AbstractThrough the postmortem examination, pathologists offer the ultimate clinico-pathologic assessment of the efficacy of medical care. Similarly, pathologists can offer a clinico-pathologic assessment of the efficacy of health care technology. Assessment by pathologists has diagnostic authority because it draws on the resources of the laboratories of surgical and necropsy pathology. In this essay we argue for enhancing the accuracy of medical device and biomaterials technology assessment by systematically collecting pathology-oriented data. We recommend the establishment of a pathology-based medical device registry to assess implantable medical device technology by accumulating reports routinely issued by pathology departments throughout the country. We further suggest the establishment of a university-based, industry-supported Medical Device and Biomaterials Pathology Institute to operate the registry, collect recovered, used health care devices, and generate definitive, pathology-based, primary data for he lth care technology assessment

<p> Problem Statement: Military members and their family members who are part of the Global War on Terrorism have experienced deployments and war for over sixteen years and with the resulting toll on both the veteran and their intimate partner. As a result, higher levels of pathology, such as PTSD, Anxiety, and Depression have been experienced by this population. While research has studied the effect of combat on military members, very little research has addressed the effects on family members. Further, no research that has been found by this researcher, has addressed the concept of resonating of pathology between the combat veteran and their intimate partner. Resonating of Pathology, or resonating pathology, for the purposes of this study, is the combat veteran and the intimate partner demonstrating the same pathology at similar levels. </p><p> The Veteran&rsquo;s Healthcare Services have begun to treat couples together and may have also witnessed this phenomenon. Understanding the relationship between combat, veteran pathology, and intimate partner pathology will have implications for practitioners and researchers. Understanding the factors that related to this phenomenon will have implications for both clinicians and researchers. </p><p> Methods: Veterans and their intimate partners from across the United States, (<i>N</i> = 398), were asked to complete a survey through the internet. The couples were asked to complete the survey separately, however, within the same session so that their results could be tied together. The veterans were asked to complete the Combat Exposure Scale (CES), the PTSD Check List for the DSM 5 (PCL&ndash;5), the Patient Health Questionnaire 9 (PHQ&ndash;9), and the Generalized Anxiety Disorder 7 (GAD&ndash;7). The intimate partners were asked to complete the PCL&ndash;5, the PHQ&ndash;9, and the GAD&ndash;7. </p><p> Results: Results suggest that there was a moderate relationship between combat experience and pathology for combat veterans. While other factors were examined to determine what was included to predict pathology within the veteran, combat experience seemed to be the primary factor for predicting PTSD, Anxiety, and Depression in the combat veteran. </p><p> Results also suggested that there was a relationship between the pathology of combat veterans and the pathology their intimate partners, as measured in this research. Not only was there resonating, or resonating of pathology, within the couples, but this tended to occur within categories of pathology such as moderate and severe levels of Anxiety and Depression. While multiple factors such as frequency of communications, number of children, and types of communication were examined, combat experiences tended to be the primary factor for predicting pathology in both the veteran and the intimate partner. </p><p> Conclusion: Combat veterans and their intimate partners appear to be experiencing the phenomenon of resonating, or resonating, of pathology. Couple-analysis demonstrates that this phenomenon is being experienced as a couple and suggests that could have implications for future research and clinical practice. Demographic factors did not seem to influence the pathology for either the veteran or their partner. Combat experience does, however, seem to be a predictor for pathology in not only the veteran but also for the intimate partner as well. </p><p>

Thesis (PhD)--Stellenbosch University, 2014.<br>ENGLISH ABSTRACT: Tuberculosis (TB) is a chronic pulmonary infection characterized by granulomatous inflammation, caseating necrosis and a propensity to develop fibrosis and cavitations. Pulmonary TB (PTB) lesions may develop in a variety of ways and can be grouped into primary, secondary, progressive primary and miliary TB based on their pathogenesis and morphological appearance. The Western Cape Province, South Africa, has a high TB burden with increasing TB notification rates. At Stellenbosch University (SU), approximately 90% of cadavers used for medical dissections come from impoverished communities where TB is a major health problem in terms of morbidity. The aim of the present study was to assess the health status of a cadaver population (n=127) at the Faculty of Medicine and Health Sciences (FMHS), SU, with special reference to TB lesion distribution and prevalence. For this study, full-body digital X-rays of 127 cadavers (87 males; 40 females; average age, 47.1 years) were obtained with the Lodox® Statscan® Imaging System after embalming and prior to dissection. A complete pathology report of six organ systems including the skeletal system was used in combination with histological examination, molecular analysis and radiological findings to investigate the prevalence and association between TB and systemic pathology. Samples for histological purposes were removed from organs with pathology lesions. For molecular studies, five different nucleic acid extraction methods were used to extract DNA from the formalin-fixed paraffin-embedded cadaver samples. Pulmonary samples were subjected to a line probe assay (LPA) and polymerase chain reactions (PCR) to determine mycobacterial genotypic distribution. Two independent radiologists examined the chest X-rays and their findings were compared with the pulmonary findings. PTB was a common finding in the cadaver population (76.4%) with males more commonly affected. A female predilection was observed for extrapulmonary TB. Statistically, TB was associated with pulmonary pathology, including pneumonia and bronchiectasis. Systemic pathology commonly encountered in the present study included neoplasms, coronary artery disease, colonic diverticula, hepatic triaditis, cirrhosis, glomerulosclerosis, pyelonephritis and a variety of healed maxillofacial and appendicular skeletal fractures. Extracted nucleic acid concentrations, as determined with the NanoDrop® spectrophotometer, ranged between 10ng/μl and 1000ng/μl. The standard salting-out method was found to be the most cost-effective and therefore the preferred method for nucleic acid extraction. The HAIN® MTBDRplus® kit was effective in determining the presence of mycobacterial species belonging to the Mycobacterium tuberculosis complex (MTBC). The sensitivity to first-line drugs could not be determined as a result of DNA degradation. Spoligotyping was unsuccessful, as incomplete and unidentifiable hybridization of the 43 spacers occurred. The RD105 and MUB02/RD105 PCR results were non-reproducible and non-specific. Pulmonary cavitation and pleural thickening were the only findings that were positively correlated with the radiological findings (p<0.05). To our knowledge, this is the first study to extensively investigate TB and systemic pathology including histopathology, molecular techniques and postmortem radiology in cadavers from low socio-economic backgrounds from a high TB burden area. This study therefore provides a more complete and thorough understanding of the prevalence, distribution and morphology of TB lesions as well as the association between TB and systemic pathology.<br>AFRIKAANSE OPSOMMING: Tuberkulose (TB) is a kroniese, pulmonêre infeksie gekenmerk deur granulomateuse ontsteking, verkasende nekrose en ’n neiging om fibrose en kavitasies te ontwikkel. Pulmonêre TB (PTB) letsels kan op verskeie wyses ontwikkel en kan, gebaseer op patogenese en morfologiese voorkoms, geklassifiseer word as primêre, sekondêre, progressiewe primêre of miliêre TB. Die Wes-Kaap provinsie, Suid-Afrika, het ’n hoë TB las met toenemende TB aanmeldingskoerse. Ongeveer 90% van die kadawers wat by die Universiteit Stellenbosch (US) gebruik word vir mediese disseksies kom van verarmde gemeenskappe waar TB ’n groot gesondheidsprobleem is in terme van die siektesyfer. Die doel van hierdie studie was om die gesondheidstoestand te bepaal van ’n kadawerpopulasie (n=127) by die Fakulteit Geneeskunde en Gesondheidswetenskappe (FGG), US, met spesifieke verwysing na die verspreiding en voorkoms van TB letsels. Digitale X-strale van die hele liggaam van 127 kadawers (87 manlik; 40 vroulik; gemiddelde ouderdom, 47.1 jaar) na balseming en voor disseksie is met behulp van die Lodox® Statscan® Imaging System bekom. ’n Volledige patologie-verslag van ses orgaanstelsels, die skeletstelsel ingesluit, is in kombinasie met histologiese ondersoeke, molekulêre ontleding en radiologiese bevindinge gebruik om die voorkoms van en assosiasie tussen TB en sistemiese patologie te ondersoek. Monsters vir histologiese studie is van organe met patologiese letsels geneem. Vir molekulêre studies is vyf verskillende nukleïensuur ekstraksie-metodes gebruik om DNS uit die formalien-gefikseerde paraffien-ingebed kadawermonsters te ekstraheer. Pulmonêre monsters is onderwerp aan ’n lyn-ondersoek-toets (“line probe assay”) en polimerase-kettingreaksies (PKR) om mikobakteriële genotipiese verspreiding te bepaal. Twee onafhanklike radioloë het die bors X-strale ondersoek en hulle bevindinge is vergelyk met die pulmonêre bevindinge. PTB is ’n algemene bevinding in die kadawerpopulasie (76.4%) met mans wat meer dikwels geaffekteer is. ’n Vroulike voorkeur vir ekstrapulmonêre TB is waargeneem. TB is statisties geassosieër met pulmonêre patologie, longontsteking en brongiëktase. Sistemiese patologie wat algemeen gevind is in die huidige studie sluit in neoplasmas, koronêre hartsiekte, kolon divertikula, lewer triaditis, sirrose, glomerulosklerose, piëlonefritis en ’n verskeidenheid geneesde maksillofasiale en appendikulêre skeletale frakture. Geëkstraheerde nukleïensuur-konsentrasies, soos bepaal met die NanoDrop® spektrofotometer, het gewissel tussen10ng/μl en 1000ng/μl. Daar is gevind dat die standaard uitsoutings-metode die mees koste-effektief en dus die voorkeur-metode nukleïensuur ekstraksie-metode was. Die HAIN® MTBDRplus® toets was effektief om die aanwesigheid van mikobakteriële spesies wat aan die Mycobacterium tuberculosis kompleks behoort, te bepaal. Sensitiwiteit vir eerste-linie middels kon nie bepaal word nie as gevolg van DNS degradasie. Spoligotipering was onsuksesvol omdat onvoltooide en onidentifiseerbare hibridisasie van die 43 merkers plaasgevind het. Die RD105 en MUB02/RD105 PCR resultate was nie-herhaalbaar en nie-spesifiek. Pulmonêre kavitasie en pleurale verdikking was die enigste bevindinge wat positief gekorreleer is met die radiologiese bevindinge (p<0.05). Na ons wete is hierdie studie die eerste een wat TB en sistemiese patologie ekstensief ondersoek en gebruik maak van histopatologie, molekulêre tegnieke en nadoodse radiologie in kadawers van lae sosio-ekonomiese agtergronde vanaf ’n area met ’n hoë TB las. Hierdie studie verskaf dus ’n meer volledige en deeglike begrip van die voorkoms, verspreiding en morfologie van TB letsels sowel as die assosiasie tussen TB en sistemiese patologie.

Abstract:
The Radiology Department of the Montreal General Hospital was testing a digital system for gastrointestinal examinations. The goal of the study was to evaluate its potential in clinical situations, with the purpose of complementing or replacing the old fluoroscopic system available at the moment.<br>The core of our study was to image 18 surgical specimens with gastrointestinal pathology utilizing 3 different techniques: high resolution shielded-cabinet, the Siemens digital system and the conventional analog hard copy system.<br>A skin dose exposure test was conducted by the department with the purpose of assessing the benefits of both the digital and the conventional systems.<br>We concluded in this study that the many advantages of the digital system--lower skin exposure, image reconstruction and manipulation, fast access to images, overcome its inferior resolution as compared with the analog hard-copy generation system.

Abstract:
This thesis examines the usefulness of voluntary health insurance (VHI) as a lever for improving health system performance. It posits that VHI may further health policy goals if it addresses gaps in statutory coverage, if it does not make those who rely on statutory coverage worse off, and if those who need VHI have access to it. The thesis presents four studies that analyse markets for VHI in the European Union; developments in public policy towards VHI, including the implications of the EUlevel regulatory framework for VHI; the impact of VHI on health system performance; the effects of allowing people to choose between statutory and voluntary health insurance; and VHI’s influence on consumer mobility where insurers compete to offer statutory benefits. The thesis finds that while VHI is critical to financial protection in some countries, it does not always address key gaps in statutory coverage or reach those who need it, and the depth of its coverage has declined over time, even in heavily regulated markets. VHI has a regressive effect on equity in health financing, lowers equity in the use of health services and does not seem to have a positive effect on efficiency, partly because insurers in many countries lack appropriate incentives. What is more, a failure to align incentives across VHI and statutory health insurance can undermine the efficiency of public spending on health. Many of VHI’s negative effects can be attributed to poor policy design. Policy makers can try and ensure VHI contributes to rather than undermines health system performance through the following mechanisms: better understanding of VHI’s interaction with the health system; stronger policy design, focusing on aligning incentives in pursuit of health policy goals and ensuring efficiency in the use of public resources; willingness and capacity to regulate the market to secure financial and consumer protection; and regular monitoring and evaluation.

Abstract:
This study aims at analyzing Turkish health policy from a Primary Health Care perspective as pronounced in Alma-Ata, 1978. The Alma-Ata Declaration has long been regarded as a watershed in the health field and 134 countries, including Turkey, have endorsed the Declaration showing their support for the views expressed in the Document. However, although the international community gave its full support, in practice, different interpretations and implementation of the principles have emerged. Turkey, one of the countries where health has rarely occupied the agenda, has been undergoing radical reforms since the mid 1980s with the ultimate aim of achieving Health for all by the Year 2000 through Primary Health Care. There is full commitment at the national policy-making level to endorse policies coherent with the principles of Alma-Ata. However, not all policies adopted seem to be consonant with what was declared in Alma-Ata requiring a detailed analysis of the policies suggested and implemented. In the light of this, the aims of the study are: (1) to analyze Turkish health policy since the 1960s from a Primary Health Care perspective with the aim of exploring the Turkish response to Alma-Ata; (2) to explore the perceptions of Turkish health policy-makers about Primary Health Care and related issues; (3) to discuss the prospects for Primary Health Care in Turkey. Basic principles of the Primary Health Care approach as declared in Alma-Ata have been taken as a guideline in analyzing Turkish health policy and the perceptions of the Turkish health policy-makers. These principles and their implementation, or the way they are perceived, have guided the research in answering the question ''what are the prospects for Primary Health Care in Turkey?" The nature of the research, based on document analysis and semi-structured interviews, has necessitated a qualitative stance. It was concluded that the Turkish version of Primary Health Care differs from the Declaration in a number of ways. A number of possible reasons for this have been offered. The perceptions of the policy-makers on certain issues that are closely related with the Approach, inter alia, has been found as one of the most possible explanations behind the current situation and a need to alter the ascendant approach towards health issues in general has been emphasized.

Aim/Purpose: [This Proceedings paper was revised and published in the 2018 issue of the journal Issues in Informing Science and Information Technology, Volume 15] The aim of this study was to concept test a novel instructional aid called Virtual Pathology Learning Resource (VPLR), which was used as a vehicle to communicate information, and enhance teaching and learning of basic sciences (Anatomy, Physiology and Pathology) to allied health science students at a South Australian university. Background: Pathology was traditionally taught using potted specimens to independently review macroscopic features of disease. However, this approach alone was found inadequate and ineffective. For one, the potted specimens were not easily accessible for all students. VPLR is a new teaching platform comprising of digitised human normal and human pathology specimens (histology, histopathology), patient case studies, short answer and critical thinking questions, and self-assessment quizzes. Using authentic learning theory as an educational approach, this learning resource was developed to enhance the teaching and learning of Pathology. Methodology: A cross-sectional study design was used. A survey, administered at the conclusion of the course, gathered qualitative and quantitative data concerning the perceptions and experiences of the students about VPLR. The online tool SurveyMonkey was utilised so that students could respond anonymously to a web link that displayed the questionnaire. The effectiveness of the program and its perceived impact on students was assessed using a 18-item questionnaire seeking agreement or disagreement with statements about VPLR, and open-ended questions querying the best things about VPLR, benefits to be derived, and areas for improvement. Descriptive and frequency analyses were performed. Contribution: The VPLR approach involved rich learning situations, contextualised content, and facilitated greater understanding of disease concepts and problems. Findings: In a sample of 103 Medical Radiation students, 42% of students (N=43) responded to the post-intervention survey. The majority of students reported highly positive effects for each component of the VPLR. The overall results indicated that this tool was an effective strategy in teaching Pathology as it assisted students' gaining knowledge and developing professional imaging skills. Recommendations for Practitioners: As students found VLPR to be beneficial, it is recommended that the same approach be applied for teaching of Pathology to other allied health students, such as Nursing. Other universities might consider adopting this innovation for their courses. Recommendation for Researchers: Applying VPLR to other allied health science students will be undertaken next. This innovation will be appropriate for other health science students with particular emphasis on case-based or problem-based learning, and combined with clinical experiences. Impact on Society: In reshaping the way of teaching a science course, students are benefited by a greater depth of understanding of content, and increase motivation with study. These are important to keep students engaged and prepared for practice. VPLR may impact on education and technology trends so that continuous exploration and possibilities of initiatives are ongoing to help students be successful learners. Other impacts are the new forms of learning discovered, and the renewed focus on group work and collaboration and the use of technology in innovation. Future Research: Future directions of this research would be to conduct a follow-up of this cohort of students to determine if the impacts of the innovation were durable, that means the change in perceptions and behaviour are sustained over time.
