Notfallmäßige Vorstellung eines Ehepaars in der zentralen Notaufnahme. Der Mann gibt an, seit 7 Tagen an hochfieberhaften Temperaturen, trockenem Reizhusten und körperlicher Schwäche zu leiden, während die Ehefrau ähnliche Symptome in milderer Ausprägung bietet. In der klinischen Untersuchung fallen feinblasige Rasselgeräusche über beiden Lungen beider Patienten und eine respiratorische Insuffizienz des Ehemanns auf. Untersuchungen und Diagnose Laborserologisch Erhöhung der Inflammationsparameter sowie der Transaminasen und der Laktatdehydrogenase. Röntgenologisch pulmonale Infiltrate unterschiedlicher Ausprägung bei beiden Patienten. Im weiteren Verlauf Nachweis einer SARS-CoV-2-Infektion der Ehepartner mit schwerem Verlauf beim männlichen Patienten. Therapie und Verlauf Intubation und Beatmung des männlichen Patienten bei mittelschwerem ARDS mit langer intensivmedizinischer Behandlung. Genesung der weiblichen Patientin unter Sauerstofftherapie. Diskussion Diese beiden Fälle zeigen den Ausgangspunkt einer endemischen Verbreitung von SARS-CoV-2 im Landkreis Heinsberg (NRW) und bis zu diesem Zeitpunkt den ersten Fall eines lebensbedrohlichen Verlaufs in Deutschland.

KurzfassungMit dem deutlichen Anstieg der Zuwanderung Geflüchteter seit Mitte der 2010er-Jahre stehen die Städte und Gemeinden mit ihrer kommunalen Ausländer- und Migrationspolitik vor neuen Herausforderungen der gesellschaftlichen Integration. Die Bewältigung dieser Herausforderungen erfolgt im Zusammenspiel verschiedener, zum Teil neuer lokaler Akteure aus dem öffentlichen (z. B. kommunale Verwaltung, Wohlfahrtsorganisationen), privatwirtschaftlichen (z. B. Arbeitgeber) und zivilgesellschaftlichen Bereich (z. B. ehrenamtliche Initiativen). In diesem Beitrag werden die verschiedenen Logiken zentraler, am Integrationsprozess beteiligter Akteure beleuchtet, um aufzuzeigen, wer unter welchen Voraussetzungen die Integrationsbedingungen vor Ort in welcher Weise gestaltet und steuert. Die Untersuchung wurde in zwei unterschiedlichen lokalen Kontexten, der Stadt Köln und dem Kreis Heinsberg, mit dem Ziel durchgeführt, Unterschiede und Gemeinsamkeiten lokaler Governance aufzuzeigen. Die Ergebnisse beruhen auf 29 Experteninterviews mit Vertretern der Stadt- und Kreisverwaltungen, der Stadtgesellschaften und der Landesebene, die im Rahmen beruflicher oder ehrenamtlicher Tätigkeit am Integrationsprozess Geflüchteter beteiligt sind. Im Ergebnis zeigen sich bei einigen Maßnahmen der Integrationspolitik verschiedene Handlungslogiken. Dies trifft erstens für das Verhältnis zwischen staatlichen Akteuren der Bundes- und Landesebene und kommunalen Akteuren zu. Dies trifft zweitens aber auch auf das Verhältnis zwischen kommunalen und zivilgesellschaftlichen Akteuren zu. Dabei spielen die räumlichen und situativen Kontexte eine differenzierende Rolle.

Abstract Purpose The described work aimed to avoid cancellations of indispensable treatments by implementing active patient flow management practices and optimizing infrastructure utilization in the radiation oncology department of a large university hospital and regional COVID-19 treatment center close to the first German SARS-CoV‑2 hotspot region Heinsberg in order to prevent nosocomial infections in patients and personnel during the pandemic. Patients and methods The study comprised year-to-date intervention analyses of in- and outpatient key procedures, machine occupancy, and no-show rates in calendar weeks 12 to 19 of 2019 and 2020 to evaluate effects of active patient flow management while monitoring nosocomial COVID-19 infections. Results Active patient flow management helped to maintain first-visit appointment compliance above 85.5%. A slight appointment reduction of 10.3% daily (p = 0.004) could still significantly increase downstream planning CT scheduling (p = 0.00001) and performance (p = 0.0001), resulting in an absolute 20.1% (p = 0.009) increment of CT performance while avoiding overbooking practices. Daily treatment start was significantly increased by an absolute value of 18.5% (p = 0.026). Hypofractionation and acceleration were significantly increased (p = 0.0043). Integrating strict testing guidelines, a distancing regimen for staff and patients, hygiene regulations, and precise appointment scheduling, no SARS-CoV‑2 infection in 164 tested radiation oncology service inpatients was observed. Conclusion In times of reduced medical infrastructure capacities and resources, controlling infrastructural time per patient as well as optimizing facility utilization and personnel workload during treatment evaluation, planning, and irradiation can help to improve appointment compliance and quality management. Avoiding recurrent and preventable exposure to healthcare infrastructure has potential health benefits and might avert cross infections during the pandemic. Active patient flow management in high-risk COVID-19 regions can help Radiation Oncologists to continue and initiate treatments safely, instead of cancelling and deferring indicated therapies.

Abstract Aim of the study During pandemics, the whole population is simultaneously confronted with the same health threat, resulting in enormous public interest. The current COVID-19 pandemic has left the world in a unique state of crisis. The aim of this analysis was to explore whether Google searches can be used to retrospectively retrace the COVID-19 pandemic in Germany and to detect local outbreaks by reflecting public interest in the virus. Methods Google Trends was used to explore the relative search volume (RSV) related to “coronavirus” from January 2020 to July 2020 in Germany. The RSV ranging between 0-100 was compared to new SARS-CoV-2 infections per day on a national level and to the cumulative infection numbers on a state level, as well as to important infectiological and political events. Results The most striking search peaks occurred after the first reported SARS-CoV-2 infection in Germany (January 27), during a major local outbreak in Heinsberg (February 25), after school closings (March 13) and the largest peak after nationwide contact restrictions (March 22) were announced. On a state level, peaks in RSV were observed after the first reported infection in each respective state. In addition, a higher RSV was recorded in states with higher numbers of infections (r=0,6, p=0,014) such as in Bavaria (RSV=96, 391 infections/100,000 inhabitants) and Baden-Württemberg (RSV=98, 340 infections/100,000 inhabitants). The lowest RSV (n=83) and lowest number of infections (50 infections/100,000 inhabitants) was observed in Mecklenburg-Western Pomerania. Since the end of May, SARS-CoV-2 related RSV remained at low level even when numbers of infections were temporarily rising due to local outbreaks such as the outbreak in Gütersloh, North Rhine-Westphalia. Conclusion RSV related to “coronavirus” precisely reflected public interest during the beginning of the COVID-19 pandemic. As public interest has strongly declined, information distribution regarding the newest developments over the entire course of the pandemic will be a major public health challenge.

In this piece, Lindquist provides a toolkit for working with Indigenous youth through media arts. In doing so, she braids the three themes of the Symposium together: Indigenous, digital, and youth issues. Here, she presents her work as part academic article, part toolkit. The toolkit includes four examples of media arts justice activities that can be used to engage and support youth as they make connections between local and global issues. These activities were used by nehiyaw youth from Frog Lake First Nation who were attending Heinsburg Community School with support from Native Youth Sexual Health Network. Each activity includes a step-by-step guide, as well as background information on the relevance for young people as well as the scholarly community. They are grounded in both project- and place-based pedagogical approaches, and have benefits for Indigenous and non-Indigenous students. Lindquist theorizes around these examples throughout using Indigenous feminisms, reproductive justice, and education-based frameworks and thus bridges the gap between scholar and practitioner. This toolkit is emerging as a bridge between where we are at now and what we can imagine.

We whole-genome sequenced 55 SARS-CoV-2 isolates from Germany to investigate SARS-CoV-2 outbreaks in 2020 in the Heinsberg district and Düsseldorf. While the genetic structure of the Heinsberg outbreak indicates a clonal origin, reflecting superspreading dynamics from mid-February during the carnival season, distinct viral strains were circulating in Düsseldorf in March, reflecting the city’s international links. Limited detection of Heinsberg strains in the Düsseldorf area despite geographical proximity may reflect efficient containment and contact-tracing efforts.

Abstract Background The Dutch province of Limburg borders the German district of Heinsberg, which had a large cluster of COVID-19 cases linked to local carnival activities before any cases were reported in the Netherlands. However, Heinsberg was not included as an area reporting local or community transmission per the national case definition at the time. In early March, two residents from a long-term care facility (LTCF) in Sittard, a Dutch town located in close vicinity to the district of Heinsberg, tested positive for COVID-19. In this study we aimed to determine whether cross-border introduction of the virus took place by analysing the LTCF outbreak in Sittard, both epidemiologically and microbiologically. Methods Surveys and semi-structured oral interviews were conducted with all present LTCF residents by health care workers during regular points of care for information on new or unusual signs and symptoms of disease. Both throat and nasopharyngeal swabs were taken from residents suspect of COVID-19, based on regional criteria, for the detection of SARS-CoV-2 by Real-time Polymerase Chain Reaction. Additionally, whole genome sequencing was performed using a SARS-CoV-2 specific amplicon-based Nanopore sequencing approach. Moreover, twelve random residents were sampled for possible asymptomatic infections. Results Out of 99 residents, 46 got tested for COVID-19. Out of the 46 tested residents, nineteen (41%) tested positive for COVID-19, including 3 asymptomatic residents. CT-values for asymptomatic residents seemed higher compared to symptomatic residents. Eleven samples were sequenced, along with three random samples from COVID-19 patients hospitalized in the regional hospital at the time of the LTCF outbreak. All samples were linked to COVID-19 cases from the cross-border region of Heinsberg, Germany. Conclusions Sequencing combined with epidemiological data was able to virtually prove cross-border transmission at the start of the Dutch COVID-19 epidemic. Our results highlight the need for cross-border collaboration and adjustment of national policy to emerging region-specific needs along borders in order to establish coordinated implementation of infection control measures to limit the spread of COVID-19.

Abstract Background COVID-19 pandemic caused a decline in stroke care in several countries. The objective was to describe lockdown stroke care in a tertiary stroke center in Düsseldorf, Germany near Heinsberg, a German hot spot for COVID-19 in spring 2020. Methods In a retrospective, observational, single-center study, we compared all patients treated in our emergency department (ED), patients seen by a neurologist in the ED, ED patients suffering from ischemic and hemorrhagic strokes and transient ischemic attacks (TIAs) as well as stroke patients admitted to our stroke unit during lockdown in spring 2020 (16 March 2020–12 April 2020) to those cared for during the same period in 2019 and lockdown light in fall 2020 (2 November – 29 November 2020). Results In spring 2020 lockdown the mean number of patients admitted to our ED dropped by 37.4%, seen by a neurologist by 35.6%, ED stroke patients by 19.2% and number of patients admitted to our stroke unit by 10% compared to the same period in 2019. In fall lockdown light 2020 effects were comparable but less pronounced. Thrombolysis rate was stable during spring and fall lockdown, however, endovascular treatment (EVT) rate declined by 58% in spring lockdown and by 51% in fall lockdown compared to the period in 2019. Conclusions Our study indicates a profound reduction of overall ED patients, neurological ED patients and EVT during COVID-19 pandemic caused lockdowns. Planning for pandemic scenarios should include access to effective emergency therapies.
