Abstract:
We present a strategy for incrementally migrating legacy systems to object-oriented platforms. The migration process consists of six sequential phases and encompasses reverse engineering and reengineering activities. The aim of reverse engineering is to decompose programs into components implementing the user interface and components implementing application domain objects. The identification of objects is centred around persistent data stores and exploits object-oriented design metrics. Wrapping is the core of the reengineering activities. It makes new systems able to exploit existing resources, thus allowing an incremental and selective replacement of the identified objects. The migration strategy has been defined and experimented within the project ERCOLE (Encapsulation, Reengineering and Coexistence of Object with Legacy) on legacy systems developed in RPG for the IBM AS/400 environment.

Abstract:
Introduction: Coronary Artery Calcification (CAC) score may give information in cardiovascular risk stratification asymptomatic individuals. Profiles and distribution of CAC scores are still scarce in Indonesia. This study aimed to evaluate the distribution of CAC based on age and gender in asymptomatic patients. Methods: Subjects were asymptomatic Asian above 40 years-old undergoing cardiovascular check-up, including Computed Tomography (CT) CAC at Siloam Heart Institute, from April 2018 to August 2019. Data were obtained retrospectively and analyzed statistically with IBM SPSS version 22. Results: A total of 1640 patients were enrolled, with males slightly more than half. The mean age was 55,6 ± 9,6 years, with age group of 50-59 years as the majority (35,9%). Almost half of the subjects had zero CAC score. Around two-thirds of females, particularly below 50 years old, had zero CAC scores. CAC scores >400 were more prevalent in males across all age groups. The majority of healthy males had a CAC score between 0-99. There was a positive correlation between age and CAC scores in both genders. Females with CAC score >400 were found mostly after 70 years old, ten years older than males. CAC score >1000 was more prevalent in older males compared to females. Conclusion: The distribution of CAC score is remarkably affected by age and gender. Zero CAC score is found predominant in our subjects. CAC scores of ≥400 are common in males across all age groups. CAC score >1000 is more exclusively found in the elderly male

Abstract:
Since 2018 several FAIR Phase 0 beamtimes have been operated at GSI, Darmstadt. Here the new challenging technologies for the upcoming FAIR facility shall be tested while various physics experiments are performed with the existing GSI accelerators. One of these challenges concerns the performance, reliability, and scalability of the experiment data storage. Raw data as collected by event building software of large scale detector data acquisition has to be safely written to a mass storage system like a magnetic tape library. Besides this long term archive, it is often required to process this data as soon as possible on a high performance compute farm. The C library LTSM (“Lightweight Tivoli Storage Management”) has been developed at the GSI IT department based on the IBM TSM software. It provides a file API that allows for writing raw listmode data files via TCP/IP sockets directly to an IBM TSM storage server. Moreover, the LTSM library offers Lustre HSM (“Hierarchical Storage Management”) capabilities for seamlessly archiving and retrieving data stored on Lustre file system and TSM server. In spring 2019 LTSM has been employed at the FAIR Phase 0 beamtimes at GSI. For the HADES experiment LTSM was implemented into the DABC (“Data Acquisition Backbone Core”) event building software. During the 4 weeks of Ag+Ag@1.58 AGeV beam, the HADES event builders have transferred about 400 TB of data via 8 parallel 10 GbE sockets, both to the TSM archive and to the “GSI green cube” HPC farm. For other FAIR Phase 0 experiments using the vintage MBS (“Multi Branch System”) event builders, an LTSM gateway application has been developed to connect the legacy RFIO (“Remote File I/O”) protocol of these DAQ systems with the new storage interface.

Abstract:
NRCVAX is a complete system of programs, covering all aspects of crystal structure analysis from data reduction to the presentation of results. The system, which is written in a `neutral' Fortran 77, presently exists in two forms. The first runs on a VAX computer under VMS, on an 80386 PC under UNIX and under IBM VM/CMS and MVS/TSO. The second is an adaptation which runs on PC-XT, AT, PS/2 and comparable microcomputers under MS-DOS. The two versions differ somewhat in structure, but very little in code, operation or functionality except for the graphics. The many options of the programs can be selected in a highly interactive manner and because of this the system is very flexible. Most options are assigned default values, however, and it is usually safe to run the routines with a minimum of user input using the defaults. The system will accept data from a wide variety of sources and has interface routines for several other systems. Graphics in the VAX/UNIX version are based on the widely available Tektronix 4000 series protocol, while the microcomputer version supports most common display adapters. It is also possible to prepare files for a variety of plotters, dot-matrix printers and laser printers. Source code is distributed and it should not be difficult to adapt the system to any computer with virtual memory and a Fortran 77 compiler.

Abstract:
Background: Oral iron supplementation causes gastrointestinal side effects. Short-term alterations in dietary iron exacerbate inflammation and alter the gut microbiota, in murine models of colitis. Patients typically take supplements for months. We investigated the impact of long-term changes in dietary iron on colitis and the microbiome in mice. Methods: We fed mice chow containing differing levels of iron, reflecting deficient (100 ppm), normal (200 ppm), and supplemented (400 ppm) intake for up to 9 weeks, both in absence and presence of dextran sodium sulphate (DSS)-induced chronic colitis. We also induced acute colitis in mice taking these diets for 8 weeks. Impact was assessed (i) clinically and histologically, and (ii) by sequencing the V4 region of 16S rRNA. Results: In mice with long-term changes, the iron-deficient diet was associated with greater weight loss and histological inflammation in the acute colitis model. Chronic colitis was not influenced by altering dietary iron however there was a change in the microbiome in DSS-treated mice consuming 100 ppm and 400 ppm iron diets, and control mice consuming the 400 ppm iron diet. Proteobacteria levels increased significantly, and Bacteroidetes levels decreased, in the 400 ppm iron DSS group at day-63 compared to baseline. Conclusions: Long-term dietary iron alterations affect gut microbiota signatures but do not exacerbate chronic colitis, however acute colitis is exacerbated by such dietary changes. More work is needed to understand the impact of iron supplementation on IBD. The change in the microbiome, in patients with colitis, may arise from the increased luminal iron and not simply from colitis.

Abstract:
In the subtropics, bucks show seasonal breeding patterns, and their semen quality decreases during the non-breeding season. Therefore, breeders tend to improve bucks’ semen quality before the breeding season for higher conception rates. In the current study, we hypothesised that simultaneous administration of equine chorionic gonadotrophin (ECG) and melatonin would improve fresh semen quality in bucks before the breeding season. Nine Beetal bucks were randomly assigned (n=3 per treatment) to three treatments: control, melatonin, and melatonin + ECG. Melatonin implants (18 mg; BTC Lab) were placed subcutaneously at the base of the ear. Bucks in the melatonin + ECG treatment were administered ECG (400 IU; Syncro-Part, Ceva Santé Animale) intramuscularly on every fourth day until the end of the experiment. Control bucks were administered normal saline (400 IU; Otuska Pakistan) intramuscularly on every fourth day. Semen was collected twice per week using an artificial vagina (42°C) and immediately evaluated for volume, color, pH, and contaminants. Sperm concentration, motility and kinematics (curvilinear velocity, straight-line velocity, average path velocity, and amplitude of lateral head displacement), viability, DNA, and acrosomal and mitochondrial integrity were monitored using a computer-assisted semen analyzer (AndroVision, Minitube). Weekly concentrations of plasma testosterone and melatonin of all bucks were analysed using radioimmunoassay (Immunotech, Beckman Coulter Ltd.) and enzyme-linked immunosorbent assay (450nm), respectively. Comparisons within and between treatments were made using generalised linear models (repeated-measures analysis of variance). Weekly single-point variance between the treatments was determined (analysis of variance) at P ≤ 0.05 (SPSS ver. 20.0; IBM Corp.). Semen quality (volume, pH, total motility (%), and concentration) improved after Week 4 in the melatonin + ECG treatment compared with the control and melatonin treatments (P<0.05). Similarly, progressive motility (%), viability, DNA, acrosomal and mitochondrial integrity, and sperm kinematics (curvilinear velocity, straight-line velocity, average path velocity, and amplitude of lateral head displacement) improved (P<0.05) after Week 4 in the melatonin + ECG treatment. Similarly, non-viability and ratio of abnormal spermatozoa decreased by Week 3 in the melatonin + ECG treatment (P<0.05) compared with the control and melatonin treatments. Likewise, plasma testosterone concentration (ngmL−1) of bucks was higher (P<0.05) at Week 3 in the melatonin + ECG treatment (4.2±0.2) than in the melatonin (0.8±0.1) and control (1.2±0.1) treatments. Within the melatonin + ECG treatment, plasma testosterone concentration was higher (P<0.05) at Week 5 (4.9±0.2) and Week 9 (4.5±0.1) than at Week 3 (4.2±0.2). Plasma melatonin concentration (pgmL−1) increased (P<0.05) from Week 5 onward in the melatonin + eCG (12.5±0.1) and melatonin (10.2±0.1) treatments compared with the control (2.65±0.1). In conclusion, the simultaneous administration of melatonin and ECG improved fresh semen quality in Beetal bucks.

Abstract:
Background: Early pregnancy failure (EPF) is a common experience for women. Medical management allows for expulsion of the nonviable pregnancy in a controlled manner without any surgical risk. The aim of this study was to compare efficacy and safety of mifepristone followed by misoprostol with misoprostol alone in management of EPF.Methods: This was a prospective comparative interventional randomised clinical study conducted at Shri Maharaja Gulab Singh hospital, Jammu, Jammu and Kashmir India from November 2019 to October 2020. A total of 200 patients with gestational age less than 13 weeks and ultrasound diagnosis of EPF were included in the study and randomly divided into two groups, group A (100 patients) received tab. mifepristone 200 mg orally 24 hrs before the use of Tab misoprostol 800 ug per vaginally. If no expulsion occurs within 4 hours, repeat doses of 400 ug misoprostol were given per vaginally at 4-hourly interval to a maximum of 2 doses in women less than or equal to 9 weeks by ultrasound and 4 doses in women more than 9 weeks by ultrasound. Group B (100 patients) received only Tab misoprostol in similar doses without prior mifepristone. The study was performed after approval from the institutional ethical committee. The data was analysed using computer software Microsoft Excel, Statistical and IBM SPSS version 21.0. The statistical difference in mean value between two groups was tested using unpaired ‘t’ test. The qualitative data was compared using Fischer’s exact test. Results: The success rate was higher in group A 92% than group B where it was 76%. The mean induction-abortion interval and dose of misoprostol required for expulsion were 6.56±.66 hrs in group A and 10.40±4.33 hrs in group B and 1126.88±536.06 ug in group A and 1583.33±364.58 ug in group B. The patients in group A experienced significantly less side effects than those in group B, 19% versus 32% and also required fewer blood transfusions than group B, 2% versus 5%.Conclusions: In the present study we came to the conclusion that mifepristone followed by misoprostol is more effective, safe and acceptable than misoprostol alone.

Abstract:
Background: The digital world is rapidly changing, and so do the required digital skills. As physical devices and software are adapted to meet new possibilities and demands, individuals’ skills must adapt to technological advancement. Digital literacy is increasingly used in the public discourse, becoming a core requirement of students, academics, patients and healthcare professionals. Assessing nursing students’ digital literacy at the entry-level is vital to ascertain their abilities to use digital technologies. This study aimed to assess basic digital literacy among first-year nursing students at a selected university in South Africa. Methods: This cross-sectional study was conducted from 1st March 2019 to 31st May 2019 at a university in South Africa. The population consisted of 82 nursing students from the first-year students in the 4-year nursing programme. The convenience sampling technique was used to determine the participants of this study, and 76 respondents completed a structured questionnaire. Data were analysed descriptively using the Statistical Package for Social Sciences (SPSS®) software (version 25) from the International Business Machines Corporation (IBM®). A reliability test of the instrument was conducted, and Cronbach Alpha was 0.85.Results: The students’ digital literacy included: (i) basic computer skills — performing basic computer operations, MS Word and PowerPoint; (ii) internet skills — using e-mail, Moodle®, social media platforms, accessibility to the internet; and internet searches (iii) digital device usage — desktop, laptops, tablets and smartphones. Overall, the mean for internet skills was 3.61 (SD = ±0.62), 3.11 (SD = ±0.85) for computer skills, and 3.00 (SD = ±0.47) for digital device usage. Computer skills was positively correlated with internet skills (r = 0.278, p = .012) and computer skills corrected with digital device usage (r = 0.384, p < .001). The overall score for internet skills was higher in the female group than in males, with the mean of 4.00 (SD = ±0.62) and 3.61 (SD = ±0.54), respectively (t74 = -0.405, p = .019). Furthermore, the overall score for digital device usage was higher in the age group of 20 years and above with a mean of 3.19 (SD = ±0.38) than in the age group under 20 with a mean of 2.90 (SD = ±0.48), and those differences were statistically significant (t74 = -2.420, p = .018).Conclusions: An adequate digital literacy at the entry-level of the nursing programme is a foundation and a critical factor to academic success and future use of technology in nursing education and practice. Having adequate digital literacy among nursing students would positively impact their ability to perform electronic documentation, communicate and collaborate, and search for information to support evidence-based practice.

"Changes of Electroencephalographic Bicoherence during Isoflurane Anesthesia Combined with Epidural Anesthesia." Anesthesiology 97, no. 6 (December 1, 2002): 1409–15. http://dx.doi.org/10.1097/00000542-200212000-00012.

Abstract:
Background The authors previously reported that, during isoflurane anesthesia, electroencephalographic bicoherence values changed in a fairly restricted region of frequency versus frequency space. The aim of the current study was to clarify the relation between electroencephalographic bicoherence and the isoflurane concentration. Methods Thirty elective abdominal surgery patients (male and female, aged 34-77 yr, American Society of Anesthesiologists physical status I-II) were enrolled. After electroencephalogram recording with patients in an awake state, anesthesia was induced with 3 mg/kg thiopental and maintained with oxygen and isoflurane. Continuous epidural anesthesia with 80-100 mg/kg 1% lidocaine was also administered. Using software they developed, the authors continuously recorded the FP1-A1 lead of the electroencephalographic signal and expired isoflurane concentration to an IBM-PC compatible computer. After confirming the steady state of each isoflurane (end-tidal concentration at 0.3, 0.5, 0.7, 0.9, 1.1, 1.3, and 1.5%), electroencephalographic bicoherence values were calculated. Results In a light anesthetic state, electroencephalographic bicoherence values were low (generally < or = 15.0%). At increased concentrations of isoflurane, two peaks of electroencephalographic bicoherence emerged along the diagonal line (f1=f2). The peak emerged at around 4.0 Hz and grew higher as isoflurane concentration increased until it reached a plateau (43.8 +/- 3.5%, mean +/- SD) at isoflurane 0.9%. The other peak, at about 10.0 Hz, also became significantly higher and reached a plateau (32.6 +/- 9.2%) at isoflurane 0.9%; at isoflurane 1.3%, however, this peak slightly decreased. Conclusion Changes in the height of two electroencephalographic bicoherence peaks correlated well with isoflurane concentration.

Abstract:
Purpose The purpose of this paper is to conduct a state-of-the-art review of the ongoing research on the Industry 4.0 phenomenon, highlight its key design principles and technology trends, identify its architectural design and offer a strategic roadmap that can serve manufacturers as a simple guide for the process of Industry 4.0 transition. Design/methodology/approach The study performs a systematic and content-centric review of literature based on a six-stage approach to identify key design principles and technology trends of Industry 4.0. The study further benefits from a comprehensive content analysis of the 178 documents identified, both manually and via IBM Watson’s natural language processing for advanced text analysis. Findings Industry 4.0 is an integrative system of value creation that is comprised of 12 design principles and 14 technology trends. Industry 4.0 is no longer a hype and manufacturers need to get on board sooner rather than later. Research limitations/implications The strategic roadmap presented in this study can serve academicians and practitioners as a stepping stone for development of a detailed strategic roadmap for successful transition from traditional manufacturing into the Industry 4.0. However, there is no one-size-fits-all strategy that suits all businesses or industries, meaning that the Industry 4.0 roadmap for each company is idiosyncratic, and should be devised based on company’s core competencies, motivations, capabilities, intent, goals, priorities and budgets. Practical implications The first step for transitioning into the Industry 4.0 is the development of a comprehensive strategic roadmap that carefully identifies and plans every single step a manufacturing company needs to take, as well as the timeline, and the costs and benefits associated with each step. The strategic roadmap presented in this study can offer as a holistic view of common steps that manufacturers need to undertake in their transition toward the Industry 4.0. Originality/value The study is among the first to identify, cluster and describe design principles and technology trends that are building blocks of the Industry 4.0. The strategic roadmap for Industry 4.0 transition presented in this study is expected to assist contemporary manufacturers to understand what implementing the Industry 4.0 really requires of them and what challenges they might face during the transition process.

Abstract:
The IBA myQA® SRS high-resolution solid-state detector was evaluated in the context of robotic radiosurgery delivered using CyberKnife®. The performance was investigated for periodic machine delivery quality assurance (DQA) and patient-specific treatment verification. myQA® SRS is a 140 × 120 mm CMOS matrix with 400 µm resolution, allocated in a cylindrical ABS phantom topped by a hemispheric cap. Evaluations included: periodic DQA tests, angular response, dose-rate dependence and Iris variable aperture collimator field size measurements. For patient-specific QA various intracranial targets were studied (Gamma Index, 3%/1 mm agreement criteria), taking into account also the detector’s angular response. Results for periodic DQA were in accordance with the machine commissioning data. Dose-rate dependence was confirmed, and angular response tests resulted in a signal decay >5% when beams were delivered outside a ±50° amplitude cone with respect to the vertical direction. Concerning patient-specific QA, >50° angled beams elimination led to a Gamma Index passing rates improvement ranging between +3% and +115%. IBA myQA® SRS proved to be a suitable device for many CyberKnife® constancy DQA checks, providing high-resolution real-time results. Patient-specific Gamma tests showed high passing rates once angular dependence corrections were performed, even in high complexity treatments such as those for trigeminal neuralgia targets.

Abstract:
Background: Psychosocial factors, like physical, chemical and biological factors, are also the potential hazards. But, psychological factors, such as stress, are not mentioned in every kinds of occupation. Some important psychosocial manifestations found among the workers in previous studies were headache, depression, frustration, worry, low self-esteem, boredom and lack of sleep. Objective: This study was carried out to evaluate and psychosocial factors that are associated with occupational factor between brick kiln workers and grocery workers. Methods: An analytical cross-sectional study was conducted in Kathmandu Valley during February-March 2015 i, targeting all the brick kiln workers. Firstly, all the registered the brick kilns (106 in number) were visited to form the potential sampling frame. Multi-stage probability proportionate to size sampling was applied to select 400 brick kiln workers from 16 brick kilns who had been working there for more than two years. An equal size unmatched reference group was maintained from the grocery workers. Interviews were conducted by trained health workers. Data was entered in the Microsoft (MS) excel and was transferred into the International Business Machines (IBM) Corporation. Statistical Package for the Social Sciences (SPSS) Statistics 21 was used to perform analysis. Both descriptive and (mean, median and inter-quartile range) and inferential (Mann Whitney U test) statistics were computed. The level of significance was set at <0.05. Results: Nearly 20.0% of exposed workers were <19 years of age among which slightly above three fifth achieved primary education. Almost 40% of brick kiln workers were ever smokers. Positive factors of psychosocial and emotional functioning were lower and negative factors were higher among brick kiln workers. The parameters of relationships such as self-esteem, stress and personal agency and the parameters of emotions like emotional, somatic, chronic fear, anxiety, hopelessness, helplessness, abuse and mal-treatment were better and significantly better respectively, in the reference group than the exposed group. However, the social factor was significantly poor among the reference group i.e. the grocery workers as observed while conducting Mann Whitney U test at p<0.05 level. Conclusion: Among most of the parameters, brick kiln workers' psychosocial functioning was significantly poorer than the grocery workers. Providing formal and/or non-formal education as well as raising awareness about the deleterious effects of the exposure to brick kiln workers should be recommended.

Abstract:
IBM Slovenija je del globalne korporacije IBM, ki je navzoča v več kot 170 državah. Ukvarja se z informacijsko tehnologijo in je na tem področju ena vodilnih na svetu. Zaposluje približno 400 tisoč ljudi. IBM že nekaj let ustvarja in sledi globalne trende na področju IT – to so storitve v oblaku, digitalizacija, internet stvari, kognitivni sistemi in podobno.

Abstract:
Background Digital technologies are shaping medicine and public health. Objective The aim of this study was to investigate the attitudes toward and the use of digital technologies for health-related purposes using a nationwide survey. Methods We performed a cross-sectional study using a panel sample of internet users selected from the general population living in Germany. Responses to a survey with 28 items were collected using computer-assisted telephone interviews conducted in October 2020. The items were divided into four topics: (1) general attitudes toward digitization, (2) COVID-19 pandemic, (3) physical activity, and (4) perceived digital health (eHealth) literacy measured with the eHealth Literacy Scale (eHEALS; sum score of 8=lowest to 40=highest perceived eHealth literacy). The data were analyzed in IBM-SPSS24 using relative frequencies. Three univariate multiple regression analyses (linear or binary logistic) were performed to investigate the associations among the sociodemographic factors (age, gender, education, and household income) and digital technology use. Results The participants included 1014 internet users (n=528, 52.07% women) aged 14 to 93 years (mean 54, SD 17). Among all participants, 66.47% (674/1014) completed up to tertiary (primary and secondary) education and 45.07% (457/1017) reported a household income of up to 3500 Euro/month (1 Euro=US $1.18). Over half (579/1014, 57.10%) reported having used digital technologies for health-related purposes. The majority (898/1014, 88.56%) noted that digitization will be important for therapy and health care, in the future. Only 25.64% (260/1014) reported interest in smartphone apps for health promotion/prevention and 42.70% (433/1014) downloaded the COVID-19 contact-tracing app. Although 52.47% (532/1014) reported that they come across inaccurate digital information on the COVID-19 pandemic, 78.01% (791/1014) were confident in their ability to recognize such inaccurate information. Among those who use digital technologies for moderate physical activity (n=220), 187 (85.0%) found such technologies easy to use and 140 (63.6%) reported using them regularly (at least once a week). Although the perceived eHealth literacy was high (eHEALS mean score 31 points, SD 6), less than half (43.10%, 400/928) were confident in using digital information for health decisions. The use of digital technologies for health was associated with higher household income (odds ratio [OR] 1.28, 95% CI 1.11-1.47). The use of digital technologies for physical activity was associated with younger age (OR 0.95, 95% CI 0.94-0.96) and more education (OR 1.22, 95% CI 1.01-1.46). A higher perceived eHealth literacy score was associated with younger age (β=–.22, P<.001), higher household income (β=.21, P<.001), and more education (β=.14, P<.001). Conclusions Internet users in Germany expect that digitization will affect preventive and therapeutic health care in the future. The facilitators and barriers associated with the use of digital technologies for health warrant further research. A gap exists between high confidence in the perceived ability to evaluate digital information and low trust in internet-based information on the COVID-19 pandemic and health decisions.

Abstract:
AbstractImplementing molecular dynamics on the IBM SP2 parallel computer, we have studied the fracture of two-dimensional notched solids under tension using million atom systems. Brittle materials are modelled through the choice of interatomic potential function and the speed of the failure process, and our interest is to learn about the dynamics of crack propagation in ideal materials. Recent laboratory findings occur in our simulation experiments, one of the most intriguing is the dynamic instability of the crack tip as it approaches a fraction of the sound speed. A detailed comparison between laboratory and computer experiments is presented, and microscopic processes are identified. In particular, an explanation for the limiting velocity of the crack being significantly less than the theoretical limit is provided.

Abstract:
Purpose The purpose of the study is to investigate the impact of technological pillars of Industry 4.0 based on knowledge to adopt the supply chain performance of Thai small and medium-sized enterprises (SMEs) 4.0. In addition, to increase knowledge and understanding of how to apply knowledge in technology 4.0 to improve the efficiency of supply chains and organizations. Design/methodology/approach An integrated model was developed from applying knowledge in five technological pillars of Industry 4.0 such as Internet of things (IoTs), cloud computing, big data and analytics, additive manufacturing and cyber-security. The bibliometric analysis was used to find the relationship between the technological pillars of Industry 4.0 and the literature review. The survey questionnaires were sent to Thai SME 4.0 (manufacturing aspect). Of these, 240 useable responses were received, resulting in a response rate of 65.84%, after then, the exploratory factor analysis (EFA), confirmatory factor analysis (CFA), structural equation modeling (SEM) and validity were used to evaluate the model through IBM SPSS 21 and AMOS 22. Findings EFA showed the four groups of the technological pillars of Industry 4.0, such as support human, automation, real-time and security. These groups positively impact supply chain performance (increase delivery reliability, increase resource efficiency, decrease costs in the supply chain and reduce delivery time). Another important finding is that supply chain performance positively impacts organizational performance in profitability, return on investment (ROI) and sale growth. Originality/value This study is a model development to support the supply chain performance and increase understanding related to applying knowledge in technology 4.0 that remains unclear for SME 4.0.

Abstract:
Background: Prostatic diseases are common urological conditions confronted by the urologist regularly. The clinical presentations may oftentimes be straightforward but at times challenging due to delayed patients' presentation in our setting, occasional overlap of symptoms and signs or when complications associated with the disease supervene. This study determines the difference in clinical presentations of patients histologically diagnosed with prostatic diseases among urology patients in our clinical setting. Materials and Methods: A prospective cross-sectional study conducted among 50 urology patients with indication for Transrectal ultrasound (TRUS) guided prostate biopsy at Usmanu Danfodiyo University Teaching Hospital, Sokoto, North-western, Nigeria. Data collected into a semistructured questionnaire and studied included: sociodemographic parameters, clinical symptoms and signs at presentation, International prostate symptoms score (IPSS), co-morbidities, Digital rectal examination (DRE), and histological findings. Results were analyzed using the computer software, IBM statistical package for the social sciences (SPSS) version 25 (SPSS Inc; Chicago, IL, USA). Relationships between variables were determined using Pearson's chi-square test, and Fisher's exact tests as appropriate. Results: A total of 50 patients were enrolled over the study period with a mean age of 67.32 ± 9.11 years and a range of 50-96 years. Thirty (60.0%) patients had benign prostatic hyperplasia (BPH) while 20 (40.0%) patients had prostatic adenocarcinoma (PCa). One (3.3%) patient with BPH had associated features of chronic prostatitis. The clinical presentations found to be statistically significant in favour of prostatic adenocarcinoma were weight loss, body weakness, cachexia, pallor, and abnormal digital rectal examination. Conclusion: The presence of weight loss, body weakness, cachexia, clinical pallor, and suspicious digital rectal examination at presentation are significant findings to raise a high index of suspicion of the prostate carcinoma. Hence, physicians and urologists coming in contact with prostatic disease patients should be well guided by this in our environment. Keywords: Clinical presentations, prostatic diseases, prostatitis, benign prostatic hyperplasia, prostatic adenocarcinoma
