Abstract:
Introduction: The majority of syncope patients transported to the emergency department (ED) by emergency medical services (EMS) are low-risk with very few suffering serious adverse events (SAE) within 30-days and over 50% are diagnosed with vasovagal syncope. These patients can potentially be diverted by EMS to alternate pathways of care (primary care or syncope clinic) if appropriately identified. We sought to identify high-risk factors associated with SAE within 30-days of ED disposition as a step towards developing an EMS clinical decision tool. Methods: We prospectively enrolled adult syncope patients who were transported to 5 academic EDs by EMS. We collected standardized variables at EMS presentation from history, clinical examination and investigations including ECG and ED disposition. We also collected concerning symptoms identified and EMS interventions. Adjudicated SAE included death, myocardial infarction, arrhythmia, structural heart disease, pulmonary embolism, hemorrhage and procedural interventions. Multivariable logistic regression was used for analysis. Results: 990 adult syncope patients (mean age 58.9 years, 54.9% females and 16.8% hospitalized) were enrolled with 137 (14.6%) patients suffering SAE within 30-days of ED disposition. Of 42 candidate predictors, we identified 5 predictors that were significantly associated with SAE on multivariable analysis: ECG abnormalities [OR=1.77; 95%CI 1.36-2.48] (non-sinus rhythm, high degree atrioventricular block, left bundle branch block, ST-T wave changes or Q waves), cardiac history [OR=2.87; 95%CI 1.86-4.41] (valvular or coronary heart disease, cardiomyopathy, congestive heart failure, arrhythmias or device insertions), EMS interventions or concerning symptoms [OR=4.88; 95%CI 3.13- 7.62], age >50 years [OR=3.18; 95%CI 1.68-6.02], any abnormal vital signs [OR=1.58; 95%CI 1.03-2.42] (any EMS systolic blood pressure >180 or <100 mmHg, heart rate <50 or >100/minute, respiratory rate >25/minute, oxygen saturation <91%). [C-statistic: 0.81; Hosmer Lemeshow p=0.30]. Conclusion: We identified high-risk factors that are associated with 30-day SAE among syncope patients transported to the ED by EMS. This will aid in the development of a clinical decision tool to identify low-risk patients for diversion to alternate pathways of care.

Introduction: Syncope accounts for 1% of all annual emergency department (ED) visits in Canada with only 10.3% suffering serious adverse event (SAE) within 30-days. However, 66% are transported to ED by Emergency Medical Services (EMS). Our objectives were to assess 30 day SAE among syncope patients transported by Emergency medical services (EMS), assess the need to develop an EMS clinical decision aid, and estimate anticipated health care savings by diverting patients from the ED to alternative care pathways. Methods: We conducted a prospective cohort study at four tertiary care EDs from Feb 2012 to Feb 2013. We included patients ≥16 years of age with syncope and who arrived to the ED via EMS. We collected patient demographics, medical history, 30 day SAE, EMS time points (call received, EMS arrival on scene, EMS departure from scene, time of transfer of care in the ED), critical EMS interventions, and ED length of stay. We assessed for the occurrence of any SAE (death, arrhythmia, other cardiac and non-cardiac conditions) within 30 days of ED disposition. We used descriptive analysis, unpaired two-tailed t-test and chi-square test. Ethics approval was obtained at all study sites. Results: Of 1,475 ED patients with syncope during the study period, 992 (67.3%) arrived by EMS. Mean times (SD) for EMS arrival to the scene, patient assessment at the scene and transfer of patient from scene to the ED were 10.1 (6.4), 18.9 (8.3), and 14.6 (11.5) minutes respectively. Only two patients had critical interventions enroute (pacing and defibrillation). Overall 138 (13.9%) patients suffered a SAE; 32 (3.2%) detected by EMS, 58 (5.8%) detected during ED evaluation, 48 (4.8%) after ED disposition. The average ED length of stay was 5.9(4.2) hours. Based on average of cost from two sites, we estimated that total cost of transporting syncope patients from the scene to the ED to be $4 million in Canada. The total cost of ED care for syncope patients transported by EMS in Canada was calculated at $21.5 million. Conclusion: A substantial proportion of patients arriving to the ED via EMS suffer no SAE within 30 days. Correspondingly, our results suggest a need for an EMS clinical decision aid to divert low-risk syncope patients to alternative care pathways such as family physicians or rapid access clinics. If developed and implemented, this tool can potentially reduce EMS burden, ED crowding, and reduce healthcare costs.

Abstract:
This paper presents a technique for automated curation of a domain-specific knowledge base or lexicon for resource-constrained domains, such as Emergency Medical Services (EMS) and its application to real-time concept extraction and cognitive assistance in emergency response. The EMS responders often verbalize critical information describing the situations at an incident scene, including patients' physical condition and medical history. Automated extraction of EMS protocol-specific concepts from responders' speech data can facilitate cognitive support through the selection and execution of the proper EMS protocols for patient treatment. Although this task is similar to the traditional NLP task of concept extraction, the underlying application domain poses major challenges, including low training resources availability (e.g., no existing EMS ontology, lexicon, or annotated EMS corpus) and domain mismatch. Hence, we develop EMSContExt, a weakly-supervised concept extraction approach for EMS concepts. It utilizes different knowledge bases and a semantic concept model based on a corpus of over 9400 EMS narratives for lexicon expansion. The expanded EMS lexicon is then used to automatically extract critical EMS protocol-specific concepts from real-time EMS speech narratives. Our experimental results show that EMSContExt achieves 0.85 recall and 0.82 F1-score for EMS concept extraction and significantly outperforms MetaMap, a state-of-the-art medical concept extraction tool. We also demonstrate the application of EMSContExt to EMS protocol selection and execution and real-time recommendation of protocol-specific interventions to the EMS responders. Here, EMSContExt outperforms MetaMap with a 6% increase and six times speedup in weighted recall and execution time, respectively.

AbstractIntroductionEmergency Medical Service (EMS) systems are vital in the identification, assessment, and treatment of trauma, stroke, myocardial infarction, and sepsis patients, improving early recognition, resuscitation, and transport. Emergency Medical Service personnel provide similar care for patients with syncope. The role of EMS in the management of patients with syncope has not been reported.Hypothesis/ObjectiveThe objective of this study was to describe the management of out-of-hospital syncope by prehospital providers in an urban EMS system.MethodsThis was a retrospective cohort study of consecutively enrolled patients over 18 years of age, over a two-year period, who presented by EMS with syncope, or near-syncope, to a tertiary care emergency department (ED). Demographics included comorbidities, history, and physical exam findings documented by prehospital providers, as well as the interventions provided. Data were collected from standardized patient care records for descriptive analysis.ResultsOf the 723 patients presenting with syncope to the ED, 284 (39.3%) were transported by EMS. Compared to non-EMS patients, those who arrived by ambulance were older (mean age 65 [SD = 18.5] years versus 61 [SD = 19.2] years; P = .019). There were no statistically significant differences in cardiovascular comorbidities (hypertension, coronary artery disease, diabetes mellitus, stroke, or congestive heart failure) between EMS and non-EMS patients. The most common chief complaints were fainting (50.0%) and dizziness (44.7%). The most common intervention provided was cardiac monitoring (55.6%), followed by administration of normal saline infusion (50.5%), oxygen (41.9%), blood glucose check (41.5%), and electrocardiogram (EKG; 40.5%).ConclusionEmergency Medical Service personnel transport more than one-third of patients presenting to the ED with syncope. Introduction. Data on emergency interventions in poisonings are scarce. Objective. To determine the effectiveness of antidote therapy in acute poisoning-related emergency medical services (EMS) interventions. Methods. A prospective observational study included all poisoning-related intervention cases over 3 years (1999-2001) in the Celje region, Slovenia, covering 125,000 inhabitants. Data were recorded on an EMS form. Results. Psychoactive agents were present in 56.5% out of 244 poisoning-related EMS interventions. Prescription drugs were a cause of intoxication in 93 (39.2%) cases alone or in combination with alcohol or illegal drugs. More than one fifth of poisonings were due to the use of illegal drugs in 52 (21.9%) cases, 43 (18.1%) out of them heroin related. At the time of EMS arrival, more patients who ingested illegal drugs were in coma or comatose than the rest. 24 (45.3%) vs. 32 (17.3%) of poisoned patients were in coma (p<0.001). Glasgow Coma Scale (GCS) at the first contact was lower in patients who ingested illegal drugs than in the remaining patients (9.0 vs. 11.6, p=0.001). In 23.2% of the cases, an antidote was administered. In 29 (12.2%) naloxone and in 16 (6.7%) flumazenil was administered. Mean GCS after intervention was higher in all cases but significantly higher in illegal drug cases, 13.4 vs. 12.2 (p=0.001), with a mean positive change in GCS of 4.5 vs. 0.6 (p<0.001). In illegal drug users, mean change after antidote administration was 8.2 vs. 0.5 without antidote administration (p<0.001). Conclusion. High rate of successful antidote use during the intervention indicated the importance of good EMS protocols and the presence of a skilled doctor in the EMS team.

Data on previous medical history, chief complaint, self-perceived illness severity, demographic information, and mode of arrival to the ED were collected on all subjects. Those who arrived to the ED via EMS also were asked reasons why they opted to call an ambulance for their illness/injury. Descriptive statistics were used to quantify survey responses, and multivariable regression was used to assess the independent effect of age on mode of ED arrival.ResultsData from 1,058 subjects were analyzed, 449 (42%) of whom arrived to the ED via EMS. Compared to adults<55 years, the unadjusted prevalence ratio for the association between age and EMS use was 1.18 (95% CI, 0.96-1.45) for subjects 55-79 years and 1.54 (95% CI, 1.18-2.02) for subjects ≥80 years. After adjustment for confounding variables, age remained a statistically significant risk factor for EMS use (P<.05).ConclusionOlder age is an independent risk factor for transportation to the ED via ambulance; however, this effect is attenuated by number of chronic medical conditions and history of depression. Abstract Polygenic mutations were induced by treating Drosophila melanogaster adult males with 2.5 mm EMS. The treated second chromosomes, along with untreated controls, were then made homozygous, and five life history, two behavioral, and two morphological traits were measured. EMS mutagenesis led to reduced performance for life history traits. Changes in means and increments in genetic variance were relatively much higher for life history than for morphological traits, implying large differences in mutational target size. Maximum likelihood was used to estimate mutation rates and parameters of distributions of mutation effects, but parameters were strongly confounded with one another. Several traits showed evidence of leptokurtic distributions of effects and mean effects smaller than a few percent of trait means. Distributions of effects for all traits were strongly asymmetrical, and most mutations were deleterious. Correlations between life history mutation effects were positive. Mutation parameters for one generation of spontaneous mutation were predicted by scaling parameter estimates from the EMS experiment, extrapolated to the whole genome. Predicted mutational coefficients of variation were in good agreement with published estimates. Predicted changes in means were up to 0.14% or 0.6% for life history traits, depending on the model of scaling assumed.

AbstractIntroductionDespite intense public awareness campaigns, many patients with ST-elevation myocardial infarction (STEMI) do not utilize Emergency Medical Services (EMS) transportation to the Emergency Department (ED). Predictors for mode of transport by EMS versus private vehicle in patients with an acute STEMI were investigated.HypothesisIt was hypothesized that patient characteristics, specifically older age, male sex, and a history of a prior cardiac intervention, would be associated with a higher likelihood of EMS utilization.MethodsA retrospective, observational cohort study was performed for all STEMI patients treated from April 1, 2007 through June 30, 2010 at an urban, academic ED with 24-hour cardiac catheterization available. Multivariable analyses with predetermined predictors (age, sex, prior cardiac intervention, weekend/evening arrival) were performed to investigate associations with mode of transport. Door-to-balloon (D2B) times were calculated.ResultsOf the 209 STEMI patients, 11 were excluded, leaving 198 for analysis. Median age was 60 years (IQR: 53-70), 138 (70%) arrived by private vehicle, and 60 (30%) by EMS. The primary analysis did not identify significant predictors for EMS, but a post-hoc model found that private insurance (OR 0.18; 95% CI, 0.07-0.45) was associated with fewer EMS transports. Although not statistically significant due to the great variability in time of arrival for STEMI patients transported by private vehicle, EMS transports had shorter D2B times. During business hours and weekend/evenings, EMS had D2B times of 50 (IQR: 42-61) and 58 minutes (IQR: 47-63), respectively, while private vehicle transports had median D2B times of 62 (IQR: 50-74) and 78 minutes (IQR: 66-106).ConclusionNo associations between mode of transport and patient age, sex, weekend/evening presentation and history of a prior cardiac intervention were identified. Privately insured patients were less likely to use EMS when experiencing a STEMI. Hypothesis: Assessments to rule out cervical spine injury performed by emergency medical services (EMS) personnel correlate well with assessments performed by emergency department (ED) physicians. Methods: EMS providers completed a data form based on their initial assessment of all immobilized adult patients. Data collected included the presence or absence of: neck pain/tenderness; altered mental status; history of loss of consciousness; drug/alcohol use; neurological deficit; and other painful/distracting injury. Immobilization was considered to be indicated if any one of the six physical findings was present. The ED physician caring for the patient completed an identical data form based on his/her assessment. Physicians and EMS providers were blinded to each other's assessments. The amount of discordance between the physician and EMS assessments was analyzed using McNemar's Chi-Square for matched pairs.Results: Five-hundred-seventy-three patients were included in the study. Physician and EMS assessments matched in 78.7% (n = 451) of the cases. In 13.6% (n = 78) of the cases, the EMS assessment indicated immobilization, but the physician assessment did not. In 7.7% (n = 44) of the patients, the physician assessment indicated immobilization, but the EMS assessment did not. The discordance between assessments was statistically significant (p <0.001). The presence of neck pain or tenderness accounted for the most discordance.

Abstract:
This thesis is about the spa town of Bad Ems in West Germany - its social and economic development. It analyses the town's rise to fame as a fashionable centre for relaxation and recuperation and the emergence of a 'spa culture' in the nineteenth century. It also studies the impact of the gradual 'democratisation' of cures i.e. how spa towns like Bad Ems changed in this century with the increase in the number of cure-guests funded by the statutory insurance bodies. This inevitably involves an examination of the system of national health provision from the late 19th century and the incorporation of spa treatment into benefit schemes. The subsequent analysis of medical knowledge and opinion, with particular reference to spa remedies and treatment considers both medical practitioners in Bad Ems and the development of the science of balneology over the past one hundred and fifty years. This analysis includes the debates and arguments about the modern cure and the growing concern since the Second World War with the efficiency and effectiveness of social insurance cures. Finally, this study looks at the cure-takers themselves, both in their relationship with the medical profession and their experience of spa life. Because no comprehensive study of Germany's spas has been attempted, this thesis aims to bring together different perspectives adopted by various disciplines. However, given the present state of research, it seemed that the only viable approach would be through a case study which analyses the town of Bad Ems at a grass-root level, though without ignoring the impact of national events and policies in Germany on cure-taking and spa culture. The findings of the research indicate that the introduction of cures as a benefit of national welfare policies ensured the survival of spas as health centres. No less importantly, today a cure is no longer the preserve of a wealthy elite as in the 19th century but available to all Germans. The success of cures in Germany today would also seem to reflect a culturally specific attitude to health and illness which stands in marked contrast to that in this country where spas have declined and where there is little interest in the forms of treatment offered by mineral springs and thermal waters.

The electronic music studio ”Elektronmusikstudion” (EMS) inStockholm was initiated in 1963. It has had several different managers throughout the years. EMS soon became a rather costly project. Furthermore there have been several conflicts among staff and users wich has contributed to EMS fairly turbulent history. One main conflict concerns the issue wether EMS should be a research facility or a studio for composers. The many different managers of EMS have contributed to a rather unstructured filing of records. To facilitate the understanding of the documents in this archive, and because there is very little written about EMS history, I’ve included a rather comprehensive description of EMS organizational history.

The subject of this one year master’s thesis is to describe and examine difficulties I’ve encountered in the archival process. My main issues comprise the organization and separation of some of the documents, especially those concerning the conception and organization of EMS and also the abundant technical documentation. Also the question of office of origin has been a concern.

Non-suicidal self-injury (NSSI) has been regarded as a dysfunctional coping strategy for managing intensely difficult feelings and is a growing area of concern in clinical and non-clinical populations alike. Individuals who engage in NSSI often report significant interpersonal difficulties, with studies showing that they have impaired social interactions. Attending to the emotional expressions of others is important for appropriate social functioning, and subtly mimicking the expressions of others is an unconscious behaviour that encourages empathy and interpersonal bonding. Differences in emotion recognition and reflection can impact social behaviour, yet little research has assessed how individuals with a history of NSSI (HNSSI) process facial expressions of emotion. In this thesis, the results of three studies designed to investigate potential differences in emotion recognition and emotion mimicry in individuals with a history of NSSI compared to controls are reported. Results from Study 1 revealed that when presented with colourful, dynamic morphing stimuli showing emotional expressions, HNSSI participants were able to correctly categorize negative expressions like anger, disgust, sadness, and the ambiguous emotion of surprise at a lower stimulus intensity compared to controls; They also correctly categorized fear with greater accuracy, though at the same intensity as controls. However, in Study 2, when static, greyscale images were obscured with varying levels of fractal noise, HNSSI individuals did not show superior signal-proportion thresholds. These results may suggest that higher-order elements of visual and cognitive processing are evoked by more realistic social stimuli. In the third study, HNSSI participants were found to produce significantly less electrical activity in the corrugator muscle in response to viewing angry stimuli, and significantly less of an expected relaxation in muscle activity in response to viewing happy stimuli. Complementing these results, it was found that endorsing social influence as a motivator for engaging in NSSI was associated with less mimicry, whereas endorsing emotion regulation as a motivator was associated with greater incongruent muscle response when viewing happy faces. These observed differences in facial mimicry between HNSSI and controls may be related to some of the observed relationship difficulties experienced by this group. Results from this research may help us to better understand NSSI behaviour, as well as help to inform and further develop therapies intended to address it.

La recerca s’ha dut a terme a partir de la informació continguda als llibres de notes que, sobres les viles d’Alcover, la Selva del Camp i Valls, es conserven a l’Arxiu Històric Arxidiocesà de Tarragona. Es tracta de tres pobles de caràcter marcadament agrari i integrats dins de la Senyoria de la Mitra de Tarragona i amb una sèrie de particularitats que permeten la realització d’un estudi comparat que supera l’àmbit estrictament local per endinsar-se en l’àmbit de la història agrària regional. Amb tot, les característiques de les fonts documentals fan que aquest sigui un treball d’evolució curta en el temps que es centra en els dos primers decennis de la catorzena centúria. D’aquesta manera, s’ha prioritzat la intensitat a la prolongació cronològica atès el gran volum de petits assentaments que han nodrit la investigació i que han hagut de ser analitzats tant des del punt de vista econòmic, com social o fins i tot dins de l’àmbit fiscal o de les estructures de govern senyorial o municipal. Justament aquesta diversitat documental ha influït clarament en la forma final de la tesi, la qual s’ha organitzat en diferents apartats amb l’objectiu de dotar el treball final d’una estructura homogènia. Aquests apartats són: el marc espacial, la petita explotació camperola, aspectes jurídics sobre les relacions econòmiques que s’estableixen al mercat de la terra, aspectes econòmics i socials sobre les transaccions al mercat de la terra, la vila, les rendes senyorials i el desenvolupament municipal, i la societat. En conclusió, aquest és un treball que ha establert tota una sèrie de línies de recerca que poden continuar sent treballades en un futur amb el buidat l’anàlisi d’aquestes sèries documentals que, en línies generals, a mesura que s’avança cronològicament, són més complertes tant des del punt de vista quantitatiu com qualitatiu.
This research wants to reconstruct the quotidian lives of the villagers of ‘El Camp de Tarragona’ district during the early years of the fourteenth century. The research was undertaken on the information contained in the notarial books preserved in the Archdiocesan Historical Archive of Tarragona about the villages of Alcover, La Selva del Camp I Valls. These three municipalities, integrated into the Lordship of the Archbishop of Tarragona, have a prominently agrarian nature and a series of features that allow to conduct a comparative study that venture into the field of the regional history. However, this is a work just focused on the first two decades of the 14th century due to the special characteristics of the documentary sources. Thus, given the large number of small contracts, it has been prioritized intensity rather than chronological extension. These documents have been analyzed from the point of view of economy, society, taxation system o governmental structures. Precisely this diversity has clearly influenced the final form of the thesis, which has been organized into different sections in order to provide it with a homogeneous structure. These sections are: the spatial context, the small peasant farming, legal aspects of economic relations established in the land market, economic and social aspects of transactions in the land market, the town, the lordly incomes, the municipal development, and society. To sum up, this is a job that has established a series of research areas that can still be studied in the future by emptying and analyzing these notarial volumes which, in general, contain better information, both quantitative and qualitative, as the researcher advances through the time.

This work studies how the class of hisendats (landowners who lived from their rents), the economic and social ruling class in the Girona society of the eighteenth century, also became a leading class in the time of the liberal revolution and the implementation of the liberal state. A case study has been carried out, that of the Carles family’s heritage, one of the most important in the province of Girona, as a way to focus on this social group in different aspects: family relationships, the economic and social basis of its power and its public performance in the sociability media and in local politics. The main source of information has been the patrimonial archive of the Carles family, combined with parochial, notarial, municipal and provincial sources. The area of study is the region of Girona, in the northeast of Catalonia (Spain), during the first half of the nineteenth century.
Aquest treball estudia com els hisendats, o grans propietaris rendistes, de classe dominant econòmica i social en la societat gironina del segle XVIII, esdevingueren també classe dirigent en el marc de la conformació del nou règim liberal. S’ha realitzat un estudi de cas, l’anàlisi del patrimoni Carles, un dels més importants de la província de Girona al segle XIX, com una manera de focalitzar la visió sobre aquest grup social en els seus diversos àmbits d’actuació: les relacions familiars, la base econòmica i social del seu poder i la seva actuació pública en els medis de sociabilitat i la política local. La font principal del treball ha estat l’arxiu patrimonial de la família Carles, que ha estat complementat amb altres fons (parroquials, notarials, municipals i provincials). Abstract:
The subject of this dissertation is the confessional revivalist organisation Evangeliska Fosterlands-Stiftelsen (EFS – approximately the Swedish Evangelical Mission Society) between 1856 and 1910. EFS was founded in 1856 in a Pietistic tradition, and its aim was to revitalise from within what was seen as a too dormant State church, and also to counteract the influence of the emerging free churches. The study has five main sections. The introductory part consists of the theoretical framework and the historical context. In the second chapter EFS’ aims and expectations are studied on a national level. The third chapter examines the content of published and distributed tracts. The fourth chapter focuses on the activities of the itinerant colporteurs, and the fifth and final part studies the work of a local EFS-congregation. The aim of the dissertation is twofold. The first objective is to describe the transformation EFS underwent during the period studied. This process is described in terms related to Jürgen Habermas’ expression “public sphere”. When EFS was founded, as a board, it can be seen as one of other middle- and upper-class associations, and as such a part of the Swedish public sphere. By 1910 EFS had begun to move away from the Swedish State church and had become more like contemporary popular movements and free churches – it had started to take the shape of an limited alternative sphere, a denomination. The second aim is to use EFS as an example to describe and analyse the changed perception of religion during the second half of the 19th century. This change is described as a process of secularisation. Secularisation is seen here as the process that turned religion into an individual, voluntary and optional act of faith, among other religious and non-religious alternatives, for ordinary people. Of particular interest is the paradoxical relation between revivalism and secularisation. Various revivalist movements emphasised the personal relation to God and the individual right to interpret the word of God. The individual choice for salvation was also stressed within revivalism. These movements also created new alternatives to the all-embracing State church. Thus both the position of the Church, and the universal claims of Christianity in general, were undermined. The transformation EFS underwent is seen as an adaptation to the rise of modern society, which became more pluralistic and hence competitive during the final decades of the 19th century. This development meant that new strategies were required for religious organisations overall, in order for them to be able to compete and flourish.

Abstract The first chapter of this edited volume traces the history of documenting spiritual care. By referring to ancient and early modern practices, the relationship between spiritual (self-) care and various forms of documentation is outlined. The focus lies on developments in the twentieth and the twenty-first century, although the question of what constitutes an adequate practice of documenting healthcare chaplaincy is as old as the profession itself. The pioneers of Clinical Pastoral Education (CPE) – Richard Cabot, Anton Boisen, Russell L. Dicks, and others – developed distinctive forms of recording for different purposes. For example, procedures of documentation that are prepared for and helpful to the pastors themselves as “self-criticism,” “self-improvement,” or even “self-revelation” have been distinguished from documentation practices that are intra- or interprofessional. Regarding more recent developments in documentation, the introduction of electronic patient records (EMRs) was critical. We present a case study from Kenya to show how the issues discussed here are encountered in a non-Euro-American context.

Abstract:
In order to establish a quantitative detection method for appearance in epileptic discharges (EDs), we propose using the model parameters in a Duffing oscillator, which is a nonlinear mathematical model. Extracting four frequency bands of delta, theta, alpha and beta waves from the time history of the electrocorticogram (ECoG) obtained from rats with induced EDs, we applied a sweep window to the time history for each band. So as to fit the equation for the Duffing oscillator to the time history of the ECoG, we used the least square method to determine the model parameters expressing characteristics of ECoG. The Duffing oscillator has three kinds of vibrational parameters and four kinds of parameters about the amplitude for the driving force with two predominant frequencies contained in ECoG. In order to examine the appearance time of the EDs and the change of ECoG characteristics, we determined the model parameters for each sweep window. When epilepsy occurs, we found that the amount of the parameters related to “conservation”, “dissipation” and “input quantities” increases. On the other hand, the parameter value corresponding to nonlinearity tends to decrease. It is found that the proposed method by the model parameters of the Duffing oscillator can be used in quantitative detection for EDs.

En el proyecto “Diseño y construcción de un documento HL7 CDA para historia clínica colombiana”, se plantea el desarrollo de un simulador basado en software para ser usado en la formación de estudiantes de la escuela de ciencias de la salud de la Universidad Nacional Abierta y a Distancia. El propósito del software es poder brindar a los estudiantes una experiencia mucho más completa que la actual, a través del diligenciamiento del documento epicrisis que incluye aspectos legislativos, conjuntos estandarizados de datos y el uso de estándares como Loinc y HL7. En adelante se procede a explicar el proceso de desarrollo del simulador y su respectiva justificación de acuerdo con las necesidades del proyecto.

Abstract:
This study was conducted to inventory, classify, and map soils and vegetation within the ecosystems of Katmai National Park and Preserve (KATM) using an ecological land survey (ELS) approach. The ecosystem classes identified in the ELS effort were mapped across the park, using an archive of Geo-graphic Information System (GIS) and Remote Sensing (RS) datasets pertaining to land cover, topography, surficial geology, and glacial history. The description and mapping of the landform-vegetation-soil relationships identified in the ELS work provides tools to support the design and implementation of future field- and RS-based studies, facilitates further analysis and contextualization of existing data, and will help inform natural resource management decisions. We collected information on the geomorphic, topographic, hydrologic, pedologic, and vegetation characteristics of ecosystems using a dataset of 724 field plots, of which 407 were sampled by ABR, Inc.—Environmental Research and Services (ABR) staff in 2016–2017, and 317 were from existing, ancillary datasets. ABR field plots were located along transects that were selected using a gradient-direct sampling scheme (Austin and Heligers 1989) to collect data for the range of ecological conditions present within KATM, and to provide the data needed to interpret ecosystem and soils development. The field plot dataset encompassed all of the major environmental gradients and landscape histories present in KATM. Individual state-factors (e.g., soil pH, slope aspect) and other ecosystem components (e.g., geomorphic unit, vegetation species composition and structure) were measured or categorized using standard classification systems developed for Alaska. We described and analyzed the hierarchical relationships among the ecosystem components to classify 92 Plot Ecotypes (local-scale ecosystems) that best partitioned the variation in soils, vegetation, and disturbance properties observed at the field plots. From the 92 Plot Ecotypes, we developed classifications of Map Ecotypes and Disturbance Landscapes that could be mapped across the park. Additionally, using an existing surficial geology map for KATM, we developed a map of Generalized Soil Texture by aggregating similar surficial geology classes into a reduced set of classes representing the predominant soil textures in each. We then intersected the Ecotype map with the General-ized Soil Texture Map in a GIS and aggregated combinations of Map Ecotypes with similar soils to derive and map Soil Landscapes and Soil Great Groups. The classification of Great Groups captures information on the soil as a whole, as opposed to the subgroup classification which focuses on the properties of specific horizons (Soil Survey Staff 1999). Of the 724 plots included in the Ecotype analysis, sufficient soils data for classifying soil subgroups was available for 467 plots. Soils from 8 orders of soil taxonomy were encountered during the field sampling: Alfisols (<1% of the mapped area), Andisols (3%), Entisols (45%), Gelisols (<1%), Histosols (12%), Inceptisols (22%), Mollisols (<1%), and Spodosols (16%). Within these 8 Soil Orders, field plots corresponded to a total of 74 Soil Subgroups, the most common of which were Typic Cryaquents, Typic Cryorthents, Histic Cryaquepts, Vitrandic Cryorthents, and Typic Cryofluvents.

En este número del Boletín de Noticias (V3N1) se muestra la agenda 2021 de la asociación (Alconpat Internacional), la cual está diseñada que para que puedan ir planeando a que eventos asistir; se hace un homenaje póstumo a dos miembros que fallecieron recientemente (Néstor Ortega de Alconpat Argentina y Luis García Andión de Alconpat España); y se muestran con detalle los eventos del Congreso CONPAT 2021, del Senior Webinars, de la 75ª Semana de Rilem y del próximo curso: “Evaluación y diagnóstico de problemas patológicos en estructuras de concreto armado”, que iniciará el 13 de abril, este último forma parte del proceso de educación continua y certificación de Alconpat Internacional. Así mismo, se encuentra una pequeña nota por el premio que recibió uno de nuestros miembros (Miguel Ángel Sanjuán Barbudo), y que además es el Jefe del Boletín Técnico de la asociación, premio recibido por la revista Materials en los “Materials 2020 Outstanding Reviewer Awards”. Por último, este año la Revista Alconpat celebra su 10 aniversario y por ello, el editor en jefe de la Revista nos hace una reseña de los 10 años de su historia (2011-2021).

Abstract:
El objetivo de la presente investigación es Interpretar las vivencias de la discapacidad, a partir de la narrativa de estudiantes universitarios de la modalidad virtual del departamento de Boyacá. Esta investigación se inscribe en el paradigma de naturaleza cualitativa, enfoque hermenéutico. Se seleccionarán a los participantes por bola de nieve o en cadena hasta la saturación teórica. Se recolectará la información por medio de una entrevista a profundidad, no estructurada basada en un derrotero temático afín a las categorías de análisis. Cada entrevista será grabada, transcrita y analizada. Posteriormente se realizará la codificación y categorización, para estructurar la historia de vida de cada participante. Este análisis e interpretación de los datos, requiere de un proceso de categorización y triangulación de los mismos, la comparación constante en cada uno de los pasos, y la ejecución de un análisis de contenido y un análisis visual. Así mismo se deberá elaborar matrices, que permitan la interpretación de la información, para finalmente comunicar los resultados obtenidos. Se realizará la comunicación de los resultados en primera medida a los participantes de la investigación quienes validarán la información y en segunda medida se realizará la comunicación relativa a los informes de corte académico.

Es aquí donde nos proponemos hacer una pausa para revisitar el camino recorrido y reunir las experiencias que al paso del tiempo hemos atesorado como extraordinarias. Estás por recorrer un proyecto editorial colaborativo que celebra los primeros diez años de la Unidad de Vinculación Artística (UVA). Diez años que se expresan en 10 fanzines o números donde cada uno representa una arista desde donde mirar esta primera década. Ubicada en el corazón de Tlatelolco, la UVA ha acompañado en sus procesos de iniciación, formación y creación artística a más de 10,000 personas. A través de nuestros talleres libres y nuestra adorada biblioteca comunitaria “Alaíde Foppa” hemos sido cómplices de niñas, niños, jóvenes, adultos y personas mayores. Para ellxs, y para los que están por venir, nuestro total agradecimiento. Doce talleristas —artistas de diversas disciplinas, interesados en la educación e investigación a través del arte— acudieron al llamado para dimensionar y proyectar nuestra historia. A través de sus voces se hacen evidentes los pilares de nuestra comunidad: la educación no formal, el reconocimiento del otro como vértice de la experiencia educativa y por último, la integración de la carga cultural que representa el barrio de Tlatelolco: nuestro territorio. Abstract:
Venezuela experimenta la recesión económica más severa y prolongada de su historia económica moderna. El país ha perdido tres cuartas partes de su Producto Interno Bruto, produciendo, como consecuencia, una degradación extrema de las condiciones materiales de vida de sus habitantes. En este contexto, no existe una evaluación apropiada de los efectos que esta profunda crisis económica ha tenido sobre los indicadores socioeconómicos de las clases medias en Venezuela. Este documento técnico hace uso de las fuentes de microdatos disponibles para analizar empíricamente el tamaño y la evolución reciente de la clase media venezolana a la luz de la reciente crisis económica. En líneas generales, se corrobora una caída abrupta y sistemática tanto en el tamaño de la clase media venezolana, así como del poder de consumo de quienes todavía forman parte de ella. En 2020 se verifica que aproximadamente 9 de cada 10 familias que era considerada de clase media a principios de la década pasada, ya no lo es. Además, el análisis dinámico de los datos muestra evidencia de que una porción muy importante de familias de clase media ha perdido su estatus sobretodo en los últimos años, tendencia comprobada por la evolución indicadores no monetarios, relacionados con los niveles multidimensionales de bienestar y seguridad económica de las familias de estratos medios. Venezuela llegó a ser el hogar de la más grande, sólida y próspera clase media de la región, hoy en día, la situación ha cambiado radicalmente: una parte importante de el acervo de capacidades y capital humano de la clase media ha alimentado la ola de emigración de venezolanos de los últimos años, mientras la otra parte, la poción que permaneció en Venezuela, sufre la erosión de sus condiciones en el marco de la depresión económica y la crisis humanitaria. En general, existe amplio consenso sobre el efecto positivo que tiene la clase media sobre el bienestar, el crecimiento económico, la equidad y la estabilidad de las economías, en este sentido, la rehabilitación de la clase media venezolana deberá formar parte de cualquier programa de recuperación futura.

Abstract:
La temporada de huracanes de 2020 fue la más activa de la historia de la región, registró un total de 30 tormentas, lo cual fue más del doble del promedio anual. De las 30 tormentas, 13 fueron huracanes y 6 huracanes de mayor grado. El huracán Eta se formó el sábado 31 de octubre. El 2 de noviembre se declaró Estado de Emergencia en los departamentos de Islas de la Bahía, Cortés, Atlántida, Yoro, Colón, Olancho, Gracias a Dios, Comayagua, Francisco Morazán, el Paraíso. El 6 de noviembre se modificó este decreto y se procedió a declarar Estado de Emergencia en todo el territorio nacional, debido a los efectos de las lluvias provocadas por la Tormenta Tropical Eta. El Estado de Emergencia estaría vigente hasta el 31 de diciembre de 2020, pudiendo prorrogarse si los efectos que dieron origen a la emergencia persistiesen. El huracán Eta se disipó el 13 de noviembre, caracterizado por un comportamiento errático, presentando numerosas variaciones en su intensidad, se estimó que dejó un total de 380 a 635 mm en casi todo el territorio hondureño. Por su parte, el huracán Iota se formó el viernes 13 de noviembre. El 18 de noviembre se modificó el Decreto Ejecutivo Número PCM-109-202 y algunos artículos del decreto PCM-113-2020, para ampliar las acciones descritas allí a “otros fenómenos climáticos Iota que ocasionaran daño a la infraestructura productiva del país a nivel nacional”. Este huracán se disipó el 18 de noviembre sobre El Salvador. Este evento, durante su formación, fue catalogado bajo las categorías 4 y 5 de huracán, pero se debilitó rápidamente al tocar tierra firme. A pesar de ello, se estimó que dejaría una precipitación total de 500 a 750 mm en la parte norte del país. Los departamentos más afectados por ambos huracanes fueron Atlántida, Cortés, Santa Bárbara y Yoro. Se estima que los efectos totales causados por la tormenta tropical Eta y el huracán Iota fueron de aproximadamente L. 52 099 millones (cuadro 1). Los daños representaron el 44% de la afectación, las pérdidas 52% y los costos adicionales 4%. Nótese que el sector privado sufrió 69% de la afectación de acervo y 97% de los flujos de producción perdidos. Los costos adicionales es la única dimensión de los efectos en la que el sector público superó al privado. Los efectos totales que tuvo el desastre en el sector público fueron de aproximadamente L. 9 049 millones.
