Abstract:
Background: Antimicrobial resistance is a global public health threat. Integrated actions are necessary to reduce multidrug-resistant organisms (MDROs) in healthcare settings, including antimicrobial stewardship, infection prevention measures, and optimal environmental hygiene. We developed a project to improve hospital hygiene that involves 3 phases: (1) diagnostic, compounded by assessment of cleanliness and identification of barriers and facilitators for environment cleanliness improvement; (2) intervention, based on review of structure and processes followed by a training program focused on major weaknesses identified; and (3) evaluation, impact of the intervention assessment. Objectives: We performed group interviews to identify barriers and facilitators for improving environment cleanliness. Methods: The project was performed by the infection control team and the housekeeping manager in a 350-bed, private hospital located in the city of São Paulo (Brazil). Two group interviews were conducted, one involving supervisors and the other involving housekeeping cleaners. All professionals were invited to participate. A semistructured questionnaire was used to guide the discussion, which was compounded by the following topics: working process, availability of human and material resources, training on institutional norms and routines, perception regarding work conditions, and quality of cleanliness. Results: In total, 33 professionals attended the interviews: 12 were supervisors and 21 were housekeeping cleaners. The main facilitator identified was a good perception by the housekeeping team regarding the project. We identified several sets of barriers: (1) human resources, such as supervisor executing the cleaning, inadequate sizing of human resources in shifts, reduced scale on Sunday and holiday shifts, and lack of professional replacement for sick leave and vacation; (2) supplies and equipment, such as torn bed linen, insufficient mops, centralized and inadequate dilution of sanitizers causing delays and impacting quality of hygiene; (3) education, such as lack of training program perceived by supervisors (management) and housekeeping cleaners (basic procedures for cleaning) and knowledge regarding who cleans what; (4) motivation and relationships, such as supervisor perceptions that housekeeping cleaners are unmotivated, and this causes absenteeism. The team feels that they are disregarded by doctors, and they have relationship problems with nursing and hospital engineering staff. Also, they are afraid of being physically assaulted by coworkers. Finally, professionals reported the perception that the hospital is not clean enough and that this is related to the short time goals imposed on the staff. Conclusions: The main barriers identified were related to education strategies and management of human and material resources. The results will support the intervention phase.Funding: NoneDisclosures: None

Abstract:
AimTo explore indigenous communities’ responses to the COVID-19 pandemic and its consequences for maternal and neonatal health (MNH) care in the Peruvian Amazon.MethodsMamás del Río is a community-based, MNH programme with comprehensive supervision covering monthly meetings with community health workers (CHW), community leaders and health facilities. With the onset of the lockdown, supervisors made telephone calls to discuss measures against COVID-19, governmental support, CHW activities in communities and provision of MNH care and COVID-19 preparedness at facilities. As part of the programme’s ongoing mixed methods evaluation, we analysed written summaries of supervisor calls collected during the first 2 months of Peru’s lockdown.ResultsBetween March and May 2020, supervisors held two rounds of calls with CHWs and leaders of 68 communities and staff from 17 facilities. Most communities banned entry of foreigners, but about half tolerated residents travelling to regional towns for trade and social support. While social events were forbidden, strict home isolation was only practised in a third of communities as conflicting with daily routine. By the end of April, first clusters of suspected cases were reported in communities. COVID-19 test kits, training and medical face masks were not available in most rural facilities. Six out of seven facilities suspended routine antenatal and postnatal consultations while two-thirds of CHWs resumed home visits to pregnant women and newborns.ConclusionsHome isolation was hardly feasible in the rural Amazon context and community isolation was undermined by lack of external supplies and social support. With sustained community transmission, promotion of basic hygiene and mask use becomes essential. To avoid devastating effects on MNH, routine services at facilities need to be urgently re-established alongside COVID-19 preparedness plans. Community-based MNH programmes could offset detrimental indirect effects of the pandemic and provide an opportunity for local COVID-19 prevention and containment.

Abstract:
Motivation of workers is a field of interest to any institution. Motivated workers displaying high levels of satisfaction, or otherwise lack of dissatisfaction, have a positive impact on the overall work performance. The study aims to measure motivation levels and the influence of factors such as motivation-hygiene on satisfaction/dissatisfaction level among workers of social care institutions. A quantitative method of assessment is used. The sample consists of 110 workers. Results show a positive and statistically significant correlation between motivation factors and the levels of satisfaction with work. Responsibility is the selected factor with the biggest impact on satisfaction by the workers of the institutions surveyed. There is a positive but not statistically significant correlation between such factors as hygiene and lack of satisfaction at work. To the workers, the responsible supervisor is the factor with the biggest impact on the lack of satisfaction at work, while administrative policies are calculated as being r (Pearson) = -0.27 and sig = 0.761, which shows that they influence the dissatisfaction levels at work. Therefore institutions must pay attention to motivation strategies with the purpose of increasing satisfaction and thus decreasing dissatisfaction levels at work.

Abstract:
Objective: This evaluation examined the effectiveness and cultural compatibility of a community health worker (CHW) training programme on water, sanitation and hygiene (WASH) in Haiti. Design: Mixed-methods including pre- and post-course tests of WASH knowledge, focus group discussions and semi-structured observation. Setting: Haiti is among the world’s most underdeveloped nations for WASH infrastructure. In rural areas of the country, fewer than half of households have access to clean water and 22% have adequate sanitation facilities. The study evaluated a ‘train-the-trainer’ programme in rural southeast Haiti. Methods: Participants were 22 CHW supervisors responsible for training and monitoring the work of rural junior health agents (JHAs). The training programme was delivered by international trainers and consisted of 1 week of classroom training and 1 week of community-based experiential learning. Paired-samples t-tests were used to assess changes in knowledge, and robust qualitative methods were used to analyse focus group discussion and observation data. Results: Participants showed statistically significant improvement in WASH knowledge from pre- to post-test. In experiential supervisor-led training sessions, participants effectively conveyed course content to JHAs, and successful home visits demonstrated the potential for content to reach community members. The programme would benefit from providing educational materials to enhance dissemination. Participants expressed dissatisfaction with translations of course content and requested more frequent training, more emphasis on leadership development and specific content areas for future training. Conclusion: The CHW training programme enhanced trainees’ WASH knowledge and capacity to educate JHAs to disseminate the knowledge to local residents. The train-the-trainer model offers a cost-effective strategy for building local capacity for health education.

Abstract:
Abstract Objectives To document the perceptions of an elementary school director in Mexico City concerning the implementation of guidelines for vendors of food and beverages in schools (the Guidelines). Methods We interviewed the director of an elementary school in order to document his perceptions of the Guidelines, his experience –including the day-to-day difficulties encountered– while implementing them, the actors involved in this process and the food and beverages sold at his school. Data analysis was performed through Atlas-ti ® software. Results The level of information reported by the director on the origins of the Guidelines was adequate. Among the causes he referenced for their introduction was the growing prevalence of obesity and diabetes, which he indicated warranted the regulation of food and beverage sales in schools. With regard to the foods and beverages permitted for sale, the information reported was only partial; he recognized that restricted foods were in fact being sold. Information on the Guidelines had been transmitted to him orally by the school district supervisor, and he was provided no list of permitted and prohibited foods and beverages. There were no documents or teaching materials available in the school which could be used for disseminating this information and for training the members of the school committee in charge of supervising the implementation of the Guidelines. To ensure compliance with the Guidelines, the director advised that a specific committee composed of parents and the school district supervisor was monitoring the quality, hygiene and nutritional value of the food. The director believed that prohibiting sugary drinks in schools had produced a substantial and positive change: students had reduced their consumption of soda with favorable health results. In addition, he affirmed that, owing to the implementation of the Guidelines, the students had access to nutritious and quality food. Conclusions Although the school director is a key actor for ensuring Guideline compliance, he lacks the necessary information for implementing the Guidelines correctly given a lack of teaching material and adequate training. Abstract:
Food hygiene is indisputably one of the most important issues in terms of public health and safety. Food workers are the leading staff members for the compliance of the basic hygiene rules. COVID-19 pandemic has attracted more attention especially for the exported goods from the higher case countries. Even though there is no evidence about the food contamination and COVID-19 risk, according to the European Food Safety Authority, food hygiene practices are important to avoid any contamination risks. Managers are known to have a significant influence on the behavior of their subordinates. In the relevant literature, it is stated that “interactional justice”, which is called the behavior of managers concerning their subordinates and providing adequate information and explanation about the decisions taken, is the most important indicator of the performance and efficiency of subordinates. From this point on, it is thought that interactional justice will affect the behavior of food workers to comply with the hygiene rules. In this research, employees' perceptions of interactional justice are planned to be measured using the informational and interpersonal justice subscales of the organizational justice scale. The tendencies to comply with the hygiene rules will be measured by asking the requirements of the HACCP food security system. At the end of this study, we expect to reach the processing operation involved workers of the investigated food companies.

The preservation and promotion of health in youth is one of the most urgent and important health and social problems at the present stage. In the Russian Federation, sanitary supervision of institutions of higher professional education is carried out both for public facilities and for universities, where there are no specially developed sanitary rules and regulations. From the positions of modern preventive medicine, it is impossible to carry out reasonable health-improving measures in educational institutions of a medical profile without hygienic assessment of microclimate conditions, chemical composition of indoor air, levels of illumination, noise and electromagnetic fields. The article is devoted to the assessment of the health state of medical students in the conditions of various social and hygienic, ecological and biological factors. The hygienic characteristic of the levels of illumination and microclimate of the classrooms is given. The protection of students' health from the negative impact of electromagnetic fields from computers is also an important medical, technical and social task. Significant emphasis in the article is given to a problem of spreading bad habits, including smoking and drinking alcohol. Abstract:
This work is devoted to the problems of hygiene and epidemiology of the Helicobacter pylori infection. In recent years among the causative agents of infectious diseases, Helicobacter pylori (lat. Helicobacter pylori, HP.) takes a special place. Analysis of domestic and foreign literature had shown that at this time there is a high prevalence of Helicobacter infection (HI). It has been diagnosed in almost half of the world’s population. Published data of the Russian researchers point out its high occurrence among the population of Russia (56-88%). Analysis of the annual report of the Federal Service for Supervision in Protection of the Rights of Consumer and Man Wellbeing of the Russian Federation for 2017 showed an effective system of epidemiological and social and hygienic control of HI to have not been developed yet. Considering multiple mechanisms, ways and factors of transmission of the causative agent of HI, directions of scientific researches on the prevention of its distribution are planned. The solution of the hygienic and epidemiological problems of preventing helicobacteriosis can be carried out only by the consolidated participation of specialists of all directions of hygiene and laboratory science, as well as epidemiologists, microbiologists, and public health specialists. Clinical observations and laboratory results suggest a significant role of HP in human pathology: it is an etiological factor of more than half of all gastritis; it is found in more than 95% of patients suffering from duodenal ulcer, in 70-80% of persons with gastric ulcer, and in 60-70% of cases with gastric cancer. Accumulated over last 3 decades data indicates that up to 60-90% of all cases of stomach cancer may be associated with this infection. Against the background of such high numbers of the detection of this pathogen, questions arise why measures of hygienic prevention of Helicobacter pylori have not yet been developed, and scientific institutions of the epidemiological and hygienic profile are not sufficiently puzzled by their development.

Abstract:
Office-based workplaces are an important but understudied context for infectious disease transmission. We examined the feasibility of two different sensors (Opos and Bluetooth beacons) for collecting person-to-person contacts and hand hygiene in office-based workplaces. Opo is an interaction sensor that captures sensor-to-sensor interactions through ultrasonic frequencies, which correspond to face-to-face contacts between study participants. Opos were additionally used to measure hand hygiene events by affixing sensors to soap and alcohol-based hand sanitizer dispensers. Bluetooth beacons were used in conjunction with a smartphone application and recorded proximity contacts between study participants. Participants in two office sites were followed for one-week in their workplace in March 2018. Contact patterns varied by time of day and day of the week. Face-to-face contacts were of shorter mean duration than proximity contacts. Supervisors had fewer proximity contacts but more face-to-face contacts than non-supervisors. Self-reported hand hygiene was substantively higher than sensor-collected hand hygiene events and duration of hand washing events was short (median: 9 seconds, range: 2.5–33 seconds). Given that office settings are key environments in which working age populations spend a large proportion of their time and interactions, a better characterization of empirical social networks and hand hygiene behaviors for workplace interactions are needed to mitigate outbreaks and prepare for pandemics. Our study demonstrates that implementing sensor technologies for tracking interactions and behaviors in offices is feasible and can provide new insights into real-world social networks and hygiene practices. We identified key social interactions, variability in hand hygiene, and differences in interactions by workplace roles. High-resolution network data will be essential for identifying the most effective ways to mitigate infectious disease transmission and develop pandemic preparedness plans for the workplace setting.

Abstract:
Social firms are located on the general labor market and employ 30–50% of severely disabled people. Findings on personal and work-related impacts for employees and supervisors during the COVID-19 pandemic are not yet available and will be investigated in the present study. Using the approach of a method triangulation, focus groups with employees and individual interviews with supervisors of several social firms from the North of Germany were combined and collected in parallel. Between July and November 2020, 16 semi-structured telephone interviews with supervisors and three focus groups with 3–6 employees each working within the same team (14 employees in total) were conducted. Both formats were recorded, transcribed, anonymized, and analyzed by using Mayring’s qualitative content analysis. Because a large proportion of the employees and supervisors interviewed worked in the gastronomy sector, they were particularly affected by the “restriction of social contacts” beginning in March 2020. Hygiene and distance regulations were implemented and personnel planning and distribution of work were adapted. Challenges were raised for employees with disabilities due to the implementation of hygiene and distance regulations, a sudden loss of work, lacking routines, additional work, a lack of movement, social contacts and financial challenges. Both employees and supervisors reported fears of infection, conflicts, additional work and fears of job loss. Additionally, supervisors dealt with less staffing, challenges in detaching from work and a strained economic situation. Overall, new insights were gained into the work-related impacts for employees and supervisors in inclusive workplaces during the current COVID-19 pandemic but further research on health-promoting structures is needed.

Abstract:
Subject. Individualized etiotropic prevention of caries in children using bacteriophages. The goal is to study the peculiarities of the carioprophylactic effect of bacteriophages that are part of the drug "Fagodent." Methodology. The study involved 30 somatically safe children of 7―12 years of age with equal gender distribution and a decompensated caries course. The intensity of caries was determined by the formula KPU+CP. Patients with confirmed cariogenic bacteria dysbiosis were selected according to the CRT-bacteria test (Ivoclar company) at CFU/ml≥105. The method involves the collection of stimulated saliva, inoculation and incubation in a thermostat at a temperature of 37 °C for 2 days, the analysis of the number of colonies on a standard scale. The Green―Vermillion Hygiene Index (OHI-S) was also taken into account with an average of 0.9, children with high rates were excluded from the study. Results. After the course administration of the drug, the colonization rate of the oral cavity with cariogenic microorganisms decreases, which is reflected in the decrease in the Green―Vermillion (OHI-S) hygiene indices from 0.9±0.14 to 0.5±0.1 (p<0.05) and the test CRT-bacteria (Ivoclar Vivadent) CFU / ml from ≥105 to ≤105 (p<0.05). Upon further observation, within 3 months after the course of phagotherapy, the levels of contamination of the oral cavity with cariogenic microorganisms and hygiene were reliably kept low. Hygienic indicators also changed after the administration of the drug “Fagodent”: the Green― Vermillion hygiene index significantly decreased from 0.9±0.14 to 0.5±0.1 after 2 weeks and to 0.7±0.1 after 3 months. Findings. Clinical studies have proven the effectiveness of the drug "Fagodent" based on bacteriophages in the complex treatment and prevention of dental caries. The proposed scheme for the individual use of the drug under the supervision of a doctor does not require special equipment, is simple to implement and shows high positive results, which allows us to recommend it for the prevention of caries in children.

Abstract:
While appropriate hand hygiene practices (HHP) are protective against infections, the paucity of evidence on global estimates and determinants of HHP in adolescents limits effective design and planning of intervention to improve HHP in young people. We examined the prevalence and correlates of HHP in adolescents. We used nationally representative data from the Global School-based Student Health Survey (2003–2017) from 92 countries. HHP were categorized as “appropriate”, “inappropriate” and “lacking” based on the information about “hand washing before eating”, “hand washing after using the toilet”, and “hand washing with soap”. Multinomial logistic regression analyses were used to assess the role of socio-demographic, health, lifestyle, school, and family-related variables in HHP. Among 354,422 adolescents (13–17 years), only 30.3% were found to practice appropriate hand hygiene. Multivariable models suggest that sedentary behavior (adjusted relative risk ratio (ARRR) 1.41, 95% CI 1.31–1.51)), and bullying victimization (ARRR 1.20, 95% CI 1.10–1.30) promoted inappropriate HHP. In contrast, parental supervision (ARRR 0.55, 95% CI 0.50–0.59) and parental bonding (ARRR 0.81, 95% CI 0.75–0.87) were protective against inappropriate HHP. From a policy perspective, hand hygiene promotion policies and programs should focus on both school (bullying, exercise) and family-level factors (parental supervision and parental bonding) factors.

Abstract:
Recent historical investigation into the rise of ‘biopolitical modernity’ in China has shed some surprising light. While it was long thought that British public health initiatives entered China via Hong Kong, the recent work of Ruth Rogaski, Philippe Chemouilli and others has established that it was actually early Japanese colonialism that played the crucial role. It was the Meiji Empire's hygiene reform projects in Taiwan and Manchuria that provided the model for Republican China. Curiously overlooked by medical historians has been one of the major early works of Japanese public health that directly inspired and guided this colonial medical enterprise. This was that of the Japanese health reformer and colonial officer, Gotō Shinpei (1857–1929), and it was undertaken in Munich as a doctoral thesis under the supervision of Max von Pettenkofer. In this article, I focus on the way in which Shinpei dealt in his thesis with the relations between centralisation and local self-administration as one of the key issues facing hygienic modernisation and colonial biopolitical control.
