Abstract:
Hemorrhoids are vascular spaces forming cushions above the anal canal which allow the passage of gas without stool, and cause clinical problems only when they become engorged and prolapse with resulting pain, bleeding, itching or discharge. Hemorrhoids require treatment only if they cause symptoms, and should be treated only as long as symptoms persist. Techniques of treatment including injection therapy, rubber band ligation, infrared coagulation, bipolar probe, heater probe, ultroid and YAG laser are described. At present, rubber banding is the most cost effective therapy. Diet education is essential to prevent recurrence.

Introduction: This study was conducted to find out determinants of preference of homeopathic treatment to allopathic one.Objectives: To determine factors influencing people’s attitude towards homeopathic treatment. Study Design: This was an descriptive crosssectional epidemiological study. Setting: At homeopathic clinics situated at Zenana Hospital road Bahawalpur Period: 1st July 2010 to 30th Nov2010. Subjects & Methods: Tool of data collection was preformed questionnaire which was used to collect relevant information. The informedconsent was taken. Eighty subjects were included in the study. Results: Out of total respondents, 87% preferred homeopathic treatment andonly 12.5% preferred allopathic treatment. This high utilization was attributed to high efficacy, low cost, better taste, less side effects and familytrend. Conclusions: People at Bahawalpur prefer homeopathic treatment to the allopathic one.

Abstract:
The paper provides an overview of various treatment options for chronic hemorrhoids based on current national and international recommendations. The publication describes the basic principles of effective conservative therapy of the disease. Methods of minimally invasive treatment options for chronic hemorrhoids are given in details: sclerotherapy, infrared coagulation, latex ligation; advantages and disadvantages of each of these methods are highlighted. Literature data indicate that surgical removal of hemorrhoids remains an effective method of treatment for patients with hemorrhoids stage III IV, who underwent unsuccessful minimally invasive procedures. The characteristics of possible complications after hemorrhoidectomy and the frequency of their occurrence are also given. The authors analyze results of using stapler hemorrhoidopexy and the technique of Doppler-controlled ligation of hemorrhoidal arteries. Despite the variety of techniques, it remains relevant to search for further treatment options for chronic hemorrhoids in order to choose an effective individual treatment tactics for each patient, taking into account various manifestations of hemorrhoidal disease.

Abstract Hemorrhoids are one of the most common diseases of the anorectal region. Previously, treatment for hemorrhoids included conservative treatment, outpatient treatment, and surgical treatment. The development of flexible reversible endoscopes has provided precise controllability and imaging, enabling further improvement and development of various endoscopic techniques to treat hemorrhoids. This article discusses several of these endoscopic techniques: rubber band ligation, sclerotherapy, and electrocoagulation. The development, efficacy, and advantages of these treatments are summarized and evaluated. It is expected that going forward, endoscopic technology will be further applied in clinical practice and may become the first-line method for the treatment of hemorrhoids.

This article is devoted to the results of applying the original minimally invasive surgical technique for the treatment of chronic internal hemorrhoids. The purpose of the study is to improve the outcomes of treatment in patients with chronic internal hemorrhoids by the simultaneous use of pathogenically substantiated low-traumatic surgical methods: dearterialization of internal hemorrhoids with Doppler guidance and clamping with latex rings. Materials and methods. Study included 204 patients with chronic hemorrhoids of the II–III degree, divided into 3 groups depending on the technique of minimally invasive surgery. The standard research methods for coloproctology were used. Results. It was found that the best short-term and long-term results of the surgical treatment of chronic internal hemorrhoids were obtained using the combined surgical technique proposed by the authors. Conclusions. The proposed combination of minimally invasive surgery may be the method of choice in the surgical treatment of chronic hemorrhoids of the II–III degree.

Homeopathic basic research is still in the screening phase to identify promising model systems that are adapted to the needs and peculiarities of homeopathic medicine and pharmacy. We investigated the potential of a common plant-pathogen system, Arabidopsis thaliana infected with the virulent bacteria Pseudomonas syringae, regarding its response towards a homeopathic treatment. A. thaliana plants were treated with homeopathic preparations before and after infection. Outcome measure was the number of P. syringae bacteria in the leaves of A. thaliana, assessed in randomized and blinded experiments. After a screening of 30 homeopathic preparations, we investigated the effect of Carbo vegetabilis 30x, Magnesium phosphoricum 30x, Nosode 30x, Biplantol (a homeopathic complex remedy), and Biplantol 30x on the infection rate in five or six independent experiments in total. The screening yielded significant effects for four out of 30 tested preparations. In the repeated experimental series, only the homeopathic complex remedy Biplantol induced a significant reduction of the infection rate (p = 0.01; effect size, d = 0.38). None of the other four repeatedly tested preparations (Carbo vegetabilis 30x, Magnesium phosphoricum 30x, Nosode 30x, Biplantol 30x) yielded significant effects in the overall evaluation. This phytopathological model yielded a small to medium effect size and thus might be of interest for homeopathic basic research after further improvement. Compared to Bion (a common SAR inducer used as positive control), the magnitude of the treatment effect of Biplantol was about 50%. Thus, homeopathic formulations might have a potential for the treatment of plant diseases after further optimization. However, the ecological impact should be investigated more closely before widespread application.

THE AIM of the study was to develop a minimally invasive technique that does not have the disadvantages of traditional surgical treatment, does not have any restrictions in patients with external hemorrhoids as in HAL-RAR method. Technique that is more effective than HAL-RAR and isolated laser destruction methods, in the treatment of patients with chronic hemorrhoids stage II-III. MATERIALS AND METHODS. We developed a combined method of treatment of the patients with hemorrhoids, that unites artery ligation under the ultrasound control with Doppler effect and submucous subdermal laser destruction of internal and external hemorrhoids. We studied the safetiy and effectiveness of the developed method. The study included 100 patients with chronic hemorrhoids stage II and III, the patients were treated with the use of combined methodology HAL-laser (study group), the method HAL-RAR (1 control group) and closed hemorrhoidectomy linear stapler (control group 2). RESULTS. The analysis showed that the combined method HAL-laser has higher efficiency than the method HAL-RAR, and thus is as safe and well tolerated in patients. Its not so efficient compare to conventional hemorrhoidectomy because of the risk of a cosmetic defect, but does not increase the risk of recurrence of the disease, reduces the risk of complications and more endurable for the patients, it also allows to reduce the time of disability.

AIM: to clarify surgeon’s preferences for the conservative treatment of chronic hemorrhoids.MATERIALS AND METHODS: an anonymous survey included 102 surgeons with an experience in chronic hemorrhoids treatment. RESULTS: an analysis of survey revealed surgeon’s preferences for local and systemic drugs for the treatment of chronic hemorrhoids. The surgeons reported more than 30 different drugs for the local treatment of hemorrhoids. Most specialists consider it necessary to prescribe systemic phlebotonics. The drug «Detralex» was most often used.CONCLUSION: a majority of surgeons use effective drugs for the treatment of chronic hemorrhoids. In any cases relative shortage of knowledge about discussed problem was revealed.

OBJECTIVE. Determine the frequency of clinical manifestations of hemorrhoids and constipation in people seeking advice about hemorrhoids. In the course of the treatment of patients with hemorrhoids phlebotropic evaluate the effectiveness of therapy micronized purified flavonoid fraction (MPFF). MATERIALS AND METHODS. This multicenter study, including screening and observation part, which is part of the International Research «CHORUS» (Chronic venous and hemorrhoid diseases evaluation and scientific research), conducted in nine centers in different regions of Russia, 80 doctors of Coloproctology. In the screening group included 2668 patients who had investigated the incidence of constipation, as a risk factor for hemorrhoids. Conservative treatment, the foundation of which was, Moffitt therapy, received 1952 patients with stage I-IV hemorrhoids. Evaluating the effectiveness of the treatment was evaluated on the basis of a questionnaire. RESULTS. The questionnaire shows that constipation suffered - 766 (28,8 %) patients. Violation of defecation patterns and changes in stool consistency was observed in 1155 (43,9%) and 633 (25.5 %), respectively. At the same time, 288 (11,1 %) indicated a tendency to loose stools and diarrhea. Conservative treatment, the foundation of which is phlebotropic MPFF therapy conducted in patients of observational group has shown its efficiency in all grades of hemorrhoids. During the entire observation period of conservative treatment was effective in 1489 (76,3 %) patients. Surgical treatment was performed in 463 (23 %) patients grade I-IV hemorrhoids, the main part of patients with grade III -199 (43,1 %) and grade IV hemorrhoids - 68 (64,2 %). CONCLUSION. Conservative treatment of hemorrhoid disease, which is the basis on phlebotropic MPFF therapy, is effective at all stages of hemorrhoids, but in patients with grade III and grade IV disease requires surgical treatment.

They had various clinical manifestations of the disease: perianal protrusion (76.4%), bleeding (82.3%), pain syndrome (71.5%). 51 patient (91.1%) had external hemorrhoids; 3 children (5.3%) - internal; 2 children (3.6%) - combined. Patients, by their clinical course, were distributed as follows: acute hemorrhoids - 33 children (58.9%), chronic one - 23 (41.1%). Ultrasound examination and sigmoidoscopy were used for diagnostics. Different curative techniques followed the developed curative algorithm: conservative therapy and surgery (open surgery and with the LigaSure apparatus) as well as sclerotherapy. Results. The most effective technique, having better cosmetic and clinical outcomes, was hemorrhoidectomy. Conservative treatment (more than 80%) can be a method of choice at early stages of the disease and at younger age groups. Outcomes after sclerotherapy are comparable with outcomes of conservative and surgical treatment. Sclerotherapy is recommended at the internal stages of hemorrhoids and in case of any contraindications to surgical treatment.

OBJECTIVES OF RESEARCH. To assess the clinical effectiveness of phlebotrophic drug Detralex (Micronized purifiedflavonoid fraction (MPFF)) during complex treatment of patients with acute hemorrhoids. MATERIALS AND METHODS. A comparative analysis of treatment outcomes of 293 patients with acute hemorrhoids was conducted. All patients were divided into two groups for further clinical observations. Standard drug therapy was performedfor 145 patients (group I). For 148 patients (group II), in addition to the standard treatment, phlebotrophic drug Detralex was additionally initiated. Research program included detection of content of acute inflammatory phase reactants and pro-inflammatory cytokines in blood plasma, evaluation of pain syndrome severity (VRS) and quality of life parameters (SF-36 questionnaire). RESULTS. Initiation of MPFF with complex treatment of patients with acute hemorrhoids was found to be contributed to faster elimination of edema and inflammation, a decrease in severity of hemorrhoids thrombosis, relief of pain syndrome and improvement in patients'quality of life, along withwell-marked regression of laboratory markers of inflammatory reaction This allows to increase the number of good outcomes of acute hemorrhoids treatment from 66,2% to 89,9%, that is by 23,7% and to reduce the number of unsatisfactory outcomes from 4,2% to 1,3%, that is by 2,9%. CONCLUSION. Inclusion of Detralex into program of acute hemorrhoids treatment contributes to a faster elimination of clinical and laboratory manifestations of the disease and allows improving the results of treatment for this category of patients.

AIM. To work up the optimal approach to the prevention and treatment of hemorrhoids in pregnant women in antenatal and postpartum women in the immediate postpartum period. MATERIAL AND METHODS. Based on the comparative results of treatment of hemorrhoids using topical and systemic medicinal drugs in 113 pregnant women determined the effectiveness of preventive and therapeutic tactics in respect of haemorrhoidal disease in pregnancy and the early postpartum period.. Treatment schemes designed for optimum efficiency and safety of treatment of acute hemorrhoids in this patient's category. RESULTS. The use of investigated schemes for treatment and prevention can significantly reduce the risk of exacerbation of the hemorrhoids and reduce the terms of relief of symptoms in pregnant and postpartum women. The experience of the Detralex (Daflon 500) demonstrates the effectiveness and good tolerability of this product when used in pregnant women for treatment and prevention of postpartum exacerbation of hemorrhoids.

Objective: Observational, prospective study to describe the homeopathic management of premenstrual syndrome (PMS) by a group of French physicians.Method: Women with PMS for >3 months were prescribed individualized homeopathic treatment. The intensity of 10 clinical symptoms of PMS was scored individually at inclusion and at a 3–6 month follow-up visit: absent = 0, mild = 1, moderate = 2, severe = 3. Total symptom score (range: 0–30) was calculated and compared for each patient at inclusion and at follow-up. PMS impact on daily activities (quality of life, QoL) was compared at inclusion and follow-up as: none, mild, moderate, severe, very severe.Results: Twenty-three women were prescribed homeopathic treatment only (mean age: 39.7 years). Folliculinum (87%) was the most frequently prescribed homeopathic medicine followed by Lachesis mutus (52.2%). The most common PMS symptoms (moderate or severe) at inclusion were: irritability, aggression and tension (87%), mastodynia (78.2%) and weight gain and abdominal bloating (73.9%); and the most common symptoms at follow-up were: irritability, aggression and tension (39.1%), weight gain and abdominal bloating (26.1%) and mastodynia (17.4%). Mean global score for symptom intensity was 13.7 at inclusion and 6.3 at follow-up. The mean decrease in score (7.4) was statistically significant (p < 0.0001). Twenty-one women reported that their QoL also improved significantly (91.3%; p < 0.0001).Conclusions: Homeopathic treatment was well tolerated and seemed to have a positive impact on PMS symptoms. Folliculinum was the most frequent homeopathic medicine prescribed. There appears to be scope for a properly designed, randomized, placebo-controlled trial to investigate the efficacy of individual homeopathic medicines in PMS.

Objective To investigate the clinical use of a large C suture in the procedure for prolapse and hemorrhoids (PPH) for treatment of mixed hemorrhoids. Methods Patients with mixed hemorrhoids (grade III or IV) who underwent treatment with a large C suture during PPH in the Affiliated Hospital (Group) of Putian University from 1 April 2018 to 31 March 2019 were enrolled in this retrospective study. The incidences of anastomotic stenosis and anastomotic hemorrhage after the operation were observed. Results The study population comprised 126 patients (46 men and 80 women) ranging in age from 30 to 78 years (mean, 46.1 ± 2.5 years). Of these patients, 60 had circular mixed hemorrhoids, 36 had grade III circular internal hemorrhoids, and 30 had grade IV circular internal hemorrhoids. The onset time among all patients ranged from 0.5 to 25 years. All patients underwent 6 months of postoperative follow-up. None of the patients with mixed hemorrhoids developed anastomotic stenosis, although two patients developed secondary anastomotic bleeding. Conclusion Placement of a large C suture during PPH is a reliable technique for treatment of mixed hemorrhoids. It is simple, effective, and applicable and can be helpful for a large number of patients in primary hospitals.

Abstract:
We conducted a randomized, placebo-cor rolled, double-blind clinical trial in order to determine the efficacy of classical homeopathic therapy in patients with chronic headaches. After 6 weeks of baseline observation, patients received either the prescribed individualized homeopathic medication or an indistinguishable placebo for 12 weeks. Outcome parameters were headache frequency, duration, and intensity, measured daily by diary. Use of medication for acure headache was also monitored. Of the 98 patients in the sample, 37 were randomized to receive placebo, 6I received individualized homeopathic remedies. Groups were comparable at the beginning of the treatment. The median age was 48.5 years; 76% suffered from migraine, 51% from tension-type headaches, and 94% were previously treated for headache. The median headache frequency was 3 days a week. Headaches were present for 23 years (median). In both groups, patients showed an improvement of one headache day less per month. The use of medication for acute headache was reduced. The headache frequency of 11 patients was reduced by more than 40%. Thirty-nine patients either did not improve or experienced aggravations. There was no significant difference in any parameter between homeopathy and placebo.
