Abstract:
18211 Background: Prognostic factors identified in advanced non small cell lung cancer are: age, gender, PS, h. SWOG univariable analysis in patients with chemotheraphy; confirmed these factors and show a relationship between the hemoglobin level and the overall survival; in addition the metastasic site number and cisplatin- based chemotheraphy (7). To analyse and compare the hemoglobin level before cisplatin- based chemotheraphy with survival in patients with advanced non- small cell lung cancer. Methods: Retrospective study conducted at the IONC of the 179 clinical record were analized, over a 5 year period. The collected data were: age, gender, PS, histologic type, stage, chemotheraphy cycles number, smooke history, number and metastasic site. We analyzed median and overal survival using Kaplan Meier, and the anemia as a prognostic implication factor with univariable and multivariable Cox regression analysis. Istologic type and TNM (1–6). Results: The mean age was 59 (40–79); 146 (81.5%) male and 33 (18.5%) women; histological types found were squamous cell carcinomas in 66 (37%), and adenocarcinoma in 113 (63%); stage IIIB in 61 (34%) and IV in 118 (66%). 147 (82%) were smokers and 32 (18%) were never smokers. All the patients had PS 0–1. Median overall survival time was 11.53 months and 13.88 months in the haemoglobin level < or > 11 gr/ 100 ml, respectively. (p=0.3). In univariable Cox regression analysis, smoking rates and chemotheraphy cycles number were predictors of survival (p=0.05 y p=0.018, respectively). Hemoglobine (p=0.55). In multivariable Cox regression analysis, only the number of cycles was predictor of survival (p=0.026). Hemoglobine (p=0.34). Conclusions: In our experience, a greater survival tendency was observed in patients with advanced non- small cell lung cancer who presented levels of Hemoglobine greater than 11 gr/dl, previous to cisplatin- based chemotherapy without statistical significance. As a marker of systemic inflammation, raised (C-reactive protein (CRP)) concentrations which are still within the normal range have been associated with an increased inflammation of chronic renal diseases (CRD). The current study aimed to establish potential determinats of raised CRP concentrations in patients who treated in Heamodialysis room,then study the relationship between CRP& some biochemical parameters related CRD We used a CRP latex reagents Kit which is based on an immunological reaction between CRP antisera bounded to the biologically inert latex particles or with CRP in the test specimens of 19 patients with (CRD) mean age 48 years ,range = 30?65 & in 21 healthy subjects as control group their age range = 30 ?45 years. The results are classified according to visible agglutination to: 1-A positive result / is indicated by the obvious agglutination pattern of the latex, in a clear solution. 2-A negative result / is indicated by no change in the latex suspension on the test slide. Then, we correlated the results of the precipitin test with the quantitative data on C-reactive protein. This study has found that: CRP concentrations in patients with CRD were increased very clearly than normal subjects , and established that CRP concentration in male was more than in female for patients specimens. Biochemical studies have shown raising (CRP) concentration is a marker of systemic inflammation. The relation between CRP & S.creatinine ,hemoglobine ,blood sugar is negative ,while positive with blood urea . Other biochemical parameter related to CRD'' blood urea, blood creatinine & hemoglobine and blood sugar'' were assayed for both subjects in order to asses the disease by compared the results. Anemia can occur in women, especially pregnant women and breastfeeding mothers. This can happen because many pregnant and breastfeeding women suffer from iron deficiency. Providing additional iron is one of the government's efforts to overcome the incidence of anemia during pregnancy, childbirth, postpartum and breastfeeding. During the pregnancy process, each pregnant woman is given 90 Fe tablets for 90 consecutive days. Consuming fruits containing vitamin C can increase non-heme absorption by four times and plays an important role in iron absorption. The aim of this service is to educate pregnant women about how to increase hemoglobin levels by consuming FE tablets containing vitamin C. One way to provide this education is through pamphlets explaining the benefits of vitamin C in increasing hemoglobin levels in pregnant women. The purpose of the evaluation in this service is to carry out pre and post tests on pregnant women. The statistical test used is the paried sample t-test. As a result, the mother's level of knowledge before and after counseling using the booklet increased. In conclusion, the presence of counseling tools in the form of pamphlets makes it easier for pregnant women to understand the existing information.

Abstract:
Background: Cesarean myomectomy (CM) is a controversial issue, even relatively contraindicated in obstetric practice. Recent reports showed that CM is not associated with increased morbidity, but those are lacking the data about risks of intensive care unit (ICU) treatment. Aim: The authors evaluated the factors affecting the duration of ICU treatment in patients after CM. Material and Methods: The study included 57 women who underwent CM and were postoperatively admitted and treated in ICU. Correlation analysis was used to estimate the effect of various parameters on the duration of ICU treatment. Results: There was a highly significant correlation between duration of ICU treatment and number of postoperative transfusions (p=0.001), duration of surgery (p=0.007), intraoprative hemorrhage (p=0.008) and myoma diameter (p=0.009). Duration of ICU treatment was also correlated with gestational age, hypertensive syndrome in pregnancy, preoperative hematocrit and hemoglobin values, number of intraoperative transfusions, postoperative hemorrhage and repeated myomectomy. Conclusions: In our report, longer ICU tretment was required in cases of perioperative hemorrhage, prolonged surgeries and those requiring perioperative transfusion. Patients who have had previous myomectomy, with lower preoperative hemoglobine and hematocrit vaues and bigger myomas are at risk of prolonged ICU treatment.

Abstract:
Anemia is a public health problem, being associated with high maternal and fetal morbidity and mortality. Identification of favorable factors in population level is extremely important. We assessed in 400 pregnant women, hospitalized in Maternity Cuza-Voda Iasi, the relationship between sociodemographic, dietary and educational factors and degree of anemia, by using a food frequency questionnaire and a nutritional knowledge questionnaire. The prevalence of anemia in the study group was 20.05%. After multiple regression analysis (made step by) there was shown that anemia is significantly correlated with total iron and calcium intake, total cholesterol and nutritional knowledge.In our group, anemia had a significant incidence, being related with common factors such as iron intake (from food and supplements) and nutritional knowledge. These data highlight the importance of counseling and education in pregnant women in anemia prevention.

In this study, 40 healty male students of Erciyes Medical Faculty were examined. Aerobic capacities of volunteers were evaluated by Physical Working Capacity 170 (PWC170) test and higher in trained group than untrained group. There were some changes in Erythrocyte count, Hemoglobine, Hematocrite, Blood volume, Cell volume and Plasma volume related to the intensity of exercise. There was no difference between trained and untrained groups. MCV, MCH and MCHC values showed no .difference by the exercise. Ali the parameters retumed to inital values, 24 hours after the exercise.

Abstract:
Colibacillosis a disease that can cause considerable economic loss, remains an important health problem. Phyllanthus (Phyllanthus niruri L) and turmeric (Curcuma domestica) are herbs that can be used as immunomodulators. This study was aimed to determine the level of safety of the combination of phyllanthus and turmeric on hematology profile and liver histopathology of layers with colibacillosis. The layers were assigned to the following of 5 groups: a) colibacillosis group without treatment, b) colibacillosis group with 500 mg/kg BW of phyllanthus, c) colibacillosis group with 300 mg/kg BW of turmeric, d) colibacillosis group with phyllanthus and turmeric combination (1:1), e) colibacillosis group with combination of phyllanthus and turmeric (1:2) . After 21 days of treatment, blood and liver sample were collected. The hematological profile (hemoglobin, hematocrit, erythrocyte and leukocyte counts) and liver histology were examined. The result were analyzed statistically by one-way ANOVA. The group that received phyllanthus had higher levels of hemoglobine, haematocrit and erythrocytes than the control group. However, no significant differences were found for the overall groups. Treatment with the combination of turmeric and phyllanthus for 21 days did not cause changes in the hematological profiles or liver histology, and therefor this herbal combination can be used as an alternative therapy for colibacillosis in layers.

Abstract The complications, especially those affecting microcirculation, exhibit great importance in the assessment and follow-up of patients with diabetes mellitus. Blood viscosity plays its most important role in the microcirculation where it contributes significantly to peripheral resistance and may cause sludging in the post capillary venules. The effect of whole blood viscosity in patients whit diabetic nephropaty is still unclear. The aim of this study was to examine the influence of glicaemic control measured by glicated hemoglobin on rheological parameters, and especially on whole blood and plasma viscosity in patients with diabetic nephropaty. Blood viscosity is an important determinant of local flow characteristics. Blood exhibits shear thinning behavior: its viscosity decreases exponentially with increasing shear rates. Estimation of whole blood and plasma viscosity was made on 42 patients, comparing different stages of diabetic nephropathy and with different values of glicated hemoglobin. Depression of the regulatory mechanisms of microvascular blood flow as well as decreased tissue perfusion indicated the restricted blood flow in microcirculatory network in diabetic nephropathy. In conclusion blood viscosity was elevated in the patients with major organ complications and not in the patients without or with early complications, in correlation with poor glicaemic control.

Abstract Aim: There are limited studies on renal involvement in beta-thalassemia patients. The aim of the present study was to investigate renal dysfunctions in pediatric patients with transfusion dependent beta thalassemia major (TD-βTM), using both conventional and early markers of glomerular and tubular dysfunctions and corralate findings to iron chelation therapy, deferasirox. Neutrofil-gelatinase associated lipocalin (NGAL) is an emerging biomarker in early diagnosis of acute kidney injury and also elevated urine NGAL is a good predictor of chronic kidney disease. We aimed to evaluate urine NGAL/creatinin ratio in thalassemia patients and corralate findings with normal controls. Methods: Fifty-three TD-βTM patients (aged 5-19 years) and 47 healthy controls (aged 3.5-18 years) were enrolled in this study. All patients were regularly transfused and all of them were using oral chelator; deferasirox. In addition to conventional renal biochemistries, serum cystatin C were measured with immunoturbodimetric technique. Fresh second morning urine samples collected and analysed for sodium, potassium, calcium, protein, creatinine, glucose, phosphate, uric acid, β2-microglobulin and NGAL. NGAL was measured with immunosorbent assay (ELISA). We calculated glomerular filtration rate by Schwartz formula, fractional excretion of sodium, tubular phosphorus reabsorption and uric acid excretion. Serum ferritin concentration was also measured to asses iron overload. Results: The mean age of our patients were 145±45 month, and controls were 158±140 month. A considerable number of patients demonstrated impaired renal functions. Glomerular hyperfiltration was detected in 9.5%, proteinuria in 40% , hypercalciuria in 60% increased urinary excretion of β-2 mikroglobulin in 62%, increased uric acid excretion in 58% of thalassemia patients. We detected renal tubular and glomerular dysfunctions is associated with especially pretransfusional low hemoglobine levels. We detected no relationship with deferasirox treatment doses and period with renal patologies. Mean levels of plasma cystatin C were elevated in the patient cohort compared to controls ( 0.9+0.2 and 0.78 + 0.06 mg/L, respectively, p<0.05) , Specificially ; 26/53 thalassemia patients (%49) presented with abnormally elevated serum cystatin C. Mean levels of urine NGAL/creatinin ratio was found to be elevated in patients compared to controls (85 + 88 and 31 + 38 ng/mg kreatinin, respectively, p<0.001). In thalassemia group negative significant correlation between ürine NGAL/creatinin ratio and pretransfusional hemoglobine levels was found (r=-0.35, p<0.01). And also we detected urine NGAL/creatinin ratio was corralate with other renal parametries; such as serum cystatin c/creatinin, urine β-2MG/creatinin, urine Ca/creatinin values. Conclusions: Our data confirm high frequency of glomerular and tubular dysfunctions in TD-βTM pediatric patients, and renal patologies was associated especially pretransfusional low hemoglobine levels. Therefore routine use of early markers of renal dysfunction such as urine NGAL is recommended for all patients. Disclosures No relevant conflicts of interest to declare.

Introduction: During pregnancy, the need for iron increases, which triggers anemia. Anemia can be described as a decline in the hemoglobin level below a critical level. Meanwhile, based on a preliminary study, 10 out of 13 pregnant women were not obedient in taking iron tablets (76, 92%). The prevalence of anemia at Tanah Kalikedinding Community Health Center in 2015 remained high. Method: The analytical study used a cross-sectional design. The group of pregnant women in the third trimester who received Fe tablets and had their Hb levels checked at the Tanah Kalikedinding Health Center Surabaya were 54 people. The sampling technique chosen was total sampling. The instruments used were questionnaire sheets, data collection sheets, and survey software. Data analysis used the Fisher exact test. Result : Out of the 54 pregnant women, 20.4% of pregnant women were obedient to taking blood-supplement pills and experienced an increase in hemoglobin levels (63.3%), 70.6% of pregnant women were not adherent, and almost all of them had decreased their. The results of a bivariate analysis using Fisher's exact showed a significance value of p-value = 0.001 <α = 0.05 (p <α). Conclusion: This meant a correlation between adherence to consuming Fe tablets and hemoglobin levels in pregnant women. Also, a p-value of 0.001 (p <α). This means there is a correlation between iron nutritional intake and consumption of blood-supplemented tablets and hemoglobin gravida levels at Puskemas Tanah Kalikedinding, Surabaya.

Abstract:
Background/Aim. Besides viral serotype, HLA haplotype and cytokine genes polymorphism are associated with clinical presentation of hemorrhagic fever with renal syndrome. Since these analyses are unavailable in routine clinical practice, the aim of this study was to assess clinical, laboratory and radiographic findings associated with clinical presentation of disease severity. Methods. A total of 30 patients (27 men and 3 women), average age 40 ? 14.9 years, treated for hemorrhagic fever with renal syndrome from January 1, 1999 to December 31, 2009 in Clinical Center of Serbia, were included in the study. Nine patients (30%) had mild, 14 (46.7%) moderate and 7 (23.3%) severe form of the disease; 24 (80%) recovered, 6 (20%) died in the acute phase of the illness, and 19 patients (63.3%) required hemodialysis. Results. The average titer of antiviral antibodies in patients infected with Belgrade serotype virus were significantly higher in those with severe clinical presentation. Hypotension, anuria, macrohaematuria, pulmonary infiltration, pleural effusion, hepatomegalia and positive meningeal signs were more frequent in the patients with severe form of the disease. Statistically significant differences between groups with mild, moderate and severe clinical picture were found in serum total protein, albumin, calcium, glutamate pyruvate and glutamate oxaloacetate transaminase on admittance; serum creatinine and phosphorus concentration on day 14 and day 21; serum sodium and calciums on day 14; hemoglobine concentration on day 21. A statistically significant correlation was found between clinical presentation of the disease severity and platelet count, white blood cell count, hemoglobine concentration, serum calcium and serum transaminases on admittance. Multivariate analysis identified variables` combinations associated with clinical presentation of the disease. Conclusion. Our study confirmed that we can distinguish patients who will manifest different severities of the disease on the basis of careful consideration of laboratory and clinical findings on admission.

Female adolescent anemia is a health problem in Indonesia. Iron has not been able to reduce anemia. One of the causes is because people are not obedient in consuming Fe tablets. Sesame seeds are a plant-based source of iron which has no side effects and is a potential iron supplement in treating anemia. The research design was a pretest-post test quasy experiment with a control group design with a purposive sampling technique. The sample consisted of 42 people who were divided into 2. Data analysis used independent test, paired test, Mann Whitney test and Wilcoxon sign rank test. The combination of 91 mg sesame meal with Fe tablet was proven to increase hemoglobin levels p value = 0.013 <0.05. Sesame seeds contain various important nutrients, namely iron, folate, flavonoids, copper and other nutrients that play a role in increasing hemoglobin levels so that they can overcome anemia.

Abstract:
The problem of diabetes, which is predicted to increase in number of people suffering from diabetes in the upcoming years, needs to be examined from a bio-psycho-social perspective. This chronic illness which is difficult to be cured, affects the emotional situation and also the adherence to oral anti-diabetes (OAD). The objective of this study is to examine the relationship between bio-physiology of the body (measured by the glycated hemoglobin level of HbA1c) and the emotional situation and adherence to OAD. Observational research with a cross-sectional approach was carried out on 76 type 2 diabetics who were treated at the Gotong Royong Hospital (RSGR) Surabaya. Research data was taken through a valid and reliable questionnaire about emotional situation and adherence to OAD. The glycated hemoglobine was measured in the RSGR laboratory. Inferential analysis was tested using Spearman correlation test. The results showed a significant negative correlation power between the emotional situation and the level of HbA1c (r = -0.256, p = 0.026), as well as between the level of adherence to OAD and the level of HbA1c (r = -0.402, p = 0.000). Those outcomes indicated that the more positive the emotional situation, as well as the higher the level of adherence to OAD, the better health outcome of the diabetic patient. These results enrich the educational material in health promotion. Enhancing the positive emotional situation and the level of adherence to OAD will decrease the morbidity and mortality rates due to diabetes, and optimize the welfare of the patient.

Kadar Hemoglobine (HB) menjadi isu penting bagi atlet. HB berperan dalam pengikatan O2 dan CO2. Sehingga HB memiliki peran yang penting. TUjuan penelitian ini adalah untuk mengetahui HB atlet putra dan putri di Kalimantan Barat yang mengikuti seleksi untuk Pra PON Papua. Metode yang dipergunakan adalah dengan ekspos fakto dengan menggunakan data tes yang telah dilakukan. Tes kadar HB dilakukan oleh Dokter dengan menggunakan tes HB. Atlet yang mengikuti seleksi sebanyak 364 atlet yang terdiri dari 25 cabang olahraga. Data dianalisa dengan statistik deskriptif. Hasil penelitian menunjukkan bahwa ternyata HB atlet putra rerata lebih tinggi yaitu 15,29 dan putri 13,51. HB terendah pada peserta seleksi atlet karate 8,90 dan HB tertinggi pada altet tinju 20.80. sehingga dapat disimpulkan secara rerata kadar HB atlet dapat dijadikan indicator untu melihat tingkat keterlatihan, kedua Kadar HB masih perlu ditingkatkan dengan intervensi latihan dan asupan nutrisi

Anemia in pregnancy can have devastating effects on both the mother and the fetus. The prevalence of anemia cases in pregnant women nationwide in 2018 was 48.9%. Anemia cases of pregnant women at Banjar Puskesmas in 2019 amounted to 62.22% and cases of anemia for pregnant women at BPM Hj. Eniyati in 2020 amounted to 67.81%. The average value of hemoglobin levels of pregnant women in the control group before treatment (pretest) was 9.85 gr / dl and after treatment (posttest) was 10.52 gr / dl. Whereas in the intervention group the average value of hemoglobin levels for pregnant women before treatment (pretest) was 9.74 gr / dl and after treatment (posttest) was 11.15 gr / dl. The statistical test results obtained p value = 0.000. There is an effect of consumption of sweet starfruit juice on hemoglobin levels in pregnant women at BPM Hj. Eniyati, Banjar District, Pandeglang Regency in 2020. It is recommended for pregnant women to check their Hb levels regularly and regularly consume Fe tablets of at least 90 tablets during pregnancy accompanied by consuming fruits containing vitamin C, one of which is sweet star fruit, which can increase hemoglobin levels.

Introduction: Thalassemia and Hemoglobinopathies are the two most common monogenic disorders involving synthesis of hemoglobin molecule. In India the prevalence of β thalassemia carriers is 3-4%. Several ethnic groups have a much higher prevalence of 4-17%. HbE is the most common hemoglobin variant found in Asian population, In India it is prevalent in NE India and Eastern region where the frequencies of HbE carriers range from 3 to over 50%. Coinheritance of HemoglobinE and βthal variants is not uncommon in regions where both are prevalent. The clinical manifestations in thalassemia is due to resultant anemia due to ineffective erythropoiesis, and is mainly treated with blood transfusion which results in deposition of iron and hemosiderin in the renal tubules , heart, liver, spleen and endocrine organs. Aim: 1. To evaluate the kidney function and liver function of βthalassemia major and heterozygous βthal-HbE. 2. Correlate the hematological and biochemical status of βthal major and βthal-HbE patients with iron overload. Materials and methods: This study designed as a case-control study, was conducted in Biochemistry Section of Central Clinical Laboratory, Gauhati Medical College & Hospital during the time period from October 2019 to July 2020. Patients suffering from βthalassemia major, HbE-βthalassemia were selected as cases and age matched controls were selected from healthy relatives of patients, and patients attending ENT, Ortho and Surgery OPD. For Liver function assessment AST, ALT, ALP, Tbil, TP was done and for Kidney function assessment urea, creatinine, uric acid, calcium was done. To assess the iron overload Iron, TIBC, Ferritin was done. Results & Observations: Hb levels in the case group were decreased to 60% of levels reported in the control group (7.08±1.66 vs 11.72±1.17). Abstract: Probiotics drinks, containing Lactobacillus, were rapidly developed because of its benefit to the health. The bacteria reduced the pathogenic microorganism growth such as virus, fungi, and protozoa. The nutritional status is influenced by food intake, immunity and infection. The children of fisherman’s village are included in the insecure nutrition group because of the low social economic status and healthy living behavior. The aim of this research was to know the influence of daily consumption of probiotics in fermented milk to the nutrition status in elementary school children. This research is quasi-experimental with pretest-posttest control group design. Subjects of 95 elementary school children were divided into 2 groups: 62 children as treatment group and 33 children as control group. Determination of nutritional status is based on anthropometry according to CDC 2000 (BMI-age) growth-chart; and the biochemistry parameter consist of the level of serum total protein, albumin, blood urea nitrogen (BUN), urine creatinine and hemoglobin. Determination of serum total protein and BUN were carried out by Microkjeldahl method, hemoglobine by cyanmet hemoglobin method, albumin by dye-binding method, and creatinine level by Jaffe reaction method. The infection status was determined by anamnesis and laboratory examination of feces which was conducted to find the parasite in the stool. Of sixty five ml fermented milk, that contains of 6.5 x 109 Lactobacillus shirota strain, was consumed every day in 37 days. The data was analyzed by Wilcoxon test for nonparametric data and paired T-test for parametric data. The result shows that after the treatment, it increased the nutritional status athropometrically from underweight to normal (12.9%) and overweight to normal (3.23%). In biochemically, it increased the total protein level from 6,111 to 6,705 mg/dL (p=0,001) and decreased the BUN from 42,709 to 41,127 mg/dL (p=0,004) significantly eventhough it was in normal range. The conclusion is that consuming fermented milk for 37 days in children is proven to enhance the nutritional status both by anthropometric and biochemistry parameter. A pulsatile, membrane type pump, TPP, was developed for use in routine cardiac surgery. The artificial ventricle consists of a polycarbonate housing with an inlet and outlet polyurethane tricuspid valve. The membrane is actuated hydraulically. For pre-clinical studies, we designed a study in sheep. After a pump run of 6 hours the animals were allowed to recover and sacrificed after 72 hours. All clinical parameters returned to normal values (p>0.05 vs. control values). During pump run we found elevated free plasma hemoglobine. However, these values returned to normal until the end of the observation period. Thereafter, the device was used in ten routine cardiac surgery procedures. All patients survived the procedure and were discharged from hospital. The postoperative course of lab parameters (kidney, liver and blood count) was no different to routine cardiac surgical procedures. This pulsatile pump system can thus be safely employed in cardiac surgery.

Heparin surface coated hollow-fiber oxygenators and tubing sets were evaluated without systemic heparinization (surface) in comparison to conventional uncoated membrane (plate) and bubble oxygenators and tubing sets with systemic heparinization (ACT>400 s). Thirteen dogs were perfused for 6 hours with a mean flow of 100 ml/kg min. and either surface (42±9 kg), plate (38±12 kg) or bubble oxygenators (41 ± 18: NS) by cavo-aortic cannulation after median sternotomy. Besides continuous monitoring of hemodynamics, blood samples for blood gas, biochemical and hematological analyses were taken before, after mixing and every 30 minutes thereafter. Throughout perfusion, arterial pH, arterial pCO2, arterial pO2 and venous oxygen saturation could be maintained within normal ranges for the three groups. Plasma hemoglobine, however, was significantly lower in heparin-coated equipment (p<0.005), which allowed cardiopulmonary bypass without systemic heparinization and therefore without return of shed blood.

StreszczenieTalasemia alfa to niedokrwistość wynikająca z mutacji w genach kodujących alfa-globinę lub w elementach regulatorowych klastra alfa-globiny. Zespół hemoglobiny Barta to najcięższa postać tej niedokrwistości, spowodowana defektem genetycznym prowadzącym do całkowitego braku syntezy alfa-globiny, najczęściej wynikającym z delecji obu kopii genów z każdego allelu. U chorych nie są syntetyzowane dwie dominujące hemoglobiny niezbędne dla prawidłowej ontogenezy – HbF w okresie płodowym oraz postnatalnie HbA. Hemoglobiną dominującą jest hemoglobina Barta, składająca się wyłącznie z łańcuchów gamma-globiny. Choroba ujawnia się w okresie prenatalnym w postaci niedokrwistości oraz obrzęku płodu. Przypadek tej postaci talasemii alfa został zdiagnozowany i był skutecznie leczony prenatalnie w jednym z ośrodków w Polsce. W pracy przedstawiono jego opis kliniczny oraz zaprezentowano wyniki badań biochemicznych i molekularnych.

e16130 Background: Some patients may develop hormone refractory prostate (HRPC) cancer with neuroendocrine features, although they were not recognizable in the initial biopsy. Serum chromogranin is a marker for tumors with neuroendrocrine differentiation. Purpose: To analyze if serum levels of chromogranin A may identify poor prognosis HRPC patients in patients without neuroendocrine features in the initial biopsy. Methods: Circulating chromogranin A was prospectively analyzed in 34 patients when they were diagnosed under the standard criteria of HRPC. A patient was considered to have higher than normal levels of serum chromogranin if levels were above 100 ug/L. Patients were also assessed for serum PSA, enolase, serum alkaline phosphatase, albumin, and hemoglobin. Results were correlated with clinical prognostic factors as ECOG, PSA double time and Gleason score. We analyzed the time to hormone-refractory disease since diagnosis, and overall survival, as well as pattern of relapse (visceral vs. nonvisceral disease). Results: Median age was 63 (49–85). Median PSA was 33 (0.04–1455). At baseline, 16 patients were chromogranin-negative (46%), and 18 patients were chromogranin-positive (54). Chromogranin A did not correlate with either enolase or PSA. Patients with positive (>100 ug/L) had stadistically significant lower levels of hemoglobine (13.8 vs 12.3), and a trend to have more visceral disease vs locoregional diasease or bone metastases, a lower albumine level (39.85 vs 45.05), higher alkaline phosphatase (221 vs 111), and LDH (1,914.5 vs 415). In this group of patients, we did not find differences in PSA double time, time to hormone-refractory disease or Gleason score. Conclusions: The analysis of this cohort of patients suggests that serum chromogranine A correlates with other known adverse prognosis factors of survival in prostate cancer patients. No significant financial relationships to disclose.

Deli SerdangMethods: The design of this study was a quasy experiment with the design using the one group pretest posttest research design using purposive sampling technique of 15 people. The data used are primary and secondary data. Background: Medroxyprogesterone acetate (MPA) and levonorgestrel intrauterine device (LNG-IUD) are two drugs used to treat abnormal uterine bleeding in women with myoma. We organized this study to compare the effectiveness of these two treatments. Methods: This was a retrospective one-year-long cohort study of 95 women with uterine leimyoma. Fifty three women who had received LNG-IUD formed the LNG-IUD group while 42 women who received regular intramuscular injections of 150 mg MPA at every 3 months for one-year period made up the MPA group. Both groups were compared in aspect of demographic, clinical and biochemical characteristics. Results: At the end of one year, the LNG-IUD group had significantly smaller fibroid size, lower visual analogus scale score for pelvic pain, for dysmenorrhea and dyspareunia than the MPA group. There were a significant reduction in the number of patients with menorrhagia and a significant increase in serum hemoglobine levels both in LNG-IUD and MPA groups at the end of the one-year long study period. Conclusions: LNG-IUD appears as a good choice for the reduction in fibroid size and associated pelvic pain.

Chemically cross-linked hemoglobins are designed to be resistant to renal excretion, even in the absence of haptoglobin. The manner in which binding to haptoglobin influences the pharmacokinetics of acellular cross-linked and native hemoglobins was investigated after intravenous injection of radiolabeled native human hemoglobin and trimesyl-(Lys82)β-(Lys82)β cross-linked human hemoglobin, at trace doses, into rats. Under these conditions, there is sufficient plasma haptoglobin for binding with hemoglobin. In vitro binding assayed by size-exclusion chromatography for bound and free hemoglobin revealed that, at <8 μM hemoglobin, native human hemoglobin was completely bound to rat haptoglobin, whereas only ∼30% of trimesyl-(Lys82)β-(Lys82)β cross-linked hemoglobin was bound. Plasma disappearance of low doses (0.31 μmol/kg) of native and cross-linked hemoglobins was monoexponential (half-life = 23 and 33 min, respectively). The volume of distribution (40 vs. 19 ml/kg) and plasma clearance (1.22 vs. 0.4 ml·min−1·kg−1) were higher for native than for cross-linked hemoglobin. Native and cross-linked human hemoglobins were found primarily in the liver, and not in the kidney, heart, lung, or spleen, mostly as degradation products. These pharmacokinetic findings suggest that the binding of hemoglobin to haptoglobin enhances its hepatocellular entry, clearance, and distribution.

One of the alternatives to overcome iron deficiency anemia is the fortification of foods that are consumed by many people, namely sweet potatoes. The aims of this research was to determine the effect of consumption of sweet potatoes processing on hemoglobin levels of multiparous mothers. This research was conducted for 1 year consisting of the first stage, namely determining the research sample based on the criteria of 32 samples, the second stage was giving informed consent and determining the intervention given, the third stage with the type of research used was Quasi-Experimental research with pretest-posttest only control group design, The design was to measure the hemoglobin levels before and after the intervention giving sweet potatoes for 2 weeks in the control group and the intervention group. The fourth stage was processing the data by using the paired T-test and independent T-test. The results of statistical tests used the paired T-test showed that in the control group the value of ρ = 0.005 <α = 0.05 and the intervention group ρ = 0.000 <α = 0.05. And the results of statistical tests used the independent T-test obtained a value of ρ = 0.008 <α = 0.05, which means that there is an influence between sweet potatoes procesing and an increase of Hb levels for multiparous pregnant women at Minasa Upa Public Health Center of Makassar.

ObjectiveThe aim of present study was to evaluate our clinical decision support system (CDSS) for predicting risk of diabetic retinopathy (DR). We selected randomly a real population of patients with type 2 diabetes (T2DM) who were attending our screening programme.Methods and analysisThe sample size was 602 patients with T2DM randomly selected from those who attended the DR screening programme. The algorithm developed uses nine risk factors: current age, sex, body mass index (BMI), duration and treatment of diabetes mellitus (DM), arterial hypertension, Glicated hemoglobine (HbA1c), urine–albumin ratio and glomerular filtration.ResultsThe mean current age of 67.03±10.91, and 272 were male (53.2%), and DM duration was 10.12±6.4 years, 222 had DR (35.8%). The CDSS was employed for 1 year. The prediction algorithm that the CDSS uses included nine risk factors: current age, sex, BMI, DM duration and treatment, arterial hypertension, HbA1c, urine–albumin ratio and glomerular filtration. The area under the curve (AUC) for predicting the presence of any DR achieved a value of 0.9884, the sensitivity of 98.21%, specificity of 99.21%, positive predictive value of 98.65%, negative predictive value of 98.95%, α error of 0.0079 and β error of 0.0179.ConclusionOur CDSS for predicting DR was successful when applied to a real population.

Abstract Context.—Apparent hemoglobinopathies caused by blood transfusions rarely have been reported in the scientific literature. Objective.—To interpret the abnormal hemoglobins appearing as small peaks on hemoglobin chromatograms or electrophoresis membranes. Design.—In the clinical laboratories of a university hospital and a metropolitan hospital affiliated with a medical school, we interpreted hemoglobin chromatograms and electrophoresis membranes; correlated them with patients' medical, laboratory, and transfusion records; and when possible, identified the abnormal hemoglobin in the donors' transfusion segments. Results.—We detected 52 incidences of apparent hemoglobinopathies in 32 recipients caused by blood transfusion, of which 46 were hemoglobin C, 4 were hemoglobin S, and 2 were hemoglobin O-Arab. When first detected, the abnormal hemoglobins in recipients ranged from 0.8% to 14% (median, 5.6%). Multiple transfusions with abnormal hemoglobins occurred in 11 patients with 2 patients receiving hemoglobin C blood 5 separate times. One patient received hemoglobin C and later S, and another patient received C and later O-Arab. Conclusions.—Apparent hemoglobinopathies caused by blood transfusions are far more common than previously reported and represent diagnostic challenges. Misdiagnosis could lead to unnecessary testing, treatment, and counseling. If a hemoglobinopathy from a unit of transfused blood is identified in a recipient, we recommend notifying the donor of that abnormality.

As hemoglobinopatias são doenças causadas por mutações que afetam os genes de globinas, que resultam em alterações estruturais e/ou funcionais das moléculas de hemoglobina. Dentre as hemoglobinas variantes, as mais frequentes na população brasileira são a hemoglobina S e C. O objetivo é detectar hemoglobinas anormais em uma população acadêmica de Alfenas-MG. Para a realização do estudo, foram coletadas 336 amostras de sangue periférico, hemolisadas com saponina e clorofórmio. Em seguida, foi realizada a eletroforese de hemoglobina em pH alcalino para qualificação de hemoglobinas normais e grande parte das anormais. A confirmação da hemoglobina S foi feita pela prova de falcização. Do total de amostras analisadas, 97,62% (n=328) apresentam perfil eletroforético compatível com hemoglobinas AA e 2,38% (n=8) compatível com hemoglobinas AS (traço falciforme). No Brasil, a prevalência média de Hb AS é próxima de 2% na população total. Portanto, os resultados encontrados reafirmam a média encontrada na população brasileira de pessoas com o traço falciforme.

New tools for the rapid diagnosis of hemoglobinosis could encourage the extension of their screening in Africa. Our goal was to assess the analytical performances of a rapid hemoglobin S and C detection test, the Sickle Scan. This was a cross-sectional study carried out in March 2019 at the Yopougon Teaching Hospital. The subjects followed for hemoglobinosis as well as the subjects seeking out an electrophoresis of their hemoglobin were included. We carried out the hemogram, the electrophoresis of hemoglobin at alkaline pH (reference method) coupled with the metabisulfite sickling test (Emmel test) and the rapid detection test to be evaluated. This immunochromatographic test is capable of detecting hemoglobins A, S, and C, and to infer the hemoglobin phenotype from there. The study recruited 191 individuals. The test detected hemoglobins S and C with a sensitivity of 99.4% and 97.7% respectively; a specificity of 93.3% and 99.3%. The positive likelihood ratio for hemoglobins S and C was 15 and 144 respectively. The negative likelihood ratio was 0.01 for hemoglobin S, and 0.02 for hemoglobin C. The intrinsic characteristics obtained make this test an interesting screening tool for hemoglobinosis S and C. Keywords: Diagnostic test, hemoglobinosis, Abidjan, sensitivity, specificity.

One of the risk factors of Acute Coronary Syndrome (ACS) still controversial is Lipoprotein(a). Lp(a) is one of the lipid componentshighly homologous to plasminogen and which may compete with it in the fibrinolytic pathway and has an atherogenic effect. Prior to thestudy many variaties in results have been shown. These variations are related to different population and ethnics, thus, the researcherswere triggered to investigate the role of Lp(a) on the ACS in the Indonesian population. This case control study was conducted at theSardjito General Hospital, Yogyakarta, Indonesia consisting of 40 participants in ACS as the case group and other 40 persons sufferingfrom Stable Angina Pectoris (SAP) as a control group. This study lasted from May−December 2011. The Lp(a) was measured usingturbidimetric immunoassay method while other laboratory results were obtained from the medical records. The results of this studyshowed that high Lp(a) level (more than 30 mg/dL) was the risk factor of ACS (RR=2.818, CI: 1.069–7.426). There was no difference ofthe baseline characteristics such as: the history of hypertension, diabetes mellitus, smoking, as well as in other laboratory parameters suchas: lipid profile, hemoglobine and uric acid level in the case as well as the control group. Significant differences were found in leucocytenumber, creatinine and blood glucose level. The median level of those parameters was found higher in the case group.

We tested the hypothesis that chemical modifications used to produce stable, oxygen-carrying, Hb-based blood substitutes can induce cytotoxicity in endothelial cells in culture because of altered redox activity. We examined the interaction of hydrogen peroxide with nonmodified hemoglobin (HbA0) and two chemically modified hemoglobins, α-cross-linked hemoglobin (α-DBBF) and its polymerized form (poly-α-DBBF). Hydrogen peroxide-induced cell death (as assessed by lactate dehydrogenase release) in bovine aortic endothelial cells (BAEC) was completely inhibited by all three hemoglobin preparations, consistent with their known pseudoperoxidase activity [hemoglobin consumes peroxide as it cycles between ferric (Fe3+) and ferryl (Fe4+) hemes]. However, reaction of the modified hemoglobins, but not HbA0, with hydrogen peroxide induced apoptotic cell death (as assessed by morphological changes and DNA fragmentation) that correlated with the formation of a long-lived ferrylhemoglobin. A preparation of ferryl-α-DBBF free of residual peroxide rapidly induced morphological changes and DNA fragmentation in BAEC, indicative of apoptotic cell death. Redox cycling of chemically modified hemoglobins by peroxide yielded a persistent ferryl iron that was cytotoxic to endothelial cells.

Two dimers combine to form the functioning alpha 2 beta 2-tetramer. This model of hemoglobin assembly explains the different levels of positively charged and negatively charged mutant hemoglobins that are encountered in heterozygotes and the effect of alpha-thalassemia and heme deficiency states in modifying the level of the variant hemoglobin as well as Hb A2. This paper describes a low-resolution system for identifying variant hemoglobins with great sensitivity and specificity. After electrophoresis of the hemoglobin sample in a gel, fixation is used to entrap the hemoglobin. The gel is dried, incubated with a monoclonal antibody against the desired hemoglobin, then incubated with a second antibody against the first antibody which is conjugated with the enzyme beta-d-galactosidase. An enzyme overlay membrane containing a fluorogenic substrate is then placed on the gel surface, incubated, and removed, yielding an immunofluorescent print. The entire procedure takes only two hours, and by virtue of fluorescent detection gives sharper band resolution and greater sensitivity than conventional dye methods. The system clearly distinguishes SS sickle-cell hemoglobin from heterozygous and "S-like" hemoglobins. The technique therefore holds promise as a powerful probe for allelic variants.

Entretanto, existem várias limitaçõespara seu uso. The World Health Organization considers Diabetes Mellitus (DM) apandemic and, consequently, a serious public health issue. Classically,laboratory tests of Fasting Glucose and Oral Glucose Tolerance Test areperformed for the diagnosis of DM. Recently, the American Diabetes Association(ADA) advocated the use of Glycated Hemoglobin (HbA1C),with a threshold of 6.5%, for this purpose. This work aims to review theliterature on the use of Glycated Hemoglobin in the diagnosis of diabeticpatients. HbA1C is the gold standard laboratory test for the metabolicfollow-up of diabetics patients and presents several advantages, such as:it represents the glycemic mean in the period from 30 to 90 days precedingthe test; presents a greater convenience because the patient doesnot need to fast; has a greater pre-analytical stability when compared tothe other tests used with this function; is less susceptible to daily disturbances.However, there are several limitations to its use. The result of theexamination can be altered by several factors, such as the presence ofvariant hemoglobins, hemolytic anemias, nutritional anemias, uremia,pregnancy and acute blood loss. In addition, the literature shows differencesin sensitivity and specificity of HbA1C when evaluated in patientsof different ethnicities and age groups. The 6.5% threshold should not beconsidered ideal for all populations. Until a determination of cutoff valuesthat have proven diagnostic efficacy, the use of Fasting Glucose andthe Oral Glucose Tolerance Test is more reasonable for the evaluation ofpatients with suspected of DM.

Hemoglobin A1c (HbA1c) is used routinely to monitor long-term glycemic control in people with diabetes mellitus, as HbA1c is related directly to risks for diabetic complications. The accuracy of HbA1c methods can be affected adversely by the presence of hemoglobin (Hb) variants or elevated levels of fetal hemoglobin (HbF). The effect of each variant or elevated HbF must be examined with each specific method. The most common Hb variants worldwide are HbS, HbE, HbC, and HbD. All of these Hb variants have single amino acid substitutions in the Hb β chain. HbF is the major hemoglobin during intrauterine life; by the end of the first year, HbF falls to values close to adult levels of approximately 1%. However, elevated HbF levels can occur in certain pathologic conditions or with hereditary persistence of fetal hemoglobin. In a series of publications over the past several years, the effects of these four most common Hb variants and elevated HbF have been described. There are clinically significant interferences with some methods for each of these variants. A summary is given showing which methods are affected by the presence of the heterozygous variants S, E, C, and D and elevated HbF. Methods are divided by type (immunoassay, ion-exchange high-performance liquid chromatography, boronate affinity, other) with an indication of whether the result is artificially increased or decreased by the presence of a Hb variant. Laboratorians should be aware of the limitations of their method with respect to these interferences.
