Paton, Robin William. "Does selective ultrasound imaging of 'at risk' hips and clinically unstable hips, in developmental dysplasia of the hip (DDH) produce an effective screening programme?" Thesis, Lancaster University, 2011. http://ethos.bl.uk/OrderDetails.do?uin=uk.bl.ethos.618306.

Full text
Abstract:
Aim: A 10 year prospective, observational, longitudinal cohort study was undertaken. All clinically unstable hip joints and those with 'risk factors' were selectively screened for Developmental Dysplasia of the Hip (DOH) in a specialised Paediatric Orthopaedic clinic. The main 'risk factors' included breech presentation, strong family history, certain foot deformities [Congenital Talipes Equino Varus (CTEV), Congenital Talipes Calcaneo-valgus (CTCY), postural talipes equinovarus (TEV)] and oligohydramnios. Other lesser 'risk factors' (torticoliis, metatarsus adductus) and miscellaneous factors (clicky hip joints, limited hip abduction) were in addition screened. Method: From the 37,510 births there were 130 referred as clinically unstable and 2826 neonates/infants whose hips were sonographically screened. Data recorded included the presence of clinical hip instability or 'risk factors', the gender, the race, the age, the sonographic results (primary and secondary scans), type of referral (GP or Paediatricians) and the side of pathology. Pathological DDH comprised Graf Type Ill, IV sonographic abnormalities and irreducible hip dislocation. Results: Males with 'pure' risk factors (without clinical instability) were not at risk and their hips should not be routinely imaged by ultrasound (Breech 1:594, family history 1:99, CTCV 0). There was an equivocal case to be made for selective hip screening in females with certain risk factors (without clinical instability, Breech 1:32, family history 1:28, CTCV 1:52). Possible 'risk factors' of TEV, CTEV, 'clicky' hips and oligohydramnios were not identified as 'true' risk factors. Oligohydramnios is defined as a lack of amniotic fluid around the foetus Primary neonatal clinical hip screening was more effective at identifying cases of pathological DDH than 'at risk' sonographic screening (clinical screen in g: sensitivity of 66%, the specificity of 99.77% and PPY of 27.97%) The General Practitioner 6-8 week clinical hip check was ineffective as a screening programme and is not recommended (only identified one non- surgically treatable case in 10 years). Conclusion: Selective sonographic screening of conventional 'at risk' factors does not improve the effectiveness of the established 'best practise' clinical hip joint screening programmes (with the outcome measure of irreducible hip dislocation).

Abstract:
The DiGeorge syndrome also known as 22q11.2 microdeletion syndrome, is the most common deletion in humans. This deletion is linked to a non-allelic homologous recombination that occurs during meiosis and involves sequences called LCRs for "Low Copy Repeats". Depending on the LCRs involved, different deletions are observed, inducing the loss of approximately 40 genes. The absence of genotype/phenotype correlation in patients and the phenotypical differences regardless of the size of the microdeletion suggests the involvement of additional parameter. The hypothesis of epigenetic changes associated with the onset or variability of symptoms has been suggested but never investigated. In order to tackle this question, we decided to focus our attention of the role of the HIRA histone chaperone encoded by a gene located in the 22q11.2-deleted region. HIRA is involved in the deposition of the H3.3 histone variant, one of the main histone in the brain. In order to determine whether HIRA is implicated in the neurological manifestations in DiGeorge patients and particularly in schizophrenia, we developed and optimized a new protocol for the direct differentiation of human induced pluripotent stem cell (hiPSCs) into neural progenitors, cortical and dopaminergic neurons. In parallel, we developed a new protocol for hiPSCs differentiation toward the skeletal muscle lineage and the production of multinucleated muscle fibers. Altogether, these results open new perspectives for the modeling of a large number of pathologies, and in the context of our laboratory, the exploration of epigenetic mechanisms associated with phenotypic variability in different genetic diseases

The current research is an invitation to the reader to sail into what has been lived for me, as a researcher, reaching the hip-hop young cultural movement in the Lucia do Livramento Mayvorne´s elementary school, in the Monte Serrat hill in Florianópolis. Through the whole investigative process, I tried to have a look on this young cultural movement and on the adolescents here called: Daisy, Rose, Violet, Clove, Dahlia, jasmine, sunflower, Bromeliad, Lily and Cactus that brought their experiences of this culture giving substance to the Research. I understood that gestures, corporal motion and vestry style came imbued with the black identity, marked by their braid hairs and body swaying. The concept of identity says about the social, cultural, ethnic and gender belonging, which is constructed in family, social and institutional atmospheres, over comparisons and assays in the socio-cultural inter-relations. I have seen in my body and in the pleasure for dance complicity for the research. This research is composed by the discourse of the adolescents, physical education teachers, arts teacher and the school principal, in connection to the body and dance, which are the main object of this research, where I searched for my reflections, having the theoretical-methodological allusion of the cultural researches as support. On the dance and body experience, the adolescents manifested their importance by photographing the choreographies of the hip-hop young cultural movement. Stays for the reading of the complete research, the reflection of this culture on the discourse of black adolescents that were here investigated.

Abstract:
HIMA! / NOW! - translation from Armenian In light of recent and ongoing protests, demonstrations and riots in Ukraine, Egypt, Israel or Armenia, some actual questions arise such as how is the built environment used as an area of protest, how it is chosen as a focal point or path for resistance, what are the common characteristics of revolutionary spaces and how the environment effects on success. Lack of democracy from the government and in everyday life of Armenia society, total corruption and feeling of impunity oligarchy, the government and most of governmental institutions, weak economic growth accompanied by alarming number of emigration as well as successful scenarios in neighboring Georgia and Ukraine, force to predict a inescapable eruption of protests Armenia in near future. History shows that protests and civil disobedience are inevitable and necessary expressions of dissent in any democratic nation and country. However there are different passive tools to improve democratic institutions in the society and country in general such as public forums, open discussions and debates, freedom of speech and media etc. With this project I want to create a great Park with generous program embracing main democratic principles as a Public Space and Forum for everyone in capital of Armenia, Yerevan, BUT in case of nascent civil resistance the Park will become a space as a tool with urban inventory that people can use, manipulate, claim in order to defend their values and save own life's. This will be a Playground/Fortress for Democracy !

Groves, Dawn. "Geometric variances in hip osteoarthritis and tribology of the natural hip." Thesis, University of Leeds, 2015. http://etheses.whiterose.ac.uk/10415/.

Full text
Abstract:
Osteoarthritis (OA) of the hip joint is a common form of arthritis that often requires surgical intervention using total hip replacement (THR), as treatment using early interventional techniques is still poorly understood. It is hoped that gaining a better understanding of hip geometry, and that using this information in future in-vitro simulations, will contribute to the evidence base regarding the aetiology of OA and the use of early surgical interventions to prevent or delay the onset of hip OA. This thesis aimed to investigate geometric variations in patients with known hip OA and compare them to asymptomatic participants of the same age. The thesis also aimed to develop a novel in-vitro simulation model for the tribological testing of complete natural hip joints. Twenty nine participants were recruited into the study (n=15 control and n=14 hip OA), and the geometry of their hip joints was explored and compared using high resolution (3.0 T) MRI and 3D shape matching software (EndPoint), which in the main, had previously been used for investigating the knee joint. In the study group, obvious shape differences such as superior flattening of the femoral head, differences in the sphericity of the head and acetabulum, and a significantly smaller degree of acetabular anteversion were observed when compared to the asymptomatic group. Experimental work began by validating a new pendulum friction simulator (Mk B) using THR bearings and hemiarthroplasties. One major modification and several minor modifications were required before the simulator could be used to develop the novel complete natural hip joint in-vitro simulation model, which was done successfully using porcine tissue. This model and the novel sample potting methodology developed alongside it can be used in future in-vitro tribological studies of the natural hip joint, and information gained from the MRI study can be used as geometric parameters for future in-vitro simulations.

The purpose of this study was to characterize metallurgical and tribological events occurring at the articulating surfaces of all metal implants in order to gain understanding of the wear characteristics of Co-Cr-Mo alloys. The surfaces of fifteen implant heads (or balls), made of either cast, low carbon wrought or high carbon wrought Co-Cr-Mo material, were examined using scanning electron and atomic force microscopes. In the fast part of the study, six of the implants were examined prior to simulator testing, three after 3 million cycles of testing at 3 times body weight, and six after 6 million cycles of testing, also at 3 times body weight. In the second part of the study, the six implants that were examined prior to testing in the first part, were progressively wear tested at 5 times body weight and were periodically examined until 2 million cycles of testing. Sachs, Aaron Dickinson. "The hip-hopsploitation film cycle: representing, articulating, and appropriating hip-hop culture." Diss., University of Iowa, 2009. https://ir.uiowa.edu/etd/591.

Full text
Abstract:
In this dissertation, I examine the articulation of hip-hop in the mid-1980s as it emerged onto the national stage of American popular culture. Using Articulation Theory, I weave together an argument explaining how and why hip-hop went from being articulated as a set of multicultural and inclusive practices, organized around breaking, graffiti, and DJing, to being articulated to a violent, misogynistic, and homophobic hyper-masculine representation of blackness as essentially rap music culture. In doing so I also argue that there are real political, social, racial, cultural, and ideological implications to this shift in articulation; that something is at stake in defining hip-hop as both black and rap music culture. I put forward this argument by making three distinct steps over the course of this dissertation. First, I identify a change in how hip-hop was represented and thus articulated in popular media. Through an intertextual analysis of the hip-hopsploitation genre films I show that early hip-hop was being represented primarily as a set of cultural practices cohering around breaking, graffiti, and DJing rather than the now dominant articulation as rap music culture. Next I set forth one possible reason for this shift within the limiting conditions set by the available media technologies and means of commodification. The visual nature of hip-hop's early articulation coupled with the economic inaccessibility of consumer home video made breaking and graffiti difficult to commodify compared to rapping as an aural element. Using "technological determinist" theorists like McLuhan, Innis, and Kittler, I argue that understanding how hip-hop as been historically constructed requires analyzing the limiting effect that the material conditions of media technologies have on the production of hip-hop. Finally, I offer a second, racial and cultural reason for this shift in articulation, and begin identifying some of the significance of this shift. A key aspect of the articulation of hip-hop as rap music is the further connection to blackness. This connection may function to maintain white patriarchal hegemony by displacing it on the black body via rap music: a complex dynamic of disidentification and appropriation.

Hines, M. "Hip-spine interaction in low back pain : the role of the hip extensors." Thesis, London South Bank University, 2017. http://researchopen.lsbu.ac.uk/2757/.

Full text
Abstract:
Despite the high prevalence and global burden of low back pain (LBP), the pathogenesis is poorly understood. Without a better understanding of what structures are involved in the development and chronicity of LBP, the value and efficacy of clinical assessments and physical therapy interventions are limited. Although there is a clear link between the lumbar spine, pelvis and hip extensors during movement in both LBP and healthy subjects, there is limited evidence regarding whether it is passive or active components that are influenced. There is a need for improved prognostic evaluation of patients with LBP, including whether altered hip biomechanics are the result of structural, passive elements, or neuromuscular, active components of movement. Such evaluations will be beneficial for researchers, clinicians and physical therapists. The purpose of the present investigation is initially to demonstrate how a handheld measuring device can be adapted for use in measuring passive hip moments during supine leg raising. Comparisons are then made between subjects with LBP and healthy controls. A validated dynamic biomechanical model is used to calculate passive hip moments at a variety of knee angles, from which a predictive equation is derived, which is specific to each subject. Following a gait analysis protocol, the predictive equation is used to calculate passive hip extensor moments during the hip flexion component of gait. Comparisons are made between passive hip extensor moments, total hip moments, power and work done, in subjects with and without LBP. The present investigation demonstrated the high accuracy of a handheld force transducer for the measurement of passive hip moments. There were no statistically significant differences in passive hip extensor biomechanical properties between subjects with LBP and healthy controls. However, assessment during walking demonstrated significant differences in passive hip extensor moments between subjects with LBP and controls. Further differences were identified in total hip moments, power and work done, despite no differences in gait parameters. It is plausible that the passive and active components of movement interact, although further research is required to determine whether such interactions are consistent and predictable. It was observed that the passive contribution to hip biomechanics during the swing phase of gait is considerable, and should be incorporated into dynamic modelling. Differentiating between passive and active components may be particularly useful for researchers, clinicians and physical therapists, for evaluating which components are influenced by LBP and for assessing the efficacy of component-specific interventions. Future research should expand on this research to include a wider range of LBP patients, with different severity and disability of LBP, to develop a more complete range of data on how passive and active components are influenced and the range of interactions during common movements. Other research should attempt to determine which interventions are most appropriate for targeting changes to passive and active components independently, and in accordance with patient adaptations to LBP. The modelling, experimental procedures and customised equipment used in the present investigation are appropriate for use in assessing passive contributions to joint biomechanics during movement.

Abstract:
In recent years the interest of companies in the use of recycled materials for the manufacture of their products has increased considerably. The main reasons for this are, firstly, the economic savings offered when compared to the progressive increase in the price of the raw material and secondly, the pressure for a company to implement an environmental policy, as part of its 'corporate social responsibility', in tune with a growing sensitivity of public opinion in this area. <br/>This thesis has focussed on studying the feasibility of the use of recycled thermoplastic materials for applications in the consumer electronics sector. In the first part, it centres on the feasibility of the substitution of the virgin materials High-Impact Polystyrene (HIPS) and the mix of polymers poly(ether phenylene) with High-Impact Polystyrene (PPE/HIPS), for the same materials of recycled origin, coming from industrial waste. In the second part, it studies the feasibility of substitution of the virgin materials studied in the first part, by a different recycled material. This recycled material being the pol (ethylene terephtalat) (PET) which comes from the selective collection of post consumption drink bottles. <br/>The research work has studied the variation of mechanical and thermal properties of the original recycled materials and of the mixtures of virgin and recycled material, the effects of the degradation provoked by the successive transformation processes and the influence of mineral fillers (fibres and glass beads) and halogen-free phosphor-based flame retardants, on the properties of the materials. <br/>The study, carried out on these three recycled materials, has allowed the following: analysis of the factors that intervene in the investigation of recycled materials; knowledge regarding the possibilities of application in the case of household electronics components and it has created the bases for being able to establish a methodology for carrying out future studies on the introduction of recycled materials in other technological applications.

Abstract:
This study examined the relationship between hip strength and hip, pelvis, and trunk kinematics in healthy runners. Whole body kinematic data were collected while subjects ran in the laboratory. Isometric hip abduction, flexion, external rotation, and internal rotation torques were measured bilaterally using a dynamometer. Subjects were divided into strong and weak groups for each muscle strength parameter. Differences in hip, pelvis, and trunk motion were then examined using independent sample t-tests. Pearson correlation coefficients were used to assess these relationships for all subjects. Most notably, runners with weak abductors displayed greater hip adduction and pelvic rotation compared to the strong abductor group, while runners with weak external rotators displayed greater trunk rotation compared to the strong external rotator group. Moderate, negative correlations were observed for the above relationships. While data from this study help clarify the relationship between hip strength and running kinematics, no causal conclusions can be made.

Abstract:
In their breakthrough work, Calegari and Geraghty have shown how to bypass some serious restrictions of the original method by Taylor-Wiles, thus allowing us to attack more general modularity conjectures and related questions. Their method hinges on two conjectures, one is related to the problem of attaching Galois representations to torsion classes in the cohomology of Shimura varieties and the other to the requirement that these cohomology groups, localised at an appropriate ideal are non zero only in a certain range. The first conjecture is addressed in a great generality by Peter Scholze, but the second remains elusive. Recently, for coherent cohomology, inspired by the classical Hida theory, Vincent Pilloni has proposed a method consisting of p-adically interpolating the entire complex of coherent sheaves of automorphic forms on the Siegel threefold. This serves as a way to get around the second conjecture above and plays a crucial role in a recent work, where they show that abelian surfaces over a totally real field are potentially modular. In this thesis, we adapt the argument of Pilloni to construct a Hida complex interpolating classes in higher cohomology groups of the Picard modular surface. In a future work, we hope to use this to obtain some similar modularity results for abelian three-folds arising as Jacobians of some Picard curves

Abstract:
Mestrado em Bioquímica<br>Bacterial resistance is a major issue in research related to environmental and public health, due to the ineffectiveness of antibiotics, resulting from a subpopulation of bacterial cells entering a dormant state. Multiple studies revealed that toxin-antitoxin systems are involved in such bacterial persistence. An example of such a toxin-antitoxin system is the HipAB complex, where HipA, and its cognate antitoxin are intimately related. HipB is very unstable and it is prone to degradation by proteases (e.g. Lon protease), leaving HipA exerting its toxic effect on cells. To study the conformations and interactions from HipAB in Shewanella oneidensis, it was necessary to overexpress and purify the proteins and to perform cross linking reactions which were analysed by mass spectrometry. The cross linking reactions were performed with BS3 (Bis[sulfosuccinimidyl] suberate), an in solution cross linker, tested at different concentrations (0.5mM, 1mM, 2mM) to find more efficient reaction conditions. By SDS-PAGE analysis it was possible to conclude that the best cross linker concentration is the highest tested. Furthermore, some predictions from possible intra- and intermolecular binding sites were made, unfortunately it was only possible to identify an interaction between HipA(143) and HipA(264).<br>A resistência bacteriana é um ponto importante de estudo em investigação relacionado com o ambiente e a saúde pública, devido à ineficiência dos antibióticos, que resulta de uma subpopulação de células bacterianas que entram num chamado estado dormente.

Abstract:
The evolution of humans to an upright stance has led to a change in the postural relationship between the human pelvis and the lower limb. However, despite the uncovering of the femoral head anteriorly, the bipedal hip remains inherently stable due to its bony anatomy and strong ligamentous support. Deviation from normal osseous anatomy results in a loss of constraint to dynamic movement with pathological translation of the femoral head (1,2). A similar scenario is expected with compromise of the surrounding soft tissue structures. A presentation of nine clinical cases describes the condition thought to result from compromise of the anterior hip ligaments. A triad of clinical signs are described. To investigate the patho-mechanical mechanism in these patients, a magnetic resonance imaging (MR.I) study of symptomatic hips was performed. Although increased joint , translation was not demonstrated in this study, distinctive appearances of the iliofemoral ligament were identified. Results of a cadaveric study then go on to demonstrate the stabilizing roles of the anterior capsular ligaments. With damage to the anterior capsule, increased translation of the femoral head was seen not only throughout movement range, but specifically correlated with the functional positions of the iliofemoral and pubofemoral ligaments. The concept of instability of the hip due to soft tissue laxity is postulated to result in progressive damage to the labrum and chondral surfaces as the femur abuts the acetabular rim during extra-physiological motion. Chronic capsular injury may destabilise previously asymptomatic hips with subsequent development of pain in young, active patients. Knowledge of the function of the capsule and its reinforcements is critical if hip surgeons are to manage these patients appropriately and understand the implications of capsular compromise on long-term function.

Yazdi, Far Mahshid. "Studies of hip impingement diagnosis." Thesis, Brunel University, 2014. http://bura.brunel.ac.uk/handle/2438/13868.

Full text
Abstract:
Hip impingement is a hip associated abnormality which develops among young and middle-aged individuals. It reduces the activity of those affected and if it is not detected at early stage, it can result in osteoarthritis. In this thesis a reliable framework for studying impingement detection is developed. Current clinical methods in detecting hip impingement involve measuring three angles, first the patient’s leg being flexed until maximum angle, second patient’s leg being flexed until 90° then adducted until maximum angle, and third patient’s leg being flexed until 90° afterwards internally rotated until the maximum angle also known as FADIR (Flexion, adduction in 90° flexion and internal rotation in 90° flexion) test. This is a manual method and relies heavily on surgeons experience and even pain tolerance of the patient and the method is prone to error. The use of computational programmes are known to be more accurate and reliable as the kinematic of contact can easily be studied using the digitised bones of the hip joint assuming that the impingement is determined by bone to bone contact kinematics. Current impingement studies assume that the kinematics of hip joint can be studied by assuming the centre of rotation is fixed for hip joint. For highly conforming joints this assumption is acceptable but for cases where conformity is poor the presence of soft tissue and soft tissue loading becomes very important. The important need in orthopaedics field is to develop a model without too much simplification. In this thesis for the first time the complete computational model of hip with soft tissue has been used to detect the impingement in a specific patient. The effect of centre of rotation and soft tissue are considered on impingement detection. In this study the femur, acetabulum, cartilage and ligaments of specific patients were modelled in MIMICs (Materialise' Interactive Medical Image Control System) using both MRI and CT scan. 3D hip models with and without soft tissues of normal hip, hip with impingement and hip with impingement after reshaping were modelled. The hip models were meshed in 3-Matic. The hip models were imported to Abaqus and boundary conditions were applied. Impingement zone and impingement angle was detected in Abaqus. Different centre of rotation was applied to consider the effect of centre of rotation to detect impingement. Experimental studies were set up to validate the hip models. Mocap, Wiimote, MotionNode and goniometer were used together at the same time to measure the flexion, adduction and internal rotation in 90⁰ of flexion in twenty two healthy volunteers. Validity and reliability of all of the methods were calculated. It is the first time that reliability and validity of Wiimote and MotionNode are considered to be used in medical application. Our results show that the model with soft tissue is closer to the experimental results. It shows that the soft tissue in hip model affects hip impingement angle and hip biomechanics. This finding also shows that, if the boundary condition is closer to the real hip, then the results of computer-aided program will be more reliable.
