frequency of diseases. Medical schools and hospitals collaborated to conduct domestic and foreign health education volunteer activities. Social accountability must be prioritized, considering the importance of medical schools’ functions and roles in a high knowledge-information society. Both doctors’ medical capacity and competence to actively participate in developing medical care and health policies for the community should be strengthened.

Abstract:
Since 2001, China has been an aging society; it is expected to become superaged by 2033. This rapid aging trend poses a challenge to the elderly regarding their pension services and healthcare. Primary healthcare has great potential for serving older adults in the community, yet it is not popular. This study used 1977 samples from the 2018 China Health and Retirement Longitudinal Study database to explore the use of outpatient services in primary care institutions among the middle-aged and elderly. Using a structural equations model, we constructed a framework to explore pathways leading to primary outpatient use. We discovered that the supply of primary health services had a significant direct and mediating effect on the utilization of primary outpatient services, and that community pension services may indirectly discourage it. In addition, the supply of primary health services has a suppressor effect between medical insurance and primary outpatient utilization. Health insurance directly promotes primary outpatient utilization, while the supply of primary care institutions suppresses the positive influence of medical insurance on the utilization of primary outpatient services. Therefore, community pension services should pay attention to differentiated services. Moreover, adjusting the coordinated development of medical insurance and the supply of primary healthcare could enhance the positive effects of medical insurance for outpatients.

Doctors as medical workers are at the forefront of health services for Covid-19 patients. During the pandemic, doctor is the profession that has the highest risk in handling Covid-19 patients. In this case, it is appropriate if doctors get legal protection in carrying out their profession. So, what is the actual form of legal protection that doctors have received so far in handling Covid-19 patients? Have the existing regulations accommodated this protection?. The method of research which used in this research was normatif by examining the law which is conceptualized as a norm or rule that applied in society, and becomes a reference for everyone's behavior. The results of the study indicated that there were preventive and repressive measures which were as means of legal protection for doctors during Pandemic. Preventive efforts can be interpreted as steps or ways that can be taken to prevent an event that has legal consequences in the form of fulfilling obligations as a doctor, namely by carrying out the profession in accordance with professional standard, professional service standard, and standard operating procedures, completing administration in medical practice such as informed consent. and medical records, getting vaccines, providing incentives. While repressive efforts are defined as steps or method which taken if an event that results in law has occurred in the form of providing compensation to doctors who died, giving awards for services, bearing medical expenses for exposed doctors, and prosecution for criminal acts towards people who do not orderly implement health protocols. The legal protection of medical workers in handling Covid-19 is evident in the fact that these medical workers have received legal protection in the form of supervision and guidance carried out by the Central Government, Regional Government or their Work Agencies. It shows that existing regulations have accommodated legal protection for doctors during the Covid-19 pandemic.

In 2012 there were 200 cases of public service disputes to be criminalized in East Java. They occured as a logical consequence of Act Number 14 Year 2008 leading to some consequences that public service is required to give satisfaction to the society The problem of health services in Indonesia cannot be separated from the low competence of the medical personnel, infrastructure and medical equipment, human resources, and complex regulations which are not easy to implement. Due to the problem above, a research focused on such policies to improve the capacity building by optimizing the role of Primary Health Center (PHC) as First Level Health Facility (FLHF) especially for Participants Social Health Insurance Provider (SHIP) is highly considered to carry out. The Social Health Insurance Provider is a legal entity established to administer the Health Insurance program, and the Primary Health Center is a health service facility that organizes some efforts on public and individual health at the first level. In ensuring the satisfaction of adequate services, FLHF has been working with PHC as the implementer of health services for SHIP participants. Because of it, PHC becomes the forefront to provide the health services to the community, especially, to SHIP participants. To increase the satisfaction of SHIP participants, it is necessary to note and find some ways out to the problems related to the improvement of Human Resources, Health Facilities, Service system, Information and supervision.

Abstract:
This article presents the results of research and development of an innovation ecosystem for for-profit medical services provided to residents of the Kaliningrad region at the medical centre of the Immanuel Kant Baltic Federal University. These services contribute to the sustainable development of regional healthcare in line with the current green growth trends. A universal model of an ecosystem for providing medical services to residents of a region is proposed, along with a model for a platform ecosystem of a university medical centre. The desk and field marketing research enabled the first stage of ecosystem creation – the formation of the value proposition, which is the provision of “complex case” medical services. This approach to medical care will preserve regional human resources and encourage residents to pay more attention to their health, the environment, and natural resources. Another positive result will be harmonising the environmental needs of society with global trends in sustainability and green growth.

Studies and reports on uptake of health services in Nepal and other low-income countries often focus on limitations due to physical factors, such as travel distance to health facility, or lack of medical facilities or electricity at the health care centre or focus on resources, such as lack of service providers, or lack of appropriately trained staff. In this editorial article, we highlight the importance of discrimination as a reason for people not seeking available health care. Discrimination is particularly a barrier to service usage among the most deprived people in society, such as the Dalit community in Nepal and South Asia more generally. We discuss the caste-based discrimination in Nepal and its effects on health outcomes of those groups who experience such discrimination.

Abstract Research and innovation in personalized medicine (PM) are extensive and expanding, with several pharmacogenetic/pharmacogenomic (PGx) testing options currently available for a wide range of health problems. However, PGx-guided therapy faces many barriers to full integration into clinical practice and acceptance by practitioner/patient: utilization and uptake by payers in real-world practice are being discussed, and the criteria to guide clinicians and policy makers in PGx test selection are not fully incorporated. This review focuses on the advances of pharmacogenomics to individualize treatments, the relationship between pharmacogenetics and pharmacometabolomics, the new paradigm of the Big Data, the needs and barriers facing PGx clinical application and the situation of PGx testing in health national services. It is based on lectures presented by speakers of the European Society of Pharmacogenomics and Personalised Therapy (ESPT) Fourth Conference, held in Catania, October 4th, 2017.

Abstract:
Nigerian women living with HIV/AIDS do face social, psychological and physical challenges in their daily lives. The objectives of this study were to determine how elements of the socio-medical approach have contributed to managing their health problems in Calabar metropolis, Nigeria. Six hundred women, aged 15-60 years, were sampled from Hearth-to-Heart Centers for their opinions on the socio-medical approach to managing their condition. The women expressed reasonable satisfaction with the availability and accessibility of socio-medical services: all of them received counselling, 92.2% were counselled on adherence, 81.7% received anti-retro viral services and 64.2% enjoyed family planning services. Some of the routine challenges highlighted by the women were poverty and fear of rejection by society. Also, the use of family planning does hinder their adherence to antiretroviral treatment. Overall, the socio-medical approach has been successful in managing their illness; more women should thus avail themselves of the opportunity of enjoying good health despite their HIV status. Abstract:
Recent research has shown the efficacy of screening for serious medical conditions from data collected while people interact with online services. In particular, queries to search engines and the interactions with them were shown to be advantageous for screening a range of conditions including diabetes, several forms of cancer, eating disorders, and depression. These screening abilities offer unique advantages in that they can serve a broad strata of the society, including people in underserved populations and in countries with poor access to medical services. However, these advantages need to be balanced against the potential harm to privacy, autonomy, and nonmaleficence, which are recognized as the cornerstones of ethical medical care. Here, we discuss these opportunities and challenges, both when collecting data to develop online screening services and when deploying them. We offer several solutions that balance the advantages of these services with the ethical challenges they pose.

Abstract:
Investigation of the key indicators of health promotion policies for an aging society can shed light on the priority of the government’s health promotion efforts. This study applied the fuzzy analytic hierarchy process (AHP) method in an empirical analysis of the key indicators of the health promotion policies for Taiwan’s aging society. Based on a review of the literature and expert interviews, this paper first conducted a preliminary study and evaluation of major factors affecting health promotion policies and found 4 major evaluation aspects and 16 evaluation indicators. After employing an AHP expert questionnaire in an empirical investigation, the following findings were made: (1) “healthy living” was the most important evaluation aspect for Taiwan’s health promotion policies for its aging society. (2) The six leading key indicators of health promotion policies were “promotion of personal health awareness and behavior,” “promotion of home medical services,” “guaranteeing the economic security of the elderly,” “planning a family caregiver support service system,” “well-planned health promotion plans for the elderly,” and “training long-term care service personnel and providing professional medical care.” Following discussion, recommendations concerning these six key indicators are made as a reference for future evaluation of health promotion policies for an aging society.

Abstract:
This article deals with issues that are directly related to the regulatory legal regulation of the health care system in the Russian Federation. The paper analyzes the norms regulating both the provision of medical services itself and the developing activities of entrepreneurs in the field of providing medical services on a paid basis. Today, the state's policy is aimed at forming a healthy lifestyle of civil society, at making them aware of the value of life and health. in the context of the emergence of new risks and, in this regard, the introduction of new restrictions in the life of society. In this light, the development of private medical practice already has a very different meaning. The fear of people when contacting doctors in state polyclinics, the need for a long time to stay in queues for tests, leads to the desire of people to increasingly turn to private clinics, in which the appointment is made by appointment at a certain set time, and tests can be passed not according to the strictly limited two hours in state medical institutions, but choose a more suitable and convenient time. Problems of providing medical services and medical care were analyzed during data collection through interviews and interviews with patients of polyclinics and ordinary citizens who want to express their opinions. The analysis of the legislation that changed in late 2020 - early 2021 brought some information on positive and not quite innovations that are already in effect on the territory of the state. The paper analyzes the regulatory legal acts that allow the state institution of the Russian healthcare system to provide medical services on a paid basis. The problems that require legislative regulation for the foundations of the activities of private medical institutions for the provision of medical care, medical services and counseling of citizens who have applied to them are considered. The results of the study show that it is possible to make certain changes in the activities of the health care system of the Russian Federation by organizing co-financing of patients from insurance organizations, associated with the use of a progressive taxation scale, as well as the obligation of private clinics to provide certain types of services, for example, counseling, in the direction issued by a state medical institution in the absence of a narrow specialist. Consequently, the development of domestic health care, taking into account a fairly wide range of services provided under compulsory health insurance policies, can be assessed positively. To regulate the activities and development of private business in the organization and operation of medical institutions, it is necessary to introduce rules that allow you to control the tariffs for services and assistance provided, contracts and agreements offered to those who apply, as well as to introduce a system of expert assessments to facilitate the resolution of disputes arising on the work of medical institutions on a commercial basis.

Abstract:
Abstract This paper summarizes the role of Public Health specialists within the UK Defence Medical Services (DMS). The armed forces have a need for expert advice on health improvement, health protection and healthcare public health. The first professor of military hygiene, Dr Edmund Parkes, was a leading pioneer in the public health movement of the late 19th century. Since then, the armed forces have evolved the term ‘hygiene’, though ‘health’, to ‘well-being’. Military doctors with an interest in medical administration and the health of populations have longstanding links with their civilian peers, through the Society of Medical Officers of Health and its successor the Faculty of Public Health. The specialty of Public Health in the armed forces is multidisciplinary, following the same educational pathway as civilian peers. The speciality has made important contributions during global health emergencies including the Ebola outbreak in 2014 and the recent COVID-19 pandemic. Although a small cadre, within the DMS and in the UK Public Health workforce, they have an important role in keeping our armed forces ready for operations. We celebrate the 50th anniversary of the Faculty of Public Health and expect that the DMS will contribute to the further development of the specialty.

Abstract:
The article reveals the essence of health capital based on a review and theoretical analysis of scientific works of scientists on this topic. Based on the analysis of its development factors, negative trends in life expectancy and mortality caused by the pandemic were identified in Kazakhstan. The article also presents the results of quantitative studies conducted by interviewing patients, as well as an expert survey. These studies have identified existing problems in health care and made recommendations to address them. The pandemic and its consequences have seriously affected both the country’s economy and the quality of health capital in Kazakhstan. Along with other social and economic factors, the pandemic has led to a decrease in life expectancy and an increase in mortality in the country. Studies have found that not all patients receiving treatment in medical institutions of Kazakhstan are satisfied with the quality of medical services received. The poor quality of health services not only exacerbates the diseases of patients, but also leads to significant economic costs for health care and society as a whole. The quality of medical services will be improved by increasing financing for healthcare, digitalization and the introduction of artificial intelligence in the industry, improving the skills of doctors and their salaries, using modern medical equipment, staffing medical personnel, timely prevention and promotion of a healthy lifestyle. The identified negative trends in the development of health capital in Kazakhstan point to the objective need to increase health spending, which are profitable investments that contribute to both increasing the productivity of workers and improving the health of the individual. To improve the quality of health capital, it is necessary to move from the concept of improving health care to the concept of monitoring, premature prevention and strengthening public health.

Abstract:
As untact communication is promoted in the era of the COVID-19 pandemic, special attention is paid to remote medical examination and customized healthcare trends. General digital healthcare services among social community members positively affect individuals’ healthcare and reduce medical social services’ burden, contributing to the development of society. Accordingly, it is necessary to induce healthcare behaviors through the continuous usage of digital healthcare services among social community members and to examine significant social impact factors in this regard. This study empirically analyzes the impact of three social impact factors – social capital, social support, and social value – on the continuous usage of digital healthcare service with healthcare behaviors and e-health literacy as media. To this end, a survey was conducted among 363 individuals who had used digital healthcare services in Korea, and the statistical data were analyzed. Social capital and social value were found to affect healthcare behaviors, e-health literacy, and continuous usage intentions, but social support did not. Based on this result, it was confirmed that the factors regarded by digital healthcare service users as necessary were the values and perceptions shared in society and the group, information and active communication rather than direct public support.

Medical and long-term care costs are increasing all over the world. In this study, we investigated the characteristics of groups with high cost of medical and long-term care to define targets for curbing social security costs. As a result, for the population covered by the National Health Insurance, a large portion of medical costs were incurred for mental disorders, malignant neoplasms, and lifestyle-related diseases. For those covered by the Late Elderly Health Insurance System, most medical costs were incurred for lifestyle-related diseases, femoral fractures, neurological diseases, mental disorders, pneumonia, malignant neoplasms, and Alzheimer’s disease. From multiple regression analysis, the hospitalization days, use of advanced medical treatment, outpatient days, and high long-term care level influenced the increased costs. On the other hand, disease characteristics had only a very low effect. These findings suggest that the target population has complex medical and long-term care needs because they have multiple diseases.

Abstract:
Hospital Based Health Technology Assessment (HB-HTA) is a new policy implemented in Poland to allow for a more practical and contextualized assessment related to the use of specific medical procedures, devices, or equipment. It requires changes in governance relating to the healthcare sector. One of the forms of governance improvement is to involve society in the process of creating public services. This can be implemented, e.g., by applying the pragmatic model of public responsiveness. The aim of this research was to identify and analyze forces which will shape a dynamic process in determining the implementation of HB-HTA. The results obtained in the Gioia analysis led to the identification of the main forces driving and restraining the implementation of HB-HTA. The grouping and interpretation allowed for the twelve most important dimensions to be distinguished, which were recognized as conceptual categories necessary to build theories that describe the studied phenomenon. This study contributes to the development of the idea of responsiveness in public management theory and in health care services, and ultimately helps to better enable the adjustment of health services to the dynamically changing needs of Polish society.

AbstractIntroduction: On 26 December 2003, a catastrophic earthquake measuring 6.6 on the Richter scale devastated large areas of the city of Bam in south-eastern Iran. More than 40,000 people died, tens of thousands were injured, and almost 20,000 homes were destroyed. Many national and international search-and-rescue teams were dispatched to the area to provide medical and health services and assist in the evacuation of survivors to undamaged areas.Problem: The purpose of the study was to evaluate the opinions of survivors about medical responses provided, and the process of reconstruction of health infrastructures. Methods: This was a descriptive study performed two years after the earthquake. Stratified, two-stage area sampling was used to enroll 211 survivors into the survey. A designed questionnaire was applied to evaluate the respondents' opinions about medical and health responses. The respondents were asked to score their satisfaction on a variety of services on a five-point scale, with 1 being “very poor” and 5 being “very good”.Results: Family members and relatives comprised the majority of first responders for those injured or trapped (127, 60.2%). Field hospitals deployed by the Red Crescent, international relief teams, and military forces were the first medical facilities for 98 (46.4%) of the casualties. As denoted by the mean values for the satisfaction scores, transportation by aircraft to the backup hospitals received the highest score (4.2), followed by international assistance (4.1), first medical care (3.5), search and rescue (3.3), primary transportation (3.1), and reconstruction and the quality of access to the infrastructures of the city (2.6). Two years after the earthquake, 151 (71.5%) respondents still were living in connexes (temporary accommodations or shelters for victims to live in; resemble a small hotel), only 33 (15.6%) had access to safe drinking water, and 44 (20.9%) did not have sufficient supplies of sanitary food. Conclusions: In addition to reinforcing the medical and health infrastructures of a society in accordance with geographical and architectural characteristics, effective air evacuation and relief missions carried out by experienced international relief teams can play an important role in the appropriate management of approximately 30,000 casualties after a catastrophic event, such as experience with the Bam Earthquake.

Most women want to be involved in their healthcare decisions and need timely, free, and accurate information. We conducted a needs assessment in Douglas County, Kansas by systematically reviewing regional organizations that might offer services and resources related to women’s health in midlife. We compared these resources to a benchmark for menopausal care available online nationwide (Gennev.com). We documented the primary purpose of each organization (e.g., cosmetic, wellness, medical care), services and resources offered (e.g., hormone therapy, counseling, non-pharmacological treatments), methods of outreach (e.g., blogs, classes), target audience, costs, and types of service providers (e.g., physician, counselor). We surveyed 9 regional websites: 5 offered medical care, 3 cosmetic and wellness services, 2 were municipal organizations, 2 offered mental health/social support. Four organizations offered services targeted specifically towards perimenopausal women. The most commonly offered services were hormone replacement therapy (44%), nutritional supplements (33%), and weight loss programs (33%). Very few offered educational resources (1) or menopause assessments (1) and none offered tailored psychosocial support for the perimenopause transition. The services offered were expensive with no free services and very few free resources. Organizations were primarily staffed by medical providers, only 1 organization had Menopause Practitioners certified by the North American Menopause Society. Our results demonstrate a need for comprehensive educational and support services for perimenopausal women to fulfill the need for timely, accessible, and accurate information during this understudied health transition.

Abstract:
In the context of the era of big data, the management of Alzheimer’s patients has aroused widespread concern in the society. What should the elderly and people with dementia do and how the society should accommodate these special groups have aroused heated discussions in the society. The goal of the medical-nursing integration model is to realize the integrated model of medical care-nursing-rehabilitation-old care, and to better satisfy the needs by providing medical services, rehabilitation care, and health management. The medical needs of the elderly can help the elderly maintain a healthy state; the elderly care needs can be better met through elderly care services, and the burden on families and society can be reduced. With the advancement of the medical-care integration model, new solutions have been provided for the management of senile dementia patients. Therefore, under the medical-care integration model, this paper managed Alzheimer’s patients based on the big data algorithm based on association rules and compared it with the management of Alzheimer’s patients under the traditional model, and drew the following conclusions: compared with the management satisfaction of Alzheimer’s patients under the traditional model, family members and patients’ total satisfaction with nursing management under the medical-care integration model was greatly improved; compared with the management of Alzheimer’s patients under the traditional model, the incidence of adverse events in the management of Alzheimer’s patients under the medical-care integration model was greatly reduced, which was reduced to 17.6%; under the medical-care integration model, the anxiety and depression scores of the elderly patients with dementia decreased rapidly; under the medical-care integration model, the living ability of senile dementia patients has been greatly improved; the use of the medical-care integration model to manage senile dementia patients has greatly improved the sleep quality, mental quality, and quality of life of senile dementia patients.

Abstract:
Research on the potential accessibility of medical services has made great progress, but there is a large gap between the analysis results and the actual feelings of residents. With the refinement of urban management, the need for actual accessibility calculations reflecting the current status of medical service levels is becoming stronger. In modern society, as people work and live at an increasingly fast pace, people increasingly focus on time saving. However, in addition to travel time and distance, personal perceptions of medical facilities and access habits also influence residents’ choice of specific hospitals for medical treatment. With the combined effect of these factors, the actual status of accessibility of medical facility services is formed. In order to improve estimates of the actual accessibility and narrow the gap with residents’ subjective perceptions, this study leverages realistic data, such as real-time navigation prediction data that approximates residents’ actual travel time to hospitals and information on residents’ subjective behaviors in choosing specific hospitals for medical treatment. Finally, a new approach is proposed to further improve the existing Gaussian two-step floating catchment area (Ga2SFCA) method by fully respecting the important effects of distance cost and time cost, and combining them by using a weighted mean.

Abstract:
As human lifespan increases and the need for elderly care grows, the demand for healthcare services and its associated costs have surged, causing a decline in the operational efficiency of universal healthcare. This has created an imbalance in medical services across different regions, posing a long-standing challenge for the public. To address this issue, strategies to enhance the capacity, efficiency, and quality of healthcare services in various regions must be developed. The appropriate allocation of medical resources is a fundamental requirement for countries to establish a robust healthcare system. This empirical study utilized data envelopment analysis (DEA) to evaluate the efficiency of medical service capacity and identify potential improvement strategies for counties and cities in Taiwan during the period from 2015 to 2020. The results of this study show that (1) the annual average efficiency of medical service capacity in Taiwan is approximately 90%, indicating that there is still room for a 10% improvement; (2) among the six municipalities, only Taipei City has sufficient healthcare capacity, whereas the efficiency of the remaining municipalities needs improvement; and (3) most counties and cities demonstrated increasing returns to scale, indicating a need to scale up the capacity of medical services as appropriate. Based on the findings of this study, it is recommended that medical personnel be increased accordingly to balance the workload, a favorable working environment be provided to stabilize the medical workforce, and urban–rural medical disparities be balanced to improve service quality and reduce cross-regional health services. These recommendations are expected to provide a reference for society as a whole to promote and enhance public health policies, leading to a continual improvement of the quality of medical services.

Abstract:
Taiwan is expected to become a superaged society by 2026. Community pharmacies have recently joined Taiwan’s primary care system; they have great potential to provide professional healthcare services. This study examined whether the services provided by community pharmacists enhance medication adherence, enable the identification and solution of drug therapy problems, and are accepted by community residents. The Department of Public Health, Taoyuan City, collaborated with the Taoyuan Pharmacist Association over 11 months in 2018 in enabling pharmacists to dispense prescriptions and provide medication adherence consultations, cognitive services, and home and institutional medical care services. This study designed four satisfaction questionnaires to assess the feasibility and performance of these services. Regarding the services related to medication knowledge and adherence, 92.10% of the patients reported overall satisfaction, and all understanding and ability scores were improved in more than 95% of patients. The number of patients highly cooperative regarding their medication had risen from 14 to 234 after the intervention, and the number with low medication adherence had dropped from 533 to 33. More than 90% of respondents indicated that the institutional medical care services had significantly improved their medication knowledge and behaviors. The feasibility of the incorporation of integrated the public health services model into age-friendly pharmacies was confirmed by this study.

Abstract:
From 1879, evangelical missions aimed specifically at Jews began providing free medical services to the newly arrived immigrant community in London's East End. This article focuses on three specific medical missions to Jews belonging to the London Society for Promoting Christianity among the Jews, the British Society for the Propagation of the Gospel amongst the Jews and the Mildmay Mission to the Jews. It considers the particular attractions of these medical missions in terms of what they were able to offer the immigrant Jew that existing state and voluntary medical services did not provide, alongside the cost and possible risk posed by attendance. The article questions whether the popularity of evangelical medical missions within the Jewish East End is as surprising as it may first appear, if the limited health care options available to the nineteenth-century poor are considered in conjunction with the additional obstacles facing Jewish immigrants, such as cultural and religious differences, anti-Jewish prejudice and most notably the language barrier.
