Abstract:
Mental health professionals need to understand how the psychosocial and mental health needs of HIV-infected youth evolve over time and to be able to identify salient clinical challenges that present with each developmental stage. It is also important to understand that HIV/AIDS affects children’s lives indirectly, by the presence of HIV/AIDS in a family member, even if the child is not HIV infected. This chapter uses a developmental perspective to introduce key mental health objectives in the lives of developing HIV-infected children and adolescents and provides an overview of epidemiological, psychosocial, and clinical parameters to be considered in their clinical care and management. The chapter also addresses issues facing perinatally and behaviorally HIV-infected children and adolescents. Separate sections of the chapter discuss biopsychosocial factors salient to children and adolescents who are affected by HIV infection in the family.

Abstract:
HIV-infected infants and children have a different, more progressive disease course compared to that of adults given that early infection leads to sustained, high-magnitude viremia with significant seeding of reservoirs in the first months of life. Early diagnosis of HIV infection is pivotal in the management of infants and prevention of HIV-associated morbidity and mortality. The availability of potent pediatric antiretroviral formulations encompassing different classes of drugs for infected infants and young children is limited. Significant advancements have been achieved in the area of infant post-exposure prophylaxis. Early antiretroviral treatment is still the mainstay of pediatric HIV infection, particularly for infants younger than age 12 months, but it is also highly recommended for older children. Early treatment of young infants diagnosed soon after birth appears to be the best approach to reduce the seeding of viral reservoirs and potentially attain prolonged periods of HIV remission off antiretrovirals.

An estimated 185 million individuals have been infected with hepatitis C virus (HCV) worldwide. Although often clinically silent for decades, chronic HCV infection predisposes to late-onset complications, including liver cirrhosis and hepatocellular carcinoma. Mother-to-child transmission (MTCT) of HCV affects approximately 5% of children born to viremic mothers and is the primary route of HCV infection in young children. While some vertically acquired HCV infections are resolved during the first years of life, many persist indefinitely. Chronically infected children tend to be asymptomatic and have mild liver disease, but they face a risk of progression to advanced liver disease in adulthood. Current diagnostic and management strategies leave most infected children undiagnosed and untreated. Widespread use of newly-available direct-acting antiviral therapies has the potential to substantially reduce the global burden of HCV, including vertically acquired HCV, but an effective vaccine likely will be required to achieve this ultimate goal.

Abstract:
Lymphocytic interstitial pneumonitis is a common respiratory complication of HIV infection in children but a rare complication in HIV-infected adults. The diagnosis requires histologic confirmation by biopsy. Antiretroviral therapy has been used with success for treatment. Nonspecific interstitial pneumonitis (NSIP) presents with dyspnea, nonproductive cough, and fever in patients with CD4+ T cell counts >200 cells/mm3. The diagnosis of NSIP requires histologic confirmation by biopsy. The optimum treatment remains unclear. The prevalence of pulmonary arterial hypertension (PAH) is higher in HIV-infected patients compared to the general population. Potential therapies for HIV-associated PAH include HIV antiretroviral therapy as well as supportive therapy, including oxygen, diuretics, and anticoagulation.

Advances in HIV medical care and antiretroviral therapy transformed AIDS from a rapidly devastating fatal illness into a chronic illness for persons with access to care, leading to vast changes in the health of individuals, couples, their children, extended families, and social networks. In addition, adherence to antiretroviral therapy and viral suppression have reduced the likelihood of transmission of HIV, and the use of pre-exposure prophylaxis (PrEP) in an HIV-negative partner offers an additional option to prevent seroconversion. Significant biopsychosocial challenges remain, however, for couples who are dissimilar (serodiscordant) in HIV serological status and young adults with HIV. Many young adults and serodiscordant couples who are engaged in care and virally suppressed need support as they plan to have children or re-enter careers and social networks. There are few studies of couples with similar (seroconcordant) or serodiscordant HIV serological status. This chapter focuses on the impact of HIV on serodiscordant couples in which only one member is infected. After reviewing some of the literature, clinical issues that emerge in evaluating and treating couples are presented.
