Abstract:
Conflicting results have been published considering the role of head-up tilt test (HUTT) positivity as a prognostic factor in patients with hypertrophic cardiomyopathy (HCM). The relationship between HCM patients' genotype and their HUTT results has not been previously reported.The aim of this study was to evaluate patients with HCM and their HUTT results in regard to its value for outcome prediction and to investigate the relation of patients' genotype and their HUTT results.Seventy-four (51 ± 15 years; 42% women; median follow-up 72 months) HCM patients were divided into two groups based on their HUTT results and were retrospectively analyzed. In 67 (90.5%) subjects included in the analysis, next-generation sequencing-based genomic testing was performed. A composite end point of unexplained syncope, heart failure hospitalization, and death was defined.A total of 14 patients (18.9%) had positive HUTT (HUTT+), whereas 60 (81.1%) had negative HUTT (HUTT–). Except for the New York Heart Association functional class (p = 0.01), both groups had similar characteristics. Positive genotype was evenly distributed between the two groups. Composite end point occurred in 5 patients (35.7%) in HUTT+ group versus 14 (23.3%) patients in HUTT– group (p = 0.33).We did not find a relationship between HUTT results and long-term outcome. We found no association between HUTT results and genotype.

Abstract:
Aim: The aim of this study was to investigate the association of autonomic nervous system abnormalities on head-up tilt table test (HUTT) with generalized joint hypermobility, expressed by Beighton score (BS). Methods: This was a prospective study that included 115 consecutive patients (91 females; mean age 34.35 ± 14.11) referred either for the HUTT or testing of the cardiovascular autonomic reflexes together with HUTT. Generalized joint hypermobility was evaluated according to the BS system after which HUTT was performed. Clinically significant BS was considered if ≥4. Results: Fifteen patients (15.1%) had BS ≥4. Results of the HUTT were normal in 58 (50.4%) patients and in 57 (49.6%) patient HUTT was abnormal. Fifteen (13.0%) patients fulfilled criteria for orthostatic hypotension, 30 (26.1%) for reflex syncope and 21 (18.3%) for postural orthostatic tachycardia syndrome. Patients with pathological findings on HUTT had significantly higher BS compared to patients with normal HUTT (median 1 vs. 0, p = 0.001). There was a significant association between participants with BS ≥4 and pathological HUTT (χ[1] = 6.392, p = 0.011). Results of the multivariate regression analysis revealed that increase in the BS is associated with the increased likelihood of HUTT pathology (Exp[B] 1.44, 95% CI 1.084–1.922, p = 0.012), while increase in age is associated with lower risk of HUTT pathology (Exp[B] 0.968, 95% CI 0.939–0.998, p = 0.036). Conclusion: There is an association between autonomic nervous system abnormalities on HUTT test and generalized joint hypermobility.

Abstract:
The paper presents possible applications of entropy measures in analysis of biosignals recorded during head up tilt testing (HUTT) in patients with suspected vasovagal syndrome. The study group comprised 80 patients who developed syncope during HUTT (57 in the passive phase of the test (HUTT(+) group) and 23 who had negative result of passive phase and developed syncope after provocation with nitroglycerine (HUTT(−) group)). The paper focuses on assessment of monitored signals’ complexity (heart rate expressed as R-R intervals (RRI), blood pressure (sBP, dBP) and stroke volume (SV)) using various types of entropy measures (Sample Entropy (SE), Fuzzy Entropy (FE), Shannon Entropy (Sh), Conditional Entropy (CE), Permutation Entropy (PE)). Assessment of the complexity of signals in supine position indicated presence of significant differences between HUTT(+) versus HUTT(−) patients only for Conditional Entropy (CE(RRI)). Values of CE(RRI) higher than 0.7 indicate likelihood of a positive result of HUTT already at the passive phase. During tilting, in the pre-syncope phase, significant differences were found for: (SE(sBP), SE(dBP), FE(RRI), FE(sBP), FE(dBP), FE(SV), Sh(sBP), Sh(SV), CE(sBP), CE(dBP)). HUTT(+) patients demonstrated significant changes in signals’ complexity more frequently than HUTT(−) patients. When comparing entropy measurements done in the supine position with those during tilting, SV assessed in HUTT(+) patients was the only parameter for which all tested measures of entropy (SE(SV), FE(SV), Sh(SV), CE(SV), PE(SV)) showed significant differences.

Background: Head-up tilt table testing (HUTT) is time-consuming and associated with increased patient morbidity. Hemodynamic changes that occur during the early phase of HUTT may be predictive of neurocardiogenic syncope. Methods: A retrospective chart review was performed in 119 consecutive African Americans ( 57 ± 19) who underwent HUTT for evaluation of syncope of unknown etiology. Positive responses were defined as the development of symptoms linked with a systolic blood pressure (BP) <90 mm Hg, heart rate <50 b.p.m. or sinus arrest >3 s. Hemodynamic variables during the passive phase of HUTT were analyzed and results were then classified as a function of various predictors. Results: Sixty-two subjects (52%) had positive HUTT, and 57 (48%) had negative HUTT. Early changes in BP variables from baseline significantly predicted HUTT responses (p < 0.05). There was also a significant interaction between age and BP. An algorithm based on age and BP was developed which had positive and negative predictive values of 67.7 and 93%, respectively, with an accuracy of 79.8%. Conclusion: A novel algorithm utilizing the patients' age and changes in both systolic and diastolic BP during the early phase of HUTT enables the prediction of HUTT results without the use of vasoactive stimulation, allowing for rapid diagnosis, decreased patient morbidity and reduction in costs.

Abstract:
Syncope is commonly encountered in daily clinical practice. Depending on its etiology (benign or life-threatening conditions or environmental triggers), syncope can be neurally mediated (reflex), cardiac, or orthostatic. Furthermore, neurologic disease can cause symptoms that mimic syncope. However, there is limited research on neurally mediated syncope (NMS), which is considered a benign disorder, and close follow-ups are rarely performed. NMS can cause serious clinical events, including severe trauma and car accidents. The head-up tilt test (HUTT) is the gold standard for diagnosing NMS; however, its clinical significance remains unknown, and its relevance to NMS prognosis requires further research. This retrospective study aimed to assess the clinical significance of the HUTT for NMS. We reviewed the charts of 101 patients who underwent HUTT at Tokai University Hospital in Japan between January 2016 and March 2019. During the HUTT, 72 patients (69.2%) experienced syncope. Patients were followed up for 886.1 ± 457.7 days (interquartile range: 518–1293 days). The syncope recurrence rate was 16.9%; however, no significant difference was observed between the two groups (HUTT positive vs. negative) (13.8% vs. 18.1%, p = 0.772). Four of 29 (13.9%) and two of 72 (2.8%) patients in the negative and positive HUTT groups, respectively, experienced cardiac events (p = 0.019). Negative HUTT results may assist in anticipating unexpected clinical events within a few years. A negative HUTT result may allow us to reconsider the NMS diagnosis based on clinical information. Close outpatient follow-up of patients with negative HUTT results is warranted.

Background: Electrocardiography (ECG) and the head-up tilt test (HUTT) are vital in clinical work-up in children with vasovagal syncope (VVS). Ventricular repolarization parameters (QT) measured during the HUTT can be indicative of electrical instability; however, these parameters are not frequently assessed. This study aimed to investigate if ventricular repolarization parameters measured during the HUTT could be indicative of future ventricular arrhythmias in children with syncope. Methods: The shape and amplitude of the T-wave and parameters of the repolarization period (QT, QTpeak, Tpeak-Tend) were evaluated in a resting ECG performed on the first day of hospitalization and in ECGs performed during three phases of the HUTT. Results: In the after-tilt phase of the HUTT, 19/30 children displayed a change in T-wave morphology. QTc was significantly longer in VVS I compared to that in VVS II patients, but not in the controls (p = 0.092). Conclusions: We need further follow-up studies to establish the clinical importance of abnormal dynamics of the repolarization period in children with VVS and negative HUTT. Therefore, children with abnormal T-wave refraction and prolonged duration of the TpTe should remain under the care of a cardiological outpatient.

The High Definition Systems Usability Test Tool (HUTT) was developed to demonstrate and assess different human-computer interface (HCI) concepts for generic operator console positions within an advanced naval tactical display environment. To facilitate a “rapid prototyping” approach to interface development, HUTT was developed as a general purpose tool. The HUTT can be used by the human factors engineer as a rapid prototyping tool enabling quick construction and evaluation of alternative interface concepts. Changes to the organization and operation of a prototype can be made quickly, ensuring that more time can be spent on evaluation as compared to development. The demonstration will focus on the uses and operation of the HUTT. Demonstration participants will be shown how to develop, install and modify the OSF/Motif compliant graphics (pull-down menu structure and window contents) and the functionality of the HUTT to reflect changes in interface concepts.

Background. Head-up tilt testing (HUTT), a well-established tool in the diagnosis of vasovagal syncope, is time-consuming, and every provoked vasovagal reaction may result in consolidating the reflex mechanism. Therefore, identification of parameters that could shorten the duration of HUTT and prevent fainting is desirable. Quantitative complexity theory (QCT) may provide holistic information on the cardiovascular reaction in HUTT. The aim of the present article was to evaluate the prognostic value of complexity in comparison with traditional haemodynamic parameters (HR and BP) in predicting the HUTT outcome. Methods. Eighty-one healthy volunteers (74 men; mean age: 37.8 years) were included in this retrospective analysis of data collected within the project realized in Department of Cardiology and Internal Diseases, Military Institute of Medicine between January 2012 and October 2014. The subjects underwent HUTT, with beat-to-beat haemodynamic monitoring with a Niccomo™. The chosen haemodynamic parameters (including BP, HR, stroke volume, cardiac output, systemic vascular resistance) have been used in complexity analysis. Results. HUTT was positive in 54 (66.7%) study participants. The values of complexity were already higher in fainting subjects than those were in nonfainting ones 300 s before HUTT termination (HUTT_end), with a significant upward trend starting 150 s before (pre)syncope. An area under the curve (AUC) over 0.700 was observed for complexity from 120 s before HUTT_end, with a sensitivity of 63% and specificity of 78% at this time point. The prognostic value of complexity was superior to that of the HR and mean arterial pressure (MAP). Conclusions. Complexity has been shown to be a sensitive marker of cardiovascular haemodynamic response to orthostatic stress and proved to be superior over HR and BP in predicting HUTT outcomes.

Xenophon’s lexical choices reflect the influence both of this ‘Great Attic’ (which developed into koine Greek) and of the non-parochial historiographical tradition inaugurated by Thucydides.

Abstract:
The decision making process of avalanche hazard forecasting was studied during the ski season of 1995 at Mount Hutt. The objective was to derive the main contributing factors of avalanche forecasting and then assess the potential of Geographic Information Systems (GIS) as a tool to assist with the decision making process. A snow distribution model was developed utilising the contributing factors that were identified. Interviews were conducted with the three principle avalanche forecasters from the Mount Hutt Ski Patrol. Each forecaster was presented with a series of storm scenarios, and then asked what information they wanted to know and when they wanted it, while making their avalanche hazard assessment and forecast. The information requested by each forecaster were very similar. However, there was a distinct difference with how each forecaster interpreted the available information. They often used different approaches when assessing snow stability, but ultimately arrived at the same conclusion about the potential avalanche hazard. The key contributing factors identified in this research were wind speed and direction, snowfall intensity, the amount of new snow, and the surface conditions upon which the new snow was being deposited. A GIS model using Grid was developed to assess the spatial distribution of snow during two storm scenarios. The model appeared to work well, given that it was only designed to assess the potential of GIS. The resulting output of the model would have provided the forecasters with a more accurate estimate of snow distribution. Forecasters have to estimate the spatial distribution of snow and its stability, based on experience and know knowledge. GIS has the potential to provide accurate estimates of this distribution, that could ultimately assist conventional avalanche forecasters.

Abstract:
A study is made of seismic wave propagation at the edge of sedimentary basins and valleys, and the subsequent amplification of earthquake shaking that occurs. The aim of this thesis is to determine the geological and seismic conditions that control the occurrence and nature of basin-edge effects. In the first half of the thesis, elastic SH-wave propagation is investigated at the edge of a two-dimensional semi-infinite homogeneous layer above a homogeneous half-space, first with a geometrical ray-path and wavefront analysis and then with finite-element modelling of plane incident Ricker wavelets. It is shown that Love waves generated at an edge produce characteristic patterns of amplification across the layer in both the time and frequency domains. In addition, occurrences of localised amplification at the edge of the layer are categorised into three different classes based on their mechanisms of development. The Airy-phase edge effect occurs when the input frequency is close to the fundamental frequency of the layer; the wedge effect occurs above shallow sloping edges; and the basin-edge effect occurs above deep basins adjacent to steeply dipping edges. The second half of the thesis makes a case study of seismic amplification and edge effects in the Lower Hutt Valley. It is found that the anti-plane seismic response is characterised by multi-dimensional resonance across the full width, and the basin-edge effect and strong differential motion adjacent to the vertically-dipping Wellington fault. Love waves generated at each edge are the primary cause of such spatially varying ground motions. Modelling results are compared with an analysis of weak ground motions recorded on a dense linear array of instruments across the fault-bounded edge of the valley. A high degree of similarity is found. Discussion is also made concerning the use of the twodimensional elastic SH-wave analysis for predicting seismic response during strong ground shaking.

Abstract:
The development of distributed, reactive and collaborative services is quite challenging. Rapidly composing services for collaborative learning activities require some development methods and tools. This thesis presents an extension of the City Guide application, a platform that supports situated collaborative services developed by Surya Bahudar Kathayat his PhD thesis: <i>On the Development of Situated Collaborative Services</i>. The application was developed using the engineering method SPACE and its development tool Arctis. In the extension made, two new services instant messaging and group chat have be composed and integrated into the application. The two services have been identified as basic support services that are required for a true collaborative learning experience. Using the architecture of the application, the instant messaging and group chat services were developed as components that could be incorporated with other components within the application. The results show the instant messaging and group chat service as standalone functionalities that handle their own message routing within the application and hence did not require the use of any other messaging protocols. The results also show that by using this architecture with the necessary Arctis modifications, the City Guide application opens up for unexplored possibilities where new services can be rapidly developed and integrated.

Le ressort de celui de Dijon comprend la Côte-d’Or, la Haute-Marne et la Saône-et-Loire. Pour le personnel judiciaire, Cambacérès a choisi la continuité : les membres du tribunal d'appel sont d'anciens avocats au parlement de bourgogne, élus juges sous la révolution. D'adapter successeur du parlement de Bourgogne, la cour d'appel doit définir sa propre image. Les rapports avec l'exécutif ne sont pas faciles : conflits fréquents avec le préfet, tutelle pesante du ministre de la justice ; de plus, la cour se trouve durement ébranlée par chaque changement de régime politique. To create - one of the most important innovations of the judicial reform in year eight is the creation of twenty courts of appeal. The following departments: Côte d'Or, Haute-Marne and Saône-et-Loire fall within the competence of the court of appeal of Dijon. Concerning the judicial staff, Cambacérès chose continuity: court of appeal's members are former barristers of the burgundy parliament elected judges during the French revolution. To remodel - successor of the burgundy parliament, the court of appeal has to define its own image. Relations with the executive power are not easy: frequent conflicts with the prefect, tough supervision by the minister of justice. In addition, at each change of government, the court is highly shaken. To innovate - magistrates became state employees : - they are appointed by the minister of justice who chooses from three candidates proposed by the two chiefs of the court, - they wish to make a good career for themselves in magistracy that is to say climbing in the hierarchy within which the court of appeal enjoys and enviable position. Finally comparative study of court magistrates'carreers of Dijon, Pau and Rennes shows an obvious likeness

<p>This project originates from the need to provide seismic resilient solution for water supply to Wellington and Porirua. With other factors influencing the design, the pipeline crossing must withstand seismic loads including the rupture event of the Wellington Fault with movement of +/-6.5 m parallel to the river stream</p><p>The option study for the pipeline crossing concluded on using a bridge structure spanning the river and the geological Fault. This network arch bridge structure selected is provided with enough movement capacity to withstand the effects of the fault rupture movement without failure.</p><p>The length of the bridge structure is defined so to match the differential rotations between the supports to the allowable limits for the pipeline flex joints. To resist these seismic effects, the structure is provided with seismic restrainers that, at the same time as supporting the seismic load, provide enough rotation capability to accommodate movements on the foundations due to the Fault’s ruptureevent.</p>

Abstract:
This paper considers the first stage of Nova Huta New Town built near Krakow in the 1950s. In contrast to UK and US new settlements of the post war period it is a high density apartment block development which was ignored in the literature for more than half a century because its design, based on a system of streets, is in contrast with contemporary forms of development, either low density garden city or higher density free standing apartment blocks. A discussion of its neglect and the recent rediscovery of its qualities, both in Poland and by exponents of the US New Urbanism (part of the Urban Morphology spectrum somewhat neglected by ISUF) leads to a systematic investigation of the development, its influences and how this project conceived in a radically different political and economic context, matches or departs from the tenets of the Charter for the New Urbanism. The extent to which the context has determined the differences leads to a conclusion discussing the enduring qualities and contemporary relevance of inherited urban forms. This project shows a creative approach to the familiar scavenger hunt game. It involved the implementation of an iPhone application, HUNT, with Augmented Reality capability for the users to play the game as well as an administrative website that game organizers can use to create and make available games for users to play. Using the HUNT mobile app, users will first make a selection from a list of games, and they will then be shown a list of objects that they must seek. Once the user finds a correct object and scans it with the built-in camera on the smartphone, the application will attempt to verify if it is the correct object and then dis-play associated multi-media AR content that may include images and videos overlaid on top of real world views. HUNT not only provides entertaining activities within an environment that players can explore, but the AR contents can serve as an educational tool.
