Full textHodgkin’s Lymphoma (HL) is one of the most frequent lymphomas occurring in childhood. In young children, there is a high predominance in boys and frequent association with Epstein-Barr Virus (EBV). Cohort studies have shown that patients affected by several immune deficiency syndromes - e.g. X-linked lymphoproliferative syndrome (XLP), functional deficit of Fas/FasL pathway and common variable immunodeficiency (CVID) - are risk factors of HL. We intend to search for qualitative and quantitative immune deficiencies as susceptibility factors to child's HL in a prospective study related to Euronet –PHL C1 protocol. Eighty-three patients at diagnosis of HL have been analysed. Median age of the study population is 13 years, (5-18 years). Gender-ratio M/F is 1.1 with a larger male predominance before the age of 10 (gender-ratio of 3). The search for a defect of NKT population that would be suggesting of XLP was negative in all patients. A moderate expansion of circulating TCRαβ+ double negative cells (DNT) has been detected in 5 patients. This expansion has been further explored in the hypothesis of a defect of Fas/FasL pathway by plasmatic quantification of Fas ligand and Il-10. This led to the exclusionof the diagnosis of ALPS. An unexpected high frequency of B-cell lymphopenia has been detected in 31 out of 83 patients (37%). Peripheral B cell lymphopenia was associated with the following poor prognostic factors: advanced stages (p<0.04), low hemoglobin (p<0.06) and B symptoms (p<0.01). B-cell lymphopenia was not statistically correlated with morphology (subtype, amount of tumor cells and necrosis). Remarkably, B-lymphocytic counts were significantly higher in patients with in situ EBV (<0.05).Only a B lymphopenia with low IgG level suggesting DICV was detected. We extended the analysis to all the 395 patients included in the protocol EURONET, so we identified 4 patients with CVID. These cases will be further explored by molecular analyses. In parallel, the specific T-cells response against EBV was studied by flow cytometry in 15 patients and ELISPOT assay in 9 patients with HL. Flow cytometry , suggested a decrease in production of IL-2 by CD4 T cells in patients with high EBV viral load in response to EBV latent and lytic-cycle peptides and autologous lymphoblatoid cells lines compared to controls or patients with LH-EBV-. The ELISPOT-IFNγ assay was used to determine the frequency of T cells that produced IFNγ in response to peptides. One patient demonstrated inappropriate EBV-specific T-cell IFNγ production (<10 IFNγ secreted T cells and >1,000 EBV copies per 250000 PBMCs). These cases will be further explored by molecular analyses.Our findings confirm the known epidemiological data of HL now mainly associated to NS subtype in children and adolescents and EBV status in HL at this age. We show that peripheral B cell lymphopenia in paediatric and adolescent HL patients is frequent and associated with poor prognosis factors. We confirm the association between CVID and HL

Full textThe progression of treatment in lymphoma leads to an interest in the quality of life ofsurvivors. Chronic fatigue is the most frequently described problem among survivors. Inhodgkin lymphoma, the level of fatigue is of particular importance to patients between20 and 40 years of age and faces problems of return to employment. In the first part of thisthesis, we will deal with the causes of fatigue and are evolutionary for hodgkin lymphomaand non-hodgkin lymphoma. In particular, we will highlight the impact of comorbiditieson fatigue levels as well as differences in fatigue patterns between survivors of hodgkinlymphoma and non-hodgkinian lymphoma. In Part Two, we will deal with the integrationof fatigue into the assessment of treatment in hodgkin lymphoma with low-inclusion andhigh survival problems

Full textIntroduction and objectives: Living with a child suffering from a chronic health condition may influence the psychological health of each family member in different way. Daily life is organized around the special care of the child affected. In the specific case of severe hemophilia, the treatment give rhythm to family life and the risk of hemorrhagic accident is at the heart of the parental concern. This could potentially impact on the psychological health of their siblings. The specificity of severe hemophilia as the transmission mode, intravenous treatment and high hemorrhagic risks raises question about question about their influence on the psychological health of siblings in terms of quality of life and psychopathological disorder. Methods: A comparative study has been conducted between a clinical group composed of 55 children aged between 8 and 17 years old and having a brother suffering from severe hemophilia and a control group composed of 55 children paired. Auto and hetero questionnaires have been administrated to evaluate the psychological health of the siblings and mediating variables. Non directive interviews have been conducted on parents. Results and conclusion: Siblings of children suffering from severe hemophilia have an altered psychological health compared to children from the general population with unique psychopathological disorders. Taking into account the traumatic effect of the sibling’s experiences appears necessary to elaborate a therapeutic device adapted to their needs. Specific variables related to the child and his parents are mediating this traumatic effect but a further study needs to be done to understand these results

Full textThis research project attempts to demonstrate that it is not so much the objective situation (here, the nature of the sickness, suffering from cancer or insulin-dependent diabetes) that influences the strategies adopted by the subjects as the representation they make of it. Relating different levels of analysis (socio-biological factors, psychological resources, representation of the sickness, transient states and personalisation strategies) brings in new considerations allowing the reactions and actions of adolescents suffering from a serious sickness to be better understood. The general hypothesis postulates the implementation of coping strategies and the subject's reactions to his or her illness (anxiety state, difficulties forming projects and changes in perspectives for the future) do not mainly depend on the nature of the sickness but on the representation the patient makes of it together with sociobiographical (age, sex and the parents' socio-professional level) and psychological (anxiety trait, self-esteem, self future and style of project) variables. The level of anxiety is mainly related to the meaning the sickness assumes for the patient but also to the nature of the sickness itself and the style of the subject's project. The subject's difficulties in devising projects and future perspectives are not determined by the sickness itself but are influenced by the representation he or she makes of it, associated with psychological resources. Coping strategies implemented are not so much related to the nature of the sickness but rather to the representations the subject has thereof, with certain sociobiographical and psychological factors determining or predicting their implementation. Full textThe aim of the present work was to compare locomotor impaired and healthy young children with respect to their spatial search skills in different memory-for-locations tasks. Indeed, various studies have shown a correlation between self-produced locomotion in infancy and spatial development, especially in the realm of manual search for hidden objects. In theoretical part, we present models that predict or not delay in spatial cognition for motor-impaired children, namely Piagetian, dynamical, nativist and perceptual models. We present a broad spectrum of clinical and neurobiological data that support these theories. In experimental part, we present five different studies that assess spatial skills in 30-month-old children with type-2 spinal muscular atrophy (SMA) and suffering total deprivation of locomotor experience. Results suggest that locomotor impairment is not a key risk factor for a dramatic slowing down or deviation in the development of spatial skills
