Abstract:
Patients with upper abdominal pain, nausea, and vomiting are often evaluated with ultrasound to diagnose symptomatic cholelithiasis or cholecystitis. With a normal ultrasound, a hepatobiliary iminodiacetic acid (HIDA) scan with ejection fraction (EF) is recommended to evaluate gallbladder function. The purpose of this study was to evaluate whether the HIDA scan with EF was appropriately utilized in considering cholecystectomy. Over 18 months, we performed 1533 HIDA scans with EF. After exclusion, 1501 were analyzable, 438 of whom underwent laparoscopic cholecystectomy. Patients were divided into two groups: those with typical and atypical symptoms of biliary colic. Our primary endpoint was symptom resolution of those who underwent laparoscopic cholecystectomy. Symptom resolution was assessed by chart review of postop visits or readmissions. In patients with typical symptoms, resolution occurred in 66 per cent of patients with positive HIDA and 77 per cent with negative HIDA ( P = 0.292). In patients with atypical symptoms, resolution occurred in 64 per cent of patients with positive HIDA and 43 per cent with negative HIDA ( P = 0.013). A HIDA scan with EF was not useful in patients with typical symptoms of biliary colic and negative ultrasounds, and should not be used to make a decision for cholecystectomy. However, this test can be helpful in patients with atypical symptoms, as it does predict symptom improvement in this group.

Abstract:
The organ distribution data for the two hepatobiliary agents HIDA and p-butyl- IDA in mice showed substantial differences in the extent and rate of hepatobiliary clearance of radioactivity. 99mTc-HIDA and 99mTc-p-butyl-IDA were used in computer assisted serial hepatobiliary scintigraphy. In normal subjects, the data of dynamic studies showed that the kidneys were no longer seen after 2.5 min with p-butyl-IDA. Liver radioactivity decreased significantly after 10 min with HIDA compared to 35 min with p-butyl-IDA. The gall bladder was always visualized at the 10-12th min and the 25-30th min with HIDA and p-butyl-IDA respectively. 99mTc-HIDA provided superior hepatic duct images.

Abstract:
Objectives. Pathogenesis of gallstone includes bile stasis due to defect in the gallbladder muscle contraction. Our aim of the study is to find out the role of 99mTc-HIDA scan in assessment of gallbladder dyskinesia in cholelithiasis patients before laparoscopic cholecystectomy and compare the gallbladder dyskinesia with various parameters like symptoms of patients, diabetic status of patients, gallstones size and number, and cholecystitis features in histopathology report after surgery. Material and Method. This is a prospective observational study conducted at our hospital for three years. Totally 40 patients with gallstone were subjected to 99mTc-HIDA scan, to assess the ejection fraction of gallbladder. For all these patients detailed clinical history, presence of comorbid illness like diabetics, and symptomatology were elicited. For all patients, ultrasonogram of abdomen was done to assess number and size of stones. All parameters were tabulated and correlated. Result. While comparing 99mTc-HIDA scan findings with symptoms of patients, 21.2% were asymptomatic and 78.8% symptomatic patients who had ejection fraction less than 80%. All patients in EF >80% group were symptomatic only. It is not statistically significant. On comparing 99mTc-HIDA scan findings with diabetic status of the patients, 42.4% of diabetic and 57.6 % of nondiabetic patients had ejection fraction less than 80%. It is not statistically significant (0.681). While comparing 99mTc-HIDA scan findings with size of the gallstone in ultrasound, 63.6% patients with size less than 1cm and 36.4% with size more than 1cm had ejection fraction < 80%. It is statistically significant (0.048). On comparing 99mTc-HIDA scan findings with number of stones in ultrasound, 18.2% single gallstone patients and 81.8% multiple gallstone patients had EF less than 80% which is statistically significant (0.001). While comparing the 99mTc-HIDA scan findings with histopathology report after laparoscopic cholecystectomy, 21.2% non-cholecystitis patients and 78.8% cholecystitis patients had EF less than 80%, which is statistically (0.017) significant. Conclusion. 99mTc-HIDA scan can be an accurate method to diagnose the gallbladder dyskinesia. Gallbladder dyskinesia in 99mTc-HIDA scan can be used to predict large size stones and multiple stones before surgery. The sensitivity can be improved by 99mTc-HIDA scan in diagnosing cholecystitis patients.

Le travaux récent de Calegari et Geraghty ont enlevé les restrictions de la méthode originale de Taylor-Wiles, cela nous permet d’attaquer les conjectures de modularité plus générales. Leur méthode se fonde sur deux autres conjectures, l'une est reliée au problème d'attacher les représentations galoisiennes aux classes de torsion dans le groupe de cohomologie de la variété de Shimura sous entendue et l'autre à la dégrée de concentration de ces groupes de cohomologie localisés. La première conjecture a été adressée dans une grande généralité par Peter Scholze mais la seconde reste évasive. Récemment, pour la cohomologie cohérente, Vincent Pilloni a développé une version de la théorie de Hida pour les groupes de cohomologie supérieurs qui construit une interpolation p-adique du complexe de cohomologie en question. Comme une application importante, nous pouvons contourner la second conjecture au dessus et en effet dans un travail commun récent, Vincent Pilloni avec ses collaborateurs ont montré que toutes les variétés abéliennes sur un corps totalement réel est potentiellement modulaire. Dans cette thèse, nous adaptons l'argument de Vincent Pilloni pour construire un complexe qui interpole les classes de cohomologie supérieurs de la variété de Picard. Ces résultats servent comme le premier pas vers la modularité potentielle des variétés abéliennes de dimension 3 qui proviennent des Jacobiens de la courbe de Picard
In their breakthrough work, Calegari and Geraghty have shown how to bypass some serious restrictions of the original method by Taylor-Wiles, thus allowing us to attack more general modularity conjectures and related questions. Their method hinges on two conjectures, one is related to the problem of attaching Galois representations to torsion classes in the cohomology of Shimura varieties and the other to the requirement that these cohomology groups, localised at an appropriate ideal are non zero only in a certain range. The first conjecture is addressed in a great generality by Peter Scholze, but the second remains elusive. Recently, for coherent cohomology, inspired by the classical Hida theory, Vincent Pilloni has proposed a method consisting of p-adically interpolating the entire complex of coherent sheaves of automorphic forms on the Siegel threefold. This serves as a way to get around the second conjecture above and plays a crucial role in a recent work, where they show that abelian surfaces over a totally real field are potentially modular. In this thesis, we adapt the argument of Pilloni to construct a Hida complex interpolating classes in higher cohomology groups of the Picard modular surface. In a future work, we hope to use this to obtain some similar modularity results for abelian three-folds arising as Jacobians of some Picard curves

Many patients present with upper abdominal pain and receive some type of pain relieving therapy prior to gallbladder imaging. The physiologic effect of morphine and other analgesics on gallbladder function has been well‐studied. What hasn’t been studied as much are the implications on clinical practice and the decision about whether morphine is the best option to use in suspected chronic gallbladder disease. This case study serves to illustrate the influence of morphine in a patient who underwent both inpatient and outpatient hepatobiliary scintigraphy with dramatically different results. This case study perfectly shows the considerations that must be taken when using morphine because it eliminates many confounding variables; the only difference in the patient at the time of initial and subsequent presentation was the presence of morphine.

Hida a étudié l'image de la représentation galoisienne associée à une famille p-adique de Hida de formes automorphes. Il a montré que l'image d'une famille non CM de formes modulaires classiques ordinaires contient un sous-groupe de congruence. Il a aussi lié le niveau optimal du groupe de congruence à l'idéal de congruence entre la famille de Hida non-CM et des familles CM. Cette thèse se divise en deux parties. La première partie est à généraliser ce genre de résultats dans le cas ordinaire pour les familles de Hida sur les groupes réductifs sous les hypothèses techniques. La deuxième partie se consacre à étudier les cas concrets. On montre que les hypothèses techniques sont satisfaites. Donc le même type de résultats est établi automatiquement
Hida has studied the image of Galois representation associated to a p-adic Hida family of automorphic forms. He has proved that the image of a non-CM family ofordinary classic modular forms contains a congruence subgroup. He also related the optimal level of congruence subgroup to the congruence ideal between the non-CM Hida family and the CM ones. This thesis is divided into two parts. In the first part,we generalize this type of results to ordinary Hida families over reductive groups under some technical hypothesis. In the second part, we consider concrete cases. We prove that the technical hypothesis are satised for these cases. Hence the same type of results is established automatically

Abstract:
Fiqh, literally 'deep understanding', is the science of religious law in Islam. What does it mean for an Islamic jurist to 'do fiqh'? And how does an engagement with fiqh guide a jurist to produce statements of law for particular social contexts? These are perennial questions in the field of Islamic legal studies. The current thesis offers an answer to these questions from the viewpoint of jurists from the early classical Ḥanafī tradition of Central Asia. The thesis starts with an examination of Central-Asian Ḥanafī works of legal theory to extract the underlying epistemological foundations of this legal tradition. The remainder of the thesis presents a series of investigations into a leading work of legal commentary - the Hidāyah of Burhān al-Dīn 'Alī ibn Abī Bakr al-Marghīnānī (d. 593/1197) - to assess how these epistemological foundations inform the work. These investigations range from a study of the processes by which the legal cases commented on in the work were seen to be authoritative, to a study of the use of rational arguments, dialectical sequences and juristic disagreement in exploring and expositing cases of the law. The thesis also studies points of theory employed in the commentary that reveal how social context was seen to impact on the production of law. The study concludes by suggesting a general theory of Ḥanafī jurisprudence, explaining what it means to 'do fiqh' - presented as a particular form of engagement with the legal cases transmitted from the teaching circle of Abū Ḥanīfah (d. 150/767), the school's eponym - and how this fiqh engagement with Ḥanafī precedent informed the production of legal statements tailored to specific contexts - by the application of a particular filter of legal mechanisms, each of which reflects an understanding of the overarching principle of 'necessity' (ḍarūrah). The study presents a uniquely Ḥanafī legal epistemology which is underpinned by particular notions of authority, rationality and tradition.

As a pro-Second Amendment movement, the Boogaloo Boys are easily recognizable because of their Hawaiian-themed Aloha shirts and masks along with their semiautomatic weapons. Having the basic characteristics of anti-establishment far-right populists and seeing the outbreak of violence as something like a party, typically accelerationist Boogaloo Boys use these Hawaiian shirts to hide their intention to trigger a civil war to overthrow what they regard as a corrupt establishment in the US.

Abstract:
We shed new light on the short-term dynamic effects of cyclones on local economic growth in India. We proxy local GDP growth with night-time light intensity data and construct a cyclone index that varies across months and districts depending on windspeed exposures. Using local projections on highly granular data for the period 1993M1-2011M12, we find that yearly estimations hide large short-term differential impacts and that the negative impact of cyclones is the largest between 4 and 8 months after the event.
