Horner’s syndrome is caused by injury to the ipsilateral cervical sympathetic chain. The etiology is multifactorial with many traumatic, malignant and iatrogenic causes. Iatrogenic Horner’s syndrome following thyroidectomy is one such rare but well known entity. Long thoracic nerve palsy has been reported to have traumatic, iatrogenic, and idiopathic causes. The authors describe the case of a 62-year-old man who presented with progressively worsening right shoulder pain, winging of the scapula, and Horner syndrome. A chest CT scan revealed an apical pulmonary mass. To the authors' knowledge, this is the first report of a long thoracic nerve palsy caused by an apical pulmonary tumor.

Aim: To report a case of right-sided iatrogenic Horner's syndrome developed after branchial cleft cyst surgery. Presentation of the Scope: An 8 year-old boy presented with right-sided eyelid ptosis and enophthalmos, and diagnosed as having Horner’s syndrome. Discussion: Ophthalmic examination yielded miosis in the affected eye. Medical history revealed branchial cleft cyst surgery 4 years ago and mild ptosis was identified in the first postoperative day. Conclusion: Iatrogenic Horner syndrome may follow the cleft cyst surgery in on neck.
