Abstract:
Amyotrophic lateral sclerosis is a devastating condition with progressive loss of movement, speech, and respiratory function, and no available cure. Following the development of clinical symptoms and after receiving a diagnosis, patients may develop psychological morbidity, such as depression, anxiety, and hopelessness. However, many patients adjust successfully in the course of the disease and maintain good psychological well-being, so that a decline in psychological well-being does not necessarily accompany loss of physical function. There are several major determinants of good psychological adjustment to chronic and terminal disease—intrinsic factors such as coping strategies and internal locus of control, and extrinsic factors such as high (perceived and actual) social support by families and multidisciplinary professional teams. Providing care with a holistic view of the patient is probably the most effective approach to supporting patients’ psychosocial adjustment to the disease and minimizing depression, anxiety, and hopelessness.

Abstract:
This chapter focuses on depression in ill and dying patients. The symptoms of depression are the same in sick people and in those approaching death as they are in anyone else. While pervasive low mood, loss of pleasure and interest (anhedonia), and loss of energy are still core to the syndrome of depression, the triad of worthlessness or low self-esteem, helplessness, and hopelessness can sometimes be more important. The biological features of depression are also often present. However, in illness, or when death is near, some things may be different. This chapter first considers what makes depression different in people who are ill or dying before discussing a variety of case histories, based on real experience but not reflecting any particular individual, which each raise specific issues to do with depression in the ill and the dying. It suggests that the holism of a biopsychosocial and spiritual approach must be taken into account both to assist diagnosis and to ensure appropriate treatment, while also providing a less circumscribed and more humane approach to the ill and the dying.

Abstract:
Any attempt to define torture requires a sound theory-based understanding of the psychological mechanisms by which it induces “severe mental pain or suffering.” Among the different theories advanced for this purpose, learning theory of trauma is the one that has been most extensively tested and empirically validated. Substantial research with both animals and humans has shown that the unpredictability and uncontrollability of stressor events lead to helplessness and hopelessness responses, including anxiety and depression. This chapter presents a learning theory model of trauma, provides a definition of “pain or suffering,” demonstrates a contextual/cumulative approach to captivity stressors, and describes a methodology for assessment of captivity-induced pain or suffering. It also proposes a learning theory formulation of torture as “helplessness under the control of others” and reviews its implications for the distinction between torture and cruel, inhuman, or degrading treatment and emerging trends in international law in interpretation of torture.

The importance of spiritual well-being and the role of "meaning" in moderating depression, hopelessness and desire for death in terminally-ill cancer and AIDS patients has been well-supported by research, and has led many palliative clinicians to focus on the development of non-pharmacologic interventions that can help their patients address these issues. Individual Meaning-Centered Group Psychotherapy (IMCP), an intervention developed and rigorously tested by the Department of Psychiatry & Behavioral Sciences at Memorial Sloan-Kettering Cancer Center, is a seven-week program based around the work of Viktor Frankl, and which utilizes a mixture of didactics, discussion and experiential exercises that focus around particular themes related to meaning and advanced cancer. Patients are assigned readings and homework that are specific to each session's theme and which are utilized in each session. While the focus of each session is on issues of meaning and purpose in life in the face of advanced cancer and a limited prognosis, elements of support and expression of emotion are inevitable in the context of each group session.
