Abstract:
Object The term “citation classic” has been used in reference to an article that has been cited more than 400 times. The purpose of this study is to identify such articles that pertain to clinical neurosurgery. Methods A list of search phrases relating to neurosurgery was compiled. A topic search was performed using the Institute for Scientific Information Web of Science for phrases. Articles with more than 400 citations were identified, and nonclinical articles were omitted. The journals, year of publication, topics, and study types were analyzed. Results There were 106 articles with more than 400 citations relating to clinical neurosurgery. These articles appeared in 28 different journals, with more than half appearing in the Journal of Neurosurgery or the New England Journal of Medicine. Fifty-three articles were published since 1990. There were 38 articles on cerebrovascular disease, 21 on stereotactic and functional neurosurgery, 21 on neurooncology, 19 on trauma, 4 on nontraumatic spine, 2 on CSF pathologies, and 1 on infection. There were 29 randomized trials, of which 86% appeared in the New England Journal of Medicine, Lancet, or the Journal of the American Medical Association, and half concerned the prevention or treatment of stroke. In addition, there were 16 prospective studies, 15 classification or grading systems, and 7 reviews. The remaining 39 articles were case series, case reports, or technical notes. Conclusions More than half of the citation classics identified in this study have been published in the past 20 years. Case series, classifications, and reviews appeared more frequently in neurosurgical journals, while randomized controlled trials tended to be published in general medical journals.

Abstract:
Object The number of citations a published article receives is a measure of its impact in the scientific community. This study identifies and characterizes the current 100 top-cited articles in journals specifically dedicated to neurosurgery. Methods Neurosurgical journals were identified using the Institute for Scientific Information Journal Citation Reports. A search was performed using Institute for Scientific Information Web of Science for articles appearing in each of these journals. The 100 top-cited articles were selected and analyzed. Results The 100 most cited manuscripts in neurosurgical journals appeared in 3 of 13 journals dedicated to neurosurgery. These included 79 in the Journal of Neurosurgery, 11 in the Journal of Neurology, Neurosurgery and Psychiatry, and 10 in Neurosurgery. The individual citation counts for these articles ranged from 287 to 1515. Seventy-seven percent of articles were published between 1976 and 1995. Representation varied widely across neurosurgical disciplines, with cerebrovascular diseases leading (43 articles), followed by trauma (27 articles), stereotactic and functional neurosurgery (13 articles), and neurooncology (12 articles). The study types included 5 randomized trials, 5 cooperative studies, 1 observational cohort study, 69 case series, 8 review articles, and 12 animal studies. Thirty articles dealt with surgical management and 12 with nonsurgical management. There were 15 studies of natural history of disease or outcomes after trauma, 11 classification or grading scales, and 10 studies of human pathophysiology. Conclusions The most cited articles in neurosurgical journals are trials evaluating surgical or medical therapies, descriptions of novel techniques, or systems for classifying or grading disease. The time of publication, field of study, nature of the work, and the journal in which the work appears are possible determinants of the likelihood of citation and impact.

Background The number of times that a published article is cited is one indicator of its scientific impact. There is an abundance of articles published on brain arteriovenous malformation [AVM]. Bibliometric analyses are useful in determining most impactful studies within this field. The aim of this study is evaluate most cited works in brain AVM using bibliometric analyses. Methods Brain AVM specific search using Harzing publish or Perish publicly assessable software that indexes citations from Google scholar was performed in May 2017. Full length articles 100 most sited articles were reviewed base on the aim of this study. Results After detailed search 164 articles with a total number of 36776 citations over 68 year period were obtained. Further analysis performed to obtain 100 most cited works. The top 10 most cited article obtained further. The top 100 papers included were sited on average of 295 times per year ranged 104 to 1827. Publication date ranged from 1981 to 2014. Sixty-six of the 100 articles were from United States of America, 11 from Canada, over 15 from Europe .The journal publishing the highest number of the top 100 highly cited articles was Journal of Neurosurgery with 38 publications. The most corresponding authors were from neurosurgical specialty Conclusion This current report provides analysis of the most influential works in brain AVM research describing the trend of most cited works with its comprehensive list.

Bibliometric analyses are useful in determining most impactful studies within this field. The aim of this study is evaluate most cited works in transanal endoscopic microsurgery using bibliometric analysis. Methods Transanal endoscopic microsurgery specific search using Harzing publish or Perish publicly assessable software that indexes citations from Google scholar was performed in October 2017. Full length articles 50 most sited articles were reviewed based on the aim of this study. Results After detailed search 980 articles with a total number of 49543 citations over 43year period were obtained. Further analysis performed to obtain 50 most cited works with total of 6728. The top 10 most cited article obtained further. The top 50 papers included were sited on average of 320 times per year ranged 78 to 292. Publication date ranged from 1992 to 2013. Eleven of the 50 articles were from Germany, 10 from United States of America, over 20 from Europe. The journal publishing the highest number of the top 50 highly cited articles was Diseases of Colon & Rectum Journal of with 15 publications. The most corresponding authors were from colorectal specialty Conclusion This current report provides the most up-to-date analysis of the most influential works in transanal endoscopic microsurgery research describing the trend of most cited works with its comprehensive list. None of the article meant the citation classics criterion.

This study employs the Theory of Planned Behavior (TPB) to understand the role of attitudes, subjective norms, and perceived behavioral control on students’ intent to orally cite sources using complete, rather than partial, citations. Undergraduates (N = 326) enrolled in general education oral communication courses completed a self-report survey testing the TPB variables and students’ citation intentions. Findings demonstrate that subjective norms and perceived behavioral control predict student intention to cite sources and do so completely. Tensions between credibility gains and time constraints are also evident. Implications for teaching students in basic courses about oral citations are discussed.

Abstract When Stephen Ellis died in July 2015, African Studies lost one of its most prolific, provocative and celebrated scholars. Given the scale and uniqueness of his contribution, it is perhaps surprising that a collection of his writings did not appear during his lifetime. It is now possible to bring such a volume to the public. With an introduction by Tim Kelsall and an afterword by Jean-François Bayart, this collection aims to provide scholars and students with an introduction to the main themes in Ellis' work. These revolved around the roles of religion, criminality and violence in African society and politics--preoccupations that also informed his interpretation of African rebellions and resistance movements. Doctors view the best relationship with patients in highly personal and idealistic terms, admitting through discussion of their own experiences that sustained interpersonal relationships with many patients are difficult to establish. For doctors, interpersonal trust with their patients looms as the central feature of strong, effective relationships. The ability to relate to patients on deeper psychological and emotional levels was the key focus for them in their work. They also cited other roles, such as friend and expert advisor, as important in gaining patient trust. Doctors’ views and their best patient relationship experiences emphasized the benefits of dyadic care delivery, even as the notion of the relational dyad finds less support within health care, given over instead to higher volume, transactionally oriented care relationships between organizations and patients.

Many women writers have secured a solid place in the literary canon, while others have remained marginalized. This reference includes alphabetically arranged entries on roughly 70 women writers whose works are widely read in English, and on some 20 related topics. While some of the writers profiled are widely known, others have not yet received as much attention. And while most of the writers are from England and America, the volume also profiles Chilean, Brazilian, Indian, South African, Australian, French, and German authors. The writers selected are feminist, in that their works have challenged traditional gender roles, explored female oppression, or critiqued patriarchal social structures. In addition to providing biographical information, the entries include interpretative summaries of major works. Each author entry includes biographical information, an extensive summary treatment of at least one of her works, an list of her other major works, cross-references to related entries, and a list of works for further reading. What distinguishes this encyclopedia from the many other volumes addressing feminist literature or literature by women is the interpretative summary in each entry. The volume closes with a list of works cited. Entries are clearly written and are accessible to high school students and undergraduates.

The only woman to receive the Congressional Medal of Honor for her service during the Civil War, Dr. Mary E. Walker (1832-1919) was a surgeon, a public lecturer, and an outspoken champion of women's rights. The book combines memoir, interview, and archival sources to survey the musical life of the author’s hometown, Detroit, in his youth during the city’s heyday, 1940s–1960s. After an opening chapter on the formation of personal musical identity, the focus shifts to the formative role of the public school system in educating and shaping the careers of waves of highly talented youth, many of whom became leading figures in African American and classical music nationally. Next comes a panorama of the “neighborhood” subcultural musics of European, southern white, and southern black immigrants to Detroit, followed up by a close-up of the Jewish community’s special case. “Merging Traffic” considers the way that industry, labor, the counterculture, Motown, and the media brought many streams of music together. AbstractDetermining the scientific footprint of a given author is not easy, as many bibliometric metrics exist and none can capture the full extent to which someone has fed the field with knowledge and changed the course of science. Using existing databases (Web of Science and iCite from the National Institute of Health), we tried to capture the impact of the work of Richard P. Baum over the last four decades. His works led to over 11,000 citations in the literature with highly cited articles, even when using the latest more valid practice of comparing the relative citation ratio that captures the scientific productivity independent of the field of study.

Abstract The modern form of containers, as popularly known through platforms like Docker, Singularity (Apptainer), Podman or Charliecloud, to cite only a few, began to take shape over a decade ago. However, the fundamentals behind containerization and actual benefits of containers in scientific computing remain largely unclear to a vast majority of users. In fact, there is a significant gap between simplistic “Hello world” examples found online and real scientific applications. Also, rumors suggest that achieving satisfactory performances on supercomputers across multiple nodes is impossible. The aim of this paper is to explain how containers can leverage the potential of high-speed networks for inter-node communications with UCX on Fram and Betzy (from the Norwegian national e-infrastructure provider). It is also shown how to achieve near-native performance on LUMI (EuroHPC’s flagship) despite a “Slingshot-11” interconnect and proprietary library. Results obtained in the standard OSU Micro-Benchmarks tests for latency and bandwidth, and with a fully-fledged climate model, demonstrate that containerized applications work just as well as their bare-metal counterparts, are portable and provide bit-for-bit reproducibility on different platforms. Containers are therefore highly recommended to minimize deployment and porting issues i) for AaaS (Applications as a Service) coming with all the necessary software environment (rather than source code only); and ii) so that HPC users do not have to rely on anybody to install what they need and can be operational within minutes whilst still getting top performance.

Abstract It is widely recognised that Artificial Intelligence (AI) technologies present a considerable opportunity to increase efficiency, productivity, and outputs across a broad range of tasks and activities. These technologies can allow decisions to be made much faster, with less human involvement - in certain cases removing the need for a human at all. As a result, AI has been cited by policing leaders as having the potential to deliver a ‘step change’ in policing efficiency. It is also recognised, however, that there are challenges when using such technologies in domains that involve high risk and high consequence decisions - where evidence, accountability, and human oversight are paramount. The National Police Chief’s Council (NPCC) in the UK has taken an important step to mitigate risks, by developing an AI Covenant that includes key principles to consider when using AI in policing, for example, the need for AI systems to be explainable, accountable, and robust. These principles provide useful guidance, however, they are intangible and context dependent. The notion of “explainability”, for example, is inherently human-centered and involves interpretation on behalf of a human to determine whether an output is “explainable” in a given context, including the intended audience and the role of the explanation. Typical AI evaluation centres around technical benchmarks and academic scenarios, however, we cannot assess whether human-centered principles are delivered by focusing on technical evaluations of the AI systems themselves. Furthermore, in policing the users of technologies are experts in their domain, such as experienced analysts or investigators, and their interpretations of outputs from AI systems will be informed by their expertise. We cannot assess whether human-centered principles are delivered for a policing context without involving humans who are experts in that context, together with realistic decisions for them to make. This can be defined as a socio-technical approach to system evaluation which considers people, machines and context in combination. This paper explores a practical case study involving AI decision support for a policing context, which looked to evaluate the impact of AI transparency and explainability. The case study drew upon cognitive engineering principles, firstly to understand and define the decision making context, then design a supporting AI system that delivered transparency, and finally to evaluate the impact of system transparency upon expert decision making. The evaluation utilised a bespoke environment, serving as a platform for human-centered research of a socio-technical system. This paper presents some initial lessons on the value of delivering socio-technical evaluation, and core building blocks that enable such evaluations to be performed, including: A realistic context for decision making, including a recognisable narrative supported by synthetic data and real world experts. A physical or virtual environment to host aspects of the socio-technical system under evaluation, for example, expert users interacting with AI systems under observation. A multidisciplinary research team who can formulate and address research questions, drawing upon different perspectives. These components form a replicable foundation, called the Human-Centered Experimentation Platform (HEP), that can be scaled to address various human-centered research questions and different types of AI systems. Future work will look to expand and validate the method and components with other case studies, across different types of system and use cases.

Social media is increasingly used for the dissemination of scientific publications by most science, technology and management (STM) journals. Bioinformatics is one of the new areas of research emerging in the subject tree. The role of social media in increasing the awareness of published works in bioinformatics has not been previously explored. Here, a qualitative analysis of the highest trending works in bioinformatics along with a correlation analysis with their social media metrics (altmetrics) is made. A comprehensive search has been performed for articles related to the bioinformatics using the SCOPUS Database. Further, a highly-cited article with more than 1000 citations has been filtered out. Another comprehensive search for bioinformatics articles using the Altmetric database was conducted. The Altmetric database provides a total weighted score of all online mentions for an article received on Facebook, Twitter, blogs, and mainstream media sources. There is a significant correlation has been found between the patent mention and citations.

Abstract One of the current and important challenges faced by cathodic protection professionals is to access the corrosion probability of cathodically protected buried pipes subject to alternating current (AC) interference (i.e., AC corrosion). In practice, it is very common for cathodically protected pipes to be buried adjacent high voltage AC electrical power lines and electric energy distribution systems. These electric power systems can produce stray AC current in soil or induced AC current in the pipe. This AC current can cause severe corrosion on buried pipes which are supposed to be catholically protected. There are several criteria cited by the literature to evaluate the probability of AC corrosion; however, they are inefficient since failures due to AC corrosion have been reported on pipelines which displayed electric and electrochemical parameters within the acceptable limits of those criteria. This work has as an objective to propose a new thermodynamic criterion and presents all the necessary equipment for obtaining in the field the necessary electric and electrochemical parameters to apply the proposed criterion in a safe way. Those parameters are obtained from the waveform of pipe-soil interface AC+DC off potential. Two devices were developed: a probe composed of a modified permanent Cu/CuSO4 reference electrode coupled with corrosion coupons and an electronic alternating switch device. Some measurements conducted on existing pipelines are also presented.

The COVID-19-related research field has emerged with a number of papers and citations in a very short time period. Journals published COVID-19-related works have increased their impact factor (IF), which reflects the attention on COVID-19. With publications of COVID-19-related works in Web of Science, we found that COVID-19-related papers increased IF of journals but more benefits were given to the high IF journals. Highly cited COVID-19-related papers were distributed in high IF journals. This increases the inequality of IF in research category. In conclusion, our findings imply that IF is vulnerable to external events, therefore it supports to warn the use of quantitative indicators in assessment.

Taking the authors of extremely highly cited papers as the research object, 18 items of personal information on education, work, awards, and research directions of 287 authors of extremely highly cited papers were obtained through literature information retrieval and network information retrieval. Bibliometric analysis was carried out in terms of cooperation, career length, and education level. The results show that the authors of extremely highly cited papers are more and more cooperating with others; the authors of extremely highly cited papers mostly publish extremely highly cited papers when they are early in their careers; most of them have studied in well-known institutions and received doctoral education; the age of the authors of extremely cited papers will also affect the citation when publishing extremely cited papers.

This paper presents a study of bibliographical references cited in articles published by MathNet.Ru journals. Based on data obtained from mathematical portal Math-Net.Ru, we built a journals citation graph, with its vertices denoting journals, and edges representing bibliographical references (citations) between papers published in these journals. To increase the reliability of the constructed graph, we chose a 2010-2021 citation time interval, when distribution of citing papers (papers that have cited other works) had stabilized at 3500-4500 citations per year. The structure of citation ageing is investigated; it is shown that the half-life of these citations is 8 years. So, the publication date of cited papers (papers that have been cited by other works) was limited to the year 2002. The constructed citation graph was found to have a small diameter and high density, indicating that there is a high level of research collaboration in Math-Net.Ru. It is shown that there is no Matthew effect as a pronounced advantage in the citations of leading journals in relation to less well-known ones. The adequacy of the Math-Net.Ru journal citation graph as a scientific collaboration model is confirmed by comparing the ranking of journals included the citation graph with their Science Index ranking in scientific electronic library eLIBRARY.RU. The two rankings were found to have a direct moderate relationship between themselves. A number of substantive conclusions are drawn from analysis of the citation graph.

Intrusion detection schemes must be able to detect intrusion attempts at a high network bandwidth, besides having to deal with the lack of realistic training/testing data, changes traffic behavior, unreliable classifications over time and adversarial settings. In this work a new intrusion detection model, namely reliable intrusion detection, is introduced, whose main characteristic is the usage of both batch and stream learning algorithms coupled together. The proposed model advances the state-of-the-art in intrusion detection, providing reliable detection even in the presence of network traffic behavior changes and lack of model updates. The work relevance was recognized in the publication of 5 top-tier journals, 10 international and national conference papers, and 1 registered patent, being cited almost 200 times in current works in the literature.

The major alternatives for producing work from the chemical energy of fuels include combustion systems and fuel cells. Combustion systems are subject to several performance limiting constraints. Key amongst these is the fact that combustion is an uncontrolled chemical reaction and is typically highly irreversible. The requirement to operate below the metallurgical limit adds to the irreversibility or exergy consumption in practical combustion systems. Furthermore, the use of heat exchangers, which must have finite temperature differences between fluid streams, compounds the exergy consumption. The fuel cell conversion system is a major alternative to combustion systems. It operates as a direct conversion device and is often cited as having a potential for 100% second-law efficiency. Realistically, however, the chemical reactions involved are not reversible. More importantly, the available fuel resources must be reformed to make the chemical energy of the fuel convertible to work; such processes require significant exergy input that must be factored into the determination of the overall exergy conversion efficiency attainable. This paper gives a first- and second-law analysis of the alternate systems for conversion of fuel exergy to mechanical work thus providing a more realistic comparison of the potential of both systems.

Undergraduate students in media-related fields expect their courses to be focused on real-world work environments, ultimately producing media products that can be proudly shared on social media and become part of their CV. The present paper presents a combination of integrated curriculum (IC) and project-based learning (PBL) to design a year-long activity that traverses various courses through an academic year to produce a quality media outcome. It utilizes face-to-face group interviews with the students and the instructors involved in the experience, survey questionnaires, as well as a comparison of the grades before and after the implemention of the project. The results show that the quality of the media products rised, that overall satisfaction of instructors was high, but students had mixed feelings. The lack of self-assessment tools and the emergence of group conflicts considering the length of the project were cited as the main limitations.

Higher Education Institutions are increasingly aware of industry expectations regarding work-ready graduates. Work Integrated learning and co-operative education initiatives are widely acknowledged for improving professional skills and work readiness, however, graduates often lack the ‘soft’ skills (communication, collaboration, problem solving) deemed essential for enhanced productivity and innovation in the workplace, i.e. employability skills (Jackson, 2010). Anecdotal evidence from the Professional and Community Engagement (PACE) program at Macquarie University identified the difficulties that students experience in self-assessing employability skills. One research study highlighted the inflated self-perceptions and an overall lack of humility often associated with recent graduates (Papadopoulos 2010, cited in Jackson 2015). This paper discusses the theoretical and practical development of a competency development approach to learning for employment using an Assessment Centre process model currently embedded in the curriculum of one PACE unit. Developed and coordinated by post-graduate psychology students, the model provides a set of behavioural criteria by which to assess student employability skills. While there is little evidence in the literature of the use of AC's for enhancing undergraduate employability, (see Keele et al, 2010), preliminary research and evaluation findings from this project, suggest that the AC process can have a positive influence on the development of the ‘soft’ skills of employability

This research aimed to carry out a bibliometric analysis of research related to digital literacy using the Scopus-indexed articles database which was published from 2017-2023. To collect data, the author used a document Analysis or documentation instrument, and a sample of collected 665 articles from the Scopus publications was obtained through the application of the saturated sampling technique. The data were analyzed through the R Program Biblioshiny package. The first finding is that a steady rate of publications in the field of digital literacy was observed from 2017 (68 articles) up to 2022 (131 articles). The second finding is that keywords like “e-learning”, “digital literacies”, and “students” have higher bibliographical coupling indexes of 29, 26, and 23 respectively. For the third finding, the concepts of “e-learning” and “information literacy” are very important terms within their network. Besides, authors whose works had much attention have included “e-learning”, “digital storage,” “computer science”, “digital environment”, “education”, “curricula”, and “students” in their words. Based on these findings, the main recommendation is for researchers to make further investigations in order to discover why works including the keywords like “e-learning”, “digital literacies”, and “students” are mostly cited.

Introduction. Severe limitations for the application of Ti:LiNbO3 optical (channel) waveguides especially in phase sensitive devices are set by the susceptibility of this material to optically induced changes of the (effective) indices of refraction (optical damage). Therefore several groups have thoroughly investigated this photorefractive susceptibility using different methods and waveguide devices such as asymmetric Mach- Zehnder interferometers, directional couplers and straight waveguides [1-3]. In planar waveguides using a holographic method optically induced changes of Δn ≃ 10−6 could be measured [4]. The understanding of the photorefractive effects in Ti:LiNbO3 waveguides has been considerally improved by the cited work. Nevertheless, an even more sensitive method is required to study these effects, as recent developments of waveguide fabrication techniques like codoping, ion exchange, outdiffusion and substrates of higher purity have reduced the damage susceptibility considerably. A method is needed which allows a quantitative assessment of the quality of — in particular channel — waveguides with respect to their photorefractive susceptibility.

Abstract:
The goal of this work was to understand the role of the enzyme sucrose synthase (SuSy) in synthesis of cellulose and callose in plants. The work resulting from the this grant leads to a number of conclusions. SuSy clearly plays diverse roles in carbon metabolism. It can associate with the plasma membrane of cells undergoing rapid cellulose deposition, such as cotton fibers, developing maize endosperm, gravistimulated pulvini, and transfer cells of the cotton seed. It is also concentrated at sites of high callose deposition (tapetal cells; cell plates). When SuSy levels are lowered by mutation or by anti-sense technology, cell walls undergo degeneration (maize endosperm) and show reduced levels of cellulose (potato tubers). In sum, our evidence has very much strengthened the concept that SuSy does function in the plasma membrane to channel carbon from sucrose via UDP-glucose to glucan synthase complexes. Soluble SuSy also clearly plays a role in providing carbon for starch synthesis and respiration. Surprisingly, we found that the cotton seed is one unique case where SuSy apparently does not play a role in starch synthesis. Current evidence in sum suggests that no specific SuSy gene encodes the membrane-associated form, although in maize the SS 1 form of SuSy may be most important for cell wall synthesis in the early stages of endosperm development. Work is still in progress to determine what does control membrane localization - and the current evidence we have favors a role for Ca2+, and possibly also protein phosphorylation by differentially regulated protein kinases. Finally, we have discovered for the first time, a major new family of genes that encode the catalytic subunit of the cellulose synthase of plants - a result that has been widely cited and opens many new approaches for the study of this important plant function.

Workplace spirituality has been defined as a framework for organizational values that is part of the culture, connection, and future. AKA is a small woman-owned, spirit-led business. Our mission and vision direct us toward programs, research, evaluation, and communities that match our values, theories, and interests. Because spirituality is essential for wellness, well teams, and well communities, we designed this study to explore AKA team member views about spirituality and how they incorporate spirituality in their work. The following research questions to guide this study: 1) What are some spiritual practices of AKA team members? and 2) How do spiritual practices influence the inner self, outer self, and team/clients? Methods: Data for this study were collected from AKA Associates with Zoom interviews from May 2023 to July 2023. Interviews were audio recorded, transcribed using Otter.ai, cleaned, and coded using thematic analysis. Results: The research team contacted twelve AKA associates; nine were eligible to participate. Two were male, and seven were female. The thematic analysis revealed three major workplace spirituality themes: beliefs, practices, and experiences. Spiritual beliefs varied among the team. Many cited a belief in God or a higher power. Others mentioned goodness, altruism, and a Divine connection to the path they are on. Practices have to do with the outer self and what we do, what we see, and what we hear. AKA associates’ practices vary from prayer, journaling, forgiveness, burning sage, being in nature, attending church, having joy, gratitude, and involvement in church/faith communities. AKA associates talked about their varied experiences, including service, generosity, empowerment, grounding, integrity/accountability, advocacy, and authenticity. Discussion: This qualitative study found that beliefs, practices, and experiences create connections and enduring relationships. Within the AKA team, beliefs varied about spirituality. Some believed in a Creator or God; others talked about a higher power, goodness, or nothing. Spirituality in the workplace has the potential to improve the health and well-being of employees and clients served. Spiritual beliefs vary within individuals and organizations; what is most important is the experiences that result from a spiritual orientation. When personal, organizational, and spiritual values align, growth, creativity, and innovation happen.

The authors have studied daily decision-making processes in groups under uncertainty, with an exploratory field study in the medical domain. The work follows the tradition of naturalistic decision-making (NDM) research. It aims to understand how groups in this high reliability context conceptualize and internalize uncertainties, and how they handle them in order to achieve effective decision-making in their everyday activities. Analysis of the survey data shows that uncertainty is thought of in terms of issues and sources (as identified by previous research), but also (possibly a domain-specific observation) as a lack of personal knowledge or skill. Uncertainty is accompanied by emotions of fear and shame. It arises during the diagnostic process, the treatment process and the outcome of medical decision making. The most frequently cited sources of uncertainty are partly lacking information and inadequate understanding owing to instability of information. Descriptions of typical group decisions reveal that the individual himself is a source of uncertainty when a lack of knowledge, skills and expertise is perceived. The group can serve as a source of uncertainty if divergent opinions in the decision making group exist. Three different situations of group decisions are identified: Interdisciplinary regular meetings (e.g. tumor conferences), formal ward meetings and ad hoc consultations. In all healthcare units concerned by the study, only little use of structured decision making procedures and processes is reported. Strategies used to handle uncertainty include attempts to reduce uncertainty by collecting additional information, delaying action until more information is available or by soliciting advice from other physicians. The factors which ultimately determine group decisions are hierarchy (the opinion of more senior medical staff carries more weight than that of junior staff), patients’ interest and professional competence. Important attributes of poor group decisions are the absence of consensus and the use of hierarchy as the predominant decision criterion. On the other hand, decisions judged to be effective are marked by a sufficient information base, a positive discussion culture and consensus. The authors identify four possible obstacles to effective decision making: a steep hierarchy gradient, a poor discussion culture, a strong need for consensus, and insufficient structure and guidance of group decision making processes. A number of intervention techniques which have been shown in other industries to be effective in improving some of these obstacles are presented.
