Abstract:

The purpose of this study is to explore the therapist’s perspective regarding the clients and family who receive in-home therapy, their perspectives on the therapists practicing in the field, and the industry as a whole. A qualitative phenomenological study was conducted in order to explore the emergent themes from transcribed audiotaped interviews of 12 participants. The participants were all licensed therapists working with child and adolescents and their families within the home. The participants were recruited through a purposive snowball sample.

Seven major themes emerged from the study, these themes were as follows: (a) home and environment; (b) clients and their families; (c) treatment; (d) time; (e) the therapists; (f) experience; and (g) competency. Additional findings were presented, the developmentally disabled, burnout, and the work of psychologist in the in-home therapy field. This study also presented conclusions, recommendations for future research and a summary of the work as a whole.

Intensive in-home services is a family-centered, strength-based intervention program offering comprehensive treatment that includes family therapy, mental health treatment and parenting skills for caregivers, development of positive social systems, and assistance with accessing community resources for long-term support (Youth Villages, 2013b). Framed on family system theory, Intensive-In Home Services (IIHS) was developed from Cognitive Behavioral Therapy, Functional Family Therapy, Social Development, and Crisis Intervention models as a means to modify behaviors that place youth at risk for out-of-home placement. The socio-historical development of these models is examined in the literature review. The purpose of this study was to evaluate predictors that may influence outcome measures of a youth’s inclination to remain in the home after having received IIHS treatment. The significant predictors were determined to be age, race, and total length of service received. The study sampled 3,131 youth ages birth through 17 who received IIHS services in Tennessee over a 10-year period and were discharged from July 2001 to July 2010. The study was limited to youth designated under Comprehensive Child and Family Treatment (CCFT), which is classified as part of TennCare coverage. The primary goal of CCFT is to empower families to monitor and manage mental health needs and high-risk youth behaviors in order to provide permanency and longterm stability in the natural home environment (UnitedHealthcare, 2012).

Abstract:
Efforts to keep children in their own homes when they are found to be at risk within the meaning of child protection legislation have resulted in the creation of family preservation programs. Typically, these services are crisis-oriented. Short-term, intensive work with families is offered with a goal of maintaining the child in his or her own home. A family support program which provides these services in the anglophone community of Montreal was examined.
The data for this qualitative study were obtained through indepth interviews with the program staff and through an examination of agency files. The findings suggest that service is limited to those families who are assessed to be motivated--that is compliant with the objectives of the program and accepting of the intensive nature of the service. The interventions focus on individual parenting, most often the mother's parenting. For the workers, the dual role of support and scrutiny is managed within a relationship of trust.
The findings further indicate that families who are experiencing severe problems, often related to alcoholism, family violence and extreme poverty are not served by this program.

The study was undertaken at a village called Ramotswa in Botswana. Botswana is a small country in Southern Africa with a population of 1.7 million people (Botswana Housing and Population Census, 2001). The study explores HIV/AIDS clients' and caregivers' perceptions of the Community Home-Based Care support programme. The people involved in the community home-based care programme included nurses, family welfare educators (FWEs), social workers, community volunteers, and drivers. An exploratory qualitative design was used. Collection of data was through in-depth interviews, as well as a focus-group discussion. The sample was obtained through purposeful sampling, and there were nine HIV/AIDS clients, and seven caregivers who participated in the study. Results of the study were analysed themeativally. Resulst showed that many caregivers are women, and that they perform the task of care-giving under very difficult situations of poverty, social isolation, fear and stigma. They needed financial, material spiritual, as well as professional support from the home-based care team and community members.

Abstract:
The outcomes of a child protection/family support programme, the Cottage Community Care Pilot Project, were evaluated in this study. The evaluation employed a non-equivalent comparison group design of 'at risk' consenting first-time mothers in the perinatal period with babies up to 6 weeks of age. Ninety-three families were recruited and 58 of these were matched with a trained volunteer home visitor. Analysis of assessment items and questionnaires, reviews of hospital records and the Department of Community Services Child abuse and neglect notification register and focus groups with mothers and volunteers provided the information used in this study. The CCCP had an impact on particular aspects of family function, certain infant and maternal health indices and the families' use of community services, but its contribution to reducing the incidence of child abuse and neglect is less clear. Client and volunteer feedback indicated support for the programme. While home visitation by trained volunteers is not proposed as the total answer for effective child protection or family support, the findings of this evaluation suggest that there is a place for similar programmes.

The Human Immuno-deficiency Virus (HIV) that leads to Acquired Immuno-Deficiency Syndrome (AIDS) is considered to be spreading at a high rate in South Africa. Research indicates that this disease is highly prevalent among people 15 and 49. It is estimated that one in five adults in South Africa is HIV positive. Community or home-based care nurses manage the treatment of sufferers at their homes while under the care of their family members, friends, spouses and significant others. Abstract:
Standard economic models often emphasize inputs, outputs and an examination of the structures in order to conduct an economic performance evaluation. This study applies the Institutional and Development Framework (IAD) in the broader context of New Institutional Economics (NIE) in order to examine the transaction costs of delivering Community and Home Based Care and Support (CHBC) to HIV/AIDS sufferers. For purposes of unveiling the empirical reality guiding decision making processes in the CHBC service delivery, comparative qualitative research techniques of normative variable and concept formation have been adopted to draw out the relative institutional influences from the HIV/AIDS national response partnerships. The study identifies the conflict between the predominantly standardized and more rigid formal management techniques adopted by key members of the national response and the informal cultural techniques familiar to the rural communities, and a lack of motivational incentives in the CHBC structures as the key factors against CHBC capacities to draw external funding for service delivery. CHBCs are also weakened by incoherent governance structures at the district level for facilitation of funding and information flow exacerbating the community vulnerability. Rationalization of the institutional arrangements and a clarification of roles from district to community levels, a shift of focus to facilitation of informal techniques and an integration of performance enhancing incentives are the critical policy insights envisaged to spur CHBCs to work better.

Abstract:
HIV/AIDS is a pandemic infecting and affecting millions of people worldwide. South Africa is also severely affected by this disease. Because hospitals cannot cope with patients admitted daily especially with the influx caused by HIV/AIDS patients, the government has introduced home-based care of people living with HIV/AIDS. Amongst those who practice home-based care are the volunteers. This study is aimed at exploring and describing the experiences of volunteers involved in home-based care of people living with HIV/AIDS in the Lusikisiki Magisterial district in the former Transkei area of the Province of the Eastern Cape. The researcher used a qualitative methodology with the aim of finding out what it is like to be a volunteer involved in home-based care in a rural area. The data was collected by means of semi-structured one-to-one interviews or guided interviews. A total sample of thirteen research participants was acquired through purposive as well as snowball sampling. The interviews were guided by a number of broad question themes. Data was analyzed using Tesch’s framework of data analysis as described in Creswell (1994). The researcher used Guba’s model as outlined in Krefting (1991) to ensure trustworthiness of the research findings. A literature control was undertaken to find out what other researchers and authors say about the issues raised by the study. There were five themes that came out of the data analysis process and these themes are: o The experiences of volunteers with home-based care. o Factors facilitating the work of volunteers. o Problems encountered by volunteers. o Possible solutions to problems encountered. o Views of volunteers regarding home-based care. These broad themes were further reduced into sub-themes and categories. Based on the discussion of themes, sub-themes, and categories, some research findings were presented. The discussion of the themes, sub-themes and categories was supported by verbatim quotations from the participants. On the basis of research findings, conclusions and recommendations were made.

The purpose of this study was to examine participants' perceptions on the effectiveness of the "Parents in Partnership" program (PIP) of the Los Angeles County Department of Children and Family Services. Parents in Partnership (PIP) is a collaborative effort between the Department of Children and Family Services (DCFS) and parent partner paraprofessionals toward facilitation of safe reunification and permanency through education, support and mentoring of birth parents. The program's sole goal is the timely and safe reunification of children and their families. Face-to-face interviews were conducted with fourteen participants in the PIP program. Both mentors and mentees were interviewed.

Abstract:
This paper explores the utility of Weingarten's (2003) witness positions in the understanding of compassion fatigue in people who care for their own family members with AIDS. The research is embedded in Weingarten's theory of witnessing and narrative theory and practice. The literature review explores the shift from hospital-based care to community/home-based care which has led to family members assuming the role of caring for their family members with AIDS, an overview of the effects of caring for AIDS patients on caregivers and an overview of Weingarten's (2003) theory of witnessing with special emphasis on the witnessing positions and their consequences. Interviews, based on narrative theory and practice in which Weingarten's theory is rooted, gave access to the participants' experiences, which were then analysed and interpreted through a framework developed from the witnessing theory. This article demonstrates the utility of Weingarten's (2003) theory of witnessing to people who are caregivers to their own family members with AIDS. I argue that witness positions occupied by caregivers during witnessing determine whether the caregivers will experience compassion fatigue. The negative consequences related to compassion fatigue that will be reviewed could probably be prevented through active, intentional, compassionate witnessing.

Four different case management programs delivered by UnitingCare Community Options (UCCO) in the eastern suburbs of Melbourne were examined against the expectations of case management as a policy solution to a range of perceived policy problems at the micro-, meso- and macro-levels. The micro-level expectations were related to client and family experiences of the service system and outcomes. At the meso-level expectations were focused on perceived service delivery problems such as poor matching of services to the needs of ‘complex’ clients including a lack of integration, flexibility and responsiveness to clients’ needs and preferences. Perceived macro-level policy problems were concerned with a variety of issues including increasing rates of institutionalisation, increasing costs to governments, lack of economic efficiency and the desire to create market or quasi-market conditions in the community care service delivery sector. Resumo: A implantação de estratégias que articulem universidade, serviços e comunidade são práticas de ensino-aprendizagem que, cada vez mais, ganham espaço na área da saúde, estimulando instituições de ensino superior na adequação de políticas de ensino voltadas para a formação de um profissional generalista, capacitado a entender o processo saúde-doença nos diferentes níveis de atenção. O presente estudo objetivou conhecer o perfil das famílias que receberam visitas domiciliares realizadas por estudantes dos 1º e 2º anos de graduação em Medicina e em Enfermagem na disciplina de Interação Universidade Serviços Comunidade da Faculdade de Medicina de Botucatu - UNESP, durante o ano de 2011. Nesse estudo foram realizadas 124 entrevistas a usuários cadastrados nas Unidades Básicas de Saúde ou Unidades de Saúde da Família da Secretaria Municipal de Saúde do município de Botucatu - SP, que participaram das visitas domiciliares em 2011, e aplicado um questionário semi estruturado, identificando-se suas características socioeconômicas e demográficas. Para a análise estatística foi utilizado o software SPSS/WindowsÒ (versão 10.5), calculamos as porcentagens das variáveis estudadas e utilizamos o teste de qui-quadrado, com correção de Pearson quando necessário, considerando-se nível de significância de 5%. A análise dos resultados possibilitou conhecer as características dos moradores do domicílio, das condições de vida familiar, bem como o meio em que vivem e como vivem e identificamos que não há diferença significativa entre as famílias selecionadas em dois anos diferentes e consecutivos. Assim, entendemos que nossos resultados podem contribuir para o planejamento das atividades de visita domiciliar realizada pelos estudantes da Disciplina IUSC
Abstract: The establishment of strategies that articulate the university, the services and the community are teaching-learning practices which have increasingly gained space in the health area, stimulating higher education institutions to adequate the teaching policies directed to the formation of a generalist professional, qualified to understand the health-disease process at the different care levels. The present study aimed to learn the profile of families that received home visits by 1st and 2nd-year undergraduate students of Medicine and Nursing during the discipline Interação Universidade Serviços Comunidade da Faculdade de Medicina de Botucatu - UNESP, in 2011. In this study, 124 interviews were conducted to users registered at the Basic Health Units or Family Health Units of the Secretariat of Health of Botucatu Municipality - São Paulo State, which participated in the home visiting in 2011, and a semi-structured questionnaire was applied to identify socioeconomic and demographic characteristics. For statistical analysis, the software SPSS/WindowsÒ (version 10.5) was employed; we calculated the percentages of the studied variables and used the Chi-square test, with Pearson's correction when necessary, considering a significance level of 5%. Analysis of the results allowed us to learn the characteristics of the residents of the home, the living conditions of family, as well as the environment where they live, and we identified that there is no significant difference between the families selected in two different and consecutive years. Thus, we understand that our results may contribute to the planning of home visiting activities done by students of the Discipline IUSC

The establishment of strategies that articulate the university, the services and the community are teaching-learning practices which have increasingly gained space in the health area, stimulating higher education institutions to adequate the teaching policies directed to the formation of a generalist professional, qualified to understand the health-disease process at the different care levels. The present study aimed to learn the profile of families that received home visits by 1st and 2nd-year undergraduate students of Medicine and Nursing during the discipline Interação Universidade Serviços Comunidade da Faculdade de Medicina de Botucatu - UNESP, in 2011. In this study, 124 interviews were conducted to users registered at the Basic Health Units or Family Health Units of the Secretariat of Health of Botucatu Municipality – São Paulo State, which participated in the home visiting in 2011, and a semi-structured questionnaire was applied to identify socioeconomic and demographic characteristics. For statistical analysis, the software SPSS/WindowsÒ (version 10.5) was employed; we calculated the percentages of the studied variables and used the Chi-square test, with Pearson's correction when necessary, considering a significance level of 5%. Analysis of the results allowed us to learn the characteristics of the residents of the home, the living conditions of family, as well as the environment where they live, and we identified that there is no significant difference between the families selected in two different and consecutive years. Thus, we understand that our results may contribute to the planning of home visiting activities done by students of the Discipline IUSC

According to the Ministry of Health (1996:26), the most common chronic/terminal illnesses were cardiovascular disorders, diabetes, cancer, mental disorders, HIV/AIDS, tuberculosis and asthma. Long term treatment and care and the growing incidence of these conditions necessitated the introduction of home-based care (HBC). Consequently, family care givers play a major role in the provision of care to chronically/terminally ill patients and professional health care providers adopt a supervisory role. This study examined the quality of home care services provided in Botswana. The availability and accessibility of home-based care services and resources have a direct bearing on the quality of home-based care delivery system. The researcher used systems theory was used as the conceptual framework for this study. The study aimed to • determine the accessibility and availability of home-based care services in Molepolole East • investigate what the perspectives and experiences of family care givers, patients and professional health care providers of Botswana home-based care are • determine the roles of professionals health care providers, patients, and family care givers and their relationships in the context of home- based care • identify the needs of chronically/terminally ill patents and family care givers • determine the type of support given to family care givers and patients by professional health care providers and make recommendations for the improvement of home-based care • develop a model to prepare family care givers The research design combined quantitative and qualitative research methods. A sample of convenience was used to obtain information from patients' family care givers and professional health care providers. Interviews and questionnaires were used. A proposed care giving preparedness model is presented to meet needs of the family care givers. The study found that family care givers needs are neither known nor catered for by the professional health care provider. The family care givers were not adequately prepared before adopting the care-giving role. It is recommended that • The proposed preparedness care giving training model is considered for training of patients and family care givers before discharge and during HBC. • Increase patients and family care givers decision-making

Abstract:
This study examines the role of Community Based-Organisations (CBOs) in the response to HIV/AIDS as a development challenge drawing examples from the Gabane Community Home-Based Care CBO in Botswana. The study adopted qualitative methods of research and used group discussions, relative unstructured interviews, direct observation and literature review as methods of data collection. The study found out that HIV/AIDS is indeed a development problem and that it can be dealt with using some existing development approaches such as the sustainable livelihoods approaches. The study further identified specific roles that CBOs play in the response to HIV/AIDS. It also revealed the potential that CBOs have in achieving development. In addition, the study identified and outlined challenges that CBOs face in responding to HIV/AIDS. Key recommendations are that CBOs must integrate poverty reduction interventions in their activities. They must also form coalitions and strengthen their capacity to sustain their activities and manage partnerships.

These two studies investigated maternal engagement and retention in a voluntary, home-visiting program. The program screened families at the birth of their first child for risk characteristics associated with poor child and family outcomes. Higher risk families were offered regular home visits and support for up to 5 years. In the first study, a two-level hierarchical general linear model (HGLM) was used to examine the impact of poor community health and maternal isolation on mother's active engagement in the program, following initial enrollment. Data came from 4,057 mothers with firstborn infants, who enrolled in the Oregon Healthy Start (OHS) home-visiting program from 1995 through 1998. At the time of this study OHS was operating in 15 Oregon counties. Results showed that living in a county characterized by poor community health, or maternal isolation from supportive family and friendship networks, significantly reduced the likelihood of mothers actively engaging in home visits after enrollment. Hispanic mothers were significantly more likely to engage in services than mothers of other ethnicities. In the second study, a three-level hierarchical general linear model (HGLM) was used to investigate the impact of community violence, home visitor attributes, and maternal attributes on maternal retention in the OHS home-visiting program. Data came from 1,093 mothers with firstborn infants, served by 71 home visitors, residing in 12 communities being served by the OHS program prior to February 2000. Results showed that mothers who lived in communities that displayed higher levels of community violence were less likely to remain in the OHS program. Mothers were more likely to remain in the program when served by home visitors who received more active supervision. Mothers who were older were more likely to remain in the OHS program than were younger mothers. Hispanic mothers were more likely to remain in the OHS program than were mothers of other ethnic groups. Each study presents the implications for programmatic application.

Children are being removed unnecessarily from their families and placed into substitute care because service programmes lack resources to keep families intact. There are overseas programmes, which provide home services to families, with children at risk of out of home placement. Although there has been an IFPS project there is no established intensive short-term programme in South Africa that deals with families at the brink of dissolution.The objective of the research is to investigate whether an Intensive Family Preservation Programme is feasible in South Africa. The researcher received training in the United States and met with leaders in the field, to discuss the development and implementation of the programme in South Africa. Surveys obtained the attitudes and opinions of the local social workers and commissioners of Child Welfare in Durban towards the programme. The results from opinion surveys indicate that an Intensive Family Preservation Programme is feasible in South Africa.

Over the past decade, the HIV/AIDS pandemic has had a devastating effect on many communities and families in South Africa. Many families have already lost, and many more will lose, primary caregivers to the disease. This could have a serious impact on the living conditions and emotional wellbeing of families, and especially of children and adolescents. As an intern educational psychologist working in the Eldorado Park community, the researcher became aware of the difficulties and needs experienced by families that are affected by HIV/AIDS. This study therefore aims to identify the needs of families affected by HIV/AIDS, in order to assist the educational psychologist by making recommendations for weaving a circle of care to support them, within an asset-based approach. To accomplish this, the study utilised a participatory research approach within a critical theoretical paradigm and case study design. Purposeful selection was employed and as the study is concerned with HIV/AIDS which requires the utmost confidentiality, the families were identified with the help of home-based care workers. The researcher interviewed three families and eight home-based care workers in the Eldorado Park community. Detailed data analysis and interpretation of the needs highlighted during these interviews revealed a number of common themes, which are indicative of the needs of families affected by HIV/AIDS in the Eldorado Park area. These families expressed a need for awareness, love and understanding from their communities. They also wish to have greater understanding and support from schools, as well as expressing the need for opportunities to share their experiences and raise community awareness by speaking at schools and churches, and through support groups. Families furthermore require assistance with the great economic difficulties they face, largely due to illness and unemployment. A need to gain greater access to medical care, social grants and burial societies, was expressed. Difficulties with transport and documentation seemed to create a major barrier in this regard. The need for psychological and spiritual counselling as well as legal advice was highlighted. The study provides recommendations and a practical asset map to assist the educational psychologist in weaving a circle of care for families affected by HIV/AIDS.

The escalating number of people infected by HIV and AIDS poses a challenge to current resources both at the hospital and household level. Community/Home based care is a viable option in response to the scourge of HIV/AIDS to compliment secondary and tertiary care as well as providing support to affected families in coping with the stress of caring for their terminally ill family members. This study explored the effectiveness of the implementation of Community/Home based care services by focusing on three sites at Bushbuckridge i.e. Cunningmore, Maviljan and Hluvukani. Findings revealed that family members were satisfied to have community caregivers entering their homes to provide care and support to their AIDS ill family members. Significant barriers were highlighted by families and community caregivers that hinder effective implementation of the programme, for instance, cultural sensitivity. The study proffered recommendations on how to deal with these issues.

Abstract:
The aim of this research was to support new mothers in groups, so that they can function optimally as educators. During the research mothers with babies up to three months of age were used by means of a support group. This research project was done after it became evident from a literature study that new mothers suffered with unique problems during this adaption phase. In the empirical survey the researcher gave attention to the new mothers' specific needs and problems during this trimester. The researcher aimed to explore the experiences of the new mother as educator through weekly group sessions. From the results it was clear that there was a need of support from other mothers that is going through the same experiences. In this way new mothers could receive emotional support and obtain knowledge regarding relevant issues experienced during this unique life phase.
