Full textKognitiv beteendeterapi har på senare år fått allt större spridning i socialt och psykologiskt behandlingsarbete i Sverige. Det finns idag en uppsjö av KBT-baserade program som riktar sig till olika målgrupper, och en sådan målgrupp är kriminella killar i ungdomsåren. Många behandlingshem som riktar sig till denna målgrupp säger sig använda KBT. I vår studie undersöker vi hur fyra olika hem för vård eller boende (HVB) använder dessa metoder. För den teoretiska referensramen har vi sammanfattat olika studier, främst metaanalyser, som med stor samstämmighet beskriver olika riktlinjer för hur KBT bör praktiseras med denna målgrupp för att bli en effektiv behandlingsmetod. Det finns framförallt tre principer som enligt ett flertal metaanalyser kännetecknar de mest effektiva behandlingarna, nämligen riskprincipen, behovsprincipen och responsivitetsprincipen. Utifrån dessa forskningsresultat har vi utformat frågor som kan svara på hur HVB-hemmens behandlingsmodeller förhåller sig till de riktlinjer som forskarna konstaterat. Halvstrukturerade intervjuer har genomförts med föreståndare för de olika HVB-hemmen. I analysen finner vi att behandlingshemmen använder KBT-baserade metoder på olika sätt och enligt vår tolkning anpassar inget av de fyra HVB-hemmen behandlingen efter riktlinjerna för effektiv behandling i någon högre utsträckning. Enligt den forskning som vi tagit del av så är personalens utbildning helt avgörande för hur effektiv behandlingen blir. Utbildningsnivån på de behandlingshem som omfattas av studien uppfyller inte den nivå som enligt ett flertal forskare är erforderlig för att kunna bedriva ändamålsenlig behandling. I diskussionen frågar vi oss bland annat hur viktig metoden egentligen är för behandlingens resultat.

Full textThe number of unaccompanied refugee minors arriving in Sweden has risen significantly in recent years. This calls for a reception in which their, (also long term) needs are covered. In particular, this accentuates the responsibility of the group-homes where most of these children and youth are placed after their asylum claims are accepted. In this study attention is focused on how the staff in these group-homes work, gaining information on current methods being used in the group homes, and exploring how effective and sufficient these methods are considered to be. The study involves a mixed-methods design, including both interviews and survey methods. The results show that specific group-home methods in general are perceived as very useful, but not the most important aspect in their work. Unspecific methods are used significantly more frequently, and are based in the ability to adapt to different situations and clients. The most important and useful resource in this work is considered to be having a relationship built on confidence and trust with the children, and to create a place of safety for the children. The perceptions of why the relationship is important are varied, often between the group-homes. Some emphasize practical areas, other emphasize emotional and developmental areas. Those who view the relationship as emotionally positive tend to further emphasize the importance of building relationships.

Full textResidential care agencies in Sweden have been described in the Swedish media for malpractice in recent years. Previous research has shown that youth in residential care are a vulnerable group. This study tries to answer what methods are used by residential care agencies in Sweden today and how compatible these methods are with evidence-based practice. The theoretical framework for this study is social constructivism and categorization. The study is of an exploratory nature and the aim is to map residential care agencies in Sweden (HVB) that are focused on the treatment of adolescents by the age of 13-18 years. Through quantitative content analysis, the content of n = 295 HVB websites were coded and analyzed. Results show that the HVB field in Sweden is in a process of change where CBT as a method is most prominent. Methods that have scientific support for addiction and criminal behavior were mentioned to a small extent. Many HVBs did not provide information on staff which is significant when it comes to treatment and care at HVB.

Full textYouths in Residential Institutions (HVB-home) often have a problematic behavior and are in need of protection, security and boundaries. HVB-homes are through legislation required to provide care and treatment to counteract adverse development among the youth. The Inspection for Health and Care (IVO) supervise the HVB-homes. The purpose of this study was to interview staff at HVB-homes and Inspectors at IVO to investigate their perceptions of how care and treatment should be conducted. Furthermore the aim was to distinguish whether the legislation can be interpreted differently and what consequences this has for the care and the treatment for the youths regarding their needs for positive psychosocial development. To obtain data five qualitative interviews were conducted with professionals at HVB-homes and one with two inspectors at IVO. The result was analyzed according to the theoretical concepts of psychosocial development, motivation and boundaries. The result shows that to develop psychosocial maturity, young people need individual care, empowerment, boundaries and skills to control and protect themselves. Furthermore, it shows occurrences of both obstacles and opportunities in terms of the staff being able to provide the appropriate care for the youth. The staff experience frustration in the execution of the supervision and describe that they interpret the legislation differently from IVO. The staff express that they are unable to handle the youths at HVB-home. A problematizing discussion is conducted regarding the youths’ problems at HVB-homes, need of boundaries and the staffs’ ability to provide appropriate care in relation to the execution of the supervision and the two partly different interpretations of the legislation.

Full textThe aim of this study is to investigate how länsstyrelsen as a supervisory authority to private compulsory care-institutions works, to guarantee adequate institutional care of unprivileged children and youth with special problems.
In order to reach this aim the following questions are to be answered:
• Describe länsstyrelsen´s supervision of private compulsory care-institutions (HVB-hem)
• How does the officials at the three chosen county administrative boards find about the responsibility
To answer the questions of this bachelor thesis I have chosen to apply qualitative methods, by interviewing five supervisory authorities from länsstyrelsen, in three different county administrative boards in Sweden.
The findings of the study show that officials sometimes find it hard to know the exact meaning and differences between accountability and to be accountable. It also shows that they tend to think that the relationship between länsstyrelsen and the private compulsory care institutions is complicated. Because the officials have two part relationships, they have on the one hand a consultative role and on the other hand the supervising role.

Full textThis study examines how society handles children who for different reasons can’t manage, or be managed, in compulsory schools. Observations and interviews were made in three different settings: 1) a foster home with its own school, 2) a small school for troubled youth, and 3) a home for assessment and treatment, without its own school. Common for these arenas are that the children have been separated and set apart from regular school, and they take place on the boundary, or in the intersection between, teaching and treatment. The aims are to reveal how different discourses about troubled youth, education and treatment create possibilities and limitations in relation to the goal of the practices targets, their means, and the actors. The study is based on research into interaction, daily life, and processes of identity transformation. The data is collected by ethnographic methods in the form of participant observations, and the analysis is informed by discourse analysis. In these communicative practices, educators and social workers were active in an area of research that is intimately related to identity. Analysis of everyday situations in pedagogical and social practices provides valuable knowledge for understanding what is going on in this practices and what the implications are for the actors. Examinations of the studied children’s environments and activities reveal the different emphases of efforts addressing the children’s needs which are informed by (connected to) the use of different discourses. The use of informal settings, playfulness, humour, and body contact help maintain consensus and build relationships. The relationships provide a basis for exerting influence. To offer support is also a means by which the young people can be controlled. Two strategies care and control are found to be linked in what Foucault describes as an exercise of pastoral power. These relationships are also the basis for intersubjectivity, that is, shared knowledge of the situation, including self-knowledge of the actors involved. The emphasis on informal environments and flexibility in interpreting the situation allows negotiation about who the children are and what they are expected to become. The balance in child welfare activities between coercion and free choice can be described as a balancing act in which employees have great responsibility for the children. The study discusses how different discourses affect young people’s identity. Considering the various working methods based on different discourses together with the specific child welfare activities suggests that there are several different ways to define the problem situation affecting the children; the children are in this way exposed to a number of troubled identities. The results also indicate that the children can refuse to make these identities their own. The children are actors who become visible through their resistance to what threatens their motives and through what is at stake for them, that is, their normality. Interviews with children have identified possible alternative identities that depend on repertoires of identities, in which some constructions are seen as more normal than others. The children prefer seeing themselves with friends at school and as pupils in school.

Full textDenna kvalitativa studie undersöker hur personal - på hem för vård eller boende (HVB) för ensamkommande barn - upplever sina arbetsuppgifter, sin yrkesroll samt vad de menar behövs för att kunna bemöta barnens behov. Empirin består av 6 st semi-strukturerade intervjuer genomförda på 3 olika HVB. Intervjuerna har analyserats utifrån Socialstyrelsens och Inspektionen för vård och omsorgs (IVO´s) tidigare rapporter, forskning om ensamkommande barns behov, internationell personals roll och arbetsuppgifter samt teorier om vad som hjälper människor i återhämtning från psykisk ohälsa. Resultatet visar att personalens huvudsakliga arbetsuppgifter är att möta några av barnens behov, nämligen barnens behov av trygghet, vuxenstöd, vardaglig stabilitet och att känna sig hemma. För detta krävs tid och rätt kompetens. För att personalen ska kunna bemöta dessa behov framhölls personlig lämplighet, engagemang och formell kunskap som viktigt.

Full textDetta är en kvalitativ studie som berör hur personal på Hem för Vård och Boende arbetar med psykisk ohälsa hos ensamkommande barn. Syftet med studien är att identifiera på vilka sätt personal arbetar med psykisk ohälsa samt undersöka hur de skulle vilja arbeta om det fanns tillgång till obegränsat med resurser. I studien har djupintervjuer använts som datainsamlingsmetod och en socialkonstruktionistisk ansats har applicerats. Den teoretiska utgångspunkten i studien är salutogenes och har därmed tillämpats vid analys av det insamlade materialet. En reflexiv analys av empirin har genomförts och resultatet presenteras i olika teman kopplade till personalens upplevelser. I resultat och analys framkommer att respondenterna anser att det inte ligger i boendenas uppdrag att arbeta med psykisk ohälsa men att det i praktiken fallit på deras lott. Metoder som tillämpas i det vardagliga arbetet går främst ut på att bygga relationer med och skapa struktur kring ungdomarna. Samtal är det viktigaste redskapet i arbetet med psykisk ohälsa. Vidare visar resultatet att tydligare information, mer professionell hjälp och mer utbildning ligger på respondenternas önskelista. I diskussionen förs ett resonemang kring vem som egentligen har ansvaret för att arbeta med psykisk ohälsa hos ensamkommande barn och ungdomar samt att en ökad känsla av sammanhang kan vara ett förslag till minskad psykisk ohälsa hos ensamkommande ungdomar.

Full textThe bad health in Sweden is increasing, and the most frequent increase lies in the psychiatric diagnosis at both men and women. Many of those affected works in professions that have close interactions with clients or patients, such as social workers, treatment assistants and psychiatrists. The aim of the study was to examine how personnel at residential care-homes for children experience their psycho-social working environment and comparing this against a norm. The study was based on singled out questions from a standard inquiry protocol, QPS Nordic. The questions for the study was as follows; How do employees at HVB-home experience the psychosocial work environment in terms of related factors requirements, control and support? How are the HVB-home staff's estimates on the basis of the above variables compared to a national norm group? There were 24 responders to the inquiry including 14 women and 10 men. The analysis of the results was based on the survey materials item level and shows that the responding group experienced high demands in their work environment at the same time that they experience a strong support from their management.

Full textAbstract Background: An increase of unaccompanied children and adolescents has been seen in Sweden and most of them are placed in residential care homes (HVB-homes). There are no guidelines on how the meals should be organized at the HVB-homes. In a report from The Health and Social Care Inspectorate it was stated that complaints about food among the children and adolescents was common in HVB-‐homes. Method: A web-based questionnaire was sent to 101 randomly chosen HVB-homes in Sweden of which 55 directors answered. An interview was also made with a director who had answered the questionnaire. Data from the questionnaire was presented with descriptive statistics and the interview was analyzed by identifying overall themes. Result: Meals were in most cases prepared at the HVB-home, and if food came from an external company, it was most common to order from a municipality concern or from catering. The staff working at the home was responsible for planning and preparing the meals. It was also common that children and adolescents were involved in planning and preparing the food, and little over half of the directors thought it was important to involve them in the meal situation at the HVB-home. Most of the directors thought that food related situations worked well, but some indicated that food related situations did not work well. Two themes were identified in the interview, the complexity and the clarity of the questionnaire. Conclusion: The results pointed out that organizing meals in general worked well at HVB-homes, but problems related to the organization of meals could occur. Further research with larger sample is needed to examine if these results are representative.

Full textThe aim of this study has been to contribute with knowledge regarding in what way a processorientation can improve the way public sector manage contracts. As a method a qualitative multicasestudy was used. Three municipalities, and the way they manage contracts with residential carehomes for children and young persons (HVB) was studied. The study showed a complex situation surrounding the carecontract management in public sector. As a way of handling this complex situation the municipalities would gain from using a systems thinking, with the values, tools and workmethods that characterizes Total Quality Management. With an increased processorientation using processmanagement as a tool, the municipalities could improve the quality in their process for managing contracts.
