Abstract:
Vitamin D insufficiency remains a costly pandemic in veterans. Treatment requires achievement of desired 25-hydroxyvitamin D [25(OH)D] concentrations. The frequency with which 25(OH)D should be measured following treatment remains speculative. A retrospective analysis of veterans with vitamin D insufficiency was conducted. The group was stratified on the basis of initial 25(OH)D and assessed for frequency of follow-up 25(OH)D concentrations. Over 3 years, 278 patients with insufficient 25(OH)D concentrations were identified. Of these, 87 (31%) patients had subsequent levels assessed in the year following initial documentation of vitamin D insufficiency. The likelihood of follow-up testing was unrelated to the initial vitamin D level. In the patients with follow-up 25(OH)D levels, 90% eventually achieved a serum level of 30 ng/mL or greater. Veterans with vitamin D insufficiency have inadequate serial monitoring of 25(OH)D concentrations.

Abstract:
Analysis of organizational documentation, sociometric survey and observation data from a two-year field study of an R&D organization suggests that a new type of research and development (R&D) organization, called the conceptual organization, is emerging. The conceptual organization relies on and facilitates collaboration in research and development; it is based on a long-term vision that addresses large complex and challenging problems of national and global importance. Its purpose is to work towards this vision, quickly and effectively contributing to relevant dynamic knowledge bases and meeting diverse stakeholder needs with minimum capitalization and start-up costs. To achieve this, it has an explicit conceptual organizational structure in addition to a physical structure, both of which are interwoven across other external organizational and physical structures. Conceptual organizations engage scientists through the appeal of their vision and socio-technical infrastructures that encourage and facilitate collaboration. Challenges for conceptual organizations may arise due to conflicts with traditional norms and practices embedded in university and R&D settings.

Abstract:
Clostridium difficile and staphylococcal infections are associated with increased morbidity, mortality and healthcare costs. Vitamin D deficiency may also contribute to increased healthcare costs. There is increasing evidence that vitamin D may have an antimicrobial role. We examined the relationship of serum 25(OH)D levels to staphylococcal and C. difficile infections to determine if vitamin D deficiency was associated with adverse outcomes. In the outpatient setting, vitamin D deficiency in patients with C. difficile and staphylococcal infections were associated with significantly increased total outpatients costs and fee-based consultation. Laboratory expenses had a trend towards higher costs in the vitamin D-deficient group but did not reach statistical significance. The differences were most clearly seen in the in-patient group with enhanced laboratory, pharmacy and radiology costs. These differences resulted in vitamin D-deficient patients with C. difficile or staphylococcal infections having costs more than five times higher than the non-deficient patients. The total length of hospital stay was four times greater in the vitamin D-deficient group. In addition, the total number of hospitalizations was also significantly greater in the vitamin D-deficient group. Surgery costs demonstrated a tendency to be higher in the vitamin D-deficient group but failed to reach statistical significance. Vitamin D deficiency is intimately linked to adverse health outcomes and costs in Veterans with staphylococcal and C. difficile infections in North East Tennessee. We recommend that vitamin D status be checked in patients with these infections and appropriate therapy be instituted to restore vitamin D level to normal in an expeditious manner.

Abstract:
The five texts contained in Oxford, Bodleian Library, MS Douce 114 (c. 1420-50) are seminal to understanding the centrality of asceticism in medieval English devotional literature. This thesis addresses the ways in which Douce 114 can be comprehended as a ‘whole book’ and as such outlines a transformation from extreme bodily mortification (in its first text) to the mortification of mind (in its last). It suggests that the manuscript was envisioned as a spiritual tool, its contents designed to be read in order, and that the central theme of asceticism is a hermeneutical device which guides the (Carthusian) reader’s spiritual development. The introduction provides a history of Christian asceticism while the first chapter contextualises attitudes to the phenomenon in late-medieval England. Chapters two and three examine the themes of Passion devotion and imitatio Christi in the Life of Elisabeth of Spalbeek, chapter four addresses the nature of embodiment and earthly purgatory in the Life of Christina Mirabilis, and chapter five examines the inherent problem of misguided bodily imitation of spiritual exemplars in reference to the Life of Marie of Oignies. In conclusion, chapter six argues that the Life of Catherine of Siena and Henry Suso’s Seven Points of True Love and Everlasting Wisdom, which emphasise the transcendence of bodliness, clarify the true inwardly ascetical nature of the preceding texts.

Background: Amongst the few effective preventive strategies for preeclampsia (PE), a leading cause for maternal and fetal morbidity and mortality, a recent area of study involves the association between vitamin D status and PE. The best biomarker for vitamin D is 25-hydroxyvitamin D (25(OH)D), however, there is lack of consensus in the recent literature on thresholds that decrease the risk of PE. Objectives: To quantify the relationship between maternal concentration of 25(OH)D in early pregnancy and the subsequent diagnosis of PE, adjusting for confounding factors; and to determine if there is a dose-response association between maternal plasma 25(OH)D in early pregnancy and the risk of developing PE. Design: A case-control study nested in two prospective cohorts conducted in Quebec City and Halifax between 2002 and 2010. Participants: Pregnant women (n= 169 cases with pre-eclampsia and 1966 controls ). Outcome: PE. Methods: Maternal serum banked before 20 weeks of gestation was used to measure 25(OH)D using chemiluminescence immunoassay DiaSorin Liaison. Results: Women who developed PE had a significantly lower 25(OH)D concentration at a mean gestational age of 14 weeks compared with women in the control group (mean±SD 25(OH)D=47.2 ±17.7 versus 52.3 ±17.2 nmol/L, p<0.0001). Women with 25(OH)D <30 nmol/L compared to at least 50 nmol/L had a greater risk of developing PE (aOR =2.02, 95% CI, 1.13-3.62) after adjustment for pre-pregnancy body-mass index, parity, maternal age, smoking, season and year of blood collection, gestational age at blood collection and research study site (Quebec City or Halifax). A dose-response relationship explored with splines demonstrated that serum 25(OH)D less than 50 nmol/L increased the risk of PE, but that concentrations above 50 nmol/L did not offer additional statistically significant protective effects. Conclusions: Maternal vitamin D deficiency early in pregnancy defined as 25(OH)D <30 nmol/L may be an independent risk factor for PE. Vitamin D supplementation for women of childbearing age should be explored as a preventive strategy for reducing PE and for promoting a healthier pregnancy.Keywords: Vitamin D, 25-hydroxyvitamin D, preeclampsia

Toutefois, la valeur seuil de réduction du risque de PE n'a pas encore obtenu de consensus. Objectifs: 1) Quantifier la relation entre les concentrations sériques de 25(OH)D en début de grossesse et le diagnostic ultérieur de PE; 2) Déterminer s'il existe une relation dose-effet entre les niveaux sériques de 25(OH)D et la PE. Devis de recherche: étude cas-témoin à partir de deux larges études de cohorte, menées à Halifax et à Québec entre 2002 et 2010. Participants: Femmes enceintes (n= 169 cas avec PE et 1966 témoins). Issue d'intérêt: PE. Méthodologie: Les échantillons de sang recueillis avant les 20 premières semaines de grossesse ont servi à mesurer les taux sériques de 25(OH)D. Les mesures ont été réalisées à l'aide d'un analyseur automatisé d'immunoanalyse par chimiluminescence DiaSorin Liaison.Résultats: Les femmes ayant développé la PE avaient des concentrations de 25(OH)D significativement plus faibles que celles du groupe contrôle, à un âge moyen de 14 semaines de grossesse (moyenne±écart-type 25(OH)D=47.2 ±17.7 versus 52.3 ±17.2 nmol/L, p<0.0001). Les femmes avec des concentrations sériques de 25(OH)D inférieures à 30 nmol/L comparativement à ≥50 nmol/L avaient un risque accru de PE (rapports de cotes ajustés =2.02, 95% IC, 1.13-3.62) après avoir ajusté pour l'indice de masse corporelle pré-grossesse, la parité, l'âge, le tabagisme, la saison et l'année du prélèvement sanguin, l'âge gestationnel lors du prélèvement sanguin et le lieu de recherche (Québec ou Halifax). Une relation dose-effet analysée par une régression Spline a démontré que les concentrations de 25(OH)D <50 nmol/L augmentaient le risque de PE, mais que des concentrations ≥50 nmol/L n'étaient pas statistiquement significativement protectrices. Conclusion: La déficience en vitamine D en début de grossesse serait un facteur de risque de la PE. In this work we characterized two libraries containing BAC clones - EGC_Ba and EGC_Bb - obtained from fragments of genomic DNA of Eucalyptus globulus digested with HindIII and EcoRI respectively. The estimated genome coverage for the two libraries was 16X. Both libraries had inserts with average size of 115kb. Assessment of contamination by extranuclear genome was performed by hybridization of high-density colony filters, ensuring a low rate of contamination. The probability associated with the attempt to find specific sequences in the libraries was calculated at a value greater than 0.99. The identification of several genes involved in lignin biosynthetic pathway, transcription factors and peripheral regions of interest via PCR was performed using an innovative scheme of three-dimensional pools. Of the clones identified, thirteen were chosen for sequencing. The analysis of the sequences obtained allowed identifying the reduced presence of repetitive elements and microsatellites. The identified clones can be used in future researches involving the isolation of genes of QTL and genomic sequence comparison with other species of Eucalyptus.
