Abstract:
Hospice and palliative care have shared but divergent histories. By the early 20th century, a new focus on end-of-life care had also appeared. Despite a lack of definitional precision within these models, there is a fragile but emergent evidence base and a growing professionalization of the field. As awareness increases about the rising number of deaths in the world, competing claims also emerge about the manner of our dying. These go beyond the field itself to include support for assisted dying, greater emphasis on community resilience in the face of aging and death, plus growing environmental awareness of the significance of rising death rates. These require interdisciplinary thinking, as well the injection of new ideas from cognate fields in the health and social sciences.

Abstract:
The development of hospice and palliative care in Austria started with dedicated volunteers in the 1980s and is strongly linked to the development of training for professionals. In 2015, volunteers in hospice and pailliative care (HPC) in Austria numbered 3,630 (87 per cent female) spending 257.510 hours in direct patient care taking care of 12.832 patients, 147.578 hours with activities like training, supervision and fundraising; activities as broard members. HPC volunteers are organized in teams, specially trained (national curriculum with 160 hours and 40 hours practice), and as hospice teams form part of the graded hospice and palliative care system in Austria. They care for patients and their families in all settings: the patients’ homes, nursing homes, hospital wards, and palliative care units, as well as inpatient hospices and day hospices. Factors of success are: effective management and training, standards and guidelines, adequate funding, and appreciation and good cooperation between volunteers and paid staff.

Abstract:
This chapter includes an overview of the role of the hospice and palliative care approach in advance care planning (ACP). Evidence, issues, challenges, and opportunities are discussed. The challenge of overlapping definitions and consensus about the hospice, palliative care, and advance care planning is highlighted as well as the changing role of the hospice and palliative care and implications for ACP is explored. New types of conversations are discussed e.g. living with dying, managing uncertainty, expectations, ceilings of treatment, investigations and dying in hospices. The way forward such as collaboration, co-ordination, and partnerships with other providers as well as a consistency of approach (within settings, roles, or person to person) is suggested.
