This study examines the factors associated with computer use for the self-management of health among individuals diagnosed with chronic diseases (CD) in Israel. We distinguish between: (1) access to online health information, and (2) use of online health services (OHS). A geographic representative sample comprising 2008 individuals was contacted. 1406 individuals (67.6%) reported using the computer for health concerns. Limited data exists describing the long-term prognosis of patients with acute decompensated heart failure (ADHF) further stratified according to currently recommended ejection fraction (EF) findings. In addition, little is known about the magnitude of, and factors associated with, long-term prognosis for these patients. Based on previously validated and clinically relevant criteria, we defined HF-REF as patients with an EF value ≤40%, HF-PEF was defined as an EF value > 50%, and HF-BREF was defined as patients with an EF value during their index hospitalization between 41 and 49%. Cardiovascular diseases are a leading death cause in the world. Cardiac arrest is one of the most usual, and very quickly fatal, especially in out-of-hospital environments. Defibrillation, aside with cardiopulmonary resuscitation, is an effective means to restart blood circulation and heart operation, even though even these forms of treatment can help just in sadly few situations. Defibrillation was invented and first demonstrated already year 1899, but first in the 2000s portable defibrillators with good automatic functions started to penetrate daily environments of people, especially in urban settings. Nowadays the starting point is that every citizen with normal human functionality should be able to use automated defibrillators. The chapter discusses how modern information and communication technology, especially mobiles services, internet, and location services based on them, could help citizens in the first crucial step in implementing their safety competence in emergency situations by using automatic defibrillators if they could only find them.

The incidence of diabetes is increasing, and is expected to exceed one million people in Australia by the year 2010. Diabetes is currently diagnosed after the onset of symptoms. At this stage, detection of complications is a key intervention point in reducing the associated personal and community burden. These include eye, heart, kidney, and foot disease, which in many instances can be treated with good outcomes, provided the disease process is recognized early. In Australia, both national and state governments acknowledge the disadvantage faced by rural people in availing themselves of all aspects of diabetes management, from screening to regular assessment, education and health care (Colagiuri, Colagiuri, & Ward 1998). Therefore, the challenge that is the focus of this article is the early detection of diabetes complications associated with vision and cardiac function, with the eventual aim of providing a screening service that can be used in a rural or regional environment.

Abstract:
For the emergency management of cardiovascular disorders, echocardiography and thoracic ultrasound are indispensable imaging techniques at the bedside. In the intensive care environment, crucial questions, such as left and right ventricular function, valvular heart disease, volume status, aortic disease, cardiac infection, pleural effusion, pulmonary oedema, pneumothorax, and many others, can be sufficiently and reliably answered by using these techniques; in fact, it is almost impossible to manage patients with acute severe haemodynamic impairment reasonably well without prompt and repeated access to echocardiography. This is confirmed by the prominent place that echocardiography has in the guideline-based diagnosis and treatment of all major cardiovascular emergencies, from acute heart failure to acute coronary syndrome to pulmonary embolism, etc. Moreover, it is the ideal tool to follow up the patient, since repeat examinations pose no risk to the patient and demand relatively little logistics and resources. To benefit from the wealth of information that echocardiography and thoracic ultrasound can provide, modern equipment (including a transoesophageal probe) and systematic training of echocardiographers must be ensured. The availability of prompt and experienced echocardiography and thoracic ultrasound services at all times is fundamental for sound contemporary cardiovascular intensive care.
