Abstract:
Abstract Hospitals are the type of buildings which need development and expansion continuously so it can achieve all the demands of the progress in the medical field, medical devices, and the updates in the infrastructure of the hospitals. Due to the spread of many bacteria that are related to the construction and development process, many countries had to take into considerations many strict precautions to reduce the danger of the infection spread. These precautions are important either inside the building of the hospital or outside, so the maximum reduction of infection spread should be reached as there are many patients suffer from lack of immunity. This research aims to reach a rational design criterion for hospital designers which gains the required demands to combat the infection spread during construction and development process meanwhile continuing to present the medical services. It is strongly recommended to introduce flexible structural module that can be reused in different activities. Also, isolation of patient’s spaces during construction becomes a mandatory manner. Moreover, it is advised to design an adaptable ventilation system which facilitates different sectors separation, either patient, medical stuff, or workers paths.

Abstract:
The construction costs of hospital buildings are relatively high due to the need to fulfill their complex functions and avoid mishaps. In this context, this study aims to minimize the total construction costs of hospitals while still satisfying the special architectural, practical, and structural requirements specified by design codes. To this end, 48 design alternatives with two floor systems (flat slabs with and without drop panels), three column spacings, and eight concrete grades were optimized using genetic algorithms provided by Palisade Evolver. The objective function included the materials and labor costs per square meter of the floor plan. The decision variables involved the concrete dimensions and steel bars of floors and columns. The hospital buildings were subjected to gravity, earthquake, and wind loads to thoroughly examine the realistic loading conditions. The design was performed in accordance with the Egyptian code for the design and construction of concrete structures and the Egyptian guidelines for hospitals and healthcare facilities. The results revealed that using low-strength concrete, and flat slabs without drop panels could achieve the best design. The slab thickness had a governing impact on the total cost of both floor systems.

Healthcare facilities are among the most complicated facilities to plan, design, construct and operate. A new breed of hospitals is considering the impact of the built environment on healthcare worker productivity and patient recovery in their design, construction, and operation. A crucial subset of healthcare facilities are children's hospitals where the consequences of poor building system design and performance have the potential to seriously impact young lives with compromised health. Green facilities are not always pursued: they are perceived as difficult to build and costing more than equivalent conventional hospitals. This study explored the design process of the Children's Hospital of Pittsburgh of the University of Pittsburgh Medical Center (UPMC) and Penn State's Hershey Medical Center Children's Hospital to understand the critical steps and processes for green children's hospital design. Producing a series of process maps that identify the key characteristics in the complex design requirements of a green children's hospital, this paper reveals the importance of design process to design quality. More broadly, this research will help future project teams meet the complex design requirements of green children's hospitals.

Abstract:
Healthcare constructions are public buildings that necessitate technical, healing and medical conditions, require common action from a number of disciplines and accommodate differing scales within themselves. Recently, these constructions have been a field of study whose momentum have continued to speed up for varying disciplines in terms of healing factors. This condition can be evaluated in terms of the basic requirement of “well-being” for each individual, the fact that the definition of health has become more comprehensive and the changes on spatial requirements in accord with developing technology. The definition by World Health Organization (WHO) will be distinctive for designers. WHO stated that “Health is not only protection from diseases and microbes but also a state of well-being physically, mentally and socially.” Furthermore, excitement, hope and other positive emotions that can be conveyed to human soul through design can be viewed as the basis of health philosophy constructed on the condition of “well-being” Even though the concept of health involves a comprehensive definition, its perception by users indicates change over time and it can be considered that this perception is far from a holistic approach in parallel with the definition of health. Healthcare constructions include places that have negative connotations in the conscious of the society. The design of modern healing places as livable spots, environments to where feelings of belonging can be attached and environments of relaxing and healing therapeutics has become a delayed requirement. This will only start with understanding the role of place on healing. Hospitals can be regarded as type of constructions that are convenient to healing reading. Therefore, the efforts to create theoretical background have been put forward in this study in order to evaluate the relation between healthcare constructions and perception of places to provide reading over hospitals. The study will mention hospital buildings in the historical process and changes on the healing perception of people. It is impossible to design the study independent of users in this healing reading from past to present. With the evaluation carried out in this respect, today’s spatial construction perception will be evaluated over The Royal Children’s Hospital, Melbourne, Australia and providing recommendations on construction design is targeted. PurposeGiven the premise of de facto universal standards for FM, this paper aims to investigate development of facilities management (FM) at an Albanian and a Norwegian university hospital through examination of two hypotheses: the university hospital has recognised FM and established a designated FM organisation (H1) and the university hospital provides adequate food and catering services at ward kitchens and buffets (H2). Design/methodology/approachThis is an exploratory and descriptive comparative case study based on a diverse cases’ designs. FindingsThere is limited and strong support for H1 at the Albanian and Norwegian university hospitals, respectively. Both the Albanian and the Norwegian university hospitals rely on in-house production of facilities services, but the Albanian university hospital has outsourced food and catering services. FM and provision of facilities services are deeply integrated within the Norwegian university hospital’s core activities. There is also limited and strong support for H2 at the Albanian and Norwegian university hospitals, respectively. Hence, the Albanian Ministry of Health and the Albanian university hospital’s top management have a comprehensive, but not impossible, task, if the aim is to catch up with the Norwegian university hospital concerning FM. Research limitations/implicationsThis is an exploratory and descriptive comparative case study. Large N studies should be carried out both in Albania and Norway and preferably also in other countries to corroborate and develop the findings. Originality/valueThis is the first comparative study of FM at an Albanian and a Norwegian university hospital.

Abstract:
Purpose In the past decades, public hospitals in Thailand have developed gradually and been characterized by an incremental development of hospital facilities. First, this study aims to investigate the factors that have caused the incremental development and how such development has affected the hospital’s architectural layout. Second, the paper assesses the functional quality of nonclinical areas in the Maharaj Hospital to identify space management problems. Design/methodology/approach The first part of the study is based on a literature review of the Thai health-care landscape. The second part includes the functional quality assessment of nonclinical areas, walk-through observations and documentation. Obtained data were synthesized using building quality method and measurement criteria and analytical drawing techniques for design assessment. Findings The first part identified three factors: the lack of local general practitioners, the limited number of public hospitals and the implementation of Thailand’s universal coverage scheme. These factors have resulted in a dramatically high number of patients in public hospitals. The second part identified problems regarding poor accessibility, a low level of spatial flexibility and poor spatial orientation. These problems are related to a lack of appropriate strategic space planning and lack of integration of the Thai culture into hospital design processes. Practical implications An identification of space management problems is a prerequisite to the improvement of hospital facilities. Originality/value This paper presents the first study of space management problems concerning nonclinical areas in Asian hospitals.

Technological progress has had negative effects as well as positive effects if it affects the life industry and the entire ecosystem significantly through the great consumption of natural resources, and here the construction sector in general and the health sector, in particular, have a role in this. From the perspective of keeping pace with technological development, responding to environmental changes, and paying attention to hospital environments (especially since the emergence of modern epidemics), and because the construction sector is the largest consumer of energy in the world, which made international organizations move towards creating a sustainable environment in the construction of hospital buildings by reducing energy consumption. This research focused on studying the components and principles of sustainable design for hospital buildings and the environmental, economic, health, and social benefits of sustainable development in the healthcare industry. In addition to the research objective, which is to build a model as a guide to guide health care officials interested in applying sustainable design principles in hospital design, to achieve an ideal sustainable hospital environment. To achieve this goal, a comprehensive theoretical framework was built by adopting a descriptive and analytical approach and extracting the most important vocabulary and effective indicators for sustainable design in hospitals.

Abstract:
Purpose This exploratory study stems from research conducted between 2015–2018 focussing on dementia-friendly design (DFD) in hospitals (Grey T. et al. 2018). Specifically, this study focusses on facilities management (FM) staff in Irish hospitals to gain a preliminary understanding of the level of knowledge and engagement of FM in the implementation of dementia-friendly hospital (DFH) design. Design/methodology/approach A mixed-methods approach based on a series of ad hoc semi-structured interviews, and an online survey. The aims were, namely, assess the extent of FM engagement in hospital works; measure the level of awareness regarding DFD; and identify facilitators and barriers to DFD in hospital settings. Participants (74) comprised FM staff in 35 Irish acute care hospitals. The research findings are based on thematic analysis of ad hoc semi-structured interviews (participants, n = 4) and survey responses (participants, n = 13). Findings While FM staff reported to possess important knowledge for building DFH, they also mentioned a lack of engagement of FM in design processes and hospital works. Practical implications The research has gained insight into the role of FM in promoting a dementia-friendly approach. Lack of or poor engagement of FM in design processes and hospital works means not fully tapping into rich expertise that would be invaluable in the development, implementation and maintenance of DFH. Universal design is a key driver for facilitating their engagement in the design, implementation and maintenance of DFH environments. Originality/value This is the first study exploring the role of FM in supporting a DFD approach in acute care hospitals.

O objetivo central foi a análise da relação da criação e estruturação do Hospital de São Sebastião com o debate sobre a causalidade das doenças, em especial a febre amarela, e com o desenvolvimento da medicina pasteuriana. Desta forma reconstitui o processo de criação da instituição, suas características arquitetônicas, e seu papel no processo de estruturação dos aparelhos institucionais, no campo da saúde pública, especialmente no cenário das epidemias. Analisa a arquitetura da instituição, relacionando-a com os debates existentes na época sobre arquiteturas hospitalares e com as correntes médicas hegemônicas na época.

Abstract:
Construction flexibility inside a hospital building serves as an enabler for a health service to clinically progress. This research sought the opportunity of extending flexible design approaches by consolidating established knowledge into a refreshed flexibility pattern mapping. The resulting understanding provided a base for developing design propositions as examples of apt, flexible solutions. This research contributed to guiding flexibility towards enhanced practices and it partially addressed the softening of the logarithmic growth of financial burden for health services. Also, the research provided a model for a change-ready building fabric, allowing responsive opportunity towards the accelerating pace of clinical development.

Children dread to get into the harsh technological environments of today's medical environments. Hospitals are constantly evolving to keep pace with the latest medical technologies. Additions or refurbishments or a totally new set-up usually focuses on the technological requirements rather than the human element of such an undertaking. The Riley Hospital for Children in Indianapolis presents an architectural opportunity to incorporate psychology of the environment into the design and construction of a new outpatient unit. By balancing the technological requirements with the physical and pschological needs of a child patient, one can create a healing environment more conducive to a rapid recovery. This thesis explores one of the possibilities of creating such an environment.

This study has presented a history of the early development, design, and construction of the Marion, Indiana Branch of the National Home for Disabled Volunteer Soldiers. A general history of the institution of the National Home for Disabled Volunteer Soldiers provides the context for the establishment of the Marion Home. A historical review of the selection of the Marion site, its layout plan, design and construction of buildings, and landscape design between 1888 and 1900 was then provided. The development of the Marion Home is next compared with the 1890's developmental history of the Central Branch in Dayton, Ohio and the first ten years of the Danville, Illinois Branch in order to assess commonalities and differences in construction among three homes during the same period. In the conclusion, the evidence suggests that the layout and architectural design of three homes had many similarities during the 1890s and the turn of twentieth century.

Abstract:
In 1897 John Sibbald, Commissioner in Lunacy for Scotland, stated that ‘the construction of an asylum is a more interesting subject of study for the general reader than might be supposed.’ This thesis traces the development of the public asylum in Scotland from 1781 to 1930. By placing the institution in its wider social context it provides more than a historical account, exploring how the buildings functioned as well as giving an architectural analysis based on date, plan and style. Here the architecture represents more, and provides a physical expression of successive stages of public philanthropy and legislative changes during what was arguably one of the most rapidly evolving stages of history. At a time when few medical treatments were available, public asylum buildings created truly therapeutic environments, which allowed the mentally ill to live in relative peace and security. The thesis explores how public asylums in Scotland introduced the segregation or ‘classification’ of patients into separate needs-based groups under a system known as Moral Treatment. It focuses particularly on the evolving plan forms of these institutions from the earliest radial, prison-like structures to their development into self-sustaining village-style colonies and shows how the plan reflects new attitudes to treatment. While many have disappeared, the surviving Victorian and Edwardian mega-structures lie as haunting reminders of a largely forgotten era in Scottish psychiatry. Only a few of the original buildings are still in use today as specialist units, out-patient centres, and administrative offices for Scotland’s Health Boards. Others have been redeveloped as universities or luxury housing schemes, making use of the good-quality buildings and landscaping. Whatever their current use, public asylums stand today as an outward sign of the awakening of the Scottish people to the plight of the mentally ill in the nineteenth and early twentieth centuries.

Abstract:
Since the 1960s, researchers have been exploring how the design of the built environment impacts the health and well-being of occupants and users. By the 1980s, further research began to focus on healthcare facilities in particular and how design could influence patient healing and medical staff performance (Alfonsi, 2014). Evidence-Based Design (EBD) is "the process of basing decisions about the built environment on credible research to achieve the best possible outcomes" (CHD, 2016). The desired outcomes of Evidence-Based Design recommendations include improvements in the following: patient healing, patient experience and comfort, medical staff performance, and medical staff job satisfaction (CHD, 2017). Extensive research has been done on the subject of EBD; however, the question remains whether or not the latest research findings are being utilized by the design and construction industries in practice. The purpose of this research is to determine whether or not the latest scientific knowledge and research findings are being implemented into hospital renovation projects by the healthcare design and construction industries. A list of recommendations from existing EBD literature was compiled. Construction documents from 30 recent healthcare facility renovation projects across the United States were then obtained and analyzed. The findings indicate that EBD recommendations are being adopted in practice at consistently high levels. These findings also reveal that there are still areas of potential improvement which could inform those who influence or determine building and design codes, standards, and guidelines. The results are instructive to owners, designers, and contractors by providing a glimpse into how well the industry is recognizing and implementing known best practices. The findings likewise open up new opportunities for further research which could lead to additional improvement in the healthcare facilities of the future.

A crucial action of COVID-19 combat is the quick design and building of makeshift hospitals (MHs). Although adopting building information modeling (BIM) promotes the digitalization and communication of design collaboration, data security vulnerabilities (e.g., lacking traceability and transparency) are detected and have inevitably impeded the efficiency and productivity of the MH project. Such problems often lead to rework and unnecessary disputes, wasting valuable time on projects requiring ultra-fast construction speed. The emerging blockchain technology offers an immutable and traceable collaboration environment. However, limited studies have integrated blockchain in the BIM design process, especially design in emergency projects like MH. Therefore, this paper proposes a blockchain-enabled collaboration (BEC) framework for fast and secure BIM design. The framework is illustrated in an actual MH project in Hong Kong, and results show that: (1) it supports secure and automated BIM data exchange and (2) it saves 23 % of the time in a design coordination case

<p>In order to control the spread of the COVID-19 epidemic across the country, China has used all available resources to build infectious disease hospitals in various ways. These hospitals include three modes and adapt to different disease levels: temporary emergency hospitals; makeshift hospitals by transforming public buildings; and existing general wards transformed into infectious wards. Through the practice of several projects, on the basis of the original standard system, China urgently issued a series of relevant standards and guidelines to guide the construction of temporary hospitals. As one of the earliest cases of temporary emergency infectious disease hospital, the Thunder God Mountain Hospital adopted a prefabricated modular design concept in plan design, plane design and component design, and also combined the application of Building Information Modeling (BIM) and Computational Fluid Dynamics (CFD) technology. Based on industrialized module processing and manufacturing, combined with an efficient on-site construction management system, the problem was solved of completing the construction in a very short time, which played a key role in controlling the epidemic situation.</p>

Abstract:
The waiting time is a major challenge for government hospitals around the world, particularly in developing countries, due to inefficient design and limited facilities. This waiting time causes long queues, especially in service providing facilities. Hence, queuing is considered a key performance indicator when evaluating the performance of healthcare facilities. The outpatient department (OPD) is the main division of a hospital that handles a large number of patients daily. This research aimed to provide solutions for minimising waiting time in OPD premises through efficient planning and designing strategies. This study applied the queuing theory for two case studies to analyse the waiting time of the patients at the OPD. Hospital records and field observations were used to gather data. Further solutions for minimising the waiting time were identified using semi-structured interviews with hospital management and a questionnaire survey with patients at OPD. Field observations revealed that there were long waiting queues and long waiting times at the registration counter and the consultant rooms. Due to increased demand for OPD services, unnecessary arrivals, a lack of resources, and patients’ ignorance of OPD procedures were identified as causes of overcrowding. Further, to minimise waiting time at the OPD, it should leverage the efficient designs with properly placed inquiry counters, walkways, and directions; provide adequate facilities such as a spacious waiting area, and restructuring should be implemented. The findings of the research mark valuable insights into government hospitals, and the proposed solutions will be useful for hospital management.

Abstract. The ambitious KSA 2030 Vision has among others targeted the deployment of renewable ‎energies to meet KSA’s primary electric load in the residential, commercial, and industrial ‎sectors. Within this framework, the authors have evaluated the economic ‎and technical feasibility of using a combination of solar PV systems and central energy ‎storage systems in the health sectors. The combination of the two sources means ‎reducing gas emissions, and increasing the instantaneous power from renewable energy ‎sources while using energy storage to counterattack the intermittency of solar PV energy ‎that is known to cause stability issues for the grid. In addition, it reduces the dependency ‎on standby diesel generators that emit high pollution levels. ‎The paper addresses the available energy storage devices and the design of a hybrid solar ‎PV system combined with battery-stored energy. Through the study, it was found that ‎up to 15 percent of the electric consumption can be provided by the solar PV system. It ‎was found that the cost of energy competes with conventional electricity. In addition, the ‎cost of storage devices has a significant impact on the cost of energy. Moreover, low payback ‎periods were obtained for some systems.

Healthcare architecture has become an increasingly specialized field, marked by a complex interaction between people, operations and the physical environment and an ever changing landscape of regulation and reimbursement. Patient safety is often considered in a behavioral context – what can someone do differently to improve outcomes? However, as a complex system of interactions, patient safety is better advanced through a systems-thinking lens of Human Factors/Ergonomics (HFE). Attaianese and Duca commented on the use of HFE principles in design, stating that, “when the system is the built environment, the systemic approach requires that designers move from an attention exclusively reserved for building functions towards the set of actions that users actually perform and that building has to support.” This paper reports the development of a proactive Safety Risk Assessment (SRA) tool which will contribute to the 2014 Facility Guidelines Institute (FGI) Guidelines for the Design and Construction of Hospitals and Outpatient Facilities. Six hazard areas have been considered as underlying conditions to injury or harm in the design of healthcare environments: 1) Hospital Associated Infections, 2) Falls/Immobility, 3) Medication Safety, 4) Patient Handling, 5) Security, and 6) Behavioral Health/Psychiatric Injury. These categories have been developed using iterative cycles of Delphi and nominal group methods to achieve consensus of categories and question sets for inclusion in the SRA.

Due to the safety awareness arisen from natural and human-caused disasters, robustness design of building is increasingly important to ensure the stability of the building and to prevent progressive collapse. For this reason, the robustness design of innovative construction technologies such as modular construction may be essential due to its relative novel structural form and numerous joints among modules. Particularly in Singapore, Prefabricated Prefinished Volumetric Construction (PPVC) has been highly promoted in residential and commercial buildings, hostels and hospitals to boost the construction productivity and quality as well as to reduce the reliance on foreign workforce. PPVC offers high quality and efficiency because most of the finishes and mechanical and electrical services are manufactured and installed together with the modules in factory, before sending for on-site assembly. To maximize the productivity of PPVC, modular design standardization and repetition can be improved by going for high-rise. Nonetheless, there are limited studies on the robustness of PPVC high-rise building and its behavior under progressive collapse remains uncertain. Therefore, this paper investigates the robustness of steel PPVC high-rise building under column removal scenarios by conducting non-linear numerical analysis. The effects of joint design and diaphragm action between modules are studied to ensure continuity of horizontal and vertical tying. This paper provides insight on the behaviour and alternative path for load transfer under column removal scenario for future design guideline of robustness PPVC building.

The Public Works Authority (Ashghal) of Qatar is responsible for countrywide infrastructure and public building projects starting from their planning stages and through design, procurement, construction, operation, and maintenance stages. This includes highways, local roads, surface water drainage networks, foul sewers, treated sewage effluent systems, schools and hospitals. Considering the large number of projects with construction activities ongoing at the same time, it is a challenge to develop a unique and standardized tool for monitoring of environmental management performance of each project, each department or Ashghal wide. Being a supportive department to all other Ashghal departments, with a main role of ensuring the conformity of projects to environmental regulations and standards, Quality and Safety Department (QSD) of Ashghal has started several initiatives to improve environmental management performance of projects. This paper provides a brief information about some of those initiatives, such as a portal for monthly reporting of environmental management performance (EPMS), a unique annual award which was developed to increase awareness for sustainable practices in construction and assist in guiding minimization and/or reduction of adverse environmental impacts caused by construction projects (Ashghal Green Award); and a list of Ashghal approved third-party environmental service providers which will be used as a vendor list in all Ashghal projects. In addition, Ashghal QSD chemical and microbiological laboratories as well as mobile noise and air quality monitoring trucks serve to support projects departments to improve environmental impacts resulted from construction activities on the environmental sensitive receptors located within or nearby the project.
