Full textIntroduction: Multinodular Goiter is a common endocrinopathy with prevalence between 3-7%, which can be as high as 50%, in ecographical studies. Genetic and environmental factors influence this pathology development. Iodine is a major environmental factor. This is why food habits and residence area are important. Several studies show an association between Multinodular Goiter and endocrinopathies and/or immunonologic pathologies. The follow-up and treatment aren’t consensual. Objective: To characterize the hospital population with Multinodular Goiter, in the influence area of Sousa Martins’ Hospital, in Guarda. Methods: Observational, descriptive study, with an analytical component, divided in three parts: 1) an inquiry application and clinical processes analysis; 2) iodine analysis in the water of six localities of Guarda district; 3) an example of journal diet, according to food and waters’ iodine content. Statistical analysis was done with SPSS Statistics 17.0® for Windows®. Results: 104 patients were observed (101 of the feminine sex), with an average of 50,3 years old. 60% were referenced by the family doctor, 40,1% had at least one compressive symptom at the diagnosis. 56,7% were in euthyroidism, 31,7% in one of hyperthyroidism forms and 11,5% in one of hypothyroidism forms. 52% of the patients had a positive analysis for auto-immunity. Thyroid scintigraphy was asked to 53% of the patients; 12 of 16 patients with hot and cold nodules also made a fine needle aspiration biopsy. 64, 4% of patients in euthyroidism took levothyroxine. One patient was treated with radioiodine. Diabetes Mellitus was the most frequent pathology (19,2%) associated with multinodular goiter. Iodine analysis in water reveled that Guarda has the lowest average concentration 43±13µg/L and Celorico da Beira the highest 131±92µg/L, from 9/02/2010 to 27/04/2010. With only journal diet of rich iodine fish, the minimum iodine journal iodine (150 µg, World Health Organization) is achieved. With other fish and meet, the iodine value in water is fundamental to that achievement. Discussion and Conclusion: It is necessary to reevaluate the complementary diagnostic exams asked. One should reinforce the importance of risk and protection factors and food habits. One should think of some auto-immune diseases screening. It is indispensable to classify this region concerning iodine’s content, through medium urinary excretion. Iodization, in areas with low iodine, has an excellent cost-benefit and cost-efectivity ratio.

Full textIntroduction: According to the study "Pain in Europe” (1), 19% of adults suffer from chronic pain, 9% of these, are due to omalgia. The most frequent cause of omalgia is the chronic injury of the rotators cuff. Although the Rotator´s Cuff non traumatic Symptomatic Pathology (RCSP) is a common entity that can cause pain and severe functional disability, there is a lack of reliable statistics of its frequency in Portugal. Thus, the prevalence of this disease and the direct and indirect costs associated with it remain undetermined. This work aims to study the RCSP in Sousa Martins Hospital (HSM). Methods: A retrospective study was carried out by examining computer records of all the people who went to the Emergency department and all the episodes of hospital admissions and surgeries. An analysis of the clinical charts of the Orthopedics, rehabilitation and chronic pain consultations was carried out. Of all these episodes / consultations that took place, it was verified which were due to RCSP and what were the costs associated. A bibliographic review was carried out using national and international literature, on the characteristics and frequency of this pathology. Results/Discussion: During the year 2007 in the HSM, there were 85 emergency episodes due to RCSP, which generated a cost of 9 010 €. There were 37 episodes of hospitalization / surgeries due to RCSP, costing 8364.19 €. Out of these 37 episodes, there were 35 different patients, meaning that 2 patients were hospitalized / operated twice. Using a sample of the orthopedics consultations, one can say with a confidence of 95%, that there were between 194 and 397 consultations due to RCSP and that these cost between 17 686.98 € and 36 194.49 €. Fifty rehabilitation consultations were observed, they were attended by 30 different patients, meaning that 20 patients required more than one of these consultations in the same year. Of the 30 patients, 18 underwent fisiatric treatment in this hospital. The costs of the treatment and consultations were 10,976.80 €. Due to RCSP there were 49 chronic pain consultations, which 27 different patients attended, meaning that 22 patients attended this consultation more than once in the same year. The costs of these consultations were 1 470,00 €. With a 95% degree of confidence, the sum of all the episodes due to RCSP were between 416 and 619 and the total costs between €130 488.84 and € 148 996.35. Conclusions: Despite being a highly disabling disease and surpassing international statistics of prevalence for the district in question, only a minority of patients with RCSP are being followed in differentiated medical terms. The direct costs attributed to this illness are much lower than those of other diseases with a similar impact on quality of life.

Full textIntroduction: The incidence of malignant cutaneous melanoma, over the past decades, has been increasing. It affects every age group, being more frequent around fifty. The high incidence is due to life style changes and to solar exposure habits mostly during childhood and adolescence in spite of the prevention campaigns effort. Methods: A retrospective study was made through the revision of patients’ clinical processes with malignant cutaneous melanoma diagnosed in the Dermatological Service of the Hospital Sousa Martins in Guarda, between 1995 and 2007. Results: A hundred and five cases of malignant cutaneous melanoma have been diagnosed, being 67% female and 3% male. Their average age was 65 years old. The part of the body that has been most affected by cutaneous melanoma was the head (in 47% of the cases). The most predominant subtype of melanoma was the one of superficial extension (43%). The average of the Breslow tumor thickness was 3,01 millimeters. The ulceration was part of 21 situations of melanoma. Most of the cases were in the 0 stage (39%), which has correspondence with the melanoma in situ. Nineteen patients have died with cutaneous melanoma. In the follow-up 3 new cases of primitive melanoma have been diagnosed. Discussion/Conclusion: Over the last years, from 2004 to 2007 there has been a decreasing of the incidence of malignant cutaneous melanoma in the region of Guarda. Both the age group and the anatomic localization are according to the literature. The prevalence of melanoma of superficial extension is due to interrupt solar exposure. According to some literature the Breslow tumor thickness and the ulceration are important risk factors. In this part of the country, the detection was made in the beginning as most of the patients were in stage 0.

Full textINTRODUCTION: Basal cell carcinoma is the most common type of all skin malignant neoplasms, however, despite its high prevalence, metastatic phenomena are extremely rare. They are locally destructive tumors, with slow-growing and a remarkable variety of clinical aspects. The high incidence of this tumor is, mainly, due to habits of prolonged exposure to sunlight. The most affected locations are related to areas most exposed to ultraviolet radiation. OBJECTIVES AND METHODS: The main objective of this work is to make a detailed study of basal cell carcinoma in the Dermatology Department of Hospital Sousa Martins, Guarda, during the period 2010-2011. Its achievement is intended to describe the study population and to make known the reality of the intervention area of the hospital. To this end, there are several analyzed characteristics, including age and sex distribution, anatomical location of the injury, risk factors, clinical presentation at diagnosis, clinical features, diagnostic accuracy and treatment. All the cases included in the study were confirmed by histopathology diagnosis. It was performed a retrospective analysis by reviewing the medical files of patients with the diagnosis of basal cell carcinoma. Data were subjected to descriptive statistical analysis by calculating measures such as frequencies, percentages and means. In addition, it was proceeded to a literature review on the topic in question. RESULTS: It was obtained a list of 255 individuals with basal cell carcinoma and has been accounted 312 lesions. The study included 148 women (58%) and 107 men (42%) with a mean age of 77,4 years (± 11,3 [37-101]). The head and neck region was the most affected, with 273 cases (87,5%), and the predominant specific location was the nasal region with 60 cases (19,2%). The average size of the lesions was 13,59 millimeters (± 8,90 [4-50]) and the average period of their evolution was 16,19 months (± 12,17 [2-60]). The most commonly encountered subtype was nodular, in 164 cases (52,6%). Recurrences were found in 13 cases. The most widely therapeutic approach was the surgical lesion removal in an ambulatory setting. DISCUSSION/CONCLUSION: During the years 2010 and 2011, at the Dermatology Department in Hospital Sousa Martins, Guarda, the diagnosis of basal cell carcinoma, was observed more frequently in females. The age group and the anatomical location most affected, and histological subtype are consistent with current literature. The main risk factor associated with basal cell carcinoma is the intense and intermittent sun exposure, and the changes induced by carcinogenic radiation are cumulative. Thus, strategies for prevention and early diagnosis are important.

Full textA existência de incêndios em hospitais têm dificultado a evacuação de pacientes, criar perdas de vidas humanas, destruição de bens materiais e encerramento dos serviços de atendimento ao público, pelo que, se justifica a existência de estudos sobre a engenharia do incêndio que analisem com mais detalhes os riscos e evacuação de hospitais em situação de incêndio. O presente estudo modelou o risco e a evacuação para três compartimentos corta-fogo (CCF): Bloco de Operações (BO), Serviço de Urgências (SU) e Serviços de Internamentos (SI), em situação de incêndio. A análise de risco foi com base nos métodos de Gretener, Frame e Hierárquico. Por sua vez, a modelação foi através de simulações numéricas com recurso ao programa de volumes finitos Fire Dynamics Simulator (FDS) em conjunto com o módulo de simulação de evacuação Evac, onde se analisaram 27 cenários, dos quais 10 foram do CCF BO, 12 do CCF SU e 5 cenários do CCF SI. O estudo apresentou e discutiu a influência da desenfumagem na evacuação do CCF BO, a influência do efetivo no tempo de evacuação do CCF SU em situação de incêndio, assim como o impacto na evacuação de ocupantes com recurso à utilização de elevadores para o CCF SI. Por outro lado, analisou-se ainda o impacto do número de portas e da largura das mesmas para a evacuação dos CCF, o impacto do tempo de pré-movimento, a localização do foco do incêndio, o tipo de efetivo (adultos acamados e não acamados), entre outros. Com a análise do risco de incêndio concluiu-se que os três CCF estudados estão seguros contra o incêndio. Os coeficientes de segurança são quase os mesmos para os três CCF. Entretanto, com simulações com o FDS+Evac concluiu-se que, o tempo da reação dos ocupantes a um possível incêndio é o parâmetro mais crucial para se obter a maior percentagem de ocupantes a evacuarem os CCF, uma vez que, nos diferentes cenários simulados, conseguiu-se evacuar 100% dos CCF quando estes reagiram ao incêndio na sua fase inicial (aos 30 ou 45 s) e entre 50% a 70% dos CCF evacuados quando os ocupantes reagiam ao incêndio apenas a partir dos 180 s da simulação. Por outro lado, mostrou-se obrigatória o sistema de desenfumagem dentro do CCF BO (para pré-movimentos aos 180 s), conjugado com o aumento, até 2 m, das larguras das portas de evacuação, para facilitar a evacuação de acamados. Para o CCF SU mostrou-se ainda relevante a introdução de pelo menos uma porta de evacuação que conduz a uma VHE, visto que, mesmo quando os ocupantes reagiram ao incêndio tardiamente (aos 180 s) o CCF evacuou na totalidade. Por último, para o CCF SI conclui-se ainda que, a evacuação horizontal era mais rápida que a evacuação com recurso a elevadores.
Fires in hospitals have hampered the evacuation of patients, loosing of human lives, destruction of goods and closure of public services, justifying the existence of fire engineering studies that analyse in more detail the risk and evacuation of hospitals in case of fire. In the present study were modelled the fire risk and evacuation of three fire compartments (CCF): surgeries room (BO), emergency service room(SU) and the hospitalizations services (SI), in case of fire. The risk analysis was based on the methods of Gretener, Frame and hierarchical. The modelling was performed by numerical simulations using the finite volume software Fire Dynamics Simulator (FDS) in conjunction with the evacuation simulation module Evac. They have been analysed 27 scenarios, 10 of which were of the CCF BO,12 of the CCF SU and 5 of the CCF SI. The study presented and discussed the influence of smoke-cleaning system on the evacuation of the CCF BO, the influence of the time of evacuation of the CCF SU in fire situation, as well as the impact on occupant evacuation of the use of lifts for the CCF SI. On the other hand, were analysed the impact of the number of doors and their width for the evacuation of the CCF, the impact of pre-movement time, the location of outbreak of fire, the type of effective (bedridden not bedridden adults), among others. With the fire risk analysis, it was concluded that the three CCF studied are safe from the fire. The safety coefficients are almost the same for the three CCF. However, with simulations with FDS + Evac it was concluded that the reaction time of the occupants to a possible fire is the most critical parameter to get the highest percentage of occupants to evacuate the CCF, once, in the different simulated scenarios, it was possible to evacuate 100% of the CCF when they reacted to fire at an initial phase (at 30 or 45 s) and between 50% to 70% of the CCF evacuated when the occupants responded to the fire just from the 180 s of the simulation. On the other hand, it was shown to be necessary a smoke-leaning system in the CCF BO (for pre-movements of 180 s), in conjunction with the increase, up to 2 m, of the width of the doors, in order to facilitate the evacuation of the bedridden. For the CCF SU it was proved to be still relevant to introduce at least one evacuation door that leads to a VHE, because even when the occupants reacted to the fire belatedly (at 180 s) the CCF was totally evacuated. Lastly, for the CCF SI it was concluded that the horizontal evacuation was faster than the evacuation using the lifts.

Full textSuicide is a complex public health problem resulting from the interaction between biological, psychological and sociocultural factors. It is a phenomenon with an enormous socioeconomic burden, since annually, according to the World Health Organization, about one million individuals commit suicide worldwide. That number corresponds to half of all violent deaths recorded and the tendency is to increase. Attempted suicide is defined as an act carried out by an individual who seeks his own death, but for various reasons, usually unrelated to the individual, results frustrated. The most important predictive factor of suicide is attempted suicide. So attempts, even those objectively devoid of gravity, should not be undervalued. It is considered that for every person who commits suicide, 20 or more make an attempt. Thus, the analysis of the events and the profile of patients who attempt suicide and the analysis of associated risk and protective factors help assess the vulnerability of an individual or a population, in relation to suicide, allowing for preventive action. This study, of retrospective basis, of the period between 1 January 2010 and 31 December 2014, has been based on the statistical survey of data from medical records of the individuals over 18 who resorted to the Emergency Department of Hospital Sousa Martins, ULS Guarda, EPE, for whom it was deemed an attempted suicide event, prior to clinical evaluation. The aim of this work was to analyze the profile of the patient who resorts to the ER for attempted suicide, on regard, for example, to their sociodemographic and clinical variables.

Full textThis report is part of the curricular unit "Internship" inserted in the Integrated Master in Pharmaceutical Sciences of the University of Beira Interior (Covilhã), being part of the study plan for the 2nd semester of the 5th year. This work is structured in three chapters and refers to the culmination of a whole period of learning and specialized activities in the internships of hospital and community pharmacy, as well as a research work in the hospital pharmacy that intends to study the problem of the medication waste disposal in several inpatient services with single daily dose and single dose, in view of the increasing number of medication in these services and possibly find solutions towered a necessary reduction of costs and waste. The first chapter refers to the research component, which was developed at the Pharmaceutical Services of Hospital Sousa Martins, at the Local Health Unit in Guarda, Portugal, aiming at the analysis of the medication returned to the pharmaceutical services in individual unit doses in various clinical and inpatient services. The management of hospital pharmaceutical services is undoubtedly one of the major challenges and competencies of hospital pharmacists, whom are responsible for the quality, efficacy and safety of drugs. Thus, while the implementation of individualized distribution systems for inpatient services is a pharmaceutical intervention that helps rationalize drug distribution, decrease medication errors, and decrease therapy costs. However, there are still problem with the returns of drugs distributed in this way. This has been constant problem in the reality of pharmaceutical services for some time and has become more and more relevant in recent years. Thus, at a time of constant budget constraints and in which health and the national health service becoming underfunded. The possibility of this type of analysis on drug control waste over a period of ten months (September 2018 to June 2019). Could identify the volume of returns, what type of return / waste are more significant, what are the representative disposed pharmaceutical forms and drug reuse or disposal as waste by recording and analyzing the daily information collected from all drugs returned through the unit dose delivery process from the various inpatient services. This study also aims to promote a multidisciplinary discussion of the problem, defining a set of practical measures, which aim to improve behaviors, procedures and attitudes between all health professionals responsible for handing use drugs, thus adopting a proactive stance in their daily practice in order for safety use of returned drugs and achieving cost savings while maintaining the hospital medication circuit. The second chapter of this report refers to the experience in hospital pharmacy, carried out at the Pharmaceutical Services of the Cova da Beira University Hospital Center, between September and November 2018, under the guidance of Dr. Olímpia Fonseca, which intended to focus on the main work areas or skills pharmacist have as well as the legal aspects intrinsic to the hospital pharmacy practice. The third and final chapter of this report deals with the internship component in community pharmacy, which was held at the Sé pharmacy in Guarda, Portugal under the guidance of Dr. Maria João Grilo and Dr. Cláudia Gomes, this internship was held between November the 2018 and January 2019, accounting for a total of 800 hours. This chapter describes the various responsibilities and activities assigned to the pharmacist in the community context, as well as some particularities related to the legal and financial framework of this aspect. This report also presents the various works presented at congresses and interviews and which aim to discover new perspectives and approaches to this theme.
