Abstract:Some authors have questioned the moral authority of advance directives (ADs) in cases in which it is not clear if the author of the AD is identical to the person to whom it later applies. This article focuses on the question of whether the latest results of neuroimaging studies have moral significance with regard to the moral authority of ADs in patients with disorders of consciousness (DOCs). Some neuroimaging findings could provide novel insights into the question of whether patients with DOCs exhibit sufficient psychological continuity to be ascribed diachronic personal identity. If those studies were to indicate that psychological continuity is present, they could justify the moral authority of ADs in patients with DOCs. This holds at least if respect for self-determination is considered as the foundation for the moral authority of ADs. The non-identity thesis in DOCs could no longer be applied, in line with clinical and social practice.

Introduction: The evaluation of a work system is based on the ability to adapt work to the man and the man to the work. One of the aspects that interfere in the capacity of adaptation is the satisfaction in the work as the integration, the autonomy, the motivation, the involvement and the utilization of the physical and mental capacities.Objective: To analyze the level of satisfaction of medical professionals regar ding their work environment in a Social Health Organization of Brazilian Unified Health System.Methods: It is a cross-sectional, exploratory and descriptive study. For data collection, we used a questionnaire mailed through the Google Docs platform, containing 36 questions about indicators: material, personnel, quality and social.Results: A total of 51 physicians participated in the study. The findings showed significant associations between their satisfaction of work environment and relationship with their work team, unit manager and performance recognition.Conclusion: The satisfaction in the work environment of the medical professional in the Social Health Organization is associated with internal factors, as well as the need for their performance recognition by the management policy. The external factors studied, such as multiple job sites.

Abstract:
This study identified the prevalence of work-related musculoskeletal disorders among casual dockworkers. This is a retrospective and quantitative study, the data sources of which included the medical files of dockworkers from 2000 to 2009. Data were collected in a medical service for dockworkers using a previously developed form; descriptive quantitative analysis was performed. Ethical aspects were complied with. A total of 15.8% of the diagnoses identified among male workers referred to musculoskeletal disorders. Individuals older than 52 years of age and with more than 21 years working in the field predominated. The most frequent musculoskeletal diseases included low back pain (38.8%), tendinitis (19.7%), and neck pain (12.5%). These diseases can be prevented through health interventions, confirming the importance of developing clinical support in nursing in order to improve the quality of life of dockworkers.

All sea transportation facilities users in the world in general especially in Indonesia always prioritize safety, security and comfort aspects. The ship accidents such as sinking, burning, etc., are problems related to the safety and security of sea transportation.The purpose of this study it was to determine the application of occupational safetyand health inside the speedboat accordance with Ministerial Regulation No. 6 of 2018 concerning Occupational Safety and Health (OSH) in the Ministry of Maritime Affairs and Fisheries.This study used a qualitative method carried out at the port of Tengkayu II, Tarakan City, North Kalimantan were speedboat passengers as the main informants, as well as Transportation Service officers and dock workers as supporting informants.The sampling technique used accidental sampling technique. The results of the study stated that the application of OSH inside the speedboat had not been fully implemented.Passenger awareness is a fundamental cause of the non-implementation of that regulations. One aspect of applying OSH inside the speedboat in accordance with applicable regulations is to use a life jackets while in a speedboat. Passengers only wear life jackets just before leaving when inspected by officers, but will be removed if it is far from the port. Discomfort factor is the main reason why life jackets are not used. In addition, the boat captain's negligence was still found because it did not prioritize the safety of the feeder, and driving was not in accordance with the applicable standard operating procedures.

Este trabalho mostra os resultados de uma pesquisa sobre as condições sanitárias de algumas comunidades rurais, situadas no alto trecho do rio Pajeú, semiárido pernambucano. Nos períodos de estiagem, os açudes e poços são as fontes de águas mais utilizadas para o abastecimento das cisternas, equipamento de reservação presente em 100% das residências visitadas. As águas de três dessas fontes foram classificadas como doces, de acordo com a legislação em vigor.A forma de tratamento mais usada para as águas de consumo doméstico foi a aplicação de hipoclorito. Quanto aos resíduos gerados pelas famílias, onde a coleta pública municipal não está presente, o principal destino dos resíduos é a queima a céu aberto. Os dejetos de animais são, em sua maioria, utilizados como fertilizantes, embora tenha sido observada a presença desses resíduos em algumas nascentes. De forma geral, as principais práticas sanitárias encontradas nas comunidades visitadas, em sua maioria, são caracterizadas pela individualidade, devido à ausência de serviços públicos coletivos. Algumas práticas, como a queima de resíduos sólidos é diretamente nociva à saúde das pessoas, além de poder se constituir em uma importante fonte de poluição, à medida que a população e a quantidade de resíduos aumentam. Por outro lado, as famílias entrevistadas apresentaram certo nível de conhecimento quanto à necessidade de descontaminação da água usada para beber e cozinhar, e o aproveitamento agrícola dos nutrientes contidos nos dejetos de animais.Palavras-chave: saneamento básico; comunidades rurais; semiárido. Aspects of Sanitation and Use of Waterin Small Rural Communities of Pernambuco’s Semiarid ABSTRACTThis article shows the results of a research about the health conditions of some rural communities, located on the upper course of Pajeú River, in the semiarid region of Pernambuco State, Brazil. In periods of drought, dams and wells are the most used sources of water for the supply of cisterns, which are tanks or containers, for storing or holding water that are present in 100% of the visited homes. The water from three of those sources was classified as freshwater, according to the legislation. The most widely used form of treatment of water for domestic consumption was the use of hypochlorite. The waste generated by households where the municipal public collection is not available is, most of the time, burned in the open air. Animal manure is commonly used as fertilizer, although its presence has also been observed in some river sources. Overall, the main sanitary practices found in the communities visited are, in their majority, characterized by individuality, due to the absence of collective public services. Some practices, such as burning solid waste, is directly harmful to people's health, and can constitute an important source of pollution, as the population and the amount of waste increase. On the other hand, the families that were interviewed had some level of knowledge on: (i) the need for decontamination ofwater used for drinking and cooking; and (ii) the use of the nutrients contained in animal manure in agriculture.Keywords: sanitation; rural communities; semiarid.

A heuristic “3-dimensional” project is presented, accomplished by the 24 post-graduate students of our Department’s MSc. Course “Advanced Biomedical Technology”. The project has addressed, first, RT innovative Technology as reflected on Industrial Property (IP) docs, second, cardinal patient-care and medical-managerial aspects and finally, their combination with relevant Medical Protocols, Guidelines & Standards. Parts of the three modules of the 4th-trimester, i.e. “Large Therapeutic and Diagnostic Facilities Safety, Radiation Protection and Quality Assurance” (PMS_BIT_2.4.), “Biomedical Technology (BMT) of Home-Surveillance, Health-Care Management and Online Health Services” (PMS_BIT_2.5.) and “BMT-Quality Assurance, Industrial Property Rights, Technical Standards and Medical Protocols and Guidelines” (PMS_BIT_2.6.), taught by the first author, have been combined and each student has prepared and orally presented a “3-D” paper. Around 1000 pages of relevant content (over 1 GB) with numerous Figures and Tables are included in the produced Compendium. The quality of the students’ papers and presentations corroborate the need of introducing novel, contemporary and creative examination methods, attracting their interest and promoting their self-confidence.

Background Professors of nursing sometimes experience specific situations in their daily practice that conflict with their values and ethical principles and may culminate in moral distress. Moral distress occurs when one is prevented from acting according to his or her knowledge or values, or what one considers to be ethically sound. Objectives To identify the profile of professors of nursing through grouping sociodemographic characteristics and intensity of moral distress. Method Cross-sectional and exploratory study addressing 373 nurses teaching in Brazilian federal public higher education institutions. Data were collected from June to December 2018 through email, using the Google Docs tool. A moral distress scale directed to nurse educators was used. Data were analyzed using descriptive statistics, variance analysis, and cluster analysis. Ethical considerations The Institutional Review Board at the Federal University of Rio Grande approved this study. Findings Initially, four clusters emerged for each variable predicting the profile of Brazilian professors of nursing: sex; whether the individual worked in a graduate program; age; experience in years in their respective higher education institution; and intensity of moral distress. The profile of Brazilian professors of nursing was represented by the largest cluster, 36.5% (n = 136), composed of women working in graduate programs, aged 37 years old on average, having worked in their respective institutions for approximately 5 years, and presenting a moderate intensity of moral distress. Conclusion Assigning individuals into groups facilitates seeing similarities among the predictors that compose the profile of Brazilian professors of nursing, thus recognizing those workers experiencing moral distress in their daily work routine. In addition, this study’s results are expected to encourage reflection on the planning of efficacious interventions directed to the context of education and health.

<p>Apesar da importância social e econômica do uso recreacional das águas para a sociedade, os espaços disponíveis para a referida prática estão cada vez mais escassos devido à redução dos ambientes naturais, especialmente em áreas urbanas. Além disso, aqueles restantes são ameaçados por fontes de poluição diversas, expondo os usuários aos riscos do contato com águas contaminadas. Especificamente no caso de países tropicais, como o Brasil, constata-se uma precariedade de programas de monitoramento e gestão de balneabilidade em águas doces, cuja atual metodologia está limitada a critérios que não permitem uma avaliação ampla do uso recreacional, desconsiderando aspectos estéticos e fatores de riscos físicos. Deste modo, este trabalho visa levantar e discutir aspectos relacionados à importância histórica do uso recreacional das águas ao longo do desenvolvimento da sociedade, haja vista a reconhecida relevância como um dos usos mais nobres das águas e cada vez mais ameaçado pela degradação ambiental.</p><p><strong>Palavras–chave:</strong> Lazer, balneabilidade, riscos à saúde humana.</p><p><strong>Abstract </strong></p><p>Despite the social and economic importance of the recreational use of waters for society, the available spaces for this practice are increasingly scarce due to the reduction of natural environments, especially in urban áreas. In addition, those remaining are threatened by several sources of pollution, exposing users to the risk of contact with contaminated water. Specifically in tropical countries, such as Brazil, there is a lack of programs for monitoring and management of recreatonal water use in freshwaters, whose current methodology is limited to criteria that do not allow a wide evaluation of recreational use, disregarding aesthetic aspects and physical risk factors. Thus, this work aims to raise and discuss aspects related to the historical importance of the recreational use of water throughout the development of society, given the recognized relevance as one of the noblest uses of water and increasingly threatened by environmental degradation.</p><p><strong>Keywords</strong>: Leisure, water quality for recreational use, human health risks.</p>

Background Authorship teams in the health professions are typically composed of scholars who are acquainted with one another before a manuscript is written. Even if a scholar has identified a diverse group of collaborators outside their usual network, writing an article with a large number of co-authors poses significant logistical challenges. Objective This paper describes a novel method for establishing and facilitating large-scale manuscript collaborations via social media. Methods On September 11, 2020, I used the social media platform Twitter to invite people to collaborate on an article I had drafted. Anyone who wanted to collaborate was welcome, regardless of discipline, specialty, title, country of residence, or degree completion. During the 25 days that followed, I used Google Docs, Google Sheets, and Google Forms to manage all aspects of the collaboration. Results The collaboration resulted in the completion of 2 manuscripts in a 25-day period. The International Council of Medical Journal Editors authorship criteria were met by 40 collaborators for the first article (“Documenting Social Media Engagement as Scholarship: A New Model for Assessing Academic Accomplishment for the Health Professions”) and 35 collaborators for the second article (“The Benefits of Using Social Media as a Health Professional in Academia”). The authorship teams for both articles were notably diverse, with 17%-18% (7/40 and 6/35, respectively) of authors identifying as a person of color and/or underrepresented minority, 37%-38% (15/40 and 13/35, respectively) identifying as LGBTQ+ (lesbian, gay, bisexual, transgender, gender non-conforming, queer and/or questioning), 73%-74% (29/40 and 26/35, respectively) using she/her pronouns, and 20%-23% (9/40 and 7/35, respectively) identifying as a person with a disability. Conclusions Scholars in the health professions can use this paper in conjunction with the tools provided to replicate this process in carrying out their own large-scale manuscript collaborations.

AbstractThe redevelopment of former docklands, with docks frequently backfilled by end tipping through water, requires careful planning and a proper understanding of the geotechnical properties of the materials involved. An overall scheme for development of part of the former Surrey Commercial Docks Southwark, disused since 1960s, was formulated by London Docklands Development Corporation to provide a new environment encompassing housing, schooling, a shopping centre, social and recreation facilities and light industrial units.This paper presents a review of the development strategy and planning aspects of the development of Surrey Quays with particular reference to the assessment of the former wharf areas and the quality of the dockfill. Planning considerations, along with the role of geotechnics within the infrastructure development plan, are presented. The results of geotechnical investigations which included an extensive phase of field and laboratory testing, led to a ground improvement programme consisting of surcharge loading and vibro-replacement.

Os graduados representam o produto das Universidades e seu desempenho no mercado de trabalho é um dos parâmetros mais importantes para avaliar a qualidade do ensino. O objetivo desse artigo foi verificar como se deu a formação na graduação, com base na percepção da trajetória profissional trilhada por egressos de um Curso de Enfermagem. Estudo transversal, descritivo desenvolvido em abordagem quantiqualitativa. Participaram 61 egressos graduados entre os anos 2008 a 2012. Aplicou-se questionário semiestruturado utilizando processador on-line “Google Docs”. Realizada a análise descritiva dos dados quantitativos e análise de conteúdo das questões subjetivas. Os participantes eram, predominantemente, jovens e do sexo feminino; 85% haviam realizado pós-graduação; 75% afirmaram satisfação profissional; 90% começaram a trabalhar nos dois primeiros anos, a maioria em hospitais e na Atenção Básica. Da análise temática emergiram três categorias: “A instituição formadora favorece a inserção no mercado de trabalho”; “Estágio curricular supervisionado como diferencial” e “Aspectos para melhorar formação do enfermeiro”. A formação proporcionou capacitação técnico-científica e socioeducativa para o exercício profissional seguro e comprometido com a qualidade do serviço, porém deve haver maior investimento no desenvolvimento de competências ético-políticas.Palavras-chave: Avaliação Educacional. .Educação em Enfermagem. Força de Trabalho.Abstract The graduates represent the universities product and their resourcefulness in the labor market is one of the most important parameters to evaluate the teaching quality. The objective of this study was to verify undergraduate training based on the professional trajectory perception traced by a Nursing Course graduates. The authors carried out a survey study, a descriptive, qualitative and quantitative approach and cross segment, which analyzed data from 61 graduates between the years 2008-2011. Data were collected through semi structured response to a questionnaire sent to graduates by an electronic medium online processor “Google Docs”. Descriptive analysis of quantitative data and content analysis of subjective questions was performed. It is a young population and predominantly female. 85% had completed graduate studies; 75% affirmed professional satisfaction; 90% started working in the first two years, mostly in hospitals and in Primary Health Care. From the thematic analysis three categories emerged: “The training institution favors the insertion in the labor market”; “Supervised curricular internship as a differential” and “Aspects to improve nursing training”. The training provided scientific-technical and educational formation for the safe professional exercise and committed to the service quality, but there must be more investment in the development of ethical-political skills.Keywords: Educational Measurement. OBJETIVO: Conhecer os fatores de risco para a alta severidade de cárie dentária em crianças de 12 anos de idade. MÉTODOS: Partindo-se dos resultados obtidos no levantamento epidemiológico em saúde bucal, realizado em Florianópolis, em 1995, comparou-se algumas condições sociais e de comportamento entre dois grupos com severidades distintas da cárie dentária: um com alto/muito alto (n=50) e outro com muito baixos níveis da doença (n=50), através da análise de regressão logística multivariada. RESULTADOS/CONCLUSÕES: Os fatores de risco para alta severidade de cárie foram a freqüência de consumo de doces e a renda familiar. Crianças que consumiram produtos cariogênicos duas a três vezes ao dia, todos os dias, apresentaram 4,41 vezes mais chances de ter alta severidade de cárie quando comparadas com as que consumiram esses produtos no máximo uma vez ao dia -- IC95% (OR) = [1,18; 16,43]. A renda familiar foi o fator socioeconômico de maior importância. The main goal of this study is to understand and analyse the perspective of dockworkers on health and labour at the Railway Dry Port of the Municipality of Uruguaiana in the State of Rio Grande do Sul (RS), Brazil. Sixteen dockworkers participated in the study under the methodological approach of qualitative research. The fieldwork was conducted through individual interviews by applying a semi-structured research script with open-ended questions. For the data analysis, the "discourse analysis" method was adopted, leading to the definition of five thematic categories of interpretation. From the perspective of dockworkers, dock work differs in both the weight of toil, which is hard and intense, and the sense of satisfaction regarding the collaborative and collective aspect of labour, emphasising the human side of labour. Several reports on work accidents and the identification of hazards that may be avoided by implementing changes in the organisation and work conditions were also noted.

The Senneville was built in 1967. She was managed from 1973 to 1990 by Scott Misener Steamships (later Misener Transportation) and from 1990 to 1994 by Great Lakes Bulk Carriers (GLBC). With the demise of Great Lakes Bulk Carriers (GLBC) in 1994, the ship was purchased by Algoma Central Marine and the name was changed to Algoville. This paper gives a brief outline of the history of the ship, including ownership and events leading up to the need for major structural modifications and also discusses some of the design aspects leading to the decision to widen the hull from 75 ft-3 in. to 78 ft-0 in. (22.8 to 23.7 m) beam. A brief description of the widening project by Port Weller Dry Docks is presented, followed by some thoughts on the results of the widening and the successful operation of the vessel since returning to service.

As project manager at BAE Systems Naval Ships in Glasgow, Scotland, Scott Jamieson oversees the creation of two landing-ship docks for the Royal Fleet Auxiliary. Jamieson reviews his advance blueprint all the time to make sure his team uses its time effectively. He compares the initial blueprint against the present project to ensure that the plan is still pertinent. Another blueprint success example is its extensive usage in aerospace projects. According to Jack Gallagher, executive director for the Bell/Agusta 609 Tiltrotor, aerospace projects are so large that it is easy for company executives and managers to lose control. A blueprint helps to keep all aspects of a huge project on track.

"Disorders of Consciousness (DOCs) identify a diverse group of dysfunctions affecting individuals who have survived severe brain damage. Although the clinical evaluation of patients with DOC normally depends on their residual ability to create a connection with the outside world through motor behavior, in the last ten years neuroscientists have made a great effort to better characterize the patient’s consciousness even in the case of reduced or no motor reactivity. Often, in fact, patients with DOCs are not able to perform any type of movement or adequately understand the command required but they might be still conscious or at least minimally conscious. The gap between patients’ motor capability and their residual brain complexity raises important moral issues, especially about appropriateness and proportionality of interventions and treatments. We argue that Clinical Ethics Consultation may be crucial in addressing these clinical scenarios, which assume aspects of further drama when the DOC derives from a sudden and unexpected acute event such as a trauma or a cerebrovascular accident in healthy patients. In these situations, given the particular vulnerability of the patients and the unpredictability of the diseases, Ethics Consultants should be involved in the therapeutic process in order to improve the standard of care, ensuring compliance with the inclinations and desires of the patients. Moreover, Ethics Consultants should monitor that the procedures are rigorously managed in the context of fully understood consent of those who are legally entitled to make decision. "

Investigou-se a presença de fatores de risco para doenças cardiovasculares em estudo transversal em 356 crianças de 5 a 9 anos, atendidas em unidade básica de saúde de área de baixa renda da Região Metropolitana do Rio de Janeiro, Brasil. Foram avaliados: lipidograma, estado nutricional, hábitos alimentares e aspectos sócio-econômicos. Observaram-se 10,7% de sobrepeso e 68,4% com níveis alterados no lipidograma, sendo 18,6% com LDL-colesterol alto. Para descrever o perfil alimentar as respostas ao questionário qualitativo de ingestão foram submetidas à classificação multivariada, obtendo-se seis grupos, resumidamente definidos como: da cultura tradicional brasileira; moderno (produtos diet e light); frituras; doces e refrescos (misturados com outros grupos); os demais pouco definidos. A alta prevalência dos fatores de risco para as doenças cardiovasculares desde a infância e a evidência de alimentação infantil inadequada indica a necessidade de desenvolver uma estratégia preventiva, procurando atingir toda a família, de forma a alterar os padrões de ingestão de alimentos das populações de baixa renda em direção à comportamentos mais saudáveis.

It is often perceived that cultural aspects represent a limitation which must be considered when regarding the development and implementation of public health intervention activities is often perceived, usually because there are varying degrees of difficulty concerning relationships, communication and understanding between communities and experts in this particular field. Nevertheless, different social groups’ beliefs and practices still survive and carry great force when addressing the various issues of interest regarding public health, as shown by the articles in this issue of the journal on such issues as cardiovascular risk factors, smoking and alcohol consumption, breastfeeding, physical activity, health service access, nutrition, healthcare team-patient-family relationship or implementing specific risk eradication, control or mitigation actions. However, there is relatively little public health-related information concerning how this issue can be understood and addressed, possibly because consensus is still lacking between sociologists and anthropologists working in the healthcare field. It would thus seem clear that more research is needed in this field, so that, while advances have been made regarding a situational description, advances should also be made in constructing diagnostic and intervention methodologies which would be truly accessible to those who (without being experts on this topic) are taking on the operational work amongst communities, so that cultural matters cease being a barrier and become rather an opportunity for the collective reconstruction of knowledge and practice regarding public health It may be realistic to consider formal and informal regulatory aspects, a sense of identity and belonging to a particular group and implicit or explicit power and control mechanisms incorporated into a group of interest’s everyday life as themes and possible starting points for such an approach. It is thus important to approach how these aspects are expressed in the home, jobs and public spaces (i.e. the three major areas of modern life) and as embodied in speech, language, customary practices, relationships and social organisation.

In addition to education, health is a basic component of human capital. Until recently the significance of health of the population was not the subject of serious study in economic science. However, in recent research, health is increasingly treated as a factor with long-term effect on economic development. It was concluded that the realisation of various health programmes had pronounced and numerous economic effects. The improvement of health services resulted in reduced mortality rate between the developed and underdeveloped countries, which had effect on economic growth. The health of individuals is reflected in biological, psychological, and social sphere, and their interaction. The more healthy individuals in a society, the easier it is to drive economic development. Thus in the economy of health the “production “of health as an important process in human capital increases. Total health “production” is determined by numerous factors such as available income, property, degree of education, genetic predisposition and level of public health. Besides, many other factors that determine life style of an individual are also important, which influences the creation of health needs like smoking, alcohol and drug consumption etc. These factors have impact on health “production” by using the available financial resources. Here, the possibilities of new technologies to satisfy various needs for health care should also be mentioned, since they are unavoidably connected with the increasing finances. Treatment of health protection as a domain where health is “produced” leads to conclusion that it is possible, in analytical sense, to express the relation between health status (of an individual, certain group of people, or ethnic community) as a result of health protection system and factors that determine that status in the form of production function Health = F (health protection, other inputs, time). This does not refer so much to health industry, aimed at preventing diseases (although it is important as well), but, first and foremost, to prevention and healthy lifestyle (diet, physical activity, finding right measures for each thing, avoiding harmful substances and pollution of the environment). Healthy life, of course, includes sufficient free time and appropriate living standard. Health of labour is particularly affected by healthy diet, healthy working environment, appropriate daily, weekly, and annual breaks, appropriate housing, organized care of employees’ young children, organization of physical and social activities in the company, and the like. In this context, the central problem of health protection system management is how to provide maximum possible level of health status of population with the available finances allocated for health protection. Health economists, logically, cannot directly influence the improvement of health status of the population, but can be useful in increase of efficiency in the use of available finances for health care, i.e. increase the level of health status of the population by using the same amount of finances.
